Suggestions for Radiation Oncologists during the COVID-19 Pandemic
SARS-CoV-2 pandemic and COVID-19 diffusion have recently become an international public health emergency. Cancer patients, as a frail population, are particularly exposed to the risk related to infections. The clinical decision-making process and the organizational workflow of radiotherapy department should be revised in the light of the critical situation. We herein provide practical suggestions derived from the available literature and discussed during an online session held within the e-learning educational program of the European School of Oncology on March 31st 2020
Preliminary bioassays on the susceptibility of stone fruits rootstocks to Capnodis tenebrionis (L.).
Capnodis tenebrionis (L.) (Coleoptera: Buprestidae), the so called Mediterranean flat-headed root-borer, is an economically important phytophagous pest species mainly on stone fruit trees (apricot, plum, cherry, peach and nectarine). Chemicals and Entomopathogenic nematodes are used for the control of adults and neonate larvae, respectively. Further control means are under investigations in order to have more options within Integrated Pest Control strategies. This study is aimed at investigating the susceptibility of rootstocks to the larvae of C. tenebrionis. Two bioassays were carried out during 2016 and 2017. A first bioassay was based on the evaluation of a potential antixenosis action expressed by neonate larvae infesting twigs of rootstocks (Marianna 26, Barrier, Adesoto, Mylaboran 29C, GF677, Garnem, Cab 6P, Max Ma60 and Colt). This bioassay allowed to process a high number of different rootstocks in a short time. It has a preliminary value. The second bioassay assessed the antibiosis influence of the rootstocks through the breeding of larvae (since the neonate ones) on artificial diets containing bark flour of Adesoto, Cab 6P, Colt, Garnem, GF677, Max Ma60, Montclar and 29C rootstock. The first bioassay showed that Colt, Mylaboran 29C and GF677 were the most susceptible rootstocks to larval infestation of C. tenebrionis and Max Ma60 was less favorable to the pest. Concerning the effects of the diet, larvae reared on a diet containing Montclar, Cab 6P and GF 677 bark flour had a mean daily increase of their weight higher that those reared on cortex tissues of other genotypes whereas Garnem and Colt had a lower increase
Role of brachytherapy in the treatment of cancers of the anal canal. Long-term follow-up and multivariate analysis of a large monocentric retrospective series.
BACKGROUND AND PURPOSE: There are few data on long-term clinical results and tolerance of brachytherapy in anal canal cancer. We present one of the largest retrospective analyses of anal canal cancers treated with external beam radiotherapy with/without (±) chemotherapy followed by a brachytherapy boost.
MATERIALS AND METHODS: We performed a retrospective analysis of clinical results in terms of efficacy and toxicity. The impact of different clinical and therapeutic variables on these outcomes was studied.
RESULTS: From May 1992 to December 2009, 209 patients received brachytherapy after external beam radiotherapy ± chemotherapy. Of these patients, 163 were stage II or stage IIIA (UICC 2002) and 58 were N1-3. According to age, ECOG performance status (PS), and comorbidities, patients received either radiotherapy alone (58/209) or radiochemotherapy (151/209). The median follow-up was 72.8 months. The 5- and 10-year local control rates were 78.6 and 73.9 %, respectively. Globally, severe acute and late G3-4 reactions (NCI-CTC scale v. 4.0) occurred in 11.2 and 6.3 % of patients, respectively. Univariate analysis showed the statistical impact of the pelvic treatment volume (p = 0.046) and of the total dose (p = 0.02) on the risk of severe acute and late toxicities, respectively. Only six patients required permanent colostomy because of severe late anorectal toxicities.
CONCLUSION: After a long follow-up time, brachytherapy showed an acceptable toxicity profile and high local control rates in patients with anal canal cancer
Concomitant evaluation of cardiovascular and cerebrovascular controls via Geweke spectral causality to assess the propensity to postural syncope
The evaluation of propensity to postural syncope necessitates the concomitant characterization of the cardiovascular and cerebrovascular controls and a method capable of disentangling closed loop relationships and decomposing causal links in the frequency domain. We applied Geweke spectral causality (GSC) to assess cardiovascular control from heart period and systolic arterial pressure variability and cerebrovascular regulation from mean arterial pressure and mean cerebral blood velocity variability in 13 control subjects and 13 individuals prone to develop orthostatic syncope. Analysis was made at rest in supine position and during head-up tilt at 60°, well before observing presyncope signs. Two different linear model structures were compared, namely bivariate autoregressive and bivariate dynamic adjustment classes. We found that (i) GSC markers did not depend on the model structure; (ii) the concomitant assessment of cardiovascular and cerebrovascular controls was useful for a deeper comprehension of postural disturbances; (iii) orthostatic syncope appeared to be favored by the loss of a coordinated behavior between the baroreflex feedback and mechanical feedforward pathway in the frequency band typical of the baroreflex functioning during the postural challenge, and by a weak cerebral autoregulation as revealed by the increased strength of the pressure-to-flow link in the respiratory band. GSC applied to spontaneous cardiovascular and cerebrovascular oscillations is a promising tool for describing and monitoring disturbances associated with posture modification
Time, frequency and information domain analysis of heart period and QT variability in asymptomatic long QT syndrome type 2 patients.
none8siThis study was designed to characterize in time, frequency and information domains heart period (HP) and QT interval variabilities in asymptomatic (ASYMP) long QT syndrome type 2 (LQT2) subjects. HP, approximated as the temporal distance between two consecutive R-wave peaks, and QT, approximated as the temporal distance between the R-wave peak and the T-wave offset, were automatically derived from 24h Holter recordings in 10 ASYMP LQT2 patients and 13 healthy non mutation carriers (NMC) subjects. All analyses were carried out during DAY (from 2 to 6 PM) and NIGHT (from 12 to 4 AM). Mean, variance, spectral power and complexity indices at short, medium and long time scales were assessed over HP and QT beat-to-beat series. Circadian rhythmicity was evident in both NMC and ASYMP LQT2 but ASYMP LQT2 subjects were characterized by higher HP, QT interval and HP variability during both DAY and NIGHT. In addition, multiscale complexity analysis was able to differentiate the groups by showing a higher HP complexity and a lower QT complexity at long time scales in ASYMP LQT2 during DAY. On the Different Abilities of Cross-Sample Entropy and K-Nearest-Neighbor Cross-Unpredictability in Assessing Dynamic Cardiorespiratory and Cerebrovascular Interactions
Nonlinear markers of coupling strength are often utilized to typify cardiorespiratory
and cerebrovascular regulations. The computation of these indices requires techniques describing
nonlinear interactions between respiration (R) and heart period (HP) and between mean arterial
pressure (MAP) and mean cerebral blood velocity (MCBv). We compared two model-free methods
for the assessment of dynamic HP–R and MCBv–MAP interactions, namely the cross-sample entropy
(CSampEn) and k-nearest-neighbor cross-unpredictability (KNNCUP). Comparison was carried
out first over simulations generated by linear and nonlinear unidirectional causal, bidirectional
linear causal, and lag-zero linear noncausal models, and then over experimental data acquired
from 19 subjects at supine rest during spontaneous breathing and controlled respiration at 10, 15,
and 20 breaths minute^-1 as well as from 13 subjects at supine rest and during 60 head-up tilt.
Linear markers were computed for comparison. We found that: (i) over simulations, CSampEn and
KNNCUP exhibit different abilities in evaluating coupling strength; (ii) KNNCUP is more reliable
than CSampEn when interactions occur according to a causal structure, while performances are
similar in noncausal models; (iii) in healthy subjects, KNNCUP is more powerful in characterizing
cardiorespiratory and cerebrovascular variability interactions than CSampEn and linear markers. We recommend KNNCUP for quantifying cardiorespiratory and cerebrovascular coupling

Categorizing the Role of Respiration in Cardiovascular and Cerebrovascular Variability Interactions
Objective: Respiration disturbs cardiovascular and cerebrovascular controls but its role is not fully elucidated. Methods: Respiration can be classified as a confounder if its observation reduces the strength of the causal relationship from source to target. Respiration is a suppressor if the opposite situation holds. We prove that a confounding/suppression (C/S) test can be accomplished by evaluating the sign of net redundancy/synergy balance in the predictability framework based on multivariate autoregressive modelling. In addition, we suggest that, under the hypothesis of Gaussian processes, the C/S test can be given in the transfer entropy decomposition framework as well. Experimental protocols: We applied the C/S test to variability series of respiratory movements, heart period, systolic arterial pressure, mean arterial pressure, and mean cerebral blood flow recorded in 17 pathological individuals (age: 648 yrs; 17 males) before and after induction of propofol-based general anesthesia prior to coronary artery bypass grafting, and in 13 healthy subjects (age: 278 yrs; 5 males) at rest in supine position and during head-up tilt with a table inclination of 60. Results: Respiration behaved systematically as a confounder for cardiovascular and cerebrovascular controls. In addition, its role was affected by propofol-based general anesthesia but not by a postural stimulus of limited intensity. Conclusion: The C/S test can be fruitfully exploited to categorize the role of respiration over causal variability interactions. Significance: The application of the C/S test could favor the comprehension of the role of respiration in cardiovascular and cerebrovascular regulations
Assessment of Durum Wheat (Triticum durum Desf.) Genotypes Diversity for the Integrated Production of Bioethanol and Grains
Wheat straw is an abundant source of lignocellulosic biomass that is generally not utilized for biofuel production, nor for other uses. Recent EU renewable energy directive fosters bioethanol production through lignocellulosic sugars fermentation, but the cost of this process is an issue that often depends on biomass characteristics. Lignin is a class of three-dimensional polymers providing structural integrity of plant tissues. Its complex structure, together with hemicelluloses and uronic acids content, could affect the ability of hydrolyzing biomass to fermentable sugars. To get insights into this variation, a set of 10 durum wheat genotypes was analyzed to determine variation of straw digestibility to fermentable sugars. The results showed that the lignin content was the major factor determining the recalcitrance to enzymatic process. The analysis of Spearman's correlation indicated that the sugar released after enzymatic hydrolysis had a negative connection with the lignin content, while it was positively correlated with the culm length. The possible role of other cell wall components, such as arabinose and uronic acids, was also discussed. This work aimed at analyzing the diversity of lignocellulosic digestibility to fermentable sugars of wheat straw in a small germplasm collection. Some of the selected genotypes were characterized by high sugars digestibility and high grain yield, characteristics that could make biorefining of wheat straw profitable
