What are the experiences of adults returning to work following recovery from Guillain-Barré syndrome? An interpretative phenomenological analysis

Guillain-Barré syndrome (GBS) is a transient inflammatory disorder affecting peripheral nerves, characterised by weakness and numbness in limbs, upper body and face. Residual problems affect a large minority, and complicate return to work. This qualitative study explored the experiences of people who returned to work following their diagnosis of GBS and recovery, to gain insight into factors that facilitated or inhibited this process.
Method. Five people participated in in-depth interviews. Individual and common experiences were explored through interpretative phenomenological analysis.
Findings. Three recurring themes are presented: the perceived value of work; losing and recovering a familiar identity at work; and dilemmas around using support and adaptations at work. Certain individual issues also emerged but are beyond the scope of this article. Participants tended to measure their recovery in terms of returning to work yet continued to experience certain physical and psychosocial difficulties at work related to GBS, which required active coping strategies. Limited public awareness of GBS was perceived as a hindrance when returning to work.
Conclusion. This study provides a rich account of the experiences that people encounter returning to work following GBS. Rehabilitation specialists may offer more effective preparation for this process, drawing upon the issues identified
Traditional Knowledge of the Ecology of Beluga Whales (Delphinapterus leucas) in the Eastern Chukchi and Northern Bering Seas, Alaska
Traditional ecological knowledge (TEK) has been used opportunistically in biological studies of beluga whales (Delphinapterus leucas) in Alaska, but no previous research has documented this knowledge systematically. This first such effort, which took place in Norton Bay, Buckland, and Point Lay, Alaska, provided descriptions of migratory and local movements, feeding, calving, ecological interactions, and human influences on distribution and behavior. The results are consistent with those of previous studies but add considerable detail, including descriptions of avoidance and habituation responses to anthropogenic noise, which appear to depend in part on association with hunting activities. Making greater use of TEK will benefit both research and management by providing better information and by expanding the collaborative process developed through co-management. On a utilisé le savoir écologique traditionnel (SET) quand l'occasion se présentait, dans le cadre des études biologiques portant sur le bélouga (Delphinapterus leucas) en Alaska, mais ce savoir n'a jamais fait l'objet d'une étude systématique. Bulgarian sport policy 1945-1989: A strategic relation perspective
The 2008 Beijing Olympic Games have stimulated discussions about the success of different sport systems and the Chinese model in particular. Revisiting explanations of sport in the former communist countries of Eastern Europe during the Cold War seems timely, as the current Chinese model of sport was largely designed after the Soviet example established in this period. This paper examines Bulgarian sport policy between 1945 and 1989. It employs a Strategic Relation approach (Jessop, 1990) to analyse sport policy making as a strategic relation closely linked to the dominant state project of building a new stateness. It goes beyond ideological interpretations and argues that the state represents a strategic terrain where these relations have to be established in struggles, the outcomes of which are always uncertain. Furthermore, past and present struggles and their outcomes create various socio-political environments that presuppose the forms of state selectivity and intervention in sport. The process of constructing sport policy was influenced by two main categories of strategic relations: intra-state, including political, organisational and personal relations between the Party, state apparatus and various sport and non-sport organisations and their managers, and transnational, concerning ideological, political, economic and organisational relations with both communist and western countries and international sport organisations
Does the process map influence the outcome of quality improvement work? A comparison of a sequential flow diagram and a hierarchical task analysis diagram
Background: Many quality and safety improvement methods in healthcare rely on a complete and accurate map of the process. Process mapping in healthcare is often achieved using a sequential flow diagram, but there is little guidance available in the literature about the most effective type of process map to use. Moreover there is evidence that the organisation of information in an external representation affects reasoning and decision making. This exploratory study examined whether the type of process map - sequential or hierarchical - affects healthcare practitioners' judgments.Methods: A sequential and a hierarchical process map of a community-based anti coagulation clinic were produced based on data obtained from interviews, talk-throughs, attendance at a training session and examination of protocols and policies. Clinic practitioners were asked to specify the parts of the process that they judged to contain quality and safety concerns. The process maps were then shown to them in counter-balanced order and they were asked to circle on the diagrams the parts of the process where they had the greatest quality and safety concerns. A structured interview was then conducted, in which they were asked about various aspects of the diagrams.Results: Quality and safety concerns cited by practitioners differed depending on whether they were or were not looking at a process map, and whether they were looking at a sequential diagram or a hierarchical diagram. More concerns were identified using the hierarchical diagram compared with the sequential diagram and more concerns were identified in relation to clinical work than administrative work. Participants' preference for the sequential or hierarchical diagram depended on the context in which they would be using it. The difficulties of determining the boundaries for the analysis and the granularity required were highlighted.Conclusions: The results indicated that the layout of a process map does influence perceptions of quality and safety problems in a process. In quality improvement work it is important to carefully consider the type of process map to be used and to consider using more than one map to ensure that different aspects of the process are captured
Do Children\u27s Advocacy Centers improve families’ experiences of child sexual abuse investigations?
Abstract Objective The Children\u27s Advocacy Center (CAC) model of child abuse investigation is designed to be more child and family-friendly than traditional methods, but there have been no rigorous studies of their effect on children\u27s and caregivers’ experience. Data collected as part of the Multi-Site Evaluation of Children\u27s Advocacy Centers were used to examine whether CACs improve caregivers’ and children\u27s satisfaction with investigations. Methods Nonoffending caregiver and child satisfaction were assessed during research interviews, including the administration of a 14-item Investigation Satisfaction Scale (ISS) for caregivers. Two hundred and twenty-nine sexual abuse cases investigated through a CAC were compared to 55 cases investigated in communities with no CAC. Results Hierarchical linear regression results indicated that caregivers in CAC cases were more satisfied with the investigation than those from comparison sites, even after controlling for a number of relevant variables. There were few differences between CAC and comparison samples on children\u27s satisfaction. Children described moderate to high satisfaction with the investigation, while a minority expressed concerns about their experience. Conclusions The CAC model shows promise for improving families’ experiences, but to build upon this promise, agencies will need to systematize procedures for refining and adapting the model as new research becomes available
Exact Likelihood Estimation and Probabilistic Forecasting in Higher-order INAR(p) Models

Tests of the Equivalence Principle with Neutral Kaons
We test the Principle of Equivalence for particles and antiparticles, using
CPLEAR data on tagged K0 and K0bar decays into pi^+ pi^-. For the first time,
we search for possible annual, monthly and diurnal modulations of the
observables |eta_{+-}| and phi_{+-}, that could be correlated with variations
in astrophysical potentials. Within the accuracy of CPLEAR, the measured values
of |eta_{+-}| and phi_{+-} are found not to be correlated with changes of the
gravitational potential. We analyze data assuming effective scalar, vector and
tensor interactions, and we conclude that the Principle of Equivalence between
particles and antiparticles holds to a level of 6.5, 4.3 and 1.8 x 10^{-9},
respectively, for scalar, vector and tensor potentials originating from the Sun
with a range much greater than the distance Earth-Sun. We also study
energy-dependent effects that might arise from vector or tensor interactions.
Finally, we compile upper limits on the gravitational coupling difference
between K0 and K0bar as a function of the scalar, vector and tensor interaction

Recycling of cleach plant filtrates by electrodialysis removal of inorganic non-process elements.
Water use in the pulp and paper industry is very significant, and the U.S. pulp and paper industries as well as other processing industries are actively pursuing water conservation and pollution prevention by in-process recycling of water. Bleach plant effluent is a large portion of the water discharged from a typical bleached kraft pulp mill. The recycling of bleach plant effluents to the kraft recovery cycle is widely regarded as an approach to low effluent bleached kraft pulp production. The focus of this work has been on developing an electrodialysis process for recycling the acidic bleach plant effluent of bleached Kraft pulp mills. Electrodialysis is uniquely suited as a selective kidney to remove non-process elements (NPEs) from bleach plant effluent before they reach the chemical recovery cycle. Using electrodialysis for selective NPE removal can prevent the problems caused by accumulation of inorganic NPEs in the pulping cycle and recovery boiler. In this work, acidic bleach plant filtrates from three mills using different bleaching sequences based on chlorine dioxide were characterized. The analyses showed no fundamental differences in the inorganic NPE composition or other characteristics among these filtrates. The majority of total dissolved solids in the effluents were found to be inorganic NPEs. Chloride and nitrate were present at significant levels in all effluent samples. Sodium was the predominant metal ion, while calcium and magnesium were also present at considerable levels. The feasibility of using electrodialysis to selectively remove inorganic NPEs from the acidic bleach effluent was successfully demonstrated in laboratory experiments with effluents from all these three mills. Although there were some variations in these effluents, chloride and potentially harmful cations, such as potassium, calcium, and magnesium, were removed efficiently from the bleach effluents into a small-volume, concentrated purge stream. This effective removal of inorganic NPEs can enable the mills to recycle bleach effluents to reduce water consumption. The electrodialysis process also effectively retained up to 98% of the organics and can reduce the organic discharge in the mill wastewater. By using suitable commercially available electrodialysis membranes, there were no indications of rapid or irreversible membrane fouling or scale formation, even in extended laboratory scale operations up to 100 hours. Results of laboratory experiments also showed that commercially available membranes properly selected for this process would have good stability to withstand the potentially oxidative conditions of the filtrate. A pilot-scale field demonstration was also conducted at a southern mill, using the D0 filtrate from the bleach plant. During the field demonstration we found serious membrane 2 stack clogging problems, which apparently were caused by fine fibers that escaped through the 5-micron pre-filters, although such a pre-filtration method had been satisfactory in the laboratory tests. Additional R&D is recommended to address this pre-filtration or clogging issue with systems approaches integrating pre-filtration, other separation methods, and stack design. After the pre-filtration/clogging issue is overcome, laboratory development and pilot demonstration are recommended to optimize the process parameters and to evaluate the long-term process parameters. The key technical issues here include membrane lives, control and mitigation of fouling and scaling, and cleaning-in-place protocols. From the data collected in this work, a preliminary process design and economic evaluations were performed for a model mill with 1,000-ton/day pulp production that uses a bleaching sequence based on chlorine dioxide. Assuming 3 m{sup 3} acidic effluents to be treated per ton of pulp produced, the electrodialysis process would require a membrane area of about 361 m{sup 2} for this model mill. The energy consumption of the electrodialytic stack for separation is estimated to be about 750,000 to $1,000,000 for this case. These economics seem attractive, although the overall costs will include additional operating and capital costs, which cannot be estimated at this time. It is recommended that a more detailed process design and economic evaluations be performed, when additional data are available
