    External load application in gait and posture reeducation after diffuse axonal injury of the corpus callosum. A case report

    Traumatic brain injury caused car accidents is the one of the most common causes of diffuse axonal injury typically located at the grey-white matter junction, in the corpus callosum. A 58-year-old female patient Caucasian race was admitted to the Orthopedic and Rehabilitation Unit with head injury, broken right ulnar bone, numerous broken ribs and broken right iliac crest. Neurological examination resulted in right-sided hemiparesis. There were also coordination and balance disorders while sitting and standing. The patient was unable to walk. After physiotherapy treatment included external load application (ankle weights and rucksack with weights) in gait and posture reeducation, patient has improved balance, locomotion and body posture. However, application of external loads during walk and posture reeducation needs to be further investigated with greater number of participants and control group

    ETCS (ESC) and GSM-R (RSC) compatibility tests in polish conditions

    Abstract: The process of verification of the installation of the ‘on-board control-command and signalling’ subsystem is complex and requires numerous and time-consuming checks of both the overall technical documentation for a given ETCS installation on a specific vehicle, as well as checks performed during in-service tests. The experience has shown that the tests carried out in laboratories in simulated conditions and the commissioning tests of the on- board and trackside subsystems do not provide sufficient certainty in terms of the correct integration of the ‘on-board control-command and signalling’ subsystem with ‘trackside control-command and signalling’ subsystem and other subsystems. Therefore, additional tests of ETCS (European Train Control System) and GSM-R (GSM for Railways) on-board configurations with trackside ETCS and GSM-R installations on railway lines were introduced (hereinafter ESC / RSC compatibility tests). The premise of the ESC compatibility tests and RSC is to facilitate the operation of new subsystems, because their correct functioning in a given environment is fully verified at the stage of demonstrating compliance of ESC and RSC. In accordance with Polish requirements, from 1 July 2021, ESC / RSC tests are obligatory for all new, renewed and modernized projects in the field of the ‘on-board control-command and signalling’ subsystem. This obligation is a big challenge for vehicle manufacturers and users, because at that time these tests had not yet been carried out on the network managed by PKP PLK S.A. Accordingly, this article aims to introduce the issues related to the ESC and RSC tests, as well as to describe the roles and responsibilities of the various parties involved. in the implementation of the tests. Zmniejszyła się liczba  osób z objawami neurologicznymi w obu grupach. Nie odnotowano znaczących różnic w wynikach miedzy grupami poza bardziej przeciwbólowym efektem terapii w grupie I (p=0,032).WnioskiMetody fizjoterapii charakteryzują się porównywalną skutecznością w usprawnianiu pacjentów ze zmianami zwyrodnieniowymi odcinka lędźwiowego kręgosłupa. In the first group  was used physical therapy methods  and mobilizations, in the second physical therapy, mobilizations and exercises in  water. To evaluate the effectiveness of the procedure  were  used the Oswestry questionnaire, visual-analog scale,  the author's questionnaire and functional tests.ResultsThere has been a reduction of pain in both groups  of improvement (p <0.001). Reducing the rate of disability in both groups was about 10 percentage points. Has improved the mobility of the lumbar spine by an average of 1.8 cm in group I and 1.6 cm in group II. Decline in the number of people with neurological symptoms in both groups. There were no significant differences in outcome between the groups except more analgesic therapy effect  in group I (p = 0.032).Conclusions Physiotherapy methods are characterized by comparable effectiveness in improving patients with osteoarthritis of the lumbar spine. Disability efficiency and pain are reduced as a result of the applied physical therapy.     Selected physiotherapeutic techniques and immune response in low back pain

    Physiotherapy, as an element of medical rehabilitation, comprises such methods of function improvement as: massage, kinesiotherapy, physical therapy or manual therapy. In this area, medicine offers a wide range of treatment methods, practically at every stage of a patient’s recovery. Physiotherapy is used to enhance quality of life of people with disabilities, chronic diseases or after injuries, but also as a form of prevention of dysfunctions.The aim of the study  is to present the influence of physiotherapy of low back pain on factors of immune response based on literature review.Effectiveness of a given treatment is most easily noticeable in clinical practice. It is usually the patient who evaluates the efficiency of treatment, through experiencing less pain, easier performance of certain actions or overall better functioning in everyday life. Apart from registering the subjective experience of patients, the focus is on finding objective methods of evaluating effectiveness of physiotherapy and on attempts at scientific explanation of noticeable and perceptible influence of rehabilitation treatment. This also applies to the treatment of lumbar-sacral pain. The involvement of many inflammatory mediators such as nitric oxide, interleukins, matrix metalloproteinases, prostaglandin , tumor necrosis factor alpha and a group of cytokines. and a variety of cytokines have already been  identified in the dysfunction of this region

    Physical activity level and mobility of hip, pelvic and spine in sagittal plane during forward bending

    Introduction and purpose Forward bending is a part of standard evaluation of spine mobility during physical examination. More sedentary lifestyle seems to be a factor lead to limited range of motion. However influence of physical activity level on kinematics of joints during bending has not been previously investigated. In this study, kinematics of the hip joint, pelvic and the lumbar spine during forward bending were examined using the three-dimensional motion analysis. Material and method Participants were included to one of two groups: I-higher physical activity level, II-lower physical activity level on the basis of  their leisure time physical activity. Participants performed forward bending with straight knees.Results Minimal value of pelvic angle reached during motion was higher in I group than in II group (p=0.043).Second group demonstrated larger spine range of motion than the first group (p=0.021). Conclusions This results may indicate a limited motor control and core stability associated with lower level of physical activity. Causes of different spino-pelvic motion need further investigations.

    The effect of sanguinarine on the RPMI-7951 and A375 melanoma cell lines

    Background: Considering the resistance of melanoma to standard treatment protocols, the possibility of metastasis and the high mortality risk, the selection of new alternatives seems to be necessary. Compounds of natural origin are a promising option in anti-cancer therapy. One of them — sanguinarine has a wide spectrum of pro-health properties. Thus, the study aimed to assess the effect of the alkaloid on selected melanoma cellular models. Material and methods: Two types of melanoma cell lines were used in the study — A375 and RPMI-7951. The cells were treated with sanguinarine at concentrations ranging from 0.1 to 2 μM for 24 and 48 h. The influence of the alkaloid on such processes as cell death, cell cycle, organization of the main cytoskeletal proteins and migration potential was assessed. In addition, the sensitivity of selected cell lines to sanguinarine was evaluated based on the MTT assay. Results: The results showed that sanguinarine caused a dose-dependent decrease in cell survival compared to the untreated control. Further studies confirmed that it resulted from the pro-apoptotic and anti-proliferative action of the alkaloid. There were also significant changes in the organization of cytoskeletal proteins and the number of cells visible after fluorescent labelling. Moreover, in A375 cells, characteristics of entosis and mitotic catastrophe were noted. Sanguinarine-induced impaired cell migration was also confirmed. Conclusion: According to the authors’ knowledge, they are the first to present the influence of sanguinarine on the basic life processes of the RPMI-7951 cell line and supplement the knowledge regarding the A375 melanoma cells. The present results confirm the anticancer properties of the alkaloid (cytotoxicity, anti-migratory and pro-apoptotic effect)

    COVID-19 symptoms at hospital admission vary with age and sex: results from the ISARIC prospective multinational observational study

    Background: The ISARIC prospective multinational observational study is the largest cohort of hospitalized patients with COVID-19. We present relationships of age, sex, and nationality to presenting symptoms. Methods: International, prospective observational study of 60 109 hospitalized symptomatic patients with laboratory-confirmed COVID-19 recruited from 43 countries between 30 January and 3 August 2020. Logistic regression was performed to evaluate relationships of age and sex to published COVID-19 case definitions and the most commonly reported symptoms. Results: ‘Typical’ symptoms of fever (69%), cough (68%) and shortness of breath (66%) were the most commonly reported. 92% of patients experienced at least one of these. Prevalence of typical symptoms was greatest in 30- to 60-year-olds (respectively 80, 79, 69%; at least one 95%). They were reported less frequently in children (≤ 18 years: 69, 48, 23; 85%), older adults (≥ 70 years: 61, 62, 65; 90%), and women (66, 66, 64; 90%; vs. men 71, 70, 67; 93%, each P < 0.001). The most common atypical presentations under 60 years of age were nausea and vomiting and abdominal pain, and over 60 years was confusion. Regression models showed significant differences in symptoms with sex, age and country. Interpretation: This international collaboration has allowed us to report reliable symptom data from the largest cohort of patients admitted to hospital with COVID-19. Adults over 60 and children admitted to hospital with COVID-19 are less likely to present with typical symptoms. Nausea and vomiting are common atypical presentations under 30 years. Confusion is a frequent atypical presentation of COVID-19 in adults over 60 years. Women are less likely to experience typical symptoms than men

Genotyp ACTN3 R577X został powiązany z wynikami sportowymi i fenotypem mięśniowym. Gen ACTN3 kodujący białko α-aktynina-3, który ulega ekspresji w linii Z szybkich, glikolitycznych włókien mięśniowych ma związek z mocą i siłą. Czynniki genetyczne są zaangażowane w etiologię urazów tkanek miękkich i funkcjonowanie układu krążenia. Celem naszej pracy było zaprezentowanie przeglądu literatury. Niniejsze badanie pokazuje jak  polimorfizm R577X ACTN3 może wpływać na wyniki sportowe i wystąpienie urazów. Jest wyraźnie ukazane, że czynniki genetyczne mogą mieć wpływu na trening sportowy i fizjoterapię. Słowa kluczowe: genetyka, włókna mięśniowe, urazy tkanek miękkich. Summary Genetic factors can interfere with sporting performance. The ACTN3 R577X genotype has been associated with athletic results and muscle phenotypes. The ACTN3 gene that encodes the α-actinin-3 protein, which is, only expressed in the Z line of fast glycolytic muscle fibres was found to associate with power/strength performance. Genetic factors  are implicated in the etiology of soft tissue injuries and function of cardiovascular system. The aim of our study  was to present review of literature. It is demonstrated  in this study how  R577X polymorphism in ACTN3 could influence on sport results and occurrence of injuries. It is clearly shown , that genetic factors may have implication in sport training and physical therapy. Key words: genetics, muscle fibers, soft tissue injuries

    Does Application of Lymphatic Drainage with Kinesiology Taping Have Any Effect on the Extent of Edema and Range of Motion in Early Postoperative Recovery following Primary Endoprosthetics of the Knee Joint?

    Background: The surgery of knee replacement due to degenerative changes is the last step of the treatment. After surgery, a major problem in patients is pain, swelling, intraarticular hematoma, and the restriction of the mobility of the joint. The aim of this work was to determine the effect of Kinesio Taping (KT) on reducing edema of the subcutaneous tissue and improving the range of motion in the joint. Methods: 82 patients were qualified for the study. After surgery, 42 patients received postoperative edema treatment with KT bands, and 40 patients did not receive the treatment. The swelling thickness and range of mobility were measured on the third and eighth days after the operation. Results: A statistical difference between the longitudinal measurements of the KT group and the group without KT application was shown at the level of the fibula head, 25 mm below the fibula neck, and 50 mm below the fibular neck. There were no statistically significant differences in the change in knee angle between the applied and non-applied patients. Conclusion: The lymphatic application technique KT influences the absorption of subcutaneous edema after primary knee joint replacement surgery but has no influence on mobility
