Il don giovanni tenorio di carlo goldoni. Alle origini di un mito moderno
This article investigates, analyses and reassesses the poetic interpretation of the myth of Don Giovanni Tenorio by Carlo Goldoni published in 1735. Goldoni’s Don Giovanni has been largely neglected or dismissed as the product of an early period of artistic immaturity. However, Goldoni’s version of the myth inherited all the most prominent elements of earlier versions (Tirso de Molina, Giovan Battista Andreini and Molière) but adapted it to an extent that future readings of the myth would be fundamentally affected. By transforming the protagonist's religious impiety into sexual incontinence, Goldoni draws a link between Molière's drama (1665) and the version set to music by Mozart in 1787. An analysis of the text with a focus on the reception and adaptation by Goldoni of the early plays reveals the centrality of his Don Giovanni in the modern conception of the myth as well as its role as a literary source for the librettist, Lorenzo Da Ponte.Keywords: Don Giovanni Tenorio – Carlo Goldoni – Wolfgang Amadeus Mozart – Lorenzo da Ponte – Molière – eighteenth century Italian dram
"Per Un Dante Latino": The Latin Translations of the Divine Comedy in Nineteenth-Century Italy
The history of the Latin translations of Dante's Divine Comedy represents a relatively unexplored branch within the field of Dante studies. Between 1416 and 1876, Latin translations of the Commedia were written and edited by seven men of differing backgrounds and professions: Churchmen, humanistic scholars and academics. The early comprehensive attempts in both prose and verse were published between 1416 and 1431, and have recently received some scholarly attention. In contrast, four out of the seven Latin editions of Dante's Commedia stem from the eras that preceded and immediately followed Italian unification and have yet to be studied. This doctoral dissertation addresses this lacuna and examines the history of the Latin translations of the Divina Commedia within nineteenth-century Italy
ANMCO/AIAC/SICI-GISE/SIC/SICCH Consensus Document : percutaneous occlusion of the left atrial appendage in non-valvular atrial fibrillation patients: Indications, patient selection, staff skills, organisation, and training
Atrial fibrillation (AF) is the most common arrhythmia and its prevalence is increasing due to the progressive aging of the population. About 20% of strokes are attributable to AF and AF patients are at five-fold increased risk of stroke. The mainstay of treatment of AF is the prevention of thromboembolic complications with oral anticoagulation therapy. Drug treatment for many years has been based on the use of vitamin K antagonists, but recently newer and safer molecules have been introduced (dabigatran etexilate, rivaroxaban, apixaban, and edoxaban). Despite these advances, many patients still do not receive adequate anticoagulation therapy because of contraindications (relative and absolute) to this treatment. Over the last decade, percutaneous closure of left atrial appendage, main site of thrombus formation during AF, proved effective in reducing thromboembolic complications, thus offering a valid medical treatment especially in patients at increased bleeding risk. The aim of this consensus document is to review the main aspects of left atrial appendage occlusion (selection and multidisciplinary assessment of patients, currently available methods and devices, requirements for centres and operators, associated therapies and follow-up modalities) having as a ground the significant evolution of techniques and the available relevant clinical data
Surgical Aortic Mitral Curtain Replacement: Systematic Review and Metanalysis of Early and Long-Term Results
The Commando procedure is challenging, and aims to replace the mitral valve, the aortic valve and the aortic mitral curtain, when the latter is severely affected by pathological processes (such as infective endocarditis or massive calcification). Given the high complexity, it is seldomly performed. We aim to review the literature on early (hospitalization and up to 30 days) and long-term (at least 3 years of follow-up) results. Bibliographical research was performed on PubMed and Cochrane with a dedicated string. Papers regarding double valve replacement or repair in the context of aortic mitral curtain disease were included. The metaprop function was used to assess early survival and complications (pacemaker implantation, stroke and bleeding). Nine papers (540 patients, median follow-up 41 (IQR 24.5-51.5) months) were included in the study. Pooled proportion of early mortality, stroke, pacemaker implant and REDO for bleeding were, respectively 16.2%, 7.8%, 25.1% and 13.1%. The long-term survival rate ranged from 50% to 92.2%. Freedom from re-intervention was as high as 90.9% when the endocarditis was not the first etiology and 78.6% in case of valvular infection (one author had 100%). Freedom from IE recurrences reached 85% at 10 years. Despite the high mortality, the rates of re-intervention and infective endocarditis recurrences following the Commando procedure are satisfactory and confirm the need for an aggressive strategy to improve long-term outcomes

Le diverse competenze per lo sviluppo dell’accessibilità nella didattica
Nel presente contributo si descrive un’attività svolta all’interno del Gruppo di Lavoro sulle tecniche di Insegnamento e di Apprendimento (GLIA) dell’Università di Genova (UniGe). Si tratta di un corso in modalità e-learning asincrona corredato da video e test sulle tematiche di accessibilità e inclusione. L’obiettivo è quello di migliorare la didattica rivolta alle persone con disabilità o con DSA attraverso l’attenzione alle loro reali esigenze e la condivisione di contenuti teorici, di tecniche e strumenti didattici. Nel corso vengono proposti contenuti sotto forma di brevi video per rendere la comunicazione più diretta. Si intende, infatti, allargare la discussione dal GLIA, fino a raggiungere il maggior numero di persone possibile della comunità UniGe, dai docenti al personale. Il lavoro è stato svolto da un gruppo eterogeneo composto da docenti, instructional designer, membri del Settore servizi per l'inclusione degli studenti con disabilità e con DSA, componenti del servizio e-learning di Ateneo, e da PhD candidate. Al termine del contributo, si presentano i promettenti risultati delle prime fasi di test del corso precedenti al rilascio
Looking Back to Look Forward: What to Expect in a Redo Surgery for a Bioprosthesis Replacement
Redo surgeries are becoming more common because of an increased rate of bioprosthesis implantation. We performed a retrospective study on patients who underwent redo replacement of an aortic and/or mitral bioprosthesis between 2005 and 2018 to evaluate intra-hospital mortality and morbidity. Univariate analysis was performed on the propensity score variables to determine predictors of mortality. A total of 180 patients were enrolled in the study: Group A (replacement of aortic bioprosthesis) with 136 patients (75.56%) and group B (replacement of mitral bioprosthesis ± aortic bioprosthesis) with 44 patients (24.44%). NYHA class ≥ 3 and female sex were significantly more common in group B. Cardiopulmonary-bypass time and aortic cross-clamping time in group A and group B were, respectively, 154.95 ± 74.35 and 190.25 ± 77.44 (p = 0.0005) and 115.99 ± 53.54 and 144.91 ± 52.53 (p = 0.0004). Overall mortality was 8.89%. After propensity score adjustment, Group B was confirmed to have an increased risk of death (OR 3.32 CI 95% 1.02–10.88 p p p < 0.0001). At the univariate analysis, endocarditis, cardiopulmonary-bypass and aortic cross clamping time, NYHA class ≥ 3 and urgency setting were significantly associated to death. Intra-hospital outcomes were acceptable regarding mortality and complications. Patients who need redo surgery on mitral bioprosthesis have an increased risk of post-operative pulmonary and gastrointestinal complications and mortality. Therefore the choice of mitral bioprosthesis at time of first surgery should be carefully evaluated
Enduring Reactive Oxygen Species Emission Causes Aberrant Protein S-Glutathionylation Transitioning Human Aortic Valve Cells from a Sclerotic to a Stenotic Phenotype
: Aims: During calcific aortic valve stenosis (CAVS) progression, oxidative stress and endothelial dysfunction mark the initial pathogenic steps with a parallel dysregulation of the antioxidant systems. Here, we tested whether oxidation-induced protein S-glutathionylation (P-SSG) accounts for a phenotypic switch in human aortic valvular tissue, eventually leading to calcium deposition. Next, we tested whether countering this reactive oxygen species (ROS) surge would prevent these perturbations. Results: We employed state-of-the-art technologies, such as electron paramagnetic resonance (EPR), liquid chromatography-tandem mass spectrometry, imaging flow-cytometry, and live-cell imaging on human excised aortic valves and primary valve endothelial cells (VECs). We observed that a net rise in EPR-detected ROS emission marked the transition from fibrotic to calcific in human CAVS specimens, coupled to a progressive increment in P-SSG deposition. In human VECs (hVECs), treatment with 2-acetylamino-3-[4-(2-acetylamino-2-carboxyethylsulfanylthiocarbonylamino)phenylthiocarbamoylsulfanyl]propionic acid triggered highly oxidizing conditions prompting P-SSG accumulation, damaging mitochondria, and inducing endothelial nitric oxide synthase uncoupling. All the events conjured up in morphing these cells from their native endothelial phenotype into a damaged calcification-inducing one. As proof of principle, the use of the antioxidant N-acetyl-L-cysteine prevented these alterations. Innovation: Borne as a compensatory system to face excessive oxidative burden, with time, P-SSG contributes to the morphing of hVECs from their innate phenotype into a damaged one, paving the way to calcium deposition. Conclusion: Our data suggest that, in the human aortic valve, unremitted ROS emission along with a P-SSG build-up occurs and accounts, at least in part, for the morphological/functional changes leading to CAVS.
