    Arthroplasty as a Choice of Treatment in Hip Surgery

    The hip joint bears the most load in the human body. For this reason, it carries the potential risk of degenerative arthritis in individuals with a functionally active lifestyle. The main goal in the treatment of degenerative arthritis is to achieve pain relief and create a hip joint range of motion close to normal. Even today, it is not possible to transform the hip joint, which has been degenerated due to several reasons and worn out due to the physiological properties of the cartilage structure, back to its natural state. Osteotomies, resection arthroplasties and hip arthrodeses, which are designed to compensate the load distribution affecting the hip and relieve the pain, are still employed methods. Total hip arthroplasty, on the other hand, is an alternative solution for the problem. Cemented, cementless and hybrid methods are widely used for this purpose in total hip arthroplasties. The purpose of hip prosthesis surgery is to shape the bone tips and to fill the fragments with various materials and keep these two structures as separate surfaces. Total hip arthroplasty consists of a femoral component placed in the medullas of the femur and an acetabular component placed in the acetabulum.     As it is known that, at 1. and 2. degree seismic zones, earthquake risk level is almost high and these regions cover ½ of Turkey’s area. In this study, to represent of buildings in these regions, 1/3 scaled 2 story 2 bay reinforced concrete frame under magnitude of approximately cc fA10.0 axial load was tested under reversed-cyclic lateral load. The response envelope curves and lateral load bearing capacities which were obtained from tests and SAP2000 programme, were compared.Ülkemiz topraklarının alan olarak yarıya yakınını kaplayan ve ülke nüfusunun yaklaşık 2/3’ünün bulunduğu 1. ve 2. derece deprem bölgeleri olarak adlandırılan alanlarda deprem riskinin yüksek olduğu bilinmektedir.     DergiPark: 853747tredBu çalışmanın amacı; Türkiye’de 2010-2020 yılları arasında öz düzenleme becerileri konusunda yapılan tez çalışmalarının belirlenen ölçütlere göre incelenmesidir. Çalışma tarama modelinde olup, içerik analizi tekniği ile analiz edilmiştir. Çalışmanın kaynağını YÖK Ulusal Tez Merkezi’nde 2010-2020 yılları arasında yayımlanmış olan, belirlenen konu alanına uygun ve erişime açık olan lisansüstü tezler oluşturmaktadır. Araştırma kapsamında 71 lisansüstü tez incelenmiş ve belirlenen kriterlere göre analiz edilmiştir. Bu çalışma nitel bir araştırmadır. Araştırma verileri incelenirken içerik analizi kullanılmıştır. Ulaşılan veriler tablolaştırılarak SPSS 20 paket programı aracılığıyla gerekli istatistiki verilere ulaşılmıştır. Çalışmanın güvenirliğini sağlamak için kodlama güvenirliği tekrar hesaplanmıştır. Sonuçlar frekans ve yüzde yönünden hesaplanmış ve yorumlanmıştır. Araştırma sonucuna göre en fazla sayıda araştırmanın 2019 yılında yapıldığı, tezlerin çoğunun yüksek lisans tezi olduğu ve Türkçe olarak yazıldığı, en fazla tezin Gazi Üniversitesi’nde yazıldığı, çalışmaların çoğunluğunun Eğitim Bilimleri Enstitüsü’nde ve Eğitim Bilimleri Anabilim Dalı’nda yoğunlaştığı, örneklem bölgelerinde İç Anadolu Bölgesi’nin daha çok tercih edildiği, çalışmalarda daha çok nicel yöntemlerin kullanıldığı, araştırmaların daha çok öğrenciler üzerinde yapıldığı ve ölçek ve form kullanılarak veri toplandığı sonuçlarına ulaşılmıştır. Bu araştırma, alan yazında bu tür bir çalışmaya rastlanmadığı için söz konusu alan açısından önem taşımaktadır.This study aims to examine the criteria determined by the thesis study on self-regulation skills in Turkey between the years 2010-2020. The study is in a scanning model and analyzed by content analysis technique. The source of the study is the postgraduate theses published in the National Thesis Centre of Higher Education Institutions between 2010 and 2020, suitable for the specified subject area and open to access. Within the scope of the research, 71 graduate theses were examined and analyzed according to the determined criteria. This study is qualitative research. Content analysis was used while analyzing the research data. The obtained data were tabulated and the necessary statistical data were obtained through SPSS 20 package program. The coding reliability was calculated again to ensure the reliability of the study. Results were estimated and interpreted in terms of frequency and percentage. According to the results of the research, the highest number of studies were conducted in 2019, most of the theses were master's theses and written in Turkish, the most theses were written in Gazi University, most of the studies were concentrated in the Institute of Educational Sciences and the Department of Educational Sciences. It was concluded that the Anatolian Region was preferred more, quantitative methods were used in the studies, the studies were mostly conducted on students. It was concluded that the data were collected by using scales and forms. This research is of importance in terms of the field in question, since there is no such study in the literature

    Is Cyclic Exercise Performed before Tibial Fixation Effective on Grafts during Anterior Cruciate Ligament Reconstruction?

    Objectives: The necessity of cyclic exercise to pre-stretch the autograft before tibial fixation during ACL reconstruction is unknown. In this study, we evaluated whether there was a statistically significant difference between the results of patients whounderwent cyclic exercise by way of physical examinations, knee joint stability tests, and functional evaluation tests, comparedwith the patients who underwent ACL reconstructions with or without cyclic exercise.Methods: Between March 2016 and May 2018, 59 patients with at least eight months’ follow-up of an ACL reconstruction wereidentified. Thirty patients (Group 1) who underwent cyclic exercise before tibial fixation and 29 patients (Group 2) who did notundergo cyclic exercise were evaluated and compared.Results: The mean age of the patients in Group 1 and Group 2 was 25.9 (range, 18-36) years and 25.2 (range, 18-35) years, respectively. The mean follow-up period in Group 1 was 14.6 (range, 8-22) months and 13.5 months in Group 2 (range, 8-21 months).The mean Lysholm scores of Group 1 and 2 were 95.1 (range, 83-100) and 87.1 (range, 78-100), respectively. The modified Cincinnati scores of Groups 1 and 2 were 28.7 (range, 24-30) and 26.2 (range, 21-30). The mean IKDC subjective knee evaluation scoresin Groups 1 and 2 were 91.9 (range, 83-100) and 86.7 (range, 75-100). The mean thigh atrophy was 1.5 cm in Group 1 and 2.5 cm inGroup 2. In Group 1, 23 patients jumped 85% of the distance compared with the intact side in the single-legged hop test, and 12patients in Group 2 were able to hop this distance successfully.Group 1 had statistically significantly better results in Lysholm activity scores, modified Cincinnati scores, IKDC subjective kneeassessment scores, two-time IKDC activity scale results, comparison of thigh diameters, and single-legged hop tests (p<0.05). Nosignificant difference was found in other examinations and tests.Conclusion: Cyclic exercise during the operation had a positive effect on functional scores. We believe that cyclic exercise shouldbe added to the operative procedur

    The frequency of Duchenne muscular dystrophy/Becker muscular dystrophy and Pompe disease in children with isolated transaminase elevation: results from the observational VICTORIA study

    IntroductionElevated transaminases and/or creatine phosphokinase can indicate underlying muscle disease. Therefore, this study aims to determine the frequency of Duchenne muscular dystrophy/Becker muscular dystrophy (DMD/BMD) in male children and Pompe disease (PD) in male and female children with isolated hypertransaminasemia.MethodsThis multi-center, prospective study enrolled patients aged 3–216 months with serum alanine transaminase (ALT) and/or aspartate transaminase (AST) levels &gt;2× the upper limit of normal (ULN) for ≥3 months. Patients with a known history of liver or muscle disease or physical examination findings suggestive of liver disease were excluded. Patients were screened for creatinine phosphokinase (CPK) levels, and molecular genetic tests for DMD/BMD in male patients and enzyme analysis for PD in male and female patients with elevated CPK levels were performed. Genetic analyses confirmed PD. Demographic, clinical, and laboratory characteristics of the patients were analyzed.ResultsOverall, 589 patients [66.8% male, mean age of 63.4 months (standard deviation: 60.5)] were included. In total, 251 patients (188 male and 63 female) had CPK levels above the ULN. Of the patients assessed, 47% (85/182) of male patients were diagnosed with DMD/BMD and 1% (3/228) of male and female patients were diagnosed with PD. The median ALT, AST, and CPK levels were statistically significantly higher, and the questioned neurological symptoms and previously unnoticed examination findings were more common in DMD/BMD patients than those without DMD/BMD or PD (p &lt; 0.001).DiscussionQuestioning neurological symptoms, conducting a complete physical examination, and testing for CPK levels in patients with isolated hypertransaminasemia will prevent costly and time-consuming investigations for liver diseases and will lead to the diagnosis of occult neuromuscular diseases.
