    本本文报道了12例顽固性老年糖尿病足溃疡患者的康复过程,通过全身评估、足部评估及多学科联合诊治的方法,制定优化的治疗护理方案。在伤口处理方面,首次将活性碳纤维含银敷料应用其中,通过6~12周的创面护理,溃疡愈合。该新型敷料,因活性碳纤维的吸附作用、远红处线的热能功效及银离子抗感染的作用,促进了创面的愈合,体现了物理疗法在顽固性老年足溃疡中的作用,值得在更大范围内验证和推广。12 cases of intractable diabetic foot ulcer in elderly patients was reported, systemic, topical evaluation, multi-disciplinary treatment, and optimized care program could accelerate healing process. Active carbon fiber containing silver dressing was fist used in diabetic foot care, and all ulcers healed in 6~12 weeks. This new dressing promote healing through absorption of activated carbon fibers, the effect of far red lines and the effect of silver ion on anti infection, which manifesting physical therapy assist ulcer recovery

     目的  调查老年糖尿病患者足部畸形趾甲的情况,分析趾甲异常生长的影响因素。方法  对上海地区某社区卫生服务中心229例老年糖尿病患者进行足部检查,包括周围血管病变、周围神经病变、趾甲修剪情况、甲真菌感染情况及趾甲畸形的情况。结果  老年糖尿病患者足部出现趾甲畸形者118例,高达51.52%,增龄、性别、周围血管病变、周围神经病变、修剪趾甲方法及真菌感染是引起患者畸形趾甲生长的原因。结论  重视老年糖尿病患者足部趾甲的护理,做好甲真菌病的治疗,为老年患者提供专业的趾甲修剪服务,预防糖尿病足溃疡的发生与发展。Objective: To investigate the situation of toe deformities in elderly diabetic patients and then analyze the impact factors related to the abnormal growth of toenails. Methods: 229 cases from a community health service center in Shanghai were recruited. All of these patients accepted the foot examination, including determining peripheral vascular disease, peripheral neuropathy and the situation of toenail trimming, onychomycosis and toenail deformities. Results: There were 118 cases of elderly diabetics existing toenail deformities which accounted for 51.52%. Age, gender, peripheral vascular disease, peripheral neuropathy, methods of trimming toenails and fungal infection were factors resulting in toenail deformities. Conclusion: The community health service center should strengthen the care of toenails in elderly diabetic patients and treat onychomycosis appropriately while providing professional service of trimming toenails for the elderly diabetic patients

    Percutaneous closure of postinfarct muscular ventricular septal defects: A multicenter study in China

    AbstractBackgroundSurgical repair is an effective method to treat ventricular septal defect (VSD) complicating acute myocardial infarction (AMI). However, the mortality rate remains high. This study was designed to assess the immediate and mid-term results of transcatheter closure of postinfarct muscular VSDs.MethodsData were retrospectively collected from 42 AMI patients who underwent attempted transcatheter VSD closure between 2008 and 2012 in seven heart centers of China.ResultsNine patients underwent emergent VSD closure in the acute phase (within two weeks from VSD) while the others underwent elective closure. The time between VSD occurrence and closure in emergency group and elective group was 7.7±2.3 days and 35±14.5 days, respectively (p<0.01). The percentage of procedure success in the emergency group and elective group was 77.8% (7/9) and 97% (32/33), respectively (p=0.048). The hospital mortality was higher for emergent closure in comparison to elective closure (66.7% vs. 6.1%, p<0.01). During a median follow-up of 25 months (0–58 months), two patients died at 8 and 29 months, respectively, and no serious complications occurred in other patients.ConclusionInterventional postinfarct VSD closure is a safe and effective approach that can be performed with a high procedural success rate, with favorable outcomes if it can be undertaken >14 days postinfarct

    Silencing the epidermal growth factor receptor gene with RNAi may be developed as a potential therapy for non small cell lung cancer

    Lung cancer has emerged as a leading cause of cancer death in the world. Non-small cell lung cancer (NSCLC) accounts for 75–80% of all lung cancers. Current therapies are ineffective, thus new approaches are needed to improve the therapeutic ratio. Double stranded RNA (dsRNA) -mediated RNA interference (RNAi) has shown promise in gene silencing, the potential of which in developing new methods for the therapy of NSCLC needs to be tested. We report here RNAi induced effective silencing of the epidermal growth factor receptor (EGFR) gene, which is over expressed in NSCLC. NSCLC cell lines A549 and SPC-A1 were transfected with sequence- specific dsRNA as well as various controls. Immune fluorescent labeling and flow cytometry were used to monitor the reduction in the production of EGFR protein. Quantitative reverse-transcriptase PCR was used to detect the level of EGFR mRNA. Cell count, colony assay, scratch assay, MTT assay in vitro and tumor growth assay in athymic nude mice in vivo were used to assess the functional effects of EGFR silencing on tumor cell growth and proliferation. Our data showed transfection of NSCLC cells with dsRNA resulted in sequence specific silencing of EGFR with 71.31% and 71.78 % decreases in EGFR protein production and 37.04% and 54.92% in mRNA transcription in A549 and SPC-A1 cells respectively. The decrease in EGFR protein production caused significant growth inhibition, i.e.: reducing the total cell numbers by 85.0% and 78.3 %, and colony forming numbers by 63.3% and 66.8%. These effects greatly retarded the migration of NSCLC cells by more than 80% both at 24 h and at 48 h, and enhanced chemo-sensitivity to cisplatin by four-fold in A549 cells and seven-fold in SPC-A1. Furthermore, dsRNA specific for EGFR inhibited tumor growth in vivo both in size by 75.06 % and in weight by 73.08 %. Our data demonstrate a new therapeutic effect of sequence specific suppression of EGFR gene expression by RNAi, enabling inhibition of tumor proliferation and growth. However, in vivo use of dsRNA for gene transfer to tumor cells would be limited because dsRNA would be quickly degraded once delivered in vivo. We thus tested a new bovine lentiviral vector and showed lentivector-mediated RNAi effects were efficient and specific. Combining RNAi with this gene delivery system may enable us to develop RNAi for silencing EGFR into an effective therapy for NSCLC

    Integration of Brassinosteroid Signal Transduction with the Transcription Network for Plant Growth Regulation in Arabidopsis

    SummaryBrassinosteroids (BRs) regulate a wide range of developmental and physiological processes in plants through a receptor-kinase signaling pathway that controls the BZR transcription factors. Here, we use transcript profiling and chromatin-immunoprecipitation microarray (ChIP-chip) experiments to identify 953 BR-regulated BZR1 target (BRBT) genes. Functional studies of selected BRBTs further demonstrate roles in BR promotion of cell elongation. The BRBT genes reveal numerous molecular links between the BR-signaling pathway and downstream components involved in developmental and physiological processes. Furthermore, the results reveal extensive crosstalk between BR and other hormonal and light-signaling pathways at multiple levels. For example, BZR1 not only controls the expression of many signaling components of other hormonal and light pathways but also coregulates common target genes with light-signaling transcription factors. Our results provide a genomic map of steroid hormone actions in plants that reveals a regulatory network that integrates hormonal and light-signaling pathways for plant growth regulation

    Descope of the ALIA mission

    The present work reports on a feasibility study commissioned by the Chinese Academy of Sciences of China to explore various possible mission options to detect gravitational waves in space alternative to that of the eLISA/LISA mission concept. Based on the relative merits assigned to science and technological viability, a few representative mission options descoped from the ALIA mission are considered. A semi-analytic Monte Carlo simulation is carried out to understand the cosmic black hole merger histories starting from intermediate mass black holes at high redshift as well as the possible scientific merits of the mission options considered in probing the light seed black holes and their coevolution with galaxies in early Universe. The study indicates that, by choosing the armlength of the interferometer to be three million kilometers and shifting the sensitivity floor to around one-hundredth Hz, together with a very moderate improvement on the position noise budget, there are certain mission options capable of exploring light seed, intermediate mass black hole binaries at high redshift that are not readily accessible to eLISA/LISA, and yet the technological requirements seem to within reach in the next few decades for China

    BiCGCR2: A new extension of conjugate residual method for solving non-Hermitian linear systems

    In the present paper, we introduce a new extension of the conjugate residual (CR) for solving non-Hermitian linear systems with the aim of developing an alternative basic solver to the established biconjugate gradient (BiCG), biconjugate residual (BiCR) and biconjugate A-orthogonal residual (BiCOR) methods. The proposed Krylov subspace method, referred to as the BiCGCR2 method, is based on short-term vector recurrences and is mathematically equivalent to both BiCR and BiCOR. We demonstrate by extensive numerical experiments that the proposed iterative solver has often better convergence performance than BiCG, BiCR and BiCOR. Hence, it may be exploited for the development of new variants of non-optimal Krylov subspace methods

    Management of granulomatous lobular mastitis: an international multidisciplinary consensus (2021 edition)

    Granulomatous lobular mastitis (GLM) is a rare and chronic benign inflammatory disease of the breast. Difficulties exist in the management of GLM for many front-line surgeons and medical specialists who care for patients with inflammatory disorders of the breast. This consensus is summarized to establish evidence-based recommendations for the management of GLM. Literature was reviewed using PubMed from January 1, 1971 to July 31, 2020. Sixty-six international experienced multidisciplinary experts from 11 countries or regions were invited to review the evidence. Levels of evidence were determined using the American College of Physicians grading system, and recommendations were discussed until consensus. Experts discussed and concluded 30 recommendations on historical definitions, etiology and predisposing factors, diagnosis criteria, treatment, clinical stages, relapse and recurrence of GLM. GLM was recommended as a widely accepted definition. In addition, this consensus introduced a new clinical stages and management algorithm for GLM to provide individual treatment strategies. In conclusion, diagnosis of GLM depends on a combination of history, clinical manifestations, imaging examinations, laboratory examinations and pathology. The approach to treatment of GLM should be applied according to the different clinical stage of GLM. This evidence-based consensus would be valuable to assist front-line surgeons and medical specialists in the optimal management of GLM.Improving the Ability of Diagnosis and Treatment of Difficult Disease
