    Association Between Modifiable Risk Factors and Stroke Among Hispanics

    Stroke is a significant public health issue that impacts the health of Hispanic adults. Understanding the multiple risk factors of stroke is an essential strategy to control the disease among this population. The purpose of this study was to examine the association between modifiable risk factors and stroke among Hispanics. The public health prevention framework guided this study. The research questions were designed to determine whether an association exists between modifiable risk factors for stroke among Hispanics. A quantitative, cross-sectional research design was used to conduct secondary analysis of the pooled 2017–2019 Behavioral Risk Factor Surveillance System annual surveys. The sample of 111,429 participants in this study was weighted to represent the national sample of Hispanic adults aged 18 years and older. Unadjusted bivariate models indicate that both obesity and diabetes have a significant association with the diagnosis of stroke. An adjusted logistic regression indicated that the odds of a diagnosis of stroke (outcome) had a statistically significant association with obesity and diabetes (exposure) after controlling for confounding factors, alcohol consumption, smoking, and education among Hispanics. The adjusted logistic regression model suggests that among Hispanics, those with diabetes are 8 times more likely to be diagnosed with stroke (OR = 7.939, 99% CI [7.875, 8.003], p \u3c .001). The findings of this study may provide evidence for public health professionals to propose interventions that educate Hispanics on diabetes and other risk factors for stroke. Stroke prevention programs can consider the demographic characteristics, socioeconomic status, and cultural norms and values that can impact the disease management for each individual

    Recurrent vulvovaginal candidiasis: a dynamic interkingdom biofilm disease of candida and lactobacillus

    Despite the strikingly high worldwide prevalence of vulvovaginal candidiasis (VVC), treatment options for recurrent VVC (RVVC) remain limited, with many women experiencing failed clinical treatment with frontline azoles. Further, the cause of onset and recurrence of disease is largely unknown, with few studies identifying potential mechanisms of treatment failure. This study aimed to assess a panel of clinical samples from healthy women and those with RVVC to investigate the influence of Candida, the vaginal microbiome, and how their interaction influences disease pathology. 16S rRNA sequencing characterized disease by a reduction in specific health-associated Lactobacillus species, such as Lactobacillus crispatus, coupled with an increase in Lactobacillus iners. In vitro analysis showed that Candida albicans clinical isolates are capable of heterogeneous biofilm formation, and we found the presence of hyphae and C. albicans aggregates in vaginal lavage fluid. Additionally, the ability of Lactobacillus to inhibit C. albicans biofilm formation and biofilm-related gene expression was demonstrated. Using RNA sequencing technology, we were able to identify a possible mechanism by which L. crispatus may contribute to re-establishing a healthy vaginal environment through amino acid acquisition from C. albicans. This study highlights the potential formation and impact of Candida biofilms in RVVC. Additionally, it suggests that RVVC is not entirely due to an arbitrary switch in C. albicans from commensal to pathogen and that understanding interactions between this yeast and vaginal Lactobacillus species may be crucial to elucidating the cause of RVVC and developing appropriate therapies

    Islet autoantibody frequency in relatives of children with type 1 diabetes who have a type 2 diabetes diagnosis

    AimThis study aimed to evaluate characteristics of autoimmunity in individuals who have a type 2 diagnosis and are relatives of children with type 1 diabetes.MethodsPre-diagnosis samples (median 17 months before onset) from relatives who were later diagnosed with type 2 diabetes were measured for autoantibodies to glutamate decarboxylase 65 (GADA), islet antigen-2 (IA-2A), zinc transporter 8 (ZnT8A) and insulin (IAA) as well as the type 1 diabetes genetic risk score (GRS2). Associations between islet autoantibodies, insulin treatment and GRS2 were analysed using Fisher's exact and t-tests.ResultsAmong 226 relatives (64% men; mean age at sampling 41 years; mean age 54 years at diagnosis), 32 (14%) were islet autoantibody-positive for at least one autoantibody more than a decade before diagnosis. Approximately half of these (n = 15) were treated with insulin. GADA-positivity was higher in insulin-treated relatives than in non-insulin-treated relatives (12/18 [67%] vs. 6/18 [33%], p < 0.001). IAA-positivity was observed in 13/32 (41%) of relatives with autoantibodies. GRS2 scores were increased in autoantibody-positive relatives (p = 0.032), but there was no clear evidence for a difference according to treatment (p = 0.072).ConclusionThis study highlights the importance of measuring islet autoantibodies, including IAA, in relatives of people with type 1 diabetes to avoid misdiagnosis

    PSU President’s African American, African, and Black Student Success Task Force Report

    This report is in response to the President’s Charge to the Task Force on African American/African/Black Student Success at Portland State University. As included in the charge, the report assesses the strengths, weaknesses, and opportunities for PSU in continuing to serve the African American/African/Black community at PSU. In conducting its work, the Task Force reviewed a variety of reports and databases from various offices at PSU. Based on its findings, the Task Force provides recommendations under five main areas: 1) Student recruitment and retention 2) Student Experiences 3) Courses and Programming 4) Faculty and Staff at PSU 5) Best Practices The recommendations of the Task Force provide specific recommendations on those areas that reflect the institution’s strengths, weaknesses, and opportunities. The Task Force’s recommendations will provide guidance to the President, Vice President for Enrollment Management and Student Affairs, the university leadership team, and all PSU faculty and staff for engaging with and serving Oregon’s and PSU’s diverse and growing African American/African/Black community as also outlined in the President’s Charge. It is in the spirit of collaborative work that the recommendations will assist PSU in its attempts to address student, faculty, staff, and community concerns regarding the educational needs of its African American/African/Black students. Relatedly, the committee has included information on African American/African/Black staff and faculty in its report to provide a context for its recommendations regarding students

    Using trained dogs and organic semi-conducting sensors to identify asymptomatic and mild SARS-CoV-2 infections: an observational study

    Background A rapid, accurate, non-invasive diagnostic screen is needed to identify people with SARS-CoV-2 infection. We investigated whether organic semi-conducting (OSC) sensors and trained dogs could distinguish between people infected with asymptomatic or mild symptoms, and uninfected individuals, and the impact of screening at ports-of-entry. Methods Odour samples were collected from adults, and SARS-CoV-2 infection status confirmed using RT-PCR. OSC sensors captured the volatile organic compound (VOC) profile of odour samples. Trained dogs were tested in a double-blind trial to determine their ability to detect differences in VOCs between infected and uninfected individuals, with sensitivity and specificity as the primary outcome. Mathematical modelling was used to investigate the impact of bio-detection dogs for screening. Results About, 3921 adults were enrolled in the study and odour samples collected from 1097 SARS-CoV-2 infected and 2031 uninfected individuals. OSC sensors were able to distinguish between SARS-CoV-2 infected individuals and uninfected, with sensitivity from 98% (95% CI 95–100) to 100% and specificity from 99% (95% CI 97–100) to 100%. Six dogs were able to distinguish between samples with sensitivity ranging from 82% (95% CI 76–87) to 94% (95% CI 89–98) and specificity ranging from 76% (95% CI 70–82) to 92% (95% CI 88–96). Mathematical modelling suggests that dog screening plus a confirmatory PCR test could detect up to 89% of SARS-CoV-2 infections, averting up to 2.2 times as much transmission compared to isolation of symptomatic individuals only. Conclusions People infected with SARS-CoV-2, with asymptomatic or mild symptoms, have a distinct odour that can be identified by sensors and trained dogs with a high degree of accuracy. Odour-based diagnostics using sensors and/or dogs may prove a rapid and effective tool for screening large numbers of people
