Mutationen des Fanconi-assoziierten Proteins 2 (FAP2) als Ursache einer erblichen renalen Tubulopathie
Im Rahmen der vorliegenden Arbeit wurde eine neuartige, autosomal-dominant vererbte Form des renalen Fanconi-Syndroms untersucht. Neben der klassischen proximalen Tubulopathie, die bereits in der Kindheit einsetzt und eher mild ausfĂ€llt, kommt es bei allen betroffenen Patienten im Erwachsenenalter zu einem terminalen Nierenversagen und zur Ausbildung fibrotischer Schrumpfnieren. Ăber Kopplungsanalysen konnte das betroffene Gen identifiziert werden, dessen Genprodukt als Stoffwechselenzym normalerweise in den Mitochondrien frĂŒhproximaler Tubuluszellen lokalisiert ist.
Durch die Untersuchung des renalen PhĂ€notyps einer FAP2 (âFanconi-assoziiertes Protein 2â) Knockout Maus konnte eine Haploinsuffizienz als Ursache der Krankheitsentstehung ausgeschlossen werden. Diese MĂ€use zeigten keine Symptome eines Fanconi-Syndroms.
Es hat sich herausgestellt, dass mutiertes FAP2 in der mitochondrialen Matrix mit sich selbst aggregiert und sich dort in Form langer Fibrillen ablagert. Dies konnte sowohl im verwendeten Zellmodell (stabil transfizierte, induzierbare LLC-PK1 Zellen), als auch in der untersuchten Patientenbiopsie nachgewiesen werden. Als Folge davon werden ĂŒber einen bisher noch nicht im Detail bekannten Mechanismus schwere EntzĂŒndungs-prozesse, das Absterben von proximalen Tubuluszellen und eine Fibrose induziert. Als Mechanismus, der diese EntzĂŒndungsprozesse anstöĂt, wurde eine Aktivierung des NLRP3-Inflammasomenkomplexes nachgewiesen. In unserem Zellmodell konnte eine Erhöhung von NLRP3 auf mRNA-Ebene beobachtet werden, sowie auch eine Erhöhung von IL-18 auf mRNA- und auf Protein-Ebene. IL-18 ist ein bekannter potenter EntzĂŒndungsmediator, der in der Literatur bereits mit der Entstehung der Nierenfibrose in Zusammenhang gebracht wurde. Als Fibrose-Marker waren im Zellmodell die mRNA- und Protein-Expression des extrazellulĂ€ren Matrixproteins Fibronektin 1 und des mesenchymalen Markers Glattmuskel-Aktin erhöht. Die Untersuchung einer post mortem Patientenniere zeigte in Ăbereinstimmung mit den in vitro Daten eine starke Fibrose, die sich ĂŒber das gesamte Nierengewebe erstreckte.
Zusammenfassend sind sehr wahrscheinlich die Aggregate von mutiertem FAP2 die Ursache fĂŒr die Entstehung der vorliegenden Erkrankung. Interessanterweise lĂ€sst sich durch spezielle Nahrungssupplementationen die Expression von FAP2 als Stoffwechselenzym beeinflussen. So könnte eine Reduktion der Expression von mutiertem FAP2 eine Therapieoption darstellen, da dadurch die die Aggregatbildung reduziert werden kann
Quantitative Analyse des Survival of Motor Neuron(SMN)-Komplexes unter Verwendung von absoluter Quantifizierung SMN-Komplex-spezifischer Peptide durch synthetische stabilisotopenmarkierte Peptidanaloga
Der SMN-Komplex ist eine makromolekulare Einheit, die aus neun festen Untereinheiten (SMN,
Gemin2-Gemin8, Unrip) und verschiedenen transienten Faktoren (SmB/Bâ, SmE, SmF, SmG,
SmD1-D3) aufgebaut ist. Die Hauptaufgabe des ubiquitÀren Komplexes umfasst die Assemblierung
der U snRNPs im Cytoplasma und ihre Translokation in den Zellkern, wo sie am
Aufbau des SpleiĂosoms beteiligt sind. Durch Mutation oder Deletion des SMN1-Gens kann
dieser Prozess gestört sein und dadurch Spinale Muskelatrophie (SMA) auslösen. SMA ist die
zweithÀufigste autosomal rezessiv vererbbare Krankheit mit Todesfolge nach Mukoviszidose, die
zu einer Degeneration der -Motorneuronen des RĂŒckenmarks und damit zu einer SchwĂ€chung
und einem Abbau der Muskulatur fĂŒhrt.
Das Ziel der vorliegenden Arbeit war die Charakterisierung des humanen Survival of Motor
Neuron-Komplex (SMN) hinsichtlich Zusammensetzung und Stöchiometrie mittels Anwendung
von massenspektrometrischen Methoden. In diesem Zusammenhang wurde die native stöchiometrische
Zusammensetzung des zentralen SMN-Komplexes mit zwei rekombinanten Komplexen
verglichen, die beide eine in Verbindung mit SMA bekannte Mutation (Y272C, E134K)
integriert hatten. Zu diesem Zweck wurden Wildtyp-SMN und die mutierten SMN-Analoga
als GST-Fusions-Proteine zusammen mit Wildtyp-Gemin2, -Gemin6, -Gemin7 und -Gemin8 in
Escherichia coli exprimiert. Nach Ernte und Lyse der Zellen wurde SMN auf einer Glutathion-
Sepharose-Matrix angereichert und anschlieĂend mittels verschiedener Methoden (Carbamidomethylierung,
Trypsin-Verdau) fĂŒr die massenspektrometrische quantitative Analyse vorbereitet.
Des Weiteren wurden die Unterschiede zwischen dem cytoplasmatischen und dem nukleÀren
SMN-Komplex, welche durch Zellkern-Cytoplasma-Trennung von HeLa-Zellen mittels eines
modifizierten Roeder-Protokolls und anschlieĂende Co-ImmunoprĂ€zipitation gegen den SMNspezifischen
Antikörper 7B10 erhalten wurden, untersucht.
FĂŒr die Quantifizierung der SMN-Komplexe wurde Absolute Quantifizierung (AQUA) angewendet.
Synthetische stabilisotopenmarkierte Petidanaloga wurden mit den nativen Proben in
definierten Konzentrationen verdĂŒnnt und konnten wĂ€hrend der massenspektrometrischen Analyse
durch ihre Massendifferenz von 6 bis 10 Dalton im Vergleich zu den endogenen Peptiden
detektiert werden. Zuvor mussten jedoch geeignete Peptidsequenzen ausgewÀhlt werden, um
eine reproduzierbare Quantifizierung zu gewÀhrleisten. Dazu mussten die Peptide definierten
Spezifikationen entsprechen (z.B. keine leicht modifizierbaren AminosĂ€uren oder keine ĂŒberlesenen
tryptischen Schnittstellen beinhalten). Die MS-Analyse wurde mittels Selected Reaction
Monitoring (S/MRM) auf zwei unterschiedlichen Triple-Quadrupol-Systemen durchgefĂŒhrt. Zu
diesem Zweck wurden die ĂbergĂ€nge und Systemparameter (Declustering Potential und Kollisionsenergie)
fĂŒr jedes einzelne Peptid optimiert und festgelegt.
Alternativ wurde markierungsfreie Quantifizierung auf einem LTQ Obitrap Velos System angewandt. Die hohe Massengenauigkeit und SensitivitÀt des Hybrid-FT-MS-Systems erlaubt eine
Quantifizierung der entsprechenden Mutterionen direkt aus der MS1-Spur (ohne Anwendung
von Tandem-MS-Experimenten).
Beide Strategien, S/MRM und MS1, erlauben eine Quantifizierung von Peptiden mit Konzentrationen
bis 250 amol. Durch Optimierung des dargestellten Protokolls, war es möglich
den Einfluss bekannter Patientenmutationen des SMN1-Gens auf die Stöchiometrie des SMNKomplexes
zu untersuchen, sowie den Unterschied zwischen SMN-Komplexen des Cytoplasmas
und des Zellkerns zu bestimmen.
Dieses Projekt kann dazu beitragen sowohl die Assemblierung der U snRNPs besser zu verstehen,
als auch neue ZielmolekĂŒle fĂŒr Diagnose und Therapie von genetischen Krankheiten wie
SMA bereitzustellen.Summary
The SMN complex is a macromolecular protein entity consisting of nine fixed subunits (SMN,
Gemin2-Gemin8, Unrip) and several transient factors (SmB/Bâ, SmE, SmF, SmG, SmD1-D3).
The main function of this housekeeping complex comprises the assembly of U snRNPs in the
cytoplasm and their export to the nucleus where they build up the spliceosome. As a result of
mutations in the SMN1 gene this process can be defective and elicits Spinal Muscular Atrophy
(SMA). SMA is the second most common autosomal recessive hereditary disease beside cystic
fibrosis resulting in death. It leads to a degeneration of the -motor neurons of the spinal cord
and subsequently to muscular weakness and wasting.
The aim of this study was the characterization of the human Survival of Motor Neuron (SMN)
complex composition and stoichiometry by mass spectrometry. In this context, the native stoichiometric
composition of the SMN core complex was compared with two recombinant complexes,
each incorporating a known mutation (Y272C, E134K) involved in SMA. For this purpose
wildtype SMN and mutated versions thereof were expressed as GST-fusion proteins together
with full length Gemin2, Gemin6, Gemin7 and Gemin8 in E.coli. After harvesting and lysis of the
cells the SMN complex was enriched on a glutathione-sepharose resin and further prepared by
carbamidomethylation and tryptic digestion for a quantitaive massspectrometric analysis. Furthermore,
the differences between the cytoplasmic and the nuclear SMN complex both derived
from nuclei-cytoplasm separation of HeLa cells by a modified Roeder protocol and subsequent
Co-immunoprecipitations against the SMN-specific antibody 7B19 were examined .
In order to quantify the SMN core complex absolute quantification (AQUA) was applied. Stableisotope
labeled peptide analogs were spiked to the native sample in defined amounts thus introducing
a mass difference of 6-10 Da during the massspectrometric analysis. Preliminarily,
appropriate peptides that meet several conditions (e.g. no labile amino acids, no missed tryptic
digestion sites) to allow for a confident quantification were chosen. As MS-analysis was performed
by Selected Reaction Monitoring (S/MRM) on two different triple quadrupol systems,
transitions and parameters (Declustering Potential and Collision Energy) for each respective
peptide were optimized .
Alternatively, label-free quantitation on the LTQ Orbitrap Velos was applied. The high mass
accuracy and sensitivity of this hybrid FT MS allows quantifying the respective parent ions
directly from the MS1 trace.
Both strategies, S/MRM and MS1, provide a linear quantification of peptides down to a concentration
of 250 amol. By optimizing the current workflow, it was possible to determine the
stoichiometry of SMN-complexes containing several known patient mutations within the SMN1
gene as well as discovering the differences between SMN-complexes originating either from cytoplasm
or from nucleus.
This project may help to throw a light on the assembly of U snRNPs and provide new targets
in diagnosis and therapy for genetic diseases like SMA
Implementation and validation of a novel instrument for the grading of unexpected events in paediatric surgery: ClavienâMadadi classification
Background:
Inconsistent definitions of complications and unexpected events have limited accurate analysis of surgical outcomes. Perioperative outcome classifications currently used for adult patients have limitations when used for children./
Methods:
A multidisciplinary group of experts modified the ClavienâDindo classification to increase its utility and accuracy in paediatric surgery cohorts. Organizational and management errors were considered in the novel ClavienâMadadi classification, which focuses on procedural invasiveness rather than anaesthetic management. Unexpected events were prospectively documented in a paediatric surgery cohort. Results of the ClavienâDindo and ClavienâMadadi classifications were compared and correlated with procedure complexity./
Results:
Unexpected events were prospectively documented in a cohort of 17 502 children undergoing surgery between 2017 and 2021. The results of both classifications were highly correlated (Ï = 0.95), although the novel ClavienâMadadi classification identified 449 additional events (organizational and management errors) over the ClavienâDindo classification, increasing the total number of events by 38 per cent (1605 versus 1158 events). The results of the novel system correlated significantly with the complexity of procedures in children (Ï = 0.756). Furthermore, grading of events > grade III according to the ClavienâMadadi classification showed a higher correlation with procedure complexity (Ï = 0.658) than the ClavienâDindo classification (Ï = 0.198)./
Conclusion:
The ClavienâMadadi classification is a tool for the detection of surgical and non-medical errors in paediatric surgery populations. Further validation in paediatric surgery populations is required before widespread use

Understanding the Return of Genomic Sequencing Results Process: Content Review of Participant Summary Letters in the eMERGE Research Network
A challenge in returning genomic test results to research participants is how best to communicate complex and clinically nuanced findings to participants in a manner that is scalable to the large numbers of participants enrolled. The purpose of this study was to examine the features of genetic results letters produced at each Electronic Medical Records and Genomics (eMERGE3) Network site to assess their readability and content. Letters were collected from each site, and a qualitative analysis of letter content and a quantitative analysis of readability statistics were performed. Because letters were produced independently at each eMERGE site, significant heterogeneity in readability and content was found. The content of letters varied widely from a baseline of notifying participants that results existed to more detailed information about positive or negative results, as well as materials for sharing with family members. Most letters were significantly above the Centers for Disease Control-suggested reading level for health communication. While continued effort should be applied to make letters easier to understand, the ongoing challenge of explaining complex genomic information, the implications of negative test results, and the uncertainty that comes with some types of test and result makes simplifying letter text challenging
Mitochondrial physiology
As the knowledge base and importance of mitochondrial physiology to evolution, health and disease expands, the necessity for harmonizing the terminology concerning mitochondrial respiratory states and rates has become increasingly apparent. The chemiosmotic theory establishes the mechanism of energy transformation and coupling in oxidative phosphorylation. The unifying concept of the protonmotive force provides the framework for developing a consistent theoretical foundation of mitochondrial physiology and bioenergetics. We follow the latest SI guidelines and those of the International Union of Pure and Applied Chemistry (IUPAC) on terminology in physical chemistry, extended by considerations of open systems and thermodynamics of irreversible processes. The concept-driven constructive terminology incorporates the meaning of each quantity and aligns concepts and symbols with the nomenclature of classical bioenergetics. We endeavour to provide a balanced view of mitochondrial respiratory control and a critical discussion on reporting data of mitochondrial respiration in terms of metabolic flows and fluxes. Uniform standards for evaluation of respiratory states and rates will ultimately contribute to reproducibility between laboratories and thus support the development of data repositories of mitochondrial respiratory function in species, tissues, and cells. Clarity of concept and consistency of nomenclature facilitate effective transdisciplinary communication, education, and ultimately further discovery

Peer-to-Peer-Konzepte zur Vermittlung von Informationskompetenz â Entwicklung einer neuen Informationsdienstleistung fĂŒr die Campus-Bibliothek Finkenau der HAW Hamburg
Durch den Bologna-Prozess wurde die Studiendauer in einem MaĂe verkĂŒrzt, dass die Förderung der Fertigkeiten und FĂ€higkeiten hinsichtlich der Informationssuche, -bewertung und âaufbereitung nicht alleinig von den Hochschulen garantiert werden kann. Somit tritt die Hochschulbibliothek als Teaching Library sowie als Lern- und Lehr-raumgestalter zur Förderung von Informationskompetenz in den Fokus. Durch die Im-plementierung von innovativen didaktischen AnsĂ€tzen zur Förderung von IK, könnten Defizite und Probleme behoben und den Studierenden eine angenehme Lernat-mosphĂ€re angeboten werden.
Diese Arbeit beschÀftigt sich mit dem informellen, didaktischen Peer-Ansatz und des-sen praktische Umsetzung in wissenschaftlichen Hochschulbibliotheken.
Der Theoretische Abschnitt gibt zunĂ€chst einen Ăberblick ĂŒber die Hochschule fĂŒr An-gewandte Wissenschaften Hamburg und dessen Infrastruktur. Folgen wird eine theore-tische Ausarbeitung der Thematik Informationskompetenz und die Rolle der Hoch-schulbibliothek als Teaching Library. Danach erfolgt die Darstellung des informellen Peer-Ansatzes. Dort werden mögliche AusprĂ€gungen, Stand der Forschung und die Darstellung von Anwendungsbeispielen in Hochschulbibliotheken beschrieben.
Der empirische Abschnitt enthĂ€lt die Analyse der Interviews mit einem IK-Experten, der zukĂŒnftigen Leiterin der Campus-Bibliothek Finkenau, den Studierenden des Studi-engangs BIM B.A. sowie Peer-Experten.
Auf Grundlage des theoretischen und empirischen Abschnitts wird ein Peer-to-Peer-Konzept fĂŒr die neue Bibliothek entworfen

Die stetige Zunahme der PrĂ€valenz von Ăbergewicht und Adipositas weltweit hat
sich zu einem gravierenden Problem unserer Gesellschaft entwickelt.
HausÀrztInnen kommt aufgrund ihrer guten Kenntnis der besonderen Situation von
PatientInnen sowie der hÀufigen Patientenkontakte bei der PrÀvention und
Behandlung der Adipositas eine entscheidende Rolle zu. Ursachen fĂŒr die
langfristig enttÀuschenden Behandlungsergebnisse bei der Behandlung von
Ăbergewicht sind auch in der Arzt-Patienten-Interaktion zu suchen, da
unterschiedliche Sichtweise von ĂrztInnen und PatientInnen zum
Krankheitskonzept Ăbergewicht nachgewiesen wurden. Ziel der vorliegenden
Studie war es, die Sichtweisen sowohl der PatientInnen als auch der ĂrztInnen
in Bezug auf die Ursachen, die Risikowahrnehmung und den Umgang mit
Ăbergewicht durch ein qualitatives Studiendesign weiter zu vertiefen. Dazu
wurden BeratungsgesprÀche der Gesundheitsuntersuchung (Check up 35) zwischen
HausĂ€rztInnen und ĂŒbergewichtigen PatientInnen (BMI â„ 25kg/mÂČ) in Berlin und
Brandenburg auf Tonband aufgezeichnet und transkribiert. Es nahmen acht
Ărztinnen und vier Ărzte an der Studie teil. 52 dieser GesprĂ€che wurden mit
Hilfe der qualitativen Inhaltsanalyse nach Mayring ausgewertet und durch das
Datenanalyseprogramm âatlas.tiâ veranschaulicht. In Hinblick auf die
Ursachenattribution konnten sowohl bei den ĂrztInnen als auch bei den
PatientInnen verhaltensabhÀngige von nicht-verhaltensabhÀngigen Ursachen
unterschieden werden. Sowohl ĂrztInnen als auch PatientInnen thematisierten
u.a. intensiv verschiedene Aspekte zur ErnÀhrung. Es konnte festgestellt
werden, dass ĂrztInnen den Risikofaktor Ăbergewicht sehr unterschiedlich
darlegten. PatientInnen benannten unterschiedliche GefĂŒhlszustĂ€nde,
Folgeerkrankungen und Risikowahrnehmungen im Zusammenhang mit dem Ăbergewicht.
Nicht alle ĂrztInnen sahen eine Gewichtsabnahme indiziert, selbst bei
PatientInnen mit einem BMI â„ 30 kg/mÂČ. Weiterhin zeigte sich, dass die von
PatientInnen selbst eingebrachten BehandlungsvorschlÀge weitaus intensiver und
umfassender waren, wenn diese spontan LösungsansĂ€tze Ă€uĂern konnten und nicht
nur auf Àrztliche RatschlÀge reagierten. Die Analyse der GesprÀche zeigte auch
einen geschlechtsspezifischen Umgang mit der Thematik Ăbergewicht auf.
Insgesamt konnten vielschichtige Aspekte zu den Beratungsinhalten der Check
up-Untersuchung mit ĂŒbergewichtigen PatientInnen dargestellt werden. Es zeigte
sich, dass PatientInnen ein groĂes Potential von LösungsansĂ€tzen mit in das
GesprĂ€ch hineinbringen, das von ĂrztInnen fĂŒr eine individuelle
Adipositasberatung genutzt werden kann. Besonders auf Àrztlicher Seite konnten
Unstimmigkeiten ĂŒber die Risikowahrnehmung der Adipositas erkannt werden. Die
TherapieansĂ€tze der ĂrztInnen stellten sich auĂerdem teilweise als lĂŒckenhaft
dar. Es ist zu vermuten, dass sich die Behandlungserfolge der Adipositas durch
eine Optimierung dieser Aspekte verbessern lassen.The increasing prevalence of obesity is a major public health concern, since
obesity is associated with several chronic diseases. General practitioners
(GP) are uniquely placed to provide effective advice about weight management
due to their knowledge of the patientâs social context and the frequent
contacts. Reasons for the poor success in treating obesity can also be seen in
the physician-patient-interaction, because different views of perceptions of
the disease have been identified. The aim of this study was to explore the
patientsâ and the physiciansâ beliefs and perceptions regarding causes, risks
and treatment of obesity using a qualitative design. From April to September
2007, family physician encounters with obese patients (BMI â„ 25kg/mÂČ) were
audiotaped. Four female physicians and eight male physicians participated in
this study. After transcribing, a content analysis of 52 dialogues was
performed using the method developed by Mayring. Patients and GPs attributed
their overweight to both behavioural and non-behavioural-related causes.
Especially different aspects of nutrition could be identified. GPs had
different opinions about the health risks of obesity. The patients associated
variable types of moods, diseases and risks with their overweight. Physicians
had unequal opinions about addressing weight management issues, even in very
obese patients. Some obese patients did not receive any advice. The aspects
given by patients were remarkably more intense and widespread, if the patients
had the chance to make own suggestions spontaneously. Furthermore, a gender-
specific way of dealing with the topic overweight was elaborated. Summarised,
a variety of aspects in the context of counselling overweight patients could
be identified. The broad range of solutions provided by patients can be a
chance for physicians to get valuable access to the patientsâ perspectives and
to give individual advice. Particularly the GPs showed contrary opinions of
viewing obesity as health risk. The therapeutic approach revealed gaps in the
GPâs knowledge of treating obesity. It can be assumed that the success of
treating obesity can be improved if these aspects can be optimised
