Convergent construct validity and test-retest reliability of both German versions of the original and the revised Niigata PPPD Questionnaire: NPQ and NPQ-R
BackgroundPersistent Postural-Perceptual Dizziness (PPPD) is a frequent chronic functional disorder that manifests with dizziness, unsteadiness, or non-spinning vertigo present for at least 3 months. Characteristic provocation factors are moving or complex visual stimuli and exclusion of organic diseases. To assess the severity and impact of PPPD, Japanese researchers developed the Niigata PPPD Questionnaire (NPQ). The study's aim was to evaluate the concurrent construct validity and reliability [including test-retest reliability, internal consistency, standard error of measurement (SEM), and minimal detectable change (MDC)] of the German version of the NPQ (12 items) and its revised version, NPQ-R, which contains 19 items addressing additional symptoms and symptom behavior.MethodsThe Swiss Reha Rheinfelden and the German Center for Vertigo and Balance Disorders included 265 PPPD patients (mean age 50.2 ± 16.8 years, disease duration 46.3 ± 76.6 months). Patients completed the NPQ and the NPQ-R (twice), the DHI and potentially related constructs: anxiety (ABC-Scale, VSS), depression (HADS), and general health (SF-36) once. To assess the questionnaires' reliability and validity, several statistical measures were calculated, including Spearman's rank correlation coefficients, Intraclass Correlation Coefficients (ICC2, 1), Cronbach's alpha, SEM, and MDC.ResultsOn average, patients scored 29.9 ± 13.2 for NPQ and 52.3 ± 19.6 for NPQ-R. Correlations between NPQ/NPQ-R and (1) disease-specific questionnaires were rs= 0.712 and rs= 0.752 (DHI), rs=0.426 and rs= 0.0.462 (VSS-V), rs= -0.500 and rs= -0.545 (ABC-Scale), (2) anxiety-specific subscales rs = 0.394 and rs = 0.430 (VSS-A) and rs= 0.354 and rs= 0.430 (HADS-A), (3) depression-related subscales rs=0.438 and rs= 0.487 (HADS-D), and (4) general health rs ranged between rs= -0.216 and −0.578 (all SF-36 subscales). Internal consistency, test-retest reliability, SEM and MDC calculated for NPQ/NPQ-R were α = 0.88/α = 0.91, ICC=0.83 (CI 0.77 to 0.0.87), SEM 5.55/8.37, and MDC 15/23 points.ConclusionThe German versions of NPQ and NPQ-R are valid and reliable patient-reported outcome measures for assessing PPPD, demonstrating satisfactory psychometric measurement properties including convergent construct validity and reliability parameters: internal consistency, test-retest reliability, SEM, and MDC as an evaluative measure. The NPQ-R, with its additional subscales addressing associated symptoms and symptom behavior, represents both the patient and clinician perspective on PPPD-specific problems. Therefore, we recommend utilizing the NPQ-R for a comprehensive assessment of PPPD

Schlussbericht des Vorhabens "Synergetische Lehrerbil-dung im exzellenten Rahmen" (TUD-Sylber²)
Das Projekt "Synergetische Lehrerbildung im exzellenten Rahmen" (TUD-Sylber²) zielte auf eine nachhaltige Verbesserung der inhaltlichen und organisatorischen Gestaltung der Lehrkräftebildung an der TU Dresden (TUD). Im Schwerpunkt Organisationsentwicklung ging es darum, im Zentrum für Lehrerbildung, Schul- und Berufsbildungsforschung (ZLSB) Querstrukturen für die universitäre Lehrkräftebildung zu etablieren, die Koordinations- und Unterstützungsleistungen für alle an den Lehramtsstudiengängen beteiligten Fakultäten erbringen. Der Schwerpunkt Qualitätsverbesserung umfasste die Bearbeitung zentraler inhaltlicher Herausforderungen für die Lehrkräftebildung: Inklusion/Umgang mit Heterogenität, Digitalisierung/Digitale Medien in die Lehrkräftebildung und Kohärenz von Fachwissenschaft, Fachdidaktik und Bildungswissenschaft im Lehramtsstudium. Der Schwerpunkt Regionale Vernetzung diente der Stärkung der Kooperation der universitären Lehrkräftebildung mit regionalen Akteuren (z. B. Schulen, außerschulischen Lernorten) sowie mit den Verantwortlichen für Vorbereitungsdienst und Lehrkräftefortbildung. Im Laufe der Projektlaufzeit konnten bewährte Arbeitsstrukturen und -prozesse für fakultätenübergreifende Aufgaben in der TUD-Lehrkräftebildung erfolgreich etabliert werden (überschneidungsfreie Lehrveranstaltungsplanung, Qualitätssicherung der Lehramtsstudiengänge, wissenschaftliche Nachwuchsförderung, Kooperation mit außeruniversitären Partnern). Zu den zentralen Querschnittsthemen der Lehrkräftebildung wurden im Rahmen von TUD-Sylber² Lehr-Lern-Konzepte entwickelt, Materialien und Werkzeuge erstellt sowie Lehrveranstaltungen im Studium sowie Lehrkräftefortbildungen durchgeführt.

The core area Organizational development focused on establishing cross structures for teacher education within the Center for Teacher Education and Educational Research (ZLSB) that provide services for all the faculties participating in teacher education degree programs. The core area Quality improvement covered the handling of vital challenges for teacher education in terms of content: Inclusion/handling heterogeneity, digitalization/digital media in teacher education and the coherence of specialist sciences, subject-related didactics and educational science in teacher education programs. The core area Regional networking attended the consolidation of cooperations between University’s teacher educators and regional protagonists (i. e. schools, extra-school learning locations) as well as the persons responsible for preparatory service and advanced teacher training. Over project duration, reliable working structures and operating processes in the TUD’s teacher education could successfully be established for tasks that are overarching different faculties (overlap-free teaching, quality assurance for the teacher education degree programs, support programs for young academics, cooperations with partners from outside university)
Choline Kinase Alpha as an Androgen Receptor Chaperone and Prostate Cancer Therapeutic Target.
BACKGROUND: The androgen receptor (AR) is a major drug target in prostate cancer (PCa). We profiled the AR-regulated kinome to identify clinically relevant and druggable effectors of AR signaling. METHODS: Using genome-wide approaches, we interrogated all AR regulated kinases. Among these, choline kinase alpha (CHKA) expression was evaluated in benign (n = 195), prostatic intraepithelial neoplasia (PIN) (n = 153) and prostate cancer (PCa) lesions (n = 359). We interrogated how CHKA regulates AR signaling using biochemical assays and investigated androgen regulation of CHKA expression in men with PCa, both untreated (n = 20) and treated with an androgen biosynthesis inhibitor degarelix (n = 27). We studied the effect of CHKA inhibition on the PCa transcriptome using RNA sequencing and tested the effect of CHKA inhibition on cell growth, clonogenic survival and invasion. Tumor xenografts (n = 6 per group) were generated in mice using genetically engineered prostate cancer cells with inducible CHKA knockdown. Data were analyzed with χ(2) tests, Cox regression analysis, and Kaplan-Meier methods. All statistical tests were two-sided. RESULTS: CHKA expression was shown to be androgen regulated in cell lines, xenografts, and human tissue (log fold change from 6.75 to 6.59, P = .002) and was positively associated with tumor stage. CHKA binds directly to the ligand-binding domain (LBD) of AR, enhancing its stability. As such, CHKA is the first kinase identified as an AR chaperone. Inhibition of CHKA repressed the AR transcriptional program including pathways enriched for regulation of protein folding, decreased AR protein levels, and inhibited the growth of PCa cell lines, human PCa explants, and tumor xenografts. Paediatric multiple sclerosis: The experience of the German Centre for Multiple Sclerosis in Childhood and Adolescence
Multiple sclerosis (MS) is a chronic inflammatory demyelinating disorder of the central nervous system and the most common disabling neurological disease in young adults. An estimated 5% of MS patients already have first clinical symptoms before the age of 16 years (paediatric MS). In the paediatric age group comprehensive analysis of the natural clinical course and the course under treatment in a large MS cohort is still missing. We describe a cohort of paediatric MS patients treated in the German Centre for Multiple Sclerosis in Childhood and Adolescence. A total of 166 patients with definite MS who are registered in our database were analysed. The observation time was up to 14.9 years with a mean follow-up of 4.1 years. Median age was 12.4 years (range 4 to 18 years). Prior to puberty the gender ratio was almost equal, while in adolescence there was a strong female predominance as is seen in adult onset MS. Almost all patients presented with relapsing-remitting MS. The course of the disease was more benign than in adult MS with a very slow EDSS increase and complete remission after most relapses. Most patients received immunomodulative treatment with interferon-beta or glatiramer acetate and, in severe cases, natalizumab. However, adequate treatment guidelines for this age group are still lacking

Ätiologie und Pathogenese entsprechen mit hoher Wahrscheinlichkeit der adulten Form, dennoch stellt die pädiatrische MS aufgrund des frühen Erkrankungsbeginns für Patienten, Angehörige und Ärzte eine Herausforderung dar

Zoonosen sind Infektionskrankheiten, die auf natürlichem Weg vom Tier (Vertebraten) auf den Menschen übertragen werden können. Die Bedeutung der Zoonosen liegt in der Häufigkeit der Erkrankungsfälle, der hohen Letalität einzelner Zoonosen und der Möglichkeit von bisher auf das Tierreservoir beschränkten Erregern, die Artengrenze zum Menschen zu überwinden. Veränderte Bedingungen der Lebensmittelproduktion (inklusive Massentierhaltung) und der Ernährung sowie demographische, klimatische und ökologische Faktoren fördern die Verbreitung von Zoonoseerregern. Im Rahmen des seit 1. Januar 2001 geltenden Infektionsschutzgesetzes unterliegtZoonoses are infectious diseases that can be transmitted from vertebrate animals to humans. Their significance lies in the large number of cases that occur, the high case fatality ratio of certain zoonoses, and the potential for some pathogens as yet restricted to animal hosts to cross the species barrier and infect humans. Changing habits in food production (for example, intensive animal husbandry) and food consumption as well as demographic, climatic, and ecological factors contribute to the spread of zoonotic pathogens. Several zoonoses are notifiable in Germany according to the Protection Against Infection Act enacted 1 January 2001. The European Commission issued a new directive on the monitoring of zoonoses and zoonotic agents on 17 November 2003. There is ongoing need to develop further measures to prevent and control zoonotic diseases on a national as well as international basis
