    Mental health, social adversity, and health-related outcomes in sexual minority adolescents: a contemporary national cohort study

    Background Sexual minority adolescents are more likely to have mental health problems, adverse social environments, and negative health outcomes compared with their heterosexual counterparts. There is a paucity of up-to-date population-level estimates of the extent of risk across these domains in the UK. We analysed outcomes across mental health, social environment, and health-related domains in sexual minority adolescents compared with their heterosexual counterparts in a large, contemporary national cohort. Methods The Millennium Cohort Study (MCS) is a birth cohort study in the UK following up children born between Sept 1, 2000, and Jan 11, 2002 across England, Wales, Scotland, and Northern Ireland. Children recruited from the MCS have been followed up over six recruitment sweeps to date at ages 9 months, 3 years, 5 years, 7 years, 11 years, and 14 years. We analysed mental health, social, and health-related outcomes in sexual minority versus heterosexual adolescents at age 14 years. Additionally, we estimated the accumulation of multiple adverse outcomes in both groups. The primary aim of the study was to assess whether sexual minority adolescents experienced more adverse outcomes than heterosexual adolescents. Findings Between January, 2015, and April, 2016, 9885 adolescents provided a response about their sexual attraction. 629 (6%) of 9885 adolescents (481 female participants and 148 male participants) were identified as sexual minorities. 9256 (94%) of 9885 participants (4431 female and 4825 male) were attracted to the opposite sex or not attracted to the same sex and identified as heterosexual. Sexual minority adolescents were more likely to experience high depressive symptoms (odds ratio [OR] 5·43, 95% CI 4·32–6·83; p<0·0001), self-harm (5·80, 4·55–7·41; p<0·0001), lower life satisfaction (3·66, 2·92–4·58; p<0·0001), lower self-esteem (β 1·83, 95% CI 1·47–2·19; p<0·0001), and all forms of bullying and victimisation. Sexual minorities were more likely to have tried alcohol (OR 1·85, 95% CI 1·47–2·33; p<0·0001), smoking (2·41, 1·92 −3·03; p<0·0001), and cannabis (3·22, 2·24–4·61; p<0·0001), and also had increased odds of being less physically active (β 0·36, 95% CI 0·25–0·46; p<0·0001), perceiving themselves as overweight (OR 1·73, 95% CI 1·40–2·14; p<0·0001), and dieting to lose weight (1·98, 1·58–2·48; p<0·0001). Sexual minority adolescents had more co-occurring mental health outcomes (mean 1·43 of 3 outcomes, 95% CI 1·34–1·52) compared with heterosexual adolescents (0·40 of 3 outcomes, 0·38–0·41), and more total cumulative difficulties (mean 9·43 of 28 outcomes, 95% CI 9·09–9·76 in sexual minority adolescents vs 6·16 of 28 outcomes, 6·08–6·23 in heterosexual adolescents). Interpretation Sexual minority adolescents in the UK experience disparities in mental health, social, and health-related outcomes despite living in a time of substantial progress in rights for sexual minorities. These adverse outcomes co-occur, with implications for lifelong health and social outcomes. Health and educational practitioners should be aware of the increased risk for adverse outcomes in sexual minority adolescents

    Single- and double-scattering production of four muons in ultraperipheral PbPb collisions at the Large Hadron Collider

    We discuss production of two μ+μ\mu^+\mu^- pairs in ultraperipheral ultrarelativistic heavy ion collisions at the LHC. We take into account electromagnetic (two-photon) double-scattering production and for a first time direct γγ\gamma\gamma production of four muons in one scattering. We study the unexplored process γγμ+μμ+μ\gamma \gamma \to \mu^+\mu^-\mu^+\mu^-. We present predictions for total and differential cross sections.     Autistic adolescents show atypical activation of the brain′s mentalizing system even without a prior history of mentalizing problems

    AbstractSome autistic children pass classic Theory of Mind (ToM) tasks that others fail, but the significance of this finding is at present unclear. We identified two such groups of primary school age (labelled ToM+ and ToM−) and a matched comparison group of typically developing children (TD). Five years later we tested these participants again on a ToM test battery appropriate for adolescents and conducted an fMRI study with a story based ToM task. We also assessed autistic core symptoms at these two time points. At both times the ToM− group showed more severe social communication impairments than the ToM+ group, and while showing an improvement in mentalizing performance, they continued to show a significant impairment compared to the NT group. Two independent ROI analyses of the BOLD signal showed activation of the mentalizing network including medial prefrontal cortex, posterior cingulate and lateral temporal cortices. Strikingly, both ToM+ and ToM− groups showed very similar patterns of heightened activation in comparison with the NT group. No differences in other brain regions were apparent. Thus, autistic adolescents who do not have a history of mentalizing problems according to our ToM battery showed the same atypical neurophysiological response during mentalizing as children who did have such a history. This finding indicates that heterogeneity at the behavioural level may nevertheless map onto a similar phenotype at the neuro-cognitive level

    The Incidence of Infection Complicating Snakebites in Tropical Australia: Implications for Clinical Management and Antimicrobial Prophylaxis

    Objective. To define the incidence of infection following snakebite in tropical Australia and the resulting implications for the routine prescription of prophylactic antibiotics. Methods. A retrospective study of all individuals presenting to Cairns Hospital, a tertiary referral hospital in tropical Australia, after a snakebite between December 2013 and October 2020. Results. There were 732 hospitalisations, 720 (98.4%) patients presented within 8 hours of the snakebite, and 29/732 (4.0%) were envenomated. Envenomated patients were more likely to receive empirical antibiotics than nonenvenomated patients (8/29 (27.6%) versus 14/703 (2.0%), p &lt; 0.001 ), although this was frequently as a bundle of care for critically ill individuals. Superficial skin infection was diagnosed by clinicians in 6/732 (0.8%) patients during their hospitalisation; infection was diagnosed more commonly in envenomated than in nonenvenomated patients (3/29 (10.3%) versus 3/703 (0.4%), p = 0.001 ). All 3 envenomated individuals diagnosed with infection were believed to have taipan (genus Oxyuranus) bites. Five (83%) of the six patients diagnosed with infection had received empirical antibiotics at presentation; only 1/710 (0.1%) patients who received no antibiotics developed a (superficial) infection. Conclusion. Infection is a very uncommon complication of snakebite in tropical Australia. Individuals bitten by snakes in tropical Australia should not routinely receive antibiotic prophylaxis

    Local-scale environmental gradients in ‘snail-shell’ stable isotopes from Holocene Jordanian archaeological sites

    Reconstructing environments around archaeological sites is complicated by past land management practices and regional-scale climate proxies that can be contradictory and are often located at a distance from the sites themselves. Here we explore environmental information from fossil snail shells which, even when few in number on an archaeological site, may prove invaluable in constructing site-specific data. The palaeoecology of fossil snails and the stable isotopic composition of their shell carbonate can provide context-specific information on vegetation, water availability, and relative humidity during the occupation of a site. We studied terrestrial and aquatic snails from two later Neolithic archaeological sites in the Jordanian badia, Wadi al-Qattafi and Wisad Pools. At specific archaeological site-scale our study highlights the importance of aquatic snails in the reconstruction of semi-arid environments. At Wisad pools rare aquatic snails in contexts dating between ~8.0 and ~7.6 ka demonstrate episodes of wetness; moreover, their shell isotopic compositions indicate that local watercourses were well established, corroborating previous findings that during this period the immediate environs of Wisad Pools were host to C3 plant species more typical of the Mediterranean zone. Moreover, the δ18O signal in these snail shells allow tentative reconstruction of rainwater isotopic compositions and identify the effects of evaporation. Such fine-grained environmental information is much less evident from the terrestrial snail shell data alone, showing that an ensemble of snail shell data can be highly sensitive to environmental differentials across an archaeological site. Finally, at a regional palaeoclimate-scale our Wisad Pools snail shell stable isotope data are consistent with a sustained, Rapid Climate Change (RCC)-driven wetness between 8.6 and 7.6 ka concurrent with cold and wet conditions in the wider Levant
