Vaginal Misoprostol for Cervical Ripening in Term Pregnancy
Low-dose (25 mcg) intravaginal misoprostol appears to be safe and effective for cervical ripening in term pregnancy for patients without a history of cesarean section. Compared with other cervical ripening methods, misoprostol has an increased rate of vaginal delivery within 24 hours without significant differences in cesarean section rates or fetal outcomes. How Do Patients with Mental Health Diagnoses Use Online Patient Portals? An Observational Analysis from the Veterans Health Administration
Online patient portals may be effective for engaging patients with mental health conditions in their own health care. This retrospective database analysis reports patient portal use among Veterans with mental health diagnoses. Unadjusted and adjusted odds of portal feature use was calculated using logistic regressions. Having experienced military sexual trauma or having an anxiety disorder, post-traumatic stress disorder, or depression were associated with increased odds of portal use; bipolar, substance use, psychotic and adjustment disorders were associated with decreased odds. Future research should examine factors that influence portal use to understand diagnosis-level differences and improve engagement with such tools
A Simple Likelihood Method for Quasar Target Selection
We present a new method for quasar target selection using photometric fluxes
and a Bayesian probabilistic approach. For our purposes we target quasars using
Sloan Digital Sky Survey (SDSS) photometry to a magnitude limit of g=22. The
efficiency and completeness of this technique is measured using the Baryon
Oscillation Spectroscopic Survey (BOSS) data, taken in 2010. This technique was
used for the uniformly selected (CORE) sample of targets in BOSS year one
spectroscopy to be realized in the 9th SDSS data release. When targeting at a
density of 40 objects per sq-deg (the BOSS quasar targeting density) the
efficiency of this technique in recovering z>2.2 quasars is 40%. The
completeness compared to all quasars identified in BOSS data is 65%. This paper

Think Outside the Color Box: Probabilistic Target Selection and the SDSS-XDQSO Quasar Targeting Catalog
We present the SDSS-XDQSO quasar targeting catalog for efficient flux-based
quasar target selection down to the faint limit of the Sloan Digital Sky Survey
(SDSS) catalog, even at medium redshifts (2.5 <~ z <~ 3) where the stellar
contamination is significant. We build models of the distributions of stars and
quasars in flux space down to the flux limit by applying the
extreme-deconvolution method to estimate the underlying density. We convolve
this density with the flux uncertainties when evaluating the probability that
an object is a quasar. This approach results in a targeting algorithm that is
more principled, more efficient, and faster than other similar methods. We
apply the algorithm to derive low-redshift (z < 2.2), medium-redshift (2.2 <= z
3.5) quasar probabilities for all 160,904,060
point sources with dereddened i-band magnitude between 17.75 and 22.45 mag in
the 14,555 deg^2 of imaging from SDSS Data Release 8. The catalog can be used
to define a uniformly selected and efficient low- or medium-redshift quasar
survey, such as that needed for the SDSS-III's Baryon Oscillation Spectroscopic
Survey project. We show that the XDQSO technique performs as well as the
current best photometric quasar-selection technique at low redshift, and
outperforms all other flux-based methods for selecting the medium-redshift
quasars of our primary interest. We make code to reproduce the XDQSO quasar

Identifying and Characterizing a Chronic Cough Cohort Through Electronic Health Records
Background
Chronic cough (CC) of 8 weeks or more affects about 10% of adults and may lead to expensive treatments and reduced quality of life. Incomplete diagnostic coding complicates identifying CC in electronic health records (EHRs). Natural language processing (NLP) of EHR text could improve detection.
Research Question
Can NLP be used to identify cough in EHRs, and to characterize adults and encounters with CC?
Study Design and Methods
A Midwestern EHR system identified patients aged 18 to 85 years during 2005 to 2015. NLP was used to evaluate text notes, except prescriptions and instructions, for mentions of cough. Two physicians and a biostatistician reviewed 12 sets of 50 encounters each, with iterative refinements, until the positive predictive value for cough encounters exceeded 90%. NLP, International Classification of Diseases, 10th revision, or medication was used to identify cough. Three encounters spanning 56 to 120 days defined CC. Descriptive statistics summarized patients and encounters, including referrals.
Results
Optimizing NLP required identifying and eliminating cough denials, instructions, and historical references. Of 235,457 cough encounters, 23% had a relevant diagnostic code or medication. Applying chronicity to cough encounters identified 23,371 patients (61% women) with CC. NLP alone identified 74% of these patients; diagnoses or medications alone identified 15%. The positive predictive value of NLP in the reviewed sample was 97%. Referrals for cough occurred for 3.0% of patients; pulmonary medicine was most common initially (64% of referrals).
Limitations
Some patients with diagnosis codes for cough, encounters at intervals greater than 4 months, or multiple acute cough episodes may have been misclassified.
Interpretation
NLP successfully identified a large cohort with CC. Most patients were identified through NLP alone, rather than diagnoses or medications. NLP improved detection of patients nearly sevenfold, addressing the gap in ability to identify and characterize CC disease burden. Nearly all cases appeared to be managed in primary care. Identifying these patients is important for characterizing treatment and unmet needs
A standardized, evidence-based protocol to assess clinical actionability of genetic disorders associated with genomic variation
Genome and exome sequencing can identify variants unrelated to the primary goal of sequencing. Detecting pathogenic variants associated with an increased risk of a medical disorder enables clinical interventions to improve future health outcomes in patients and their at-risk relatives. The Clinical Genome Resource, or ClinGen, aims to assess clinical actionability of genes and associated disorders as part of a larger effort to build a central resource of information regarding the clinical relevance of genomic variation for use in precision medicine and research
A meta-analytic review of stand-alone interventions to improve body image
Objective
Numerous stand-alone interventions to improve body image have been developed. The
present review used meta-analysis to estimate the effectiveness of such interventions, and
to identify the specific change techniques that lead to improvement in body image.
Methods
The inclusion criteria were that (a) the intervention was stand-alone (i.e., solely focused on
improving body image), (b) a control group was used, (c) participants were randomly
assigned to conditions, and (d) at least one pretest and one posttest measure of body
image was taken. Effect sizes were meta-analysed and moderator analyses were conducted.
A taxonomy of 48 change techniques used in interventions targeted at body image
was developed; all interventions were coded using this taxonomy.
Results
The literature search identified 62 tests of interventions (N = 3,846). Interventions produced
a small-to-medium improvement in body image (d+ = 0.38), a small-to-medium reduction in
beauty ideal internalisation (d+ = -0.37), and a large reduction in social comparison tendencies
(d+ = -0.72). However, the effect size for body image was inflated by bias both within
and across studies, and was reliable but of small magnitude once corrections for bias were
applied. Effect sizes for the other outcomes were no longer reliable once corrections for
bias were applied. Several features of the sample, intervention, and methodology moderated
intervention effects. Twelve change techniques were associated with improvements in
body image, and three techniques were contra-indicated.
Conclusions
The findings show that interventions engender only small improvements in body image, and
underline the need for large-scale, high-quality trials in this area. The review identifies effective
techniques that could be deployed in future interventions
