    Optimising the data combination rule for seamless phase II/III clinical trials

    We consider seamless Phase II/III clinical trials which compare K treatments with a common control in Phase II, then test the most promising treatment against control in Phase III. The final hypothesis test for the selected treatment can use data from both Phases, subject to controlling the familywise type I error rate. We show that the choice of method for conducting the final hypothesis test has a substantial impact on the power to demonstrate that an effective treatment is superior to control. To understand these differences in power we derive optimal decision rules, maximising power for particular configurations of treatment effects. Rules with optimal frequentist properties are found as solutions to multivariate Bayes decision problems. Although the optimal rule depends on the configuration of treatment means considered, we are able to identify two decision rules with robust efficiency: a rule using a weighted average of the Phase II and Phase III data on the selected treatment and control, and a closed testing procedure using an inverse normal combination rule and a Dunnett test for intersection hypotheses. For the first of these rules, we find the optimal division of a given total sample size between Phase II and Phase III.We also assess the value of using Phase II data in the final analysis and find that for many plausible scenarios, between 50% and 70% of the Phase II numbers on the selected treatment and control would need to be added to the Phase III sample size in order to achieve the same increase in power

    Bayesian inference for the information gain model

    One of the most popular paradigms to use for studying human reasoning involves the Wason card selection task. In this task, the participant is presented with four cards and a conditional rule (e.g., “If there is an A on one side of the card, there is always a 2 on the other side”). Participants are asked which cards should be turned to verify whether or not the rule holds. In this simple task, participants consistently provide answers that are incorrect according to formal logic. To account for these errors, several models have been proposed, one of the most prominent being the information gain model (Oaksford & Chater, Psychological Review, 101, 608–631, 1994). This model is based on the assumption that people independently select cards based on the expected information gain of turning a particular card. In this article, we present two estimation methods to fit the information gain model: a maximum likelihood procedure (programmed in R) and a Bayesian procedure (programmed in WinBUGS). We compare the two procedures and illustrate the flexibility of the Bayesian hierarchical procedure by applying it to data from a meta-analysis of the Wason task (Oaksford & Chater, Psychological Review, 101, 608–631, 1994). We also show that the goodness of fit of the information gain model can be assessed by inspecting the posterior predictives of the model.     Genetic risk and a primary role for cell-mediated immune mechanisms in multiple sclerosis.

    Multiple sclerosis is a common disease of the central nervous system in which the interplay between inflammatory and neurodegenerative processes typically results in intermittent neurological disturbance followed by progressive accumulation of disability. Epidemiological studies have shown that genetic factors are primarily responsible for the substantially increased frequency of the disease seen in the relatives of affected individuals, and systematic attempts to identify linkage in multiplex families have confirmed that variation within the major histocompatibility complex (MHC) exerts the greatest individual effect on risk. Modestly powered genome-wide association studies (GWAS) have enabled more than 20 additional risk loci to be identified and have shown that multiple variants exerting modest individual effects have a key role in disease susceptibility. Most of the genetic architecture underlying susceptibility to the disease remains to be defined and is anticipated to require the analysis of sample sizes that are beyond the numbers currently available to individual research groups. In a collaborative GWAS involving 9,772 cases of European descent collected by 23 research groups working in 15 different countries, we have replicated almost all of the previously suggested associations and identified at least a further 29 novel susceptibility loci. Within the MHC we have refined the identity of the HLA-DRB1 risk alleles and confirmed that variation in the HLA-A gene underlies the independent protective effect attributable to the class I region. Immunologically relevant genes are significantly overrepresented among those mapping close to the identified loci and particularly implicate T-helper-cell differentiation in the pathogenesis of multiple sclerosis

    A short educational intervention diminishes causal illusions and specific paranormal beliefs in undergraduates

    Cognitive biases such as causal illusions have been related to paranormal and pseudoscientific beliefs and, thus, pose a real threat to the development of adequate critical thinking abilities. We aimed to reduce causal illusions in undergraduates by means of an educational intervention combining training-in-bias and training-in-rules techniques. First, participants directly experienced situations that tend to induce the Barnum effect and the confirmation bias. Thereafter, these effects were explained and examples of their influence over everyday life were provided. Compared to a control group, participants who received the intervention showed diminished causal illusions in a contingency learning task and a decrease in the precognition dimension of a paranormal belief scale. Overall, results suggest that evidence-based educational interventions like the one presented here could be used to significantly improve critical thinking skills in our students

    Of Black Swans and Tossed Coins: Is the Description-Experience Gap in Risky Choice Limited to Rare Events?

    When faced with risky decisions, people tend to be risk averse for gains and risk seeking for losses (the reflection effect). Studies examining this risk-sensitive decision making, however, typically ask people directly what they would do in hypothetical choice scenarios. A recent flurry of studies has shown that when these risky decisions include rare outcomes, people make different choices for explicitly described probabilities than for experienced probabilistic outcomes. Specifically, rare outcomes are overweighted when described and underweighted when experienced. In two experiments, we examined risk-sensitive decision making when the risky option had two equally probable (50%) outcomes. For experience-based decisions, there was a reversal of the reflection effect with greater risk seeking for gains than for losses, as compared to description-based decisions. This fundamental difference in experienced and described choices cannot be explained by the weighting of rare events and suggests a separate subjective utility curve for experience

    Childhood bullying, paranoid thinking, and the misappraisal of social threat: trouble at school

    Background:Experiences of bullying predict the development of paranoia in school-age adolescents. While many instances of psychotic phenomena are transitory, maintained victimization can lead to increasingly distressing paranoid thinking. Furthermore, paranoid thinkers perceive threat in neutral social stimuli and are vigilant for environmental risk. Aims:The present paper investigated the association between different forms of bullying and paranoid thinking, and the extent to which school-age paranoid thinkers overestimate threat in interpersonal situations. Methods: Two hundred and thirty participants, aged between eleven and fourteen, were recruited from one secondary school in the UK. Participants completed a series of questionnaires hosted on the Bristol Online Survey tool. All data were collected in a classroom setting in quiet and standardized conditions. Results: A significant and positive relationship was found between experiences of bullying and paranoid thinking: greater severity of bullying predicted more distressing paranoid thinking. Further, paranoid thinking mediated the relationship between bullying and overestimation of threat in neutral social stimuli. Conclusion: Exposure to bullying is associated with distressing paranoid thinking and subsequent misappraisal of threat. As paranoid thinkers experience real and overestimated threat, the phenomena may persist
