We have extensive collections of books, maps, journals and BGS publications. We also hold special collections of British geological photographs and archive material, chiefly the historical archives of the BGS. Published material is acquired by exchange with overseas surveys and institutes as well as by purchase. In the past our collections have been enhanced by the incorporation of other collections such as the extensive holdings of the former Overseas Geological Surveys

    Early intervention of multiple home visits to prevent childhood obesity in a disadvantaged population: a home-based randomised controlled trial (Healthy Beginnings Trial)

    <p>Abstract</p> <p>Background</p> <p>Studies have shown that a proportion of children as young as two years are already overweight. This indicates that obesity prevention programs that commence as early as possible and are family-focused are needed. This Healthy Beginnings Trial aims to determine the efficacy of a community-based randomized controlled trial (RCT) of a home visiting intervention in preventing the early onset of childhood overweight and obesity. The intervention will be conducted over the first two years of life to increase healthy feeding behaviours and physical activity, decrease physical inactivity, enhance parent-child interaction, and hence reduce overweight and obesity among children at 2 and 5 years of age in the most socially and economically disadvantaged areas of Sydney, Australia.</p> <p>Methods/design</p> <p>This RCT will be conducted with a consecutive sample of 782 first time mothers with their newborn children. Pregnant women who are expecting their first child, and who are between weeks 24 and 34 of their pregnancy, will be invited to participate in the trial at the antenatal clinic. Informed consent will be obtained and participants will then be randomly allocated to the intervention or the control group. The allocation will be concealed by sequentially numbered, sealed opaque envelopes containing a computer generated random number. The intervention comprises eight home visits from a specially trained community nurse over two years and pro-active telephone support between the visits. Main outcomes include a) duration of breastfeeding measured at 6 and 12 months, b) introduction of solids measured at 4 and 6 months, c) nutrition, physical activity and television viewing measured at 24 months, and d) overweight/obesity status at age 2 and 5 years.</p> <p>Discussion</p> <p>The results of this trial will ascertain whether the home based early intervention is effective in preventing the early onset of childhood overweight and obesity. If proved to be effective, it will result in a series of recommendations for policy and practical methods for promoting healthy feeding and physical activity of children in the first two years of life with particular application to families who are socially and economically disadvantaged.</p

    Online Photography Toolkits: Digital Approaches to Practical Delivery.

    This paper draws on pedagogical research, and in particular it focuses on a research project developed by the Photography team at the University of Northampton: Leet, S., Sherwood, T., Murphy, A., Kalpaxi, E., Franchi, E., Wardle, C., Lowe, C., Smith, J. A. (2016-2017) ‘Online Photography Toolkits: Digital Approaches to Practical Delivery’, funded by the Institute of Learning and Teaching (ILT) of the University of Northampton. The Photography team at the University of Northampton created a series of digital toolkits to enable students to access information ‘on demand’ through a number of platforms and mobile devices. The development of effective digital resources to facilitate independent asynchronous skills-oriented learning (that commonly involves a hands-on approach) has been acknowledged as an essential need and a challenge in previous Photography CAleRO workshops. The development of these resources also provides potential for greater openness, in terms of sharing resources among departments and the wider public, for educational, pedagogic, and marketing purposes. In line with the University’s Waterside move in 2018, the toolkits will provide a digital resource to aid blended and flipped learning within Photography undergraduate programmes – a way to provide access to essential technical information and support beyond the confines of conventional teaching environments. This project would be appropriate for first year students needing additional support, as well as second year and Top-Up students requiring a refresher of practical skills. Traditional teaching approaches can pose barriers to a number of students, and offer only a finite duration of contact through practical delivery. It is hoped that the implementation of online toolkits will promote inclusion and lead to improved engagement and achievement for all learners, including the significant number in our cohorts who have learning differences and physical disabilities (e.g. deafness)

    Single versus dual antiplatelet therapy following peripheral arterial endovascular intervention for chronic limb threatening ischaemia:retrospective cohort study

    ObjectivesAntiplatelet therapy following peripheral arterial endovascular intervention lacks high quality evidence to guide practice. The aim of this study was to assess the effect of three months of dual antiplatelet therapy on amputation-free survival following peripheral arterial endovascular intervention in patients with chronic limb threatening ischemia.MethodsA retrospective review of symptomatic patients undergoing primary peripheral arterial endovascular intervention over a seven-year period was performed. The primary outcome measure was amputation-free survival. A sample size calculation based on previous cohort studies suggested that 629 limbs would be required to show a difference between single and dual therapy. Kaplan-Meier estimates and multivariate logistic regression analysis of recorded baseline characteristics was performed to determine predictors of amputation-free survival. Dual antiplatelet therapy was routinely given for 3 months.Results754 limbs were treated with primary angioplasty and/or stenting over a 7-year period, 508 of these for chronic limb threatening ischemia. There was no difference in unadjusted amputation-free survival between patients with chronic limb threatening ischaemia taking single vs. dual antiplatelet therapy (69% vs. 74% respectively Log rank Chi2 = 0.1, p = .72). After adjusting for confounders, at 1 year there was also no significant difference in amputation-free survival between patients taking single vs. dual antiplatelet therapy [OR 0.8, 95% CI 0.5-1.2, p = .3]. There was no difference in rates of major bleeding between single and dual antiplatelet therapy.ConclusionsThere was no clear evidence of reduced amputation-free survival in patients with chronic limb threatening ischemia undergoing peripheral arterial endovascular intervention being treated with dual antiplatelet therapy for 3 months. This is at odds with other retrospective case series and highlights the limitations in basing clinical practice on such data. There is a need for an adequately powered, independent randomised trial to definitively answer the question

    Unraveling the directional link between adiposity and inflammation: a bidirectional mendelian randomization approach

    &lt;b&gt;Context&lt;/b&gt;: Associations between adiposity and circulating inflammation markers are assumed to be causal, although the direction of the relationship has not been proven. &lt;b&gt;Objective&lt;/b&gt;: The aim of the study was to explore the causal direction of the relationship between adiposity and inflammation using a bidirectional Mendelian randomization approach. &lt;b&gt;Methods&lt;/b&gt;: In the PROSPER study of 5804 elderly patients, we related C-reactive protein (CRP) single nucleotide polymorphisms (SNPs) (rs1800947 and rs1205) and adiposity SNPs (FTO and MC4R) to body mass index (BMI) as well as circulating levels of CRP and leptin. We gave each individual two allele scores ranging from zero to 4, counting each pair of alleles related to CRP levels or BMI. &lt;b&gt;Results&lt;/b&gt;: With increasing CRP allele score, there was a stepwise decrease in CRP levels (P for trend &#60; 0.0001) and a 1.98 mg/liter difference between extremes of the allele score distribution, but there was no associated change in BMI or leptin levels (P ≥ 0.89). By contrast, adiposity allele score was associated with 1) an increase in BMI (1.2 kg/m2 difference between extremes; P for trend 0.002); 2) an increase in circulating leptin (5.77 ng/ml difference between extremes; P for trend 0.0027); and 3) increased CRP levels (1.24 mg/liter difference between extremes; P for trend 0.002). &lt;b&gt;Conclusions&lt;/b&gt;: Greater adiposity conferred by FTO and MC4R SNPs led to higher CRP levels, with no evidence for any reverse pathway. Future studies should extend our findings to other circulating inflammatory parameters. This study illustrates the potential power of Mendelian randomization to dissect directions of causality between intercorrelated metabolic factors

    A review of microwave coal processing

    This paper reviews the application of microwave energy in coal processing and utilisation. It covers a wide variety of processes including liberation of minerals, coal drying, pre-treatment to improve grindability, liquefaction enhancements and coke making. There is also consideration of analytical techniques that use microwave energy for continuous monitoring of coal feed systems. Where possible, industrial or pilot scale examples of these processes are discussed, along with a consideration of the significant hurdles that exist scaling from bench or pilot scale systems to industrial sized units. The processes with more immediate promise for scale-up are identified, as are those that require further laboratory scale experiments to establish whether microwave processing should be pursued at a larger scale. The importance of dielectric properties of the materials to be processed is emphasised, and the reasons behind the need for a multi-disciplinary approach to the design and operation of electromagnetic experiments are explained. Microwave coal processing is a diverse area, which has the potential to aid in coal upgrading, cleaning and comminution, thus improving efficiency and reducing harmful emissions of coal usage. Other promising research areas include the investigation of microwave processing for coking, liquefaction, enhancing fluid flow in coal beds and coal characterisation. However, due to the scale of most industrial coal processing systems and the lack of availability of microwave hardware at power levels of above 100 kW, commercial applications of microwaves for coal processing are still some if not many years away. Whilst there are some interesting applications from a scientific point of view, a very strong value proposition coupled with an organisation with high acceptance of risk would be required to develop high power microwave equipment able to work effectively in dirty, harsh environments

    Ovarian cancer symptom awareness and anticipated delayed presentation in a population sample

    Background: While ovarian cancer is recognised as having identifiable early symptoms, understanding of the key determinants of symptom awareness and early presentation is limited. A population-based survey of ovarian cancer awareness and anticipated delayed presentation with symptoms was conducted as part of the International Cancer Benchmarking Partnership (ICBP). Methods: Women aged over 50 years were recruited using random probability sampling (n = 1043). Computer-assisted telephone interviews were used to administer measures including ovarian cancer symptom recognition, anticipated time to presentation with ovarian symptoms, health beliefs (perceived risk, perceived benefits/barriers to early presentation, confidence in symptom detection, ovarian cancer worry), and demographic variables. Logistic regression analysis was used to identify the contribution of independent variables to anticipated presentation (categorised as < 3 weeks or ≥ 3 weeks). Results: The most well-recognised symptoms of ovarian cancer were post-menopausal bleeding (87.4%), and persistent pelvic (79.0%) and abdominal (85.0%) pain. Symptoms associated with eating difficulties and changes in bladder/bowel habits were recognised by less than half the sample. Lower symptom awareness was significantly associated with older age (p ≤ 0.001), being single (p ≤ 0.001), lower education (p ≤ 0.01), and lack of personal experience of ovarian cancer (p ≤ 0.01). The odds of anticipating a delay in time to presentation of ≥ 3 weeks were significantly increased in women educated to degree level (OR = 2.64, 95% CI 1.61 – 4.33, p ≤ 0.001), women who reported more practical barriers (OR = 1.60, 95% CI 1.34 – 1.91, p ≤ 0.001) and more emotional barriers (OR = 1.21, 95% CI 1.06 – 1.40, p ≤ 0.01), and those less confident in symptom detection (OR = 0.56, 95% CI 0.42 – 0.73, p ≤ 0.001), but not in those who reported lower symptom awareness (OR = 0.99, 95% CI 0.91 – 1.07, p = 0.74). Conclusions: Many symptoms of ovarian cancer are not well-recognised by women in the general population. Evidence-based interventions are needed not only to improve public awareness but also to overcome the barriers to recognising and acting on ovarian symptoms, if delays in presentation are to be minimised
