Premature mortality in refractory partial epilepsy: does surgical treatment make a difference?
Background: Epilepsy carries an increased risk of premature death. For some people with intractable focal epilepsy, surgery offers hope for a seizure-free life. The authors aimed to see whether epilepsy surgery influenced mortality in people with intractable epilepsy. Methods: The authors audited survival status in two cohorts (those who had surgery and those who had presurgical assessment but did not have surgery). Results: There were 40 known deaths in the non-surgical group (3365 person years of follow-up) and 19 in the surgical group (3905 person-years of follow-up). Non-operated patients were 2.4 times (95% CI 1.4 to 4.2) as likely to die as those who had surgery. They were 4.5 times (95% CI 1.9 to 10.9) as likely to die a probable epilepsy-related death. In the surgical group, those with ongoing seizures 1 year after surgery were 4.0 (95% CI 1.2 to 13.7) times as likely to die as those who were seizure-free or who had only simple partial seizures. Time-dependent Cox analysis showed that the yearly outcome group did not significantly affect mortality (HR 1.3, 95% CI 0.9 to 1.8). Conclusion: Successful epilepsy surgery was associated with a reduced risk of premature mortality, compared with those with refractory focal epilepsy who did not have surgical treatment. To some extent, the reduced mortality is likely to be conferred by inducing freedom from seizures. It is not certain whether better survival is attributable only to surgery, as treatment decisions were not randomised, and there may be inherent differences between the groups.<br/
Plasma Turbulence in the Local Bubble
Turbulence in the Local Bubble could play an important role in the
thermodynamics of the gas that is there. The best astronomical technique for
measuring turbulence in astrophysical plasmas is radio scintillation.
Measurements of the level of scattering to the nearby pulsar B0950+08 by
Philips and Clegg in 1992 showed a markedly lower value for the line-of-sight
averaged turbulent intensity parameter is smaller than normal for two of them, but is completely nominal for
the third. This inconclusive status of affairs could be improved by

A Sawtooth-like Timeline for the First Billion Year of Lunar Bombardment
We revisit the early evolution of the Moon's bombardment. Our work combines
modeling (based on plausible projectile sources and their dynamical decay
rates) with constraints from the lunar crater record, radiometric ages of the
youngest lunar basins, and the abundance of highly siderophile elements in the
lunar crust and mantle. We deduce that the evolution of the impact flux did not
decline exponentially over the first billion years of lunar history, but also
there was no prominent and "narrow" impact spike some 3.9 Gy ago, unlike that
typically envisioned in the lunar cataclysm scenario. Instead, we show the
timeline of the lunar bombardment has a sawtooth-like profile, with an uptick
in the impact flux near 4.1 Gy ago. The impact flux at the beginning of this
weaker cataclysm was 5-10 times higher than the immediately preceding period.
The Nectaris basin should have been one of the first basins formed at the
sawtooth. We predict the bombardment rate since about 4.1Gy ago declined slowly
and adhered relatively close to classic crater chronology models (Neukum and
Ivanov (1994)). Overall we expect that the sawtooth event accounted for about
1/4 of the total bombardment suffered by the Moon since its formation.
Consequently, considering that about 12-14 basins formed during the sawtooth
event, we expect that the net number of basins formed on the Moon was about
45-50. From our expected bombardment timeline, we derived a new and improved
lunar chronology suitable for use on Pre-Nectarian surface units. According to
this chronology, a significant portion of the oldest lunar cratered terrains
has an age of 4.38-4.42 Gyr. Moreover, the largest lunar basin, South Pole
Aitken, is older than 4.3Gy, and therefore was not produced during the lunar

11 beta-Hydroxysteroid dehydrogenase type 1 contributes to the regulation of 7-oxysterol levels in the arterial wall through the inter-conversion of 7-ketocholesterol and 7 beta-hydroxycholesterol
AbstractThe atherogenic 7-oxysterols, 7-ketocholesterol (7-KC) and 7ÎČ-hydroxycholesterol (7ÎČOHC), can directly impair arterial function. Inter-conversion of 7-KC and 7ÎČOHC has recently been shown as a novel role for the glucocorticoid-metabolizing enzyme 11ÎČ-hydroxysteroid dehydrogenase type 1 (11ÎČ-HSD1). Since this enzyme is expressed in vascular smooth muscle cells, we addressed the hypothesis that inter-conversion of 7-KC and 7ÎČOHC by 11ÎČ-HSD1 may contribute to regulation of arterial function.Incubation (4â24 h) of aortic rings with either 7-KC (25 ΌM) or 7ÎČOHC (20 ΌM) had no effect on endothelium-dependent (acetylcholine) or -independent (sodium nitroprusside) relaxation. In contrast, exposure to 7-KC (but not to 7ÎČOHC) attenuated noradrenaline-induced contraction (Emax) after 4 h (0.78 ± 0.28 vs 0.40 ± 0.08 mN/mm; p < 0.05) and 24 h (2.28 ± 0.34 vs 1.56 ± 0.48 mN/mm; p < 0.05). Both 7-oxysterols were detected by GCMS in the aortic wall of chow-fed C57Bl6/J mice, with concentrations of 7-KC (1.41 ± 0.81 ng/mg) higher (p = 0.05) than 7ÎČOHC (0.16 ± 0.06 ng/mg). In isolated mouse aortic rings 11ÎČ-HSD1 was shown to act as an oxo-reductase, inter-converting 7-KC and 7ÎČOHC. This activity was lost in aorta from 11ÎČ-HSD1â/â mice, which had low oxysterol levels. Renal homogenates from 11ÎČ-HSD1â/â mice were used to confirm that the type 2 isozyme of 11ÎČ-HSD does not inter-convert 7-KC and 7ÎČOHC.These results demonstrate that 7-KC has greater effects than 7ÎČOHC on vascular function, and that 11ÎČ-HSD1 can inter-convert 7-KC and 7ÎČOHC in the arterial wall, contributing to the regulation of 7-oxysterol levels and potentially influencing vascular function. This mechanism may be important in the cardioprotective effects of 11ÎČ-HSD1 inhibitors
Anomalous Effects of "Guest" Charges Immersed in Electrolyte: Exact 2D Results
We study physical situations when one or two "guest" arbitrarily-charged
particles are immersed in the bulk of a classical electrolyte modelled by a
Coulomb gas of positive/negative unit point-like charges, the whole system
being in thermal equilibrium. The models are treated as two-dimensional with
logarithmic pairwise interactions among charged constituents; the
(dimensionless) inverse temperature is considered to be smaller than 2
in order to ensure the stability of the electrolyte against the collapse of
positive-negative pairs of charges. Based on recent progress in the integrable
(1+1)-dimensional sine-Gordon theory, exact formulas are derived for the
chemical potential of one guest charge and for the asymptotic large-distance
behavior of the effective interaction between two guest charges. The exact
results imply, under certain circumstances, anomalous effects such as an
effective attraction (repulsion) between like-charged (oppositely-charged)
guest particles and the charge inversion in the electrolyte vicinity of a
highly-charged guest particle. The adequacy of the concept of renormalized
charge is confirmed in the whole stability region of inverse temperatures and

Yet for many years the place of the author in the practice of understanding literary works has been problematized, and even on occasions eliminated. Criticism reads the âworksâ, and may or may not refer to an author whose âlifeâ contributed to their meaning. Biography seeks the author in the works, the personality that precedes the works and gives them their characteristic shape and meaning. But the form of literary biography addresses the unusual kind of âlifeâ that puts itself into âworksâ, and this is particularly challenging where the âworksâ predominate massively over the salient facts of the âlifeâ. This essay surveys the current terrain of Shakespeare biography, and considers the key questions raised by the medium: can we know anything of Shakespeare's âpersonalityâ from the facts of his life and the survival of his works? What is the status of the kind of speculation that inevitably plays a part in biographical reconstruction? Are biographers in the end telling us as much about themselves as they tell us about Shakespeare?Peer reviewe
Can forest management based on natural disturbances maintain ecological resilience?
Given the increasingly global stresses on forests, many ecologists argue that managers must maintain ecological resilience: the capacity of ecosystems to absorb disturbances without undergoing fundamental change. In this review we ask: Can the emerging paradigm of natural-disturbance-based management (NDBM) maintain ecological resilience in managed forests? Applying resilience theory requires careful articulation of the ecosystem state under consideration, the disturbances and stresses that affect the persistence of possible alternative states, and the spatial and temporal scales of management relevance. Implementing NDBM while maintaining resilience means recognizing that (i) biodiversity is important for long-term ecosystem persistence, (ii) natural disturbances play a critical role as a generator of structural and compositional heterogeneity at multiple scales, and (iii) traditional management tends to produce forests more homogeneous than those disturbed naturally and increases the likelihood of unexpected catastrophic change by constraining variation of key environmental processes. NDBM may maintain resilience if silvicultural strategies retain the structures and processes that perpetuate desired states while reducing those that enhance resilience of undesirable states. Such strategies require an understanding of harvesting impacts on slow ecosystem processes, such as seed-bank or nutrient dynamics, which in the long term can lead to ecological surprises by altering the forest's capacity to reorganize after disturbance
Horizontal Branch Stars: The Interplay between Observations and Theory, and Insights into the Formation of the Galaxy
We review HB stars in a broad astrophysical context, including both variable
and non-variable stars. A reassessment of the Oosterhoff dichotomy is
presented, which provides unprecedented detail regarding its origin and
systematics. We show that the Oosterhoff dichotomy and the distribution of
globular clusters (GCs) in the HB morphology-metallicity plane both exclude,
with high statistical significance, the possibility that the Galactic halo may
have formed from the accretion of dwarf galaxies resembling present-day Milky
Way satellites such as Fornax, Sagittarius, and the LMC. A rediscussion of the
second-parameter problem is presented. A technique is proposed to estimate the
HB types of extragalactic GCs on the basis of integrated far-UV photometry. The
relationship between the absolute V magnitude of the HB at the RR Lyrae level
and metallicity, as obtained on the basis of trigonometric parallax
measurements for the star RR Lyrae, is also revisited, giving a distance
modulus to the LMC of (m-M)_0 = 18.44+/-0.11. RR Lyrae period change rates are
studied. Finally, the conductive opacities used in evolutionary calculations of

Safer topical treatment for inflammation using 5α-tetrahydrocorticosterone in mouse models
Use of topical glucocorticoid for inflammatory skin conditions is limited by systemic and local side-effects. This investigation addressed the hypothesis that topical 5α-tetrahydrocorticosterone (5αTHB, a corticosterone metabolite) inhibits dermal inflammation without affecting processes responsible for skin thinning and impaired wound healing. The topical anti-inflammatory properties of 5αTHB were compared with those of corticosterone in C57Bl/6 male mice with irritant dermatitis induced by croton oil, whereas its effects on angiogenesis, inflammation, and collagen deposition were investigated by subcutaneous sponge implantation. 5αTHB decreased dermal swelling and total cell infiltration associated with dermatitis similarly to corticosterone after 24 h, although at a five fold higher dose, but in contrast did not have any effects after 6 h. Pre-treatment with the glucocorticoid receptor antagonist RU486 attenuated the effect of corticosterone on swelling at 24 h, but not that of 5αTHB. After 24 h 5αTHB reduced myeloperoxidase activity (representative of neutrophil infiltration) to a greater extent than corticosterone. At equipotent anti-inflammatory doses 5αTHB suppressed angiogenesis to a limited extent, unlike corticosterone which substantially decreased angiogenesis compared to vehicle. Furthermore, 5αTHB reduced only endothelial cell recruitment in sponges whereas corticosterone also inhibited smooth muscle cell recruitment and decreased transcripts of angiogenic and inflammatory genes. Strikingly, corticosterone, but not 5αTHB, reduced collagen deposition. However, both 5αTHB and corticosterone attenuated macrophage infiltration into sponges. In conclusion, 5αTHB displays the profile of a safer topical anti-inflammatory compound. With limited effects on angiogenesis and extracellular matrix, it is less likely to impair wound healing or cause skin thinning
