    Diversity of group B streptococcus serotypes causing urinary tract infection in adults

    Serotypes of group B streptococcus (GBS) that cause urinary tract infection (UTI) are poorly characterized. We conducted a prospective study of GBS UTI in adults to define the clinical and microbiological characteristics of these infections, including which serotypes cause disease. Patients who had GBS cultured from urine over a 1-year period were grouped according to symptoms, bacteriuria, and urinalysis. Demographic data were obtained by reviewing medical records. Isolates were serotyped by latex agglutination and multiplex PCRreverse line blotting (mPCR/RLB). Antibiotic susceptibilities were determined by disc diffusion. GBS was cultured from 387/34,367 consecutive urine samples (1.1%): 62 patients had bacteriuria of >10 7 CFU/liter and at least one UTI symptom; of these patients, 31 had urinary leukocyte esterase and pyuria (others not tested), 50 (81%) had symptoms consistent with cystitis, and 12 (19%) had symptoms of pyelonephritis. Compared with controls (who had GBS isolated without symptoms), a prior history of UTI was an independent risk factor for disease. Increased age was also significantly associated with acute infection. Serotyping results were consistent between latex agglutination and mPCR/RLB for 331/387 (85.5%) isolates; 22 (5.7%) and 7 (1.8%) isolates were nontypeable with antisera and by mPCR/RLB, respectively; and 45/56 (80.4%) isolates with discrepant results were typed by mPCR/RLB as belonging to serotype V. Serotypes V, Ia, and III caused the most UTIs; serotypes II, Ib, and IV were less common. Nontypeable GBS was not associated with UTI. Erythromycin (39.5%) and clindamycin (26.4%) resistance was common. We conclude that a more diverse spectrum of GBS serotypes causes UTI than previously recognized, with the exception of nontypeable GBS

    Broadband High-Efficiency Power Amplifier with Quasi-Elliptic Low-Pass Response

    © 2013 IEEE. This paper presents a broadband high-efficiency harmonic-tuned power amplifier (PA) with quasi-elliptic low-pass responses. A combination of continuous Class-F-1 and extended continuous Class-F PA modes is employed to significantly expand the design space. A quasi-elliptic low-pass matching network is proposed, which can realize a broadband impedance matching in the predefined optimal impedance region desired by the combination of PA modes. Furthermore, two transmission zeros are generated near the passband, exhibiting high skirt selectivity and providing rapid impedance transition from the passband to the stopband. A wide stopband covering up to the third harmonic is achieved which shows good harmonic suppression. Design procedures of the proposed broadband PA are presented. To verify the proposed methodology, the broadband PA is fabricated and measured. The implemented PA achieves a bandwidth of 145.2% from 0.5 to 3.15 GHz. Over this frequency range, the drain efficiency is measured as 58-74.9% with the output power of greater than 39.03 dBm and a large signal gain ranging from 8.43 to 15.67 dB. A wide stopband is realized from 3.4 to 10 GHz, showing excellent quasi-elliptic low-pass responses. The measured adjacent leakage ratios (ACLRs) using a 20-MHz LTE signal with digital pre-distortion are below -45.06 dBc

    H-alpha +[NII] Observations of the HII Regions in M81

    In a first of a series of studies of the H-alpha + [NII] emission from nearby spiral galaxies, we present measurements of H-alpha + [NII] emission from HII regions in M81. Our method uses large-field-CCD images and long-slit spectra, and is part of the ongoing Beijing-Arizona-Taipei-Connecticut Sky Survey. The CCD images are taken with the NAOC 0.6/0.9m f/3 Schmidt telescope at the Xinglong Observing Station, using a multicolor filter set. Spectra of 10 of the brightest HII regions are obtained using the NAOC 2.16m telescope with a Tek 1024 X 1024 CCD. The continua of the spectra are calibrated by flux-calibrated images taken from the Schmidt observations. We determine the continuum component of our H-alpha + [NII] image via interpolation from the more accurately-measured backgrounds (M81 starlight) obtained from the two neighboring (in wavelength) BATC filter images. We use the calibrated fluxes of H-alpha + [NII] emission from the spectra to normalize this interpolated, continuum-subtracted H-alpha + [NII] image. We estimate the zero point uncertainty of the measured H-alpha + [NII] emission flux to be \sim 8%. A catalogue of H-alpha + [NII] fluxes for 456 HII regions is provided, with those fluxes are on a more consistent linear scale than previously available. The logarithmically-binned H-alpha + [NII] luminosity function of HII regions is found to have slope α\alpha = -0.70, consistent with previous results (which allowed α=0.50.8\alpha=-0.5 \sim -0.8). From the overall H-alpha + [NII] luminosity of the HII regions, the star formation rate of M81 is found to be 0.68Myr1\sim 0.68 M_{\odot} {\rm yr}^{-1}, modulo uncertainty with extinction corrections.Comment: 18 pages, 7 figures, accepted for publication in the Astronomical Journa

    Tackling tuberculosis: insights from an international TB Summit in London

    Tuberculosis (TB) poses a grave predicament to the world as it is not merely a scientific challenge but a socio-economic burden as well. A prime cause of mortality in human due to an infectious disease; the malady and its cause, Mycobacterium tuberculosis have remained an enigma with many questions that remain unanswered. The ability of the pathogen to survive and switch between varied physiological states necessitates a protracted therapeutic regimen that exerts an excessive strain on low-resource countries. To complicate things further, there has been a significant rise of antimicrobial resistance. Existing control measures, including treatment regimens have remained fairly uniform globally for at least half a century and require reinvention. Overcoming the societal and scientific challenges requires an increase in dialog to identify key regions that need attention and effective partners with whom successful collaborations can be fostered. In this report, we explore the discussions held at the International TB Summit 2015 hosted by EuroSciCon, which served as an excellent platform for researchers to share their recent findings. Ground-breaking results require outreach to affect policy design, governance and control of the disease. Hence, we feel it is important that meetings such as these reach a wider, global audience

    Monotonicity of Fitness Landscapes and Mutation Rate Control

    A common view in evolutionary biology is that mutation rates are minimised. However, studies in combinatorial optimisation and search have shown a clear advantage of using variable mutation rates as a control parameter to optimise the performance of evolutionary algorithms. Much biological theory in this area is based on Ronald Fisher's work, who used Euclidean geometry to study the relation between mutation size and expected fitness of the offspring in infinite phenotypic spaces. Here we reconsider this theory based on the alternative geometry of discrete and finite spaces of DNA sequences. First, we consider the geometric case of fitness being isomorphic to distance from an optimum, and show how problems of optimal mutation rate control can be solved exactly or approximately depending on additional constraints of the problem. Then we consider the general case of fitness communicating only partial information about the distance. We define weak monotonicity of fitness landscapes and prove that this property holds in all landscapes that are continuous and open at the optimum. This theoretical result motivates our hypothesis that optimal mutation rate functions in such landscapes will increase when fitness decreases in some neighbourhood of an optimum, resembling the control functions derived in the geometric case. We test this hypothesis experimentally by analysing approximately optimal mutation rate control functions in 115 complete landscapes of binding scores between DNA sequences and transcription factors. Our findings support the hypothesis and find that the increase of mutation rate is more rapid in landscapes that are less monotonic (more rugged). We discuss the relevance of these findings to living organisms

    Cd44v6 high membranous expression is a predictive marker of therapy response in gastric cancer patients

    In gastric cancer (GC), biomarkers that define prognosis and predict treatment response remain scarce. We hypothesized that the extent of CD44v6 membranous tumor expression could predict prognosis and therapy response in GC patients. Two GC surgical cohorts, from Portugal and South Korea (n = 964), were characterized for the extension of CD44v6 membranous immuno-expression, clinicopathological features, patient survival, and therapy response. The value of CD44v6 expression in predicting response to treatment and its impact on prognosis was determined. High CD44v6 expression was associated with invasive features (perineural invasion and depth of invasion) in both cohorts and with worse survival in the Portuguese GC cohort (HR 1.461; 95% confidence interval 1.002–2.131). Patients with high CD44v6 tumor expression benefited from conventional chemotherapy in addition to surgery (p < 0.05), particularly those with heterogeneous CD44v6-positive and-negative populations (CD44v6_3+) (p < 0.007 and p < 0.009). Our study is the first to identify CD44v6 high membranous expression as a potential predictive marker of response to conventional treatment, but it does not clarify CD44v6 prognostic value in GC. Importantly, our data support selection of GC patients with high CD44v6-expressing tumors for conventional chemotherapy in addition to surgery.
