    Statistical investigation of procedure in hospital clinics

    This thesis is a study of the daily working routine of a hospital X-Ray department, and it describes the empirical variation observed in a real department by the use of mathematical models| these models may be used to predict the effects of changes in departmental operating policy, thereby leading to a more effective use of the available resources* An outline is made of the working procedures of a typical X-Ray department, and there is also a survey of the literature. The various types of variability to be observed are described, together with some efficiency measures which may be adopted* In Chapter k the results of survey work in the Royal Infirmary, Edinburgh are presented in the form of an analysis of the work-load on the X-Ray department, with respect to its constitution, origin and distribution over the X-Ray facilities* An investigation is made into the relationship between the time to perform a given examination, and the age, sex and mobility of the patient* It was found that considerable differences exist between the service time distributions of patients with different characteristics, and these were later used to consider if improvements in efficiency might result by dealing with patients in homogeneous groups* A number of comments are made on particular problem areas in the Royal Infirmary

    The orbit rigidity matrix of a symmetric framework

    A number of recent papers have studied when symmetry causes frameworks on a graph to become infinitesimally flexible, or stressed, and when it has no impact. A number of other recent papers have studied special classes of frameworks on generically rigid graphs which are finite mechanisms. Here we introduce a new tool, the orbit matrix, which connects these two areas and provides a matrix representation for fully symmetric infinitesimal flexes, and fully symmetric stresses of symmetric frameworks. The orbit matrix is a true analog of the standard rigidity matrix for general frameworks, and its analysis gives important insights into questions about the flexibility and rigidity of classes of symmetric frameworks, in all dimensions. With this narrower focus on fully symmetric infinitesimal motions, comes the power to predict symmetry-preserving finite mechanisms - giving a simplified analysis which covers a wide range of the known mechanisms, and generalizes the classes of known mechanisms. This initial exploration of the properties of the orbit matrix also opens up a number of new questions and possible extensions of the previous results, including transfer of symmetry based results from Euclidean space to spherical, hyperbolic, and some other metrics with shared symmetry groups and underlying projective geometry.Comment: 41 pages, 12 figure

    The relationship between Clinical Frailty Score, CT-derived body composition, systemic inflammation, and survival in patients with chronic limb-threatening ischemia

    Background: Frailty is a chronic condition with complex etiology and impaired functional performance that has been associated with altered body composition and chronic inflammation. Chronic limb-threatening ischemia (CLTI) carries significant morbidity and mortality and is associated with poor quality of life. The present study aims to examine these relationships and their prognostic value in patients with CLTI. Methods: Consecutive patients presenting as unscheduled admissions to a single tertiary center with CLTI were included over a 12-month period. Frailty was diagnosed using the Clinical Frailty Scale (CFS). Body composition was assessed using computerised tomography (CT) at the L3 vertebral level (CT-BC) to generate visceral and subcutaneous fat indices, skeletal muscle index, and skeletal muscle density. Skeletal muscle index and skeletal muscle density were combined to form the CT-sarcopenia score (CT-SS). Systemic inflammation was assessed by the modified Glasgow prognostic score (mGPS). The primary outcome was overall mortality. Results: There were 190 patients included with a median (interquartile range) follow-up of 22 (6) months (range 15–32 months) and 79 deaths during the follow-up period. One hundred patients (53%) had a CFS >4. CFS >4 (hazard ratio [HR] 2.14, 95% confidence interval [CI] 1.25–3.66, P < 0.01), CT-SS (HR 1.47, 95% CI 1.03–2.09, P < 0.05), and mGPS (HR 1.54, 95% CI 1.11–2.13, P < 0.01) were independently associated with increased mortality. CT-SS (odds ratio 1.88, 95% CI 1.09–3.24, P < 0.01) was independently associated with CFS >4. Patients with CT-SS 0 and CFS ≤4 had 90% (standard error [SE] 5%) 1-year survival, compared with 35% (SE 9%) in patients with CT-SS 2 and CFS >4 (P < 0.001). Patients with mGPS 0 and CFS ≤4 had 94% (SE 4%) 1-year survival compared with 44% (SE 6%) in the mGPS 2 and CFS >4 subgroup (P < 0.001). Conclusions: Frailty assessed by CFS was associated with CT-BC. CFS, CT-SS, and mGPS were associated with poorer survival in patients presenting as unscheduled admissions with CLTI. CT-SS and mGPS may contribute to part of frailty and prognostic assessment in this patient cohort

    Observational diagnostics of gas in protoplanetary disks

    Protoplanetary disks are composed primarily of gas (99% of the mass). Nevertheless, relatively few observational constraints exist for the gas in disks. In this review, I discuss several observational diagnostics in the UV, optical, near-IR, mid-IR, and (sub)-mm wavelengths that have been employed to study the gas in the disks of young stellar objects. I concentrate in diagnostics that probe the inner 20 AU of the disk, the region where planets are expected to form.     Evaluation of the prognostic value of computed tomography-derived body composition in patients undergoing endovascular aneurysm repair

    Background: Endovascular aneurysm repair (EVAR) is the most common mode of repair of abdominal aortic aneurysms (AAA) in the UK. EVAR ranges from standard infrarenal repair to complex fenestrated and branched EVAR (F/B-EVAR). Sarcopenia is defined by lower muscle mass and function, which is associated with inferior perioperative outcomes. Computed tomography-derived body composition analysis offers prognostic value in patients with cancer. Several authors have evaluated the role of body composition analysis in predicting outcomes in patients undergoing EVAR; however, the evidence base is limited by heterogeneous methodology. Methods: Six hundred seventy-four consecutive patients (58 (8.6%) female, mean (SD) age 74.4 (6.8) years) undergoing EVAR and F/B-EVAR at three large tertiary centres were retrospectively recruited. Subcutaneous and visceral fat indices (SFI and VFI), psoas and skeletal muscle indices, and skeletal muscle density were measured at the L3 vertebral level from pre-operative computed tomographies. The maximally selected rank statistic technique was used to define optimal thresholds to predict mortality. Results: There were 191 deaths during the median follow-up period of 60.0 months. Mean (95% CI) survival in the low SMI versus high SMI subgroups was 62.6 (58.5–66.7) versus 82.0 (78.7–85.3) months (P < 0.001). Mean (95% CI) survival in the low SFI versus high SFI subgroups was 56.4 (48.2–64.7) versus 77.1 (74.2–80.1) months (P < 0.001). One-year mortality in the low SMI versus high SMI subgroups was 10% versus 3% (P < 0.001). Low SMI was associated with increased odds of one-year mortality (OR 3.19, 95% CI 1.60–6.34, P < 0.001). Five-year mortality in the low SMI versus high SMI subgroups was 55% versus 28% (P < 0.001). Low SMI was associated with increased odds of five-year mortality (OR 1.54, 95% CI 1.11–2.14, P < 0.01). On multivariate analysis of all patients, low SFI (HR 1.90, 95% CI 1.30–2.76, P < 0.001) and low SMI (HR 1.88, 95% CI 1.34–2.63, P < 0.001) were associated with poorer survival. On multivariate analysis of asymptomatic AAA patients, low SFI (HR 1.54, 95% CI 1.01–2.35, P < 0.05) and low SMI (HR 1.71, 95% CI 1.20–2.42, P < 0.01) were associated with poorer survival. Conclusions: Low SMI and SFI are associated with poorer long-term survival following EVAR and F/B-EVAR. The relationship between body composition and prognosis requires further evaluation, and external validation of the thresholds proposed in patients with AAA is required

    γ -ray spectroscopy of astrophysically important states in Ca 39

    Background: Nova explosions synthesize elements up to A≃40, and discrepancies exist between calculated and observed abundances of Ar and Ca created in the explosion. The K38(p,γ)Ca39 reaction rate has been shown to be influential on these isotopic abundances at the endpoint of nova nucleosynthesis. The energies of the three most important resonances, corresponding to Jπ=5/2+ excited states in the Ca39 nucleus above the proton separation threshold, are uncertain and one has been measured with conflicting values [Er=679(2) versus Er=701(2) keV] in previous experiments. Purpose: Reducing the uncertainties on the resonance energies would allow for a better understanding of the reaction rate. To improve these uncertainties, we searched for γ rays from the depopulation of the corresponding excited states in Ca39. Methods: We report a new measurement of these resonance energies via the observation of previously unobserved γ-ray transitions. These transitions were observed by studying the Ca40(3He,αγ)Ca39 reaction with Gammasphere ORRUBA Dual Detectors for Experimental Structure Studies (GODDESS). The updated resonance energies were then used to calculate the K38(p,γ)Ca39 reaction rate and assess its uncertainties. Results: In total, 23 new transitions were found, including three γ-ray transitions corresponding to the three Jπ=5/2+ states of astrophysical interest at energies of 6156.2(16), 6268.8(22), and 6470.8(19) keV. These correspond to resonance energies in the K38(p,γ)Ca39 reaction of 386(2), 498(2), and 701(2) keV. Conclusions: Updated K38(p,γ)Ca39 reaction rate calculations show a reduced upper limit at nova temperatures. However, the lower-than-previously-measured energy of the 498-keV resonance and uncertainty in its resonance strength increases the upper limit of the rate close to previous estimates at 0.4 GK

    New γ -ray transitions observed in Ne 19 with implications for the O 15 (α,γ) Ne 19 reaction rate

    The O15(α,γ)Ne19 reaction is responsible for breakout from the hot CNO cycle in type I x-ray bursts. Understanding the properties of resonances between Ex=4 and 5 MeV in Ne19 is crucial in the calculation of this reaction rate. The spins and parities of these states are well known, with the exception of the 4.14- and 4.20-MeV states, which have adopted spin-parities of 9/2- and 7/2-, respectively. γ-ray transitions from these states were studied using triton-γ-γ coincidences from the F19(He3,tγ)Ne19 reaction measured with the GODDESS (Gammasphere ORRUBA Dual Detectors for Experimental Structure Studies) at Argonne National Laboratory. The observed transitions from the 4.14- and 4.20-MeV states provide strong evidence that the Jπ values are actually 7/2- and 9/2-, respectively. These assignments are consistent with the values in the F19 mirror nucleus and in contrast to previously accepted assignments

    Low Q^2 Jet Production at HERA and Virtual Photon Structure

    The transition between photoproduction and deep-inelastic scattering is investigated in jet production at the HERA ep collider, using data collected by the H1 experiment. Measurements of the differential inclusive jet cross-sections dsigep/dEt* and dsigmep/deta*, where Et* and eta* are the transverse energy and the pseudorapidity of the jets in the virtual photon-proton centre of mass frame, are presented for 0 < Q2 < 49 GeV2 and 0.3 < y < 0.6. The interpretation of the results in terms of the structure of the virtual photon is discussed. The data are best described by QCD calculations which include a partonic structure of the virtual photon that evolves with Q2.Comment: 20 pages, 5 Figure
