    ORGANISATIONAL CULTURE: A CRITICAL FACTOR DETERMINING THE SUCCESSFUL IMPLEMENTATION OF BUSINESS PROCESS REENGINEERING. An Orange Business Services perspective

    Organisations assert to have effectively embarked on business process reengineering (BPR) and indicate remarkable enhancements in their competitive position. Administrative advisors have moderately emphasised the necessity to address the people and organisational concerns, so as to successfully implement BPR. This report illustrates research into business process reengineering and organisational culture change. A literature review is initially used to describe BPR and then, using 7s model of McKinsey, full impact of BPR on an organisation is shown, predominantly in terms of its culture. Further report is focused on the possible ways of managing this crucial dimension, so that the cooperation of organizational culture can be obtained for facilitating the successful BPR implementation. The research conducted in this paper is based on both theoretical and the practical dimensions. The start of this report provides the theoretical dimension by elaborating on the literature whereas the close association with Orange Business Services and the interviews conducted will add the practical and applied dimension to this research work. This amalgamation will help serving the purpose of both the practitioner and the researcher

    Laparoscopic management of a giant prostatic utricle: A case report and review of literature

    Prostatic utricle is a Mullerian duct remnant with an incidence of 1 %. Excision of utricle is challenging because of the close proximity of seminal vesicle, ejaculatory ducts, bladder, rectum, ureter, and nerve plexus. Here, we report the case of a 23-year-old male presented with complaints of painful terminal hematuria associated with clots along with retrograde ejaculation. The abdominal and local examination was within normal limits. MRI pelvis showed a large non-communicating cystic structure present in the pelvic cavity, compressing posterior wall of the urinary bladder, seminal vesicle, anterior wall of the rectum and also causing left hydroureteronephrosis. After evaluation, the patient was diagnosed with giant prostatic utricle cyst. Laparoscopic excision of prostatic utricle cyst was done successfully. Postoperative period was uneventful and the patient was discharged in satisfactory condition. Laparoscopic excision of prostatic utricle cyst is technically challenging but with acceptable complications and good surgical results

    Perception of dental practitioners in and around Kanpur city towards forensic odontology: a cross sectional study

    Background: In the present era, forensic odontology has expanded as one of the most remarkable and commendable branches of Forensic Sciences. Through forensic odontology, a dentist plays a very important role in crime investigation of any type. The main objective of the present study was to evaluate the knowledge, percipience and practical perception of forensic odontology among the dental practitioners in and around Kanpur city.Methods: A cross-sectional study was conducted from Jan-Mar 2019 among 207 dental practitioners in and around Kanpur city including 143 BDS and 64 MDS through a questionnaire proforma. The proforma consisted of 20 questions prepared on the topic of forensic Odontology and role of dentist in the field of forensic Odontology.Results: In this study, nearly 70% of dental practitioners were aware of the role of dentist in forensics, and around 60% of dental practitioners maintain dental records with recording of personal data and clinical findings being the most frequently used method. In the present study most of the dental practitioners were not aware of significance of chelioscopy (63%) and rugoscopy (66%) in field of forensic Odontology. Nearly 70% of dentist accepted the fact that their level of knowledge regarding forensic dentistry is inadequate and nearly 40% of them were not confident in giving any opinion regarding the same.Conclusions: This study shows that although there is an adequate awareness of role of dentist in forensic Odontology, but there is lack of good knowledge, confidence and practical approach of the dental practitioners towards forensic Odontology which may be due to lack of training, experience, exposure in field of forensics. Thus, the need of the hour lies in updating the knowledge and also developing interest of the dental practitioners regarding forensic Odontology

    Stigma and self-esteem in patients of bronchial asthma

    Asthma is a debilitating chronic disease that renders individuals physically as well as mentally sick. The perception of stigmatization further leads to inappropriate control of asthma resulting in a bidirectional relationship. A prospective, cross-sectional study was conducted in a tertiary care centre of semi urban area of north India during 2021-2022. Three hundred cases of bronchial asthma and 50 healthy controls were enrolled. The asthma control test (ACT) was used to evaluate the control of asthma. Stigma and self-esteem were measured with the use of 28 item self-stigma scale, Rosenberg self-esteem scale, and stigma-related social problem scale (SPSS). The impact of sociodemographic and clinical profiles on stigma and self-esteem was evaluated. Correlation of different scales with each other was done along with. The mean age of asthmatic patients was 47.41±15.507 years with 56% being females; 49.3% of patients were well controlled and 36.3% were partly controlled. Positive subscales of 28 item self-stigma scale, Rosenberg self-esteem scale and SSPS showed statistically significant differences between cases and controls (p<0.001). ACT significantly correlated with 28 item self-stigma scale, Rosenberg self-esteem scale and SSPS, and all three stigma and self-esteem scales correlated significantly with each other

    Psychosocial Impact of Pandemic and State Imposed Lockdown on Caregivers of Patients Presenting with Respiratory Complaints Mimicking COVID-19: A Short-term Follow-up Study

    Introduction: Pandemics and subsequent lockdowns affect mental health of different subgroups of populations. In Coronavirus Disease-2019 (COVID-19), caregivers of those patients who have respiratory complaints is one such subgroup which is more vulnerable to disturbances in mental health, because of the fear that their patient’s respiratory symptoms could be because of COVID-19. Aim: To assess the psychosocial impact of COVID-19 and subsequent state imposed lockdown on the caregivers of patients presenting with respiratory complaints and also to evaluate the effect of relaxation of lockdown after following-up them over a period of time. Materials and Methods: This prospective observational study was conducted in the Department of Pulmonary Medicine, Government Medical College, Patiala, Punjab, India (tertiary care institute), from April 2020 to June 2020. Baseline assessment was done using socio-demographic proforma, lockdown related questionnaire {3 domains, summed as total score (lockdown)}, COVID-19 related questionnaire {total score (COVID-19)} and General Health Questionnaire-12-Hindi version (GHQ-12). Reassessment was done twice i.e., at 11-15 days and 41-45 days after relaxation of lockdown. Quality Of Life (QOL) at first and second follow-up versus prelockdown times (score A and C) and first follow-up versus unlockdown (score B) was also noted. Analysis was conducted using Statistical Package for Social Sciences (IBM, SPSS)version 22.0. Results: Total 65 caregivers were enrolled in the study. Mean age of the participants was 40.2±11.812 years with maximum caregivers 25 (41.7%) aged between 31-40 years. Majority (83.3%) were men. Psychological distress was experienced in 50% of caregivers at baseline and 23.7% caregivers at first follow-up (p-value=0.001). Worry for COVID-19 (p-value=0.035), Domain 1 scores (p-value <0.001), Domain 2 scores (p-value=0.003), Domain 3 scores (p-value=0.001), and Total score lockdown (p-value <0.001) decreased significantly at first follow-up. Mean C score was significantly better than mean A score (p-value <0.001). Baseline psychological distress was significantly more in those with worry for COVID-19 (p-value=0.018), poorer scores of domains 1 (p-value=0.005), domains 2 (p-value <0.001), domains 3 (p-value <0.001), total score (lockdown) (p-value <0.001) and total score (COVID-19) (p-value=0.010). Follow-up psychological distress was more in those with “worry for COVID-19” (p-value <0.001), negative thoughts (p-value=0.001), poorer follow-up scores of three domains, total score (lockdown), mean A, B and C scores (p-value <0.001). Conclusion: Caregivers experienced extreme levels of psychological distress, which decreased, but persisted even after relaxation in lockdown

    Cross Modal Distillation for Flood Extent Mapping

    The increasing intensity and frequency of floods is one of the many consequences of our changing climate. In this work, we explore ML techniques that improve the flood detection module of an operational early flood warning system. Our method exploits an unlabelled dataset of paired multi-spectral and Synthetic Aperture Radar (SAR) imagery to reduce the labeling requirements of a purely supervised learning method. Prior works have used unlabelled data by creating weak labels out of them. However, from our experiments we noticed that such a model still ends up learning the label mistakes in those weak labels. Motivated by knowledge distillation and semi supervised learning, we explore the use of a teacher to train a student with the help of a small hand labelled dataset and a large unlabelled dataset. Unlike the conventional self distillation setup, we propose a cross modal distillation framework that transfers supervision from a teacher trained on richer modality (multi-spectral images) to a student model trained on SAR imagery. The trained models are then tested on the Sen1Floods11 dataset. Our model outperforms the Sen1Floods11 baseline model trained on the weak labeled SAR imagery by an absolute margin of 6.53% Intersection-over-Union (IoU) on the test split
