    G-band Spectral Synthesis in Solar Magnetic Concentrations

    Narrow band imaging in the G-band is commonly used to trace the small magnetic field concentrations of the Sun, although the mechanism that makes them bright has remained unclear. We carry out LTE syntheses of the G-band in an assorted set of semi-empirical model magnetic concentrations. The syntheses include all CH lines as well as the main atomic lines within the band-pass. The model atmospheres produce bright G-band spectra having many properties in common with the observed G-band bright points. In particular, the contrast referred to the quiet Sun is about twice the contrast in continuum wavelengths. The agreement with observations does not depend on the specificities of the model atmosphere, rather it holds from single fluxtubes to MIcro-Structured Magnetic Atmospheres. However, the agreement requires that the real G-band bright points are not spatially resolved, even in the best observations. Since the predicted G-band intensities exceed by far the observed values, we foresee a notable increase of contrast of the G-band images upon improvement of the angular resolution. According to the LTE modeling, the G-band spectrum emerges from the deep photosphere that produces the continuum. Our syntheses also predict solar magnetic concentrations showing up in continuum images but not in the G-band . Finally, we have examined the importance of the CH photo-dissociation in setting the amount of G-band absorption.     Attitudes of Germans towards distributive issues in the German health system

    Social health care systems are inevitably confronted with the scarcity of resources and the resulting distributional challenges. Since prioritization implies distributional effects, decisions on respective rules should take citizens’ preferences into account. Thus, knowledge about citizens’ attitudes and preferences regarding different distributional issues implied by the type of financing health care is necessary to judge the public acceptance of a health system. In this study we concentrate on two distributive issues in the German health system: First, we analyse the acceptance of prioritizing decisions concerning the treatment of certain patient groups, in this case patients who all need a heart operation. Here we focus on the fact that a patient is strong smoker or a non-smoker, the criteria of age or the fact that a patient has or does not have young children. Second, we investigate Germans’ opinions towards income dependent health services. The results reveal strong effects of individuals’ attitudes regarding general aspects of the health system on priorities, e.g. that individuals behaving health demanding should not be preferred. In addition, experiences of limited access to health services are found to have a strong influence on citizens’ attitudes, too. Finally, decisions about different prioritization criteria are found to be not independent.

    Surgical treatment for colorectal cancer: Analysis of the influence of an enhanced recovery programme on long-term oncological outcomes-a study protocol for a prospective, multicentre, observational cohort study

    Introduction The evidence currently available from enhanced recovery after surgery (ERAS) programmes concerns their benefits in the immediate postoperative period, but there is still very little evidence as to whether their correct implementation benefits patients in the long term. The working hypothesis here is that, due to the lower response to surgical aggression and lower rates of postoperative complications, ERAS protocols can reduce colorectal cancer-related mortality. The main objective of this study is to analyse the impact of an ERAS programme for colorectal cancer on 5-year survival. As secondary objectives, we propose to analyse the weight of each of the predefined items in the oncological results as well as the quality of life. Methods and analysis A multicentre prospective cohort study was conducted in patients older than 18 years of age who are scheduled to undergo surgery for colorectal cancer. The study involved 12 hospitals with an implemented enhanced recovery protocol according to the guidelines published by the Spanish National Health Service. The intervention group includes patients with a minimum implementation level of 70%, and the control group includes those who fail to reach this level. Compliance will be studied using 18 key performance indicators, and the results will be analysed using cancer survival indicators, including overall survival, cancer-specific survival and relapse-free survival. The time to recurrence, perioperative morbidity and mortality, hospital stay and quality of life will also be studied, the latter using the validated EuroQol Five questionnaire. The propensity index method will be used to create comparable treatment and control groups, and a multivariate regression will be used to study each variable. The Kaplan-Meier estimator will be used to estimate survival and the log-rank test to make comparisons. A p value of less than 0.05 (two-tailed) will be considered to be significant. Ethics and dissemination Ethical approval for this study was obtained from the Aragon Ethical Committee (C.P.-C.I. PI20/086) on 4 March 2020. The findings of this study will be submitted to peer-reviewed journals (BMJ Open, JAMA Surgery, Annals of Surgery, British Journal of Surgery). Abstracts will be submitted to relevant national and international meetings. Trial registration number NCT04305314

    HIVToolbox, an Integrated Web Application for Investigating HIV

    Many bioinformatic databases and applications focus on a limited domain of knowledge federating links to information in other databases. This segregated data structure likely limits our ability to investigate and understand complex biological systems. To facilitate research, therefore, we have built HIVToolbox, which integrates much of the knowledge about HIV proteins and allows virologists and structural biologists to access sequence, structure, and functional relationships in an intuitive web application. HIV-1 integrase protein was used as a case study to show the utility of this application. We show how data integration facilitates identification of new questions and hypotheses much more rapid and convenient than current approaches using isolated repositories. Several new hypotheses for integrase were created as an example, and we experimentally confirmed a predicted CK2 phosphorylation site. Weblink: [http://hivtoolbox.bio-toolkit.com

    EPIdemiology of Surgery-Associated Acute Kidney Injury (EPIS-AKI) : Study protocol for a multicentre, observational trial

    More than 300 million surgical procedures are performed each year. Acute kidney injury (AKI) is a common complication after major surgery and is associated with adverse short-term and long-term outcomes. However, there is a large variation in the incidence of reported AKI rates. The establishment of an accurate epidemiology of surgery-associated AKI is important for healthcare policy, quality initiatives, clinical trials, as well as for improving guidelines. The objective of the Epidemiology of Surgery-associated Acute Kidney Injury (EPIS-AKI) trial is to prospectively evaluate the epidemiology of AKI after major surgery using the latest Kidney Disease: Improving Global Outcomes (KDIGO) consensus definition of AKI. EPIS-AKI is an international prospective, observational, multicentre cohort study including 10 000 patients undergoing major surgery who are subsequently admitted to the ICU or a similar high dependency unit. The primary endpoint is the incidence of AKI within 72 hours after surgery according to the KDIGO criteria. Secondary endpoints include use of renal replacement therapy (RRT), mortality during ICU and hospital stay, length of ICU and hospital stay and major adverse kidney events (combined endpoint consisting of persistent renal dysfunction, RRT and mortality) at day 90. Further, we will evaluate preoperative and intraoperative risk factors affecting the incidence of postoperative AKI. In an add-on analysis, we will assess urinary biomarkers for early detection of AKI. EPIS-AKI has been approved by the leading Ethics Committee of the Medical Council North Rhine-Westphalia, of the Westphalian Wilhelms-University Münster and the corresponding Ethics Committee at each participating site. Results will be disseminated widely and published in peer-reviewed journals, presented at conferences and used to design further AKI-related trials. Trial registration number NCT04165369

    El niño y el juego : estudio de caso de los niños participantes en la juegoteca de la Fundación Abrir la Puerta

    Resumen: El presente trabajo tiene como objetivo conocer, describir y analizar la influencia que tiene el juego en los niños, a través de la observación de las actividades lúdicas realizadas en una de las juegotecas de la Fundación Abrir la Puerta. Se utilizaron como marco analítico conceptos provenientes de la sociología de la infancia y de la teoría de la socialización, como así también definiciones de juego y sus funciones. Para la presente investigación se eligió el estudio de caso intrínseco. Se pudo describir la forma de trabajo en la que la Fundación Abrir la Puerta lleva a cabo sus actividades para una mejor compresión del análisis de los datos. Además, se conocieron las características y datos sociodemográficos de la zona en donde funciona la juegoteca. El análisis de los datos recolectados se realizó a través de observaciones, evaluaciones de los ludoeducadores a cargo del grupo y entrevistas en profundidad. Se crearon tres dimensiones en el proceso del jugar: actitudes, relaciones sociales y habilidades sociales en donde se incluyeron diferentes temáticas como la aceptación de reglas, el interés por las actividades presentadas, el poder escuchar al otro, el trabajo en equipo, la tolerancia a la frustración entre otras. Como cierre del análisis de los datos, se presentaron ejemplos de casos específicos en los que se pueden observar los cambios paulatinos que se produjeron en los niños desde el primer encuentro al último, como proceso de socialización que abarcan y sintetizan las dimensiones presentadas en el proceso del jugar. Como resultado se pudo observar la incidencia que tiene el juego en los niños en el desarrollo de actitudes, relaciones y habilidades sociales, es decir, el proceso que realizan los niños cuando se los enfrenta al juego y el desarrollo que paulatinamente se efectúa en pos de la adquisición y desarrollo de las habilidades sociales.Abstract: The objective of this research is to know, describe and analyze the influence that play has in children through the observation of the recreational activities carried out in one of the toy libraries of Fundación Abrir la Puerta. As an analytical framework, concepts about childhood sociology and socialization theory were used as well as definitions about play and its functions. As regards methodology, the case study method was chosen in the present study. There is a description of the way that Fundación Abrir la Puerta carried on its activities in order to have a better understanding of the data analysis. Also, there is a description of the area as well as sociodemographic data of the area where the toy library was set up. Data analysis was based on observations, evaluations done by the coordinators in charge of the group and personal interviews. Three dimensions of the process of play were created: attitudes, social relations and social skills. Different themes were included in these dimensions. Some of them are: accepting rules and accepting the activities proposed by the coordinators, being able to listen to others, team work and tolerance to frustration. As a closure of the data analysis, some examples were presented and specially analyzed where changes in the children attending the toy library were detected from the first day to the last one. As a result, it could be observed the incidence and importance that play has in children as regards the development of attitudes, social relations and skills, that is to say, the process that children carry on when they are playing and the development that gradually is achieved in the acquisition and development of attitudes, social relationships and skills

    REDfly: An Integrated Knowledgebase for Insect Regulatory Genomics

    We provide here an updated description of the REDfly (Regulatory Element Database for Fly) database of transcriptional regulatory elements, a unique resource that provides regulatory annotation for the genome of Drosophila and other insects. The genomic sequences regulating insect gene expression—transcriptional cis-regulatory modules (CRMs, e.g., “enhancers”) and transcription factor binding sites (TFBSs)—are not currently curated by any other major database resources. However, knowledge of such sequences is important, as CRMs play critical roles with respect to disease as well as normal development, phenotypic variation, and evolution. Characterized CRMs also provide useful tools for both basic and applied research, including developing methods for insect control. REDfly, which is the most detailed existing platform for metazoan regulatory-element annotation, includes over 40,000 experimentally verified CRMs and TFBSs along with their DNA sequences, their associated genes, and the expression patterns they direct. Here, we briefly describe REDfly’s contents and data model, with an emphasis on the new features implemented since 2020. We then provide an illustrated walk-through of several common REDfly search use cases

    Purpose: To evaluate the presence of SARS-CoV-2 specific IgA and IgG antibodies in tears of unvaccinated and anti-COVID-19 vaccinated subjects with previous history of SARS-CoV-2 infection. To compare results in tears with those in saliva and serum and correlate with clinical data and vaccination regimens. Methods: Cross-sectional study including subjects with a previous history of SARS-CoV-2 infection, both unvaccinated and vaccinated against COVID-19. Three samples were collected: tears, saliva and serum. IgA and IgG antibodies against S-1 protein of SARS-CoV-2 were analyzed with a semi-quantitative ELISA. Results: Thirty subjects, mean age 36.4 ± 10, males 13/30 (43.3%) with history of mild SARS-CoV-2 infection were included. 13/30 (43.3%) subjects had received a 2-dose regimen and 13/30 (43.3%) a 3-dose regimen of anti-COVID-19 vaccine, 4/30 (13.3%) subjects were unvaccinated. All the participants with full anti-COVID-19 vaccination (2-or 3-doses) presented detectable anti-S1 specific IgA in all 3 biofluids, tears, saliva and serum. Among unvaccinated subjects, specific IgA was detected in 3/4 subjects in tears and saliva, whereas IgG was not detected. Considering IgA and IgG antibodies titers, no differences were observed between the 2- and 3-dose vaccination regimen. Conclusions: SARS-CoV-2-specific IgA and IgG antibodies were detected in tears after mild COVID-19, highlighting the role of the ocular surface as a first line of defense against infection. Most naturally infected unvaccinated individuals exhibit long-term specific IgA in tears and saliva. Hybrid immunization (natural infection plus vaccination) appears to enhance mucosal and systemic IgG responses. However, no differences were observed between the 2- and 3-dose vaccination schedule.This research did not receive any specific grant from funding agencies in the public, commercial, or not-for-profit sectors
