    Stroke volume and cardiac output non-invasive monitoring based on brachial oscillometry-derived pulse contour analysis: Explanatory variables and reference intervals throughout life (3–88 years)

    Background: Non-invasive assessment of stroke volume (SV), cardiac output (CO) and cardiac index (CI) has shown to be useful for the evaluation, diagnosis and/or management of different clinical conditions. Through pulse contour analysis (PCA) cuff-based oscillometric devices would enable obtaining ambulatory operator-independent non-invasive hemodynamic monitoring. There are no reference intervals (RIs), when considered as a continuum in childhood, adolescence and adult life, for PCA-derived SV [SV(PCA)], CO [CO(PCA)] and CI [CI(PCA)]. The aim of the study were to analyze the associations of SV(PCA), CO(PCA) and CI(PCA) with demographic, anthropometric, cardiovascular risk factors (CVRFs) and hemodynamic parameters, and to define RIs and percentile curves for SV(PCA), CO(PCA) and CI(PCA), considering the variables that should be considered when expressing them. Methods: In 1449 healthy subjects (3–88 years) SV(PCA), CO(PCA) and CI(PCA) were non-invasively obtained (Mobil-O-Graph; Germany). Analysis: associations between subject characteristics and SV(PCA), CO(PCA) and CI(PCA) levels (correlations; regression models); RIs and percentiles for SV(PCA), CO(PCA) and CI(PCA) (parametric methods; fractional polynomials). Results: Sex, age, and heart rate would be explanatory variables for SV, CO, and CI levels. SV levels were also examined by body height, while body surface area (BSA) contributing to evaluation of CO and CI. CVRFs exposure did not contribute to independently explain the values of the dependent variables. SV, CO and CI levels were partially explained by the oscillometric-derived signal quality. RIs and percentiles were defined. Conclusions: Reference intervals and percentile for SV(PCA), CO(PCA) and CI(PCA), were defined for subjects from 3–88 years of age, results are expressed according to sex, age, heart rate, body height and/or BSA.Fil: Zócalo, Yanina.     In recent years, applied didactics spaces have been incorporated into the different scientific areas that make up the various fields of knowledge as a practice that not only aims to demonstrate the work developed by the professions in these disciplines, but also as a way to adding new content to the curriculum of primary and secondary education students with new practical strategies that are not always accessible to educational centres. It is within the framework of these new dynamics that the project Didacteca. Materiales para la formación científica, a project that aims to develop new teaching materials to understands the Archaeology and History disciplines. The project has been funded by the specific plans of the University of Granada (FIDO Plans). In order to evaluate the materials generated during the project, the first didactic experiences have been developed. The results allow us to be optimistic about the benefits of their application, in this case, in the Secondary Education and, of course, as a teaching experience for the teachers who taught the courses.En los últimos años los espacios de didáctica aplicada se han venido percolando en las distintas disciplinas científicas que componen los diversos campos de conocimiento como una práctica, que no sólo pretenden demostrar los trabajos que desarrollan las profesiones de dichas disciplinas, sino como una forma de agregar nuevos contenidos al curriculum de los estudiantes de educación primaria y secundaria con nuevas estrategias prácticas a las que no siempre tienen acceso los centros educativos. Es en el marco de estas nuevas dinámicas en las que se genera el proyecto Didacteca. Materiales para la formación científica, un proyecto que tiene por objeto el desarrollo de nuevos materiales docentes para la comprensión de la Arqueología y la Historia, promovido por los planes específicos que en los últimos años financia la Universidad de Granada (Planes FIDO). Con objeto de evaluar los materiales desarrollados en este proyecto se han realizado las primeras experiencias didácticas, las cuales nos permiten ser optimistas en relación al beneficio que supone su aplicación, en este caso, en la educación secundaria y, cómo no, como experiencia docente para el propio profesorado que impartió los cursos

    Abstract: Internet-delivered unified protocol for transdiagnostic treatment of emotional disorders in adolescents (iUP-A): Web application and study protocol for a randomized controlled trial. A characteristic of transdiagnostic treatment is that it targets simultaneously the core symptoms and underlying vulnerabilities of several disorders. The UP-A is a new empirically supported cognitive-behavioral therapy (CBT) protocol developed by Ehrenreich-May’s group for the transdiagnostic treatment approach of emotional disorders (i.e., depression, anxiety and related disorders) in adolescents (Ehrenreich-May et al., 2018), to be applied in a face-to-face format. Internet-delivered psychological treatments represent an emerging model to improve access to evidence-based CBT. A first aim of this study is to describe the development of a web application named Aprende a Manejar Tus Emociones (AMTE; Learn to Manage Your Emotions), designed to implement the UP-A in order to be delivered via internet (iUP-A). A second objective is to test the efficacy of the iUP-A for the treatment of depression and anxiety disorders compared to waiting list, in a randomized controlled trial. We investigated changes in a broad range of primary, secondary and transdiagnostic outcome measures.Keywords: Transdiagnostic; UP-A; iUP-A; internet-delivered CBT; AMTE; anxiety; depression.Resumen: Una característica del tratamiento transdiagnóstico consiste en que aborda simultáneamente los síntomas principales y las vulnerabilidades que subyacen a varios trastornos. El UP-A es un nuevo protocolo de terapia cognitivo-conductual (TCC) basada en la evidencia desarrollado por el grupo de Ehrenreich-May para un tratamiento transdiagnóstico de los trastornos emocionales (i.e., depresión, ansiedad y trastornos relacionados) en adolescentes (Ehrenreich-May et al., 2018), para ser aplicado en un formato cara-a-cara. Los tratamientos psicológicos dispensados a través de internet representan un modelo emergente para mejorar el acceso a la TCC basada en la evidencia. Un primer objetivo del presente estudio consiste en describir el desarrollo de una aplicación web denominada Aprende a Manejar Tus Emociones (AMTE), diseñada para implementar el UP-A con objeto de ser dispensado a través de internet (iUP-A). Un segundo objetivo es probar la eficacia del iUP-A para el tratamiento de la depresión y los trastornos de ansiedad en comparación con una lista de espera, en un ensayo controlado aleatorizado.     Cost-effectiveness analysis for joint pain treatment in patients with osteoarthritis treated at the Instituto Mexicano del Seguro Social (IMSS): Comparison of nonsteroidal anti-inflammatory drugs (NSAIDs) vs. cyclooxygenase-2 selective inhibitors

    <p>Abstract</p> <p>Background</p> <p>Osteoarthritis (OA) is one of the main causes of disability worldwide, especially in persons >55 years of age. Currently, controversy remains about the best therapeutic alternative for this disease when evaluated from a cost-effectiveness viewpoint. For Social Security Institutions in developing countries, it is very important to assess what drugs may decrease the subsequent use of medical care resources, considering their adverse events that are known to have a significant increase in medical care costs of patients with OA. Three treatment alternatives were compared: celecoxib (200 mg twice daily), non-selective NSAIDs (naproxen, 500 mg twice daily; diclofenac, 100 mg twice daily; and piroxicam, 20 mg/day) and acetaminophen, 1000 mg twice daily. The aim of this study was to identify the most cost-effective first-choice pharmacological treatment for the control of joint pain secondary to OA in patients treated at the Instituto Mexicano del Seguro Social (IMSS).</p> <p>Methods</p> <p>A cost-effectiveness assessment was carried out. A systematic review of the literature was performed to obtain transition probabilities. In order to evaluate analysis robustness, one-way and probabilistic sensitivity analyses were conducted. Estimations were done for a 6-month period.</p> <p>Results</p> <p>Treatment demonstrating the best cost-effectiveness results [lowest cost-effectiveness ratio 17.5pesos/patient(17.5 pesos/patient (1.75 USD)] was celecoxib. According to the one-way sensitivity analysis, celecoxib would need to markedly decrease its effectiveness in order for it to not be the optimal treatment option. In the probabilistic analysis, both in the construction of the acceptability curves and in the estimation of net economic benefits, the most cost-effective option was celecoxib.</p> <p>Conclusion</p> <p>From a Mexican institutional perspective and probably in other Social Security Institutions in similar developing countries, the most cost-effective option for treatment of knee and/or hip OA would be celecoxib.</p

    Administración telemática del Protocolo Unificado para el tratamiento transdiagnóstico de los trastornos emocionales en adolescentes (tUP-A): un estudio de casos

    The aim of this study was to provide preliminary qualitative and quantitative evidence on the clinical utility of the UP-A delivered via telehealth (tUP-A). Three adolescents diagnosed with emotional disorders received the tUP-A through weekly individual therapy sessions via video call. Changes in several quantitative and qualitative variables were examined with the adolescents, the parents and the therapists acting as informants. The results based on pre-, post-treatment and follow-up (at three and six months) show a significant reduction in anxiety and depressive symptoms in the three adolescents according to the different sources of information. Implications of the UP-A delivered via telehealth to improve access to evidence-based therapy for emotional disorders in the adolescent population are discussed.El objetivo de este estudio fue proporcionar evidencia preliminar, de tipo cualitativo y cuantitativo, sobre la utilidad clínica del UP-A aplicado de forma telemática (tUP-A). Tres adolescentes diagnosticados de trastornos emocionales recibieron el tUP-A a través de sesiones semanales individuales por videollamada. Se examinaron los cambios en diversas variables cuantitativas y cualitativas actuando como informantes los adolescentes, los padres y los propios terapeutas. Los resultados basados en los momentos pre-/post-tratamiento y seguimiento (a los 3 y 6 meses) muestran una importante reducción de la sintomatología ansioso-depresiva en los tres adolescentes de acuerdo con las diferentes fuentes de información. Se discuten las implicaciones de administrar telemáticamente el UP-A para mejorar el acceso a la terapia de los trastornos emocionales basada en la evidencia en población adolescente. &nbsp

    New Insights about How to Make an Intervention in Children and Adolescents with Metabolic Syndrome: Diet, Exercise vs. Changes in Body Composition. A Systematic Review of RCT

    Objective: To record which interventions produce the greatest variations in body composition in patients ≤19 years old with metabolic syndrome (MS). Method: search dates between 2005 and 2017 in peer reviewed journals, following the PRISMA method (Preferred Reporting Items for Systematic reviews and Meta-Analyses). The selection criteria were: diagnostic for MS or at least a criterion for diagnosis; randomized clinical trials, ≤19 years of age; intervention programs that use diet and/or exercise as a tool (interventions showing an interest in body composition). Results: 1781 clinical trials were identified under these criteria but only 0.51% were included. The most frequent characteristics of the selected clinical trials were that they used multidisciplinary interventions and were carried out in America. The most utilized parameters were BMI (body mass index) in kg/m2 and BW (body weight) in kg. Conclusions: Most of the clinical trials included had been diagnosed through at least 2 diagnostic criteria for MS. Multidisciplinary interventions obtained greater changes in body composition in patients with MS. This change was especially prevalent in the combinations of dietary interventions and physical exercise. It is proposed to follow the guidelines proposed for patients who are overweight, obese, or have diabetes type 2, and extrapolate these strategies as recommendations for future clinical trials designed for patients with MS.This research is partially supported by Generalitat Valenciana, Grant GV/2017/177

    Diseño del proceso de selección de personal, de la empresa Killis White S.A.S en el año 2022 en Colombia

    Durante este proyecto abordaremos la necesidad especifica de la compañía Killis White S.A.S con el fin de dar solución a la misma, la problemática identificada es la ausencia de un modelo de selección de personal el cual permita a la empresa tener la tranquilidad de que cuenta con el personal más calificado para cada área de la empresa, lo cual no solo aportara a las metas de la empresa sino al desarrollo de cada integrante del equipo logrando tener un adecuado clima laboral, disminuyendo la rotación de personal y por ende reduciendo los costos administrativos que se generan con la alta rotación de personal. Para resolver esta problemática encontrada realizaremos un diagnóstico con el fin de identificar las principales necesidades que tiene la empresa en la selección de personal, realizaremos un modelo guía enfocado en el adecuado reclutamiento y selección de personal, como método adicional se diseñara fichas de evaluación para los diferentes cargos que requiera la compañía. Nos apoyaremos en un enfoque mixto (cuantitativo y cualitativo) lo cual nos permitirá unos resultados más adecuados en el proceso de investigación. El resultado esperado con todo este proceso es llegar a diseñar el proceso de selección de personal para la empresa Killis White S.A.S el cual pueda ser aplicado por la empresa aportando de una manera significativa a los objetivos de la compañía e impactando de manera positiva a los nuevos colaboradores de la empresa al encontrarse con un adecuado proceso de selección que ayudan al cumplimiento de sus metas personales al tiempo que aportan a los diferentes propósitos de la compañíaDuring this project we will address the specific need of the company Killis White S.A.S in order to solve it, the problem identified is the absence of a personnel selection model which allows the company to have the peace of mind that it has the more qualified personnel for each area of the company, which will not only contribute to the goals of the company but also to the development of each member of the team, achieving an adequate work environment, reducing staff turnover and therefore reducing the administrative costs that are generated by high staff turnover. To solve this problem found, we will carry out a diagnosis in order to identify the main needs that the company has in the selection of personnel, we will carry out a guiding model focused on the adequate recruitment and selection of personnel, as an additional method, evaluation sheets will be designed for the different charges required by the company. We will rely on a mixed approach (quantitative and qualitative) which will allow us more adequate results in the research process. The expected result with all this process is to design the personnel selection process for the company Killis White S.A.S which can be applied by the company contributing in a significant way to the objectives of the company and positively impacting the new collaborators of the company by finding an adequate selection process that helps to fulfill their personal goals while contributing to the different purposes of the company
