    Circular dichroism mode splitting and bounds to its enhancement with cavity-plasmon-polaritons

    Geometrical chirality is a widespread phenomenon that has fundamental implications for discriminating enantiomers of biomolecules. In order to enhance the chiral response of the medium, it has been suggested to couple chiral molecules to resonant optical cavities in order to enhance the circular dichroism (CD) signal at the resonant frequency of the cavity. Here, we studied a distinctly different regime of chiral light-matter interaction, wherein the CD signal of a chiral medium splits into polaritonic modes by reaching the strong coupling regime with an optical microcavity. Specifically, we show that by strongly coupling chiral plasmonic nanoparticles to a non-chiral Fabry-P\ue9rot microcavity one can imprint the mode splitting on the CD spectrum of the coupled system and thereby effectively shift the initial chiral resonance to a different energy. We first examined the effect with the use of analytical transfer-matrix method as well as numerical finite-difference time-domain (FDTD) simulations. Furthermore, we confirmed the validity of theoretical predictions in a proof-of-principle experiment involving chiral plasmonic nanoparticles coupled to a Fabry-P\ue9rot microcavity

    The Emergency Surgery Frailty Index (EmSFI) in Elderly Patients with Acute Appendicitis: An External Validation of Prognostic Score

    Background: Identification of reliable risk-stratification tools is critical for surgical decision making, particularly in frail and elderly. The aim of the study is to validate the Emergency Surgery Frailty Index (EmSFI), in over 65 years old patients operated on for acute appendicitis. Methods: An observational study was conducted enrolling elderly patients with diagnosis of acute appendicitis who underwent emergency appendicectomy or right colectomy, between 2016 and 2021. All patients were treated according to the last SIFIPAC/WSES/SICG/SIMEU guidelines. Results: Overall, 61 patients were analyzed. Complication rate was higher for patients in the second EmSFI risk Class. Moreover, ROC analyses identified 3 as the best cutoff value in predicting risk of adverse postoperative events. Complication rate was higher in oldest elderly patients—over 80 years—(42.9 vs 22.5%; p 0.05) and was mainly related to medical complications (42.9 vs 12.5%, p 0.007). However, intestinal obstruction, peri-appendicular abscess on preoperative CT, peritonitis and a longer duration of surgery are related with increased risk of complications in the group of patients under 80 years. Conclusion: The EmSFI score results a valid prognostic marker for frailty status, and it may support the surgeon in emergency setting for acute appendicitis. Patients aged 80 years or older have a higher risk of complications, independent from those factors which relate to increased morbidity in younger elderly patients. Age alone is not a reliable indicator of the real surgical risk, but it must encourage the adoption of multidisciplinary collaborative models of care for this group of patients. © 2023, The Author(s)

    Solar harvesting based on perfect absorbing all-dielectric nanoresonators on a mirror

    The high-index all-dielectric nanoantenna system is a platform recently used for multiple applications, from metalenses to light management. These systems usually exhibit low absorption/scattering ratios and are not efficient photon harvesters. Nevertheless, by exploiting far-field interference, all-dielectric nanostructures can be engineered to achieve near-perfect absorption in specific wavelength ranges. Here, we propose - based on electrodynamics simulations - that a metasurface composed of an array of hydrogenated amorphous silicon nanoparticles on a mirror can achieve nearly complete light absorption close to the bandgap. We apply this concept to a realistic device, predicting a boost of optical performance of thin-film solar cells made of such nanostructures. In the proposed device, high-index dielectric nanoparticles act not only as nanoatennas able to concentrate light but also as the solar cell active medium, contacted at its top and bottom by transparent electrodes. By optimization of the exact geometrical parameters, we predict a system that could achieve initial conversion efficiency values well beyond 9% - using only the equivalent of a 75-nm thick active material. The device absorption enhancement is 50% compared to an unstructured device in the 400 nm - 550 nm range and more than 300% in the 650 nm - 700 nm spectral region. We demonstrate that such large values are related to the metasurface properties and to the perfect absorption mechanism.     Giant Sigmoid Diverticulum: A Rare Presentation of a Common Pathology

    Although colonic diverticulum is a common disease, affecting about 35% of patients above the age of 60, giant sigmoid diverticulum is an uncommon variant of which only relatively few cases have been described in the literature. We report on our experience with a patient affected by giant sigmoid diverticulum who was treated with diverticulectomy. Resection of the diverticulum is a safe surgical procedure, provided that the colon section close to the lesion presents no sign of flogosis or diverticula; in addition, recurrences are not reported after 6-year follow-up

    Reflectance anisotropy spectroscopy of magnetite (110) surfaces

    Reflectance anisotropy spectroscopy (RAS) has been used to measure the optical anisotropies of bulk and thin-film Fe3O4(110) surfaces. The spectra indicate that small shifts in energy of the optical transitions, associated with anisotropic strain or electric field gradients caused by the (110) surface termination or a native oxide layer, are responsible for the strong signal observed. The RAS response was then measured as a function of temperature. A distinct change in the RAS line-shape amplitude was observed in the spectral range from 0.8 to 1.6 eV for temperatures below the Verwey transition of the crystal. Finally, thin-film magnetite was grown by molecular beam epitaxy on MgO(110) substrates. Changes in the RAS spectra were found for different film thickness, suggesting that RAS can be used to monitor the growth of magnetite (110) films in situ. The thickness dependence of the RAS is discussed in terms of various models for the origin of the RAS signal

    High index dielectric metasurfaces and colloidal solutions: From fabrication to application

    High index dielectric nanoparticles and meta-materials have been proposed for many different applications, including light harvesting, sensing and metalenses. However, widespread utilization in practice also requires large-scale fabrication methods able to produce homogeneous structures with engineered optical properties in a cost effective manner. Here, it is presented a facile fabrication method for silicon nanoparticles which is scalable to 4-inch wafers and can produce a wide range of nanoparticle shapes on demand. We also show that the fabricated nanoparticles can be detached from their support using a simple substrate removal technique and then transferred to colloidal suspension. We will finally discuss some uses of the fabricated systems. For the metasurfaces, we will demonstrate complete absorption due to far field interference effects. For the nanoparticles colloids we will show the possibility of realizing an intrinsically chiral structure composed of a low-loss dielectric resonator and we will study optical trapping phenomena for different particle sizes and shapes

    Overview of the Lost Meteorites of Antarctica field campaigns

    The Lost Meteorites of Antarctica project was the first UK-led Antarctic meteorite recovery expedition. The project has successfully confirmed two new high-density meteorite stranding zones in the Hutchison Icefield and Outer Recovery Icefields areas and investigated the geology of three previously unvisited Antarctic nunataks (Turner Nunatak, Pillinger Nunatak, Halliday Nunatak). The project undertook meteorite searching on the ice surface via skidoo reconnaissance and systematic searching and developed a novel pulse induction metal detection system to search for englacial iron-rich meteorites trapped within the upper one meter of ice. In total, 121 meteorites have been recovered from the ice surface searching activities, which are now curated in the United Kingdom at the Natural History Museum London and are available for scientific analysis

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries
