Impacto sobre las faunas de corales y braquiópodos ligado al Evento Zlichoviense superior en la sección de La Vid de Gordón (Zona Cantábrica)
Diversos estudios en varias secciones de la Formación La Pedrosa (Grupo La Vid,
Dominio Astur-Leonés, Zona Cantábrica), realizados por García-Alcalde (1997) y Truyols-
Massoni (2006) entre otros, han analizado los cambios faunísticos ligados al evento
Zlichoviense superior (UZE), coincidente con la brusca aparición de niveles ricos en pizarras.
Entre las evidencias faunísticas de este evento destaca el relevo en las faunas de braquiópodos
y la aparición súbita de una peculiar fauna de corales. El objetivo de este trabajo es
caracterizar estos cambios faunísticos y su relación con las variaciones ambientales ligadas a
dicho evento. Para ello se ha estudiado el estratotipo del Grupo La Vid en la localidad de La
Vid de Gordón (valle del Bernesga, provincia de León)
Efficacy of rifabutin-based triple therapy as second-line treatment to eradicate helicobacter pylori infection
<p>Abstract</p> <p>Background</p> <p>Rifabutin has been found to be effective in multi-resistant patients after various treatment cycles for Helicobacter pylori (HP) infection, but it has not been analysed as a second-line treatment. Therefore, we seek to compare the effectiveness of a treatment regimen including rifabutin versus conventional quadruple therapy (QT).</p> <p>Methods</p> <p>Open clinical trial, randomised and multi-centre, of two treatment protocols: A) Conventional regime -QT- (omeprazole 20 mg bid, bismuth citrate 120 mg qid, tetracycline 500 mg qid and metronidazole 500 mg tid); B) Experimental one -OAR- (omeprazole 20 mg bid, amoxicillin 1 gr bid, and rifabutin 150 mg bid), both taken orally for 7 days, in patients with HP infection for whom first-line treatment had failed. Eradication was determined by Urea Breath Test (UBT). Safety was determined by the adverse events.</p> <p>Results</p> <p>99 patients were randomised, QT, n = 54; OAR, n = 45. The two groups were homogeneous. In 8 cases, treatment was suspended (6 in QT and 2 in OAR). The eradication achieved, analysed by ITT, was for QT, 38 cases (70.4%), and for OAR, 20 cases (44.4%); p = 0.009, OR = 1.58. Of the cases analysed PP, QT were 77.1%; OAR, 46.5%; p = 0.002. Adverse effects were described in 64% of the QT patients and in 44% of the OAR patients (p = 0.04).</p> <p>Conclusion</p> <p>A 7-day rifabutin-based triple therapy associated to amoxicillin and omeprazole at standard dose was not found to be effective as a second-line rescue therapy. The problem with quadruple therapy lies in the adverse side effects it provokes. We believe the search should continue for alternatives that are more comfortably administered and that are at least as effective, but with fewer adverse side effects.</p> <p>Trial Registration</p> <p>Current Controlled Trials ISRCTN81058036</p
Natural History of MYH7-Related Dilated Cardiomyopathy
BACKGROUND Variants in myosin heavy chain 7 (MYH7) are responsible for disease in 1% to 5% of patients with dilated cardiomyopathy (DCM); however, the clinical characteristics and natural history of MYH7-related DCM are poorly described. OBJECTIVES We sought to determine the phenotype and prognosis of MYH7-related DCM. We also evaluated the influence of variant location on phenotypic expression. METHODS We studied clinical data from 147 individuals with DCM-causing MYH7 variants (47.6% female; 35.6 +/- 19.2 years) recruited from 29 international centers. RESULTS At initial evaluation, 106 (72.1%) patients had DCM (left ventricular ejection fraction: 34.5% +/- 11.7%). Median follow-up was 4.5 years (IQR: 1.7-8.0 years), and 23.7% of carriers who were initially phenotype-negative developed DCM. Phenotypic expression by 40 and 60 years was 46% and 88%, respectively, with 18 patients (16%) first diagnosed at <18 years of age. Thirty-six percent of patients with DCM met imaging criteria for LV noncompaction. During follow-up, 28% showed left ventricular reverse remodeling. Incidence of adverse cardiac events among patients with DCM at 5 years was 11.6%, with 5 (4.6%) deaths caused by end-stage heart failure (ESHF) and 5 patients (4.6%) requiring heart transplantation. The major ventricular arrhythmia rate was low (1.0% and 2.1% at 5 years in patients with DCM and in those with LVEF of <= 35%, respectively). ESHF and major ventricular arrhythmia were significantly lower compared with LMNA-related DCM and similar to DCM caused by TTN truncating variants. CONCLUSIONS MYH7-related DCM is characterized by early age of onset, high phenotypic expression, low left ventricular reverse remodeling, and frequent progression to ESHF. Durante los últimos tres años hemos apostado por el fortalecimiento de esta
estrategia generando nuevas modalidades, diversos escenarios para el desarrollo
del voluntariado, capacitando a los 19 líderes y los voluntarios en las sedes,
siempre bajo la profunda convicción de que el mundo se puede cambiar cuando
mucha gente pequeña, en lugares pequeños, haciendo cosas pequeñas, logran
tocar la vida de las personas que más lo necesitan
Global variations in diabetes mellitus based on fasting glucose and haemogloblin A1c
Fasting plasma glucose (FPG) and haemoglobin A1c (HbA1c) are both used to diagnose
diabetes, but may identify different people as having diabetes. We used data from 117
population-based studies and quantified, in different world regions, the prevalence of
diagnosed diabetes, and whether those who were previously undiagnosed and detected
as having diabetes in survey screening had elevated FPG, HbA1c, or both. We developed
prediction equations for estimating the probability that a person without previously
diagnosed diabetes, and at a specific level of FPG, had elevated HbA1c, and vice versa.
The age-standardised proportion of diabetes that was previously undiagnosed, and
detected in survey screening, ranged from 30% in the high-income western region to 66%
in south Asia. Among those with screen-detected diabetes with either test, the agestandardised
proportion who had elevated levels of both FPG and HbA1c was 29-39%
across regions; the remainder had discordant elevation of FPG or HbA1c. In most low- and
middle-income regions, isolated elevated HbA1c more common than isolated elevated
FPG. In these regions, the use of FPG alone may delay diabetes diagnosis and
underestimate diabetes prevalence. Our prediction equations help allocate finite
resources for measuring HbA1c to reduce the global gap in diabetes diagnosis and
surveillance.peer-reviewe
Características clínicas y epidemiológicas diferenciales de las poblaciones inmigrante y autóctona con infección por el VIH en el área sanitaria de Cartagena. Periodo 1998-2008. Differential clinical and epidemiological features of the native and immigrant populations with HIV infection in the health area of Cartagena. Period 1998-2008
Objetivos: Describir y comparar las características epidemiológicas, clínicas e inmuno-virológicas de la población inmigrante y autóctona VIH+.
Pacientes y Métodos: Estudio de cohortes retrospectivo que analiza y compara las principales variables clínicas, epidemiológicas, inmuno-virológicas y los principales aspectos relacionados con el TARGA en la población inmigrante y autóctona. Resultados y Conclusiones: El modo de transmisión del VIH más prevalente en inmigrantes fue la vía heterosexual (70.5%) y en autóctonos el consumo de drogas intravenosas (54.7%). No se hallaron diferencias clínicas e inmuno-virológicas significativas en ambas poblaciones tanto en la visita basal como al final del seguimiento clínico. La adherencia al TARGA fue buena (>95%) en inmigrantes y autóctonos (80.2% vs 76.2%, p=N.S). La frecuencia de defunciones por SIDA fue similar en inmigrantes y autóctonos (33.3% vs 31.2%) y las pérdidas de seguimiento de causa no filiada fueron mayores en sujetos inmigrantes (45% vs 36%).
Abstract:
Objectives: To describe and compare the epidemiological, clinical and immuno-virological features in immigrant and native HIV + populations.
Patients and Methods: A retrospective cohort study that analyzes and compares the main clinical, epidemiological, immuno-virological aspects and the main issues that relate to HAART in the immigrant and native population.
Results and Conclusions: The mode of transmission of HIV most prevalent in immigrants was heterosexual (70.5%) and indigenous injecting drug use (54.7%). We found no significant differences in immuno-virological parameters in both populations in the baseline visit and at the end of clinical follow-up. Adherence to HAART was good (> 95%) in immigrant and native HIV+ patients (80.2% vs 76.2%), without significant differences. The frequency of AIDS deaths was similar for immigrant and native patients (33.3% vs 31.2%) and the losses to follow-up due to unknown causes were higher in immigrant patients (45% vs 36%)
A study on user experience of smart glasses for higher education students
This paper reports a study on the User experience (UX) of STEM (e.g., Science, Technology, Engineering, and Mathematics) students using smart glasses and an interactive Augmented Reality (AR) educational app. The results show that the AR app provides a good UX, despite the presence of some form factor issues. Students’ usability ratings for both the AR app and the smart glasses, are positively correlated with the students’ perceived learning. The results of this study can be used as a guideline to design and develop further immersive e-learning technologies
Studying the User Experience of an Educational AR-Based App for Smart Glasses
The steady evolution of wearable technology—along with augmented reality (AR)—has enabled important advances in teaching and learning. We present STEM Education, an interactive AR application for smart glasses. We evaluate the app’s user experience (UX) with undergraduate STEM (Science, Technology, Engineering, and Mathematics) students. More specifically, we focus on the students’ overall satisfaction, degree of technological acceptance, mental workload, and emotional response. Our observations suggest that the app improves the student’s learning quality, while promoting high levels of engagement and satisfaction. We discuss these findings and their implications in the design of similar technology for educational contexts
