    Vitamin D Status of Clinical Practice Populations at Higher Latitudes: Analysis and Applications

    Background: Inadequate levels of vitamin D (VTD) throughout the life cycle from the fetal stage to adulthood have been correlated with elevated risk for assorted health afflictions. The purpose of this study was to ascertain VTD status and associated determinants in three clinical practice populationsliving in Edmonton, Alberta, Canada -a locale with latitude of 53°30\u27N, wheresun exposure from October through March is often inadequate to generate sufficient vitamin D. Methods: To determine VTD status, 1,433 patients from three independent medical offices in Edmonton had levels drawn for 25(OH)D as part of their medical assessment between Jun 2001 and Mar 2007.     HIV pre-exposure prophylaxis (PrEP) was approved by Health Canada in 2016. Currently in Ontario, those under the age of 25 or aged 65 years or older can access PrEP free of charge through public drug coverage programs. This study assessed the cost-effectiveness of providing public coverage for PrEP to all high-risk Ontario MSM. A Markov cohort model was built from the perspective of the Ontario Ministry of Health and Long-Term Care (MOH) using a lifetime time horizon. Health states included HIV positive, HIV negative, and Dead. Transition probabilities, health state utilities and HIV treatment costs were sourced from published literature. Costs of HIV prevention were estimated through a micro-costing approach. HIV infection risk was estimated using the HIRI-MSM screening tool. Offering PrEP to all high-risk MSM through the Ontario MOH is dominant to current coverage circumstances due to a reduction in costly HIV infections.M.Sc

    Abstract: "The problem of retrofit design of multiproduct batch plants is considered in which the optimal addition of equipment to an existing plant must be determined in view of changes in the product demands. In order to circumvent the combinatorial problem of having to analyze many alternatives the problem is formulated as a mixed-integer nonlinear program (MINLP) and solved with the outer-approximation algorithm of Duran and Grossmann. By using suitable variable transformations and approximations, the global optimum solution is guaranteed. The proposed MINLP model is also extended to the case when time-varying forecasts for product demands are given. Numerical examples are presented.
