    Intrauterine growth restriction and risk of diverse forms of kidney disease during the first 50 years of life

    Background and objectives: Previous studies have shown that individuals with low birth weight (LBW) or small for gestational age (SGA) have higher risk of kidney failure. This study investigates birth-related exposures and risk of CKD and other kidney diagnoses. Design, setting, participant, & measurements: The Medical Birth Registry of Norway has registered extensive medical data on all births in Norway since 1967. The Norwegian Patient Registry has registered diagnostic codes for all admissions and outpatient visits to Norwegian hospitals since 2008. Data from these registries were linked, and risk of CKD and other groups of kidney disease were analyzed using logistic regression statistics. LBW (below the tenth percentile), SGA (birth weight below the tenth percentile for gestational age), and preterm birth (<37 weeks) were analyzed as exposures. Results: A total of 2,663,010 individuals were included. After a mean follow-up of 26 years (maximum 50 years), 4495 had been diagnosed with CKD and 12,818 had been diagnosed with other groups of kidney disease. LBW was associated with an odds ratio (OR) for CKD of 1.72 (95% confidence interval [95% CI], 1.60 to 1.90), SGA with an OR of 1.79 (95% CI, 1.65 to 1.94), and preterm birth with an OR of 1.48 (95% CI, 1.33 to 1.66). Analyses using diagnosis of CKD at stages 3–5 as end point showed similar results. Results were similar for men and women. We analyzed adjusted ORs for other groups of kidney disease and found that LBW was associated with an adjusted OR of 1.44 (95% CI, 1.33 to 1.56) for acute kidney disease, 1.24 (95% CI, 1.14 to 1.36) for GN, 1.35 (95% CI, 1.17 to 1.56) for cystic kidney disease, and 1.15 (95% CI, 1.06 to 1.25) for kidney disease resulting from kidney or urinary tract malformations. Conclusions: LBW, SGA, and preterm birth are associated with higher risk of CKD in the first 50 years of life. Risk of other groups of kidney disease was less pronounced.acceptedVersio

Perang Salib merupakan sebuah peristiwa peperangan yang terjadi dalam kurun waktu kurang lebih dua ratus tahun (1096-1292 M), yang mempertemukan Umat Islam dan Kristen Eropa demi mendapatkan kekuasaan atas wilayah Baitul Maqdis (Yerusalem). Dalam serangkaian Perang Salib ini telah memunculkan peran seorang tokoh yang cukup berjasa besar peranannya dalam mempertahankan Baitul Maqdis (Yerusalem), dia adalah seorang raja dan pahlawan umat Islam yaitu Shalahuddin Al Ayyubi. Sesuai dengan latar belakang yang telah di ungkapkan di atas, di sini penulis mencoba menguraikan rumusan masalah mengenai latar belakang perang salib dan sejauh mana peran dan perjuangan Shalahuddin Al Ayyubi dalam peristiwa Perang Salib, yang dirumuskan ke dalam pembahasan terkait latar belakang terjadinya perang salib serta peran dan perjuangan yang dimunculkan oleh Shalahuddin Al Ayyubi dalam peristiwa Perang Salib tersebut. Peristiwa ini memfokuskan pembahasannya pada peran penting seorang Shalahuddin dalam mempertahankan Baitul Maqdis (Yerusalem) saat Perang Salib III (1096 s/d 1198 M) sehingga tetap di dalam kekuasaan Umat Muslim. Untuk metode penelitian ini penulis menggunakan pendekatan Library Research dengan menggunakan metode heuristik, di mana setelah sumber-sumber informasi terkait diperoleh, berikutnya dilakukan kritik dan verivikasi kemudian dibuat alur yang logis dengan penafsiran-penafsiran agar apa yang penulis tulis memiliki alur cerita sejarah yang runtut dengan metode historiografi. Dari hasil penelitian tersebut dapat ditarik sebuah kesimpulan bahwa Perang Salib terjadi kurang lebih selama 200 tahun yang memperebutkan wilayah (Baitul Maqdis/Yerusalem) yang diangap suci oleh 3 agama besar (Yahudi, Kristen dan Islam) karena faktor Agama, faktor Politik (Kekuasaan), dan faktor Ekonomi. Shalahuddin Al Ayyubi menjadi tokoh yang paling dikenal dalam peristiwa Perang Salib ini, peran dan perjuangannya yang cukup berarti demi mempertahankan Baitul Maqdis (Yerusalem) dari serangan Pasukan Salib Eropa.     Intrauterine growth restriction, preterm birth and risk of end-stage renal disease during the first 50 years of life

    Background: Low birth weight (LBW) is associated with a higher risk of end-stage renal disease (ESRD). The relative impacts of absolute birth weight, birth weight in relation to gestational age and preterm birth are, however, uncertain. Methods: The Medical Birth Registry of Norway has since 1967 recorded data on all births. All patients with ESRD since 1980 have been registered in the Norwegian Renal Registry. Data from these registries were linked. All individuals registered in the Medical Birth Registry were included and the development of ESRD was used as endpoint in Cox regression statistics. LBW and LBW for gestational age [small for gestational age (SGA)] according to the 10th percentiles were used as the main predictor variables. Results: Of the 2 679 967 included subjects, 1181 developed ESRD. Compared with subjects without LBW, subjects with LBW had an adjusted hazard ratio (aHR) for ESRD of 1.61 (1.38–1.98). SGA had an aHR of 1.44 (1.22– 1.70). Further analyses showed that as compared with subjects who had none of the risk factors LBW, SGA and preterm birth, subjects with one risk factor had an aHR of 1.05 (0.84–1.31), subjects with two risk factors had an aHR of 1.67 (1.40–1.98) and subjects with three risk factors had an aHR of 2.96 (1.84–4.76). Conclusions: We conclude that LBW was associated with increased risk for ESRD during the first 50 years. Our analyses add to previous knowledge showing that only subjects with at least two of the risk factors LBW, SGA or preterm birth have increased risk.publishedVersio

    Systemic inflammation early after kidney transplantation is associated with long-term graft loss: a cohort study

    Background: Early graft loss following kidney transplantation is mainly a result of acute rejection or surgical complications, while long-term kidney allograft loss is more complex. We examined the association between systemic inflammation early after kidney transplantation and long-term graft loss, as well as correlations between systemic inflammation scores and inflammatory findings in biopsies 6 weeks and 1 year after kidney transplantation. Methods: We measured 21 inflammatory biomarkers 10 weeks after transplantation in 699 patients who were transplanted between 2009 and 2012 at Oslo University Hospital, Rikshospitalet, Norway. Low-grade inflammation was assessed with predefined inflammation scores based on specific biomarkers: one overall inflammation score and five pathway-specific scores. Surveillance or indication biopsies were performed in all patients 6 weeks after transplantation. The scores were tested in Cox regression models. Results: Median follow-up time was 9.1 years (interquartile range 7.6-10.7 years). During the study period, there were 84 (12.2%) death-censored graft losses. The overall inflammation score was associated with long-term kidney graft loss both when assessed as a continuous variable (hazard ratio 1.03, 95% CI 1.01-1.06, P = 0.005) and as a categorical variable (4th quartile: hazard ratio 3.19, 95% CI 1.43-7.10, P = 0.005). In the pathway-specific analyses, fibrogenesis activity and vascular inflammation stood out. The vascular inflammation score was associated with inflammation in biopsies 6 weeks and 1 year after transplantation, while the fibrinogenesis score was associated with interstitial fibrosis and tubular atrophy. Conclusion: In conclusion, a systemic inflammatory environment early after kidney transplantation was associated with biopsy-confirmed kidney graft pathology and long-term kidney graft loss.     Intervening on health literacy by knowledge translation processes in kidney transplantation: A feasibility study

    Background Patients awaiting kidney transplantation need to be prepared ahead of the upcoming transplantation by developing targeted pre- and post-transplant knowledge. On this background, we designed a new health literacy intervention, including a film and a counselling session, based on motivational interviewing for dialysis patients provided by dialysis nurses. Aim To explore patients' and nurses' experiences of the feasibility and acceptability of the intervention, focusing on the patient as a prepared knowledge actor. Design An explorative qualitative study. Participants and Methods Data included in-depth interviews with nine patients and three nurses who participated in the intervention. The interviews were audiotaped and analysed following Kvale and Brinkmann's method for thematic data analysis. Findings Three main themes were identified: a different kind of health intervention stimulating new insight; a challenging kind of health conversation and changed relationships and increased security. Conclusions Both the patients and the nurses had an overall positive attitude toward the intervention, providing a kind of dialogue to prepare dialysis patients going through kidney transplantation. The nurses found the MI methodology to be challenging. When introducing a comprehensive communication method like MI, potential training and supervision needs for the nurses must be addressed.publishedVersio

    Temporal trends in the quality of deceased donor kidneys and kidney transplant outcomes in Europe:an analysis by the ERA-EDTA Registry

    BACKGROUND: We investigated 10-year trends in deceased donor kidney quality expressed as the kidney donor risk index (KDRI) and subsequent effects on survival outcomes in a European transplant population. METHODS: Time trends in the crude and standardized KDRI between 2005 and 2015 by recipient age, sex, diabetic status and country were examined in 24 177 adult kidney transplant recipients in seven European countries. We determined 5-year patient and graft survival probabilities and the risk of death and graft loss by transplant cohort (Cohort 1: 2005–06, Cohort 2: 2007–08, Cohort 3: 2009–10) and KDRI quintile. RESULTS: The median crude KDRI increased by 1.3% annually, from 1.31 [interquartile range (IQR) 1.08–1.63] in 2005 to 1.47 (IQR 1.16–1.90) in 2015. This increase, i.e. lower kidney quality, was driven predominantly by increases in donor age, hypertension and donation after circulatory death. With time, the gap between the median standardized KDRI in the youngest (18–44 years) and oldest (>65 years) recipients widened. There was no difference in the median standardized KDRI by recipient sex. The median standardized KDRI was highest in Austria, the Netherlands and the Basque Country (Spain). Within each transplant cohort, the 5-year patient and graft survival probability were higher for the lowest KDRIs. There was no difference in the patient and graft survival outcomes across transplant cohorts, however, over time the survival probabilities for the highest KDRIs improved. CONCLUSIONS: The overall quality of deceased donor kidneys transplanted between 2005 and 2015 has decreased and varies between age groups and countries. Overall patient and graft outcomes remain unchanged

We included patients aged ≥75-84 years (termed older adults) receiving renal replacement therapy in thirteen European countries between 2005 and 2014. Country differences and time trends in access to, and allocation of kidney transplants were examined. Survival outcomes were determined by Cox regression analyses. Between 2005 and 2014, 1392 older adult patients received 1406 transplants. Access to kidney transplantation varied from ~0% (Slovenia, Greece and Denmark) to ~4% (Norway and various Spanish regions) of all older adult dialysis patients, and overall increased from 0.3% (2005) to 0.9% (2014). Allocation of kidney transplants to older adults overall increased from 0.8% (2005) to 3.2% (2014). Seven-year unadjusted patient and graft survival probabilities were 49.1% (95% confidence interval, 95% CI: 43.6; 54.4) and 41.7% (95% CI: 36.5; 46.8), respectively, with a temporal trend towards improved survival outcomes. In conclusion, in the European dialysis population aged ≥75-84 years access to kidney transplantation is low, and allocation of kidney transplants remains a rare event. Though both are increasing with time and vary considerably between countries. The trend towards improved survival outcomes is encouraging.     Familial Contributions to the Association Between Low Birth Weight and Risk of CKD in Adult Life

    Introduction Previous studies have revealed that individuals with low birth weight (LBW) have higher risk of chronic kidney disease (CKD) and that LBW and CKD cluster in families. This study investigates how familial factors affect the association between birth-related risk markers and risk of CKD. Methods The Medical Birth Registry (MBR) of Norway has registered all births in Norway since 1967. Sibling data were available through the Norwegian Population Registry. The Norwegian Patient Registry has registered diagnostic codes for all admissions and outpatient visits to Norwegian hospitals since 2008. Data from these registries were linked. Risk of CKD according to whether the individual himself or at least one of his siblings had LBW was analyzed using logistic regression statistics. Results Of 1,847,565 individuals, 3336 had been diagnosed with CKD. Compared with individuals without LBW and no siblings with LBW, individuals without LBW but who had a sibling with LBW had adjusted odds ratio (aOR) of 1.33 (1.19–1.49), those with LBW but no siblings with LBW had aOR of 1.74 (1.55–1.95), and those with LBW and a sibling with LBW had aOR of 1.77 (1.54–2.04) for CKD. Similar results were found for LBW for gestational age, but preterm birth revealed weaker associations. Conclusion Individuals who have a sibling with LBW have an increased risk of CKD later in life, and individuals who themselves have LBW have an even higher risk. Our findings suggest that there are familial contributions to the nephron endowment in utero hypothesis
