    The late-life developmental process of self-transcendence shapes eldersā€™ perspectives on self, others, the nature of this world, and of a dimension beyond the here and now. This qualitative pilot study evaluated the Psychoeducational Approach to Transcendence and Health (PATH) Program, a psychoeducational intervention to promote self-transcendence and well-being in community-dwelling women at a senior center. The intervention involved eight weekly group sessions using group processes, mindfulness practices, creative experiences, and independent at-home practice. The findings supported the underlying theory-based structure and content of the intervention and indicated the intervention may empower elders to attend to self-care, develop acceptance, and learn new skills associated with health and well-being, thus merits further study. Based on insights gained from facilitatorsā€™ and participantsā€™ experiences and perceptions, the intervention will be revised and strengthened

    Self-transcendence changes how older adults perceive themselves, their relationships with others, the material world, and the metaphysical or spiritual dimension. It is associated with multiple indicators of well-being. The purpose of this pilot study (N = 20) was to examine the feasibility and effectiveness of a psychoeducational intervention to increase self-transcendence and well-being of older adults. Data were analyzed using generalized estimating equations. All variables trended in the directions hypothesized. Self-transcendence increased in the intervention group and decreased in the control group but not significantly. The group Ɨ time interaction for life satisfaction was significant (z = 2.89, p = .004). This feasibility study supports further investigation to assess the effectiveness of the intervention in a larger sample

    The research reported here was part of a large study of the impact of age, disability, race and sex on the teaching profession in England. The basic question asked in this research was how do these factors interact with career aspirations and achievements of classteachers, promoted teachers and headteachers? There were three different data sources: a large postal survey drawn from diverse geographic regions across England with over 2000 respondents; faceā€toā€face individual interviews with over 100 teachers in 18 case study schools from across all of the main regions of England; discussions with special interest groups of teachers. Not surprisingly, the answer to the above question was complex. Nonetheless, the paper's conclusion highlights some of the noteworthy themes across this broad sample of teachers from primary, secondary and special schools

    Mild Cognitive Impairment, Incidence, Progression, and Reversion: Findings from a Community-based Cohort of Elderly African Americans

    Objective To examine the long-term outcomes of community-based elderly African Americans by following their transitions from normal cognition to mild cognitive impairment (MCI), and to dementia. Methods Participants were from the community-based Indianapolis Dementia Project. A total of 4104 African Americans were enrolled in 1992 or 2001 and followed until 2009 with regularly scheduled evaluation of cognitive assessment. A two-stage sampling was used at each evaluation to select individuals for extensive clinical assessment following the results of stage one cognitive testing. Age and gender specific incidence, progression and reversion rates for MCI were derived using the person-year method in a dynamic cohort and predicted probabilities from weighted multinomial logistic models of transitional probabilities among normal cognition, MCI and dementia. Results Annual overall incidence rate for MCI is 5.6% (95% CI: 4.6ā€“6.6%). Annual progression rate from MCI to dementia is 5.9% (95% CI: 5.3ā€“6.5%) and annual reversion rate from MCI to normal is 18.6% (95% CI: 16.7ā€“20.4%). Both MCI incidence rates and MCI to dementia progression rates increase with age, while reversion rates from MCI to normal decrease with age. Conclusion MCI progression to dementia is much more frequent in the older age groups than in the younger participants where reversion to normal cognition is more common. Future research is needed to determine factors related to the heterogeneous outcomes in MCI individuals

    A randomised, feasibility trial of an Exercise and Nutrition-based Rehabilitation programme (ENeRgy) in people with cancer

The progressive decline in function and nutritional in this population would support an approach that targets these factors. This trial aimed to assess the feasibility of an exercise and nutrition based rehabilitation programme in people with incurable cancer. Methods: We randomized community dwelling adults with incurable cancer to either a personalized exercise and nutrition based programme (experimental arm) or standard care (control arm) for 8 weeks. Endpoints included feasibility, quality of life, physical activity (step count), and body weight. Qualitative and health economic analyses were also included. Results: Fortyā€five patients were recruited (23 experimental arm, 22 control arm). There were 26 men (58%), and the median age was 78 years (IQR 69ā€“84). At baseline, the median BMI was 26 kg/m2 (IQR: 22ā€“29), and median weight loss in the previous 6 months was 5% (IQR: āˆ’12% to 0%). Adherence to the experimental arm was >80% in 16/21 (76%) patients. There was no statistically significant difference in the following between trial arms: step count āˆ’ median % change from baseline to endpoint, per trial arm (experimental āˆ’18.5% [IQR: āˆ’61 to 65], control 5% [IQR: āˆ’32 to 50], P = 0.548); weight āˆ’ median % change from baseline to endpoint, per trial arm (experimental 1%[IQR: āˆ’3 to 3], control āˆ’0.5% [IQR: āˆ’3 to 1], P = 0.184); overall quality of life āˆ’ median % change from baseline to endpoint, per trial arm (experimental 0% [IQR: āˆ’20 to 19], control 0% [IQR: āˆ’23 to 33], P = 0.846). Qualitative findings observed themes of capability, opportunity, and motivation amongst patients in the experimental arm. The mean incremental cost of the experimental arm versus control was Ā£ā€319.51 [CI āˆ’7593.53 to 6581.91], suggesting the experimental arm was less costly. Conclusions: An exercise and nutritional rehabilitation intervention is feasible and has potential benefits for people with incurable cancer. A larger trial is now warranted to test the efficacy of this approach.12pubpub

    Eelgrass donor sites: potentially overlooked impacts of restoration in Puget Sound

    Eelgrass (Zostera marina) is an important habitat in the Salish Sea and restoration efforts are being undertaken around the region to increase eelgrass abundance and resilience. Eelgrass restoration is typically performed by transplanting whole shoots or dispersing viable seeds collected from reproductive shoots to a site. Most of the restoration efforts in the Pacific Northwest utilize whole shoots harvested from donor meadows and transplanted into restoration areas, but little work has been done to look at the impacts of the harvest on the donor stock. In response to the lack of existing data for Puget Sound, Washington Department of Natural Resources and Pacific Northwest National Laboratoryā€™s Marine Sciences Laboratory conducted a controlled harvest experiment in two regions of the Salish Sea at sites associated with ongoing restoration activities. These meadows were harvested under different pressure (i.e., different percentage of plants taken from 0 to 50%) using traditional harvesting techniques. The meadows were then monitored for two years for changes in density. The results indicated that the eelgrass meadows were surprisingly resilient to all levels of harvest under ideal conditions and in small harvest areas. Interpretation and implications of these results will be discussed, as well as potential considerations for choosing potential donor sites for future restoration efforts

    MicroRNAs in cardiac arrhythmia: DNA sequence variation of MiR-1 and MiR-133A in long QT syndrome.

    Long QT syndrome (LQTS) is a genetic cardiac condition associated with prolonged ventricular repolarization, primarily a result of perturbations in cardiac ion channels, which predisposes individuals to life-threatening arrhythmias. Using DNA screening and sequencing methods, over 700 different LQTS-causing mutations have been identified in 13 genes worldwide. Despite this, the genetic cause of 30-50% of LQTS is presently unknown. MicroRNAs (miRNAs) are small (āˆ¼ 22 nucleotides) noncoding RNAs which post-transcriptionally regulate gene expression by binding complementary sequences within messenger RNAs (mRNAs). The human genome encodes over 1800 miRNAs, which target about 60% of human genes. Consequently, miRNAs are likely to regulate many complex processes in the body, indeed aberrant expression of various miRNA species has been implicated in numerous disease states, including cardiovascular diseases. MiR-1 and MiR-133A are the most abundant miRNAs in the heart and have both been reported to regulate cardiac ion channels. We hypothesized that, as a consequence of their role in regulating cardiac ion channels, genetic variation in the genes which encode MiR-1 and MiR-133A might explain some cases of LQTS. Four miRNA genes (miR-1-1, miR-1-2, miR-133a-1 and miR-133a-2), which encode MiR-1 and MiR-133A, were sequenced in 125 LQTS probands. No genetic variants were identified in miR-1-1 or miR-133a-1; but in miR-1-2 we identified a single substitution (n.100A> G) and in miR-133a-2 we identified two substitutions (n.-19G> A and n.98C> T). None of the variants affect the mature miRNA products. Our findings indicate that sequence variants of miR-1-1, miR-1-2, miR-133a-1 and miR-133a-2 are not a cause of LQTS in this cohort

    Rhyolitic tephra horizons in northwestern Europe and Iceland from the AD 700s-800s: a potential alternative for dating first human impact

    The distribution and geochemistry of four rhyolitic tephra horizons from Iceland dated to the ad 700sā€“800s is assessed. These include the rhyolitic phase of the LandnĆ”m tephra (ad 870s), the ad 860 layer, a previously unrecorded tephra called the GA4ā€“85 layer (c. ad 700ā€“800) and the TjĆÆrnuvĆ­k tephra (c. ad 800s). The ad 860 and GA4ā€“85 layers were first found in peat bogs in north Ireland. They are here correlated with equivalent horizons on Iceland which were found below the LandnĆ”m tephra (c. ad 870s). This time period is considered important in the North Atlantic region, because it coincides with a phase of human settlement in Iceland and the Faroe Islands. The establishment of a detailed tephrochronology may provide a tool for exact dating of sediment successions and sediments associated with archaeological excavations. Caution must be taken especially on Iceland where the LandnĆ”m tephra is often used for dating archaeological sites. This investigation show that several rhyolitic tephra horizons occur close in time to the LandnĆ”m tephra, and that mistakes can be made if detailed geochemical analyses are not carried out, especially in areas which are distal to the source of the LandnĆ”m tephra (the Veidivƶtn and Torfajƶkull volcanic systems, southern Iceland)

    Pseudoachondroplasia and Multiple Epiphyseal Dysplasia: A 7-Year Comprehensive Analysis of the Known Disease Genes Identify Novel and Recurrent Mutations and Provides an Accurate Assessment of Their Relative Contribution

    Pseudoachondroplasia (PSACH) and multiple epiphyseal dysplasia (MED) are relatively common skeletal dysplasias resulting in short-limbed dwarfism, joint pain, and stiffness. PSACH and the largest proportion of autosomal dominant MED (AD-MED) results from mutations in cartilage oligomeric matrix protein (COMP); however, AD-MED is genetically heterogenous and can also result from mutations in matrilin-3 (MATN3) and type IX collagen (COL9A1, COL9A2, and COL9A3). In contrast, autosomal recessive MED (rMED) appears to result exclusively from mutations in sulphate transporter solute carrier family 26 (SLC26A2). The diagnosis of PSACH and MED can be difficult for the nonexpert due to various complications and similarities with other related diseases and often mutation analysis is requested to either confirm or exclude the diagnosis. Since 2003, the European Skeletal Dysplasia Network (ESDN) has used an on-line review system to efficiently diagnose cases referred to the network prior to mutation analysis. In this study, we present the molecular findings in 130 patients referred to ESDN, which includes the identification of novel and recurrent mutations in over 100 patients. Furthermore, this study provides the first indication of the relative contribution of each gene and confirms that they account for the majority of PSACH and MED. Hum Mutat 33:144ā€“157, 2012.
