    Amavata is a chronic disease. Due to tremendous pain in Amavata, patients daily life get disturbed. Also the morbidity of disease disturbs routine work of the patient. The sign and symptoms of Amavata is nearer to resemble with Rheumatoid arthritis in modern science, R.A is more than just arthritis. The prevalence of R.A. is approximately 0.8% of the population (range 0.3 to 2.1%), woman are affected three times more often than man. Family studies indicate a genetic predisposition. Now a days, due to changed life style and improper dietary habits like pattern of spicy food, irregular timing of meals, over eating etc. causes Agnimandya and it leads to production of Ama. All these faulty eating habits are almost always accompanied with faulty vihara like improper or over exercise, late night parties, suppression of natural urges, excessive traveling etc. these are causes of vitiation of Vata. This vitiated Vata carries Ama to the all over the body especially the Kapha sthanas and produces symptoms like Sandhi shotha, Sandhi shula, Stabdhata1 (stiffness) and other systemic sign and symptoms. This dreadful disease is called as Amavata.Tritiya Alambushadi Churn provided highly significant relief in pain (73.41%). joint score (59.26%), tenderness (73.91%), stiffness (76.22%), swelling of joints (78.53%), local temperature (77.61%) and improvement in grip strength (73.61%),functional activity (79.66%). i.e. overall total improvement is 73.63%. On the basis of these observations, administration of Tritiya Alambushadi Churna is effective for the management of Amavata

    ABSTRACT: H1N1 influenza is an emerging threat that is life threatening to pregnant women in the third trimester. Pregnant women are a high-risk group for morbidity and mortality from influenza (H1N1). During the current pandemic of H1N1 influenza, few cases of H1N1 have been reported in pregnancy. We report a case of H1N1 influenza in a 23-year old female with 28 weeks of gestation, who developed acute respiratory distress syndrome, required mechanical ventilation and eventually recovered.     In Ayurveda context, eleven types Kshudrakushtha are described and Vicharchika (Eczema) is one of them. In ancient text, there described many lines of treatmentsof Vicharchika (Eczema) like leech application, local application and many oral medicines. Gandhak Rasayanais one of the oral medicines described for many skin diseases.For this study, 30 patients of Shushka vicharchika (Dry Eczema)having complaint of Kandu, Daha, Ruja, Sparshaasahatva, Tvakvaivarnya were selected.Vicharchika (Eczema)is Kaphajavyadhi and Raktadushti is also observed. Gandhak Rasayana (Bharat Bhaishajya Ratnakar 1153) mainly acts on skin, blood and it is Rasayana in action. So it more or less acts on all Doshas and Dhatus. Gandhak isushnaveeryatmaka and Katu rasa Vipaki so acts as best Kaphghna and Kledaghna. During making of Gandhak Rasayana different twelve Drawyabhavanas are given to Shudhagandhak. These drugs helps to destroy the Samprapti of Vichrchika due to its Ushnavirya and Katukashay rasa. Some Bhavanadravyas has property of Madhur rasa, Sheet Virya due to which they acts as Pittaghna and Raktaprasadana and helps to cure Vicharchika (Eczema).Daha, Kandu, Ruja and Sparshaasahatva were significantly reduced. Also significant relief in Tvak-vaivarnyata was observed

    Machine-learning assisted optimized power-saving techniques for battery-operated Wi-Fi Internet of Things (IoT) devices are provided. These techniques use various IoT device information including Manufacturer Usage Description (MUD) data along with a clustering algorithm to provide an automatic and dynamic computation of Target Wake Time (TWT) power save schedule for the devices. These techniques may be used by next generation 802.11ax access points to more efficiently schedule the target wake time for the 802.11ax supported IoT devices, to thereby prolong the lifetime of battery operated IoT devices connected to the 802.11ac access points

    Evaluation of external ventricular drain complications and the use of a procedure-targeted image-guidance system

It can be done either in the operating room or at the bedside to relieve increased intracranial pressure or deliver medication. At the bedside, the procedure is normally performed freehand, with the occasional use of a Ghajar guide for guidance support. In the operating room, ventriculostomy may be performed with an image-guidance system, whether optical or electromagnetic. The most common complications of ventriculostomy are hemorrhage and infection. It is unclear whether catheter placement accuracy and the number of passes of the catheter for each placement are correlated with ventriculostomy complications. Our goals are 1) to evaluate the current state of practice, including complications of ventriculostomy, and 2) to evaluate a targeted image guidance system for use with ventriculostomy - the Smart Stylet. To address these goals, an Institutional Review Board-approved retrospective cross-sectional study was conducted at the Brigham and Women's Hospital (BWH) to characterize the practice of external ventricular drain placements using data from the patient electronic medical record. Post-procedure catheter location was measured on post-procedure CT and MRI imaging studies. Most cases were performed in the operating room and the operative reports provided all procedure-related information. Microbiology reports were collected within a four-week interval following catheter placements to evaluate presence of invading pathogens. All imaging studies, microbiology reports, and operative reports were reviewed manually. The rest of the medical records were not reviewed and, therefore, cerebrospinal fluid leak and shunt malfunction were not evaluated. Catheter placement accuracy and the numbers of passes for each placement were assessed. We evaluated whether these metrics were associated with the occurrence of procedure complications. A procedure-targeted image guidance system in development stage, the Smart Stylet, was implemented for use on a ventricular phantom model with a right-sided midline shift. Smart Stylet consists of an electromagnetic tracking system and ventriculostomy catheter connected to a PC and display. The operator of the Smart Stylet can interface with the system via a custom designed module in BWH's 3DSlicer software system. The system was tested for accuracy by calculating targeting error and reporting the precision of catheter placement. Precision was measured using pair-wise distances among experimental groups. The system was reviewed and commented on by three novices and two neurosurgical residents from the Massachusetts General Hospital by using the NASA-TLX grading scale questionnaire and a targeted survey. The phantom model was designed to gauge whether further tests in animals and cadavers are warranted using Smart Stylet. Patients with trauma were more likely to have catheters misplaced (OR = 9.13±2.31; p<0.05). It seems there is an opportunity to improve patient care if catheter placement is made more accurate and reliable. Use of the Smart Stylet system in a phantom study provided improvements in mean pair-wise distance and accuracy for catheter placement at the sub-centimeter level. A blinded operator achieved statistically significant improvement in targeting error using the right frontal approach (p<0.0 5). The operator also significantly improved mean pairwise distances using left and right frontal approaches (p<0.05). Novice operators and neurosurgical residents both showed improvements in targeting accuracy for catheter placement when using the system for the first time. However, the improvements were not statistically significant. Novices' pair-wise distances were significantly better with Smart Stylet guidance using the left frontal approach (p<0.05). Improved guidance techniques, such as the Smart Stylet approach, can potentially decrease ventriculostomy complications if they can be easily integrated into clinical use at low cost.by Vaibhav Devidas Patil.S.M

    Incidental finding of dysgerminoma in a full-term pregnancy: a case report

    Ovarian tumors are reported in 1 of every 200 pregnancies. Dysgerminoma with pregnancy is extremely rare, with a reported incidence of about 0.2-1 per 100,000 pregnancies. They are generally discovered due to their large size related complications like torsion, infarction, and obstruction of vaginal delivery. These rapidly growing tumors can have a heterogeneous presentation and lead to peripartum complications and morbidity. We present a unique case where a dysgerminoma went undiagnosed until labor in a patient who otherwise had an uncomplicated prenatal course. The purpose is to report and discuss our case, as further studies are needed to confirm the best management especially in the management of the incidentally diagnosed tumor

    Evaluation of prescribing patterns of teaching and non teaching hospitals by undergraduate medical students in Pune, India

    Background: This study was carried out in various hospitals to analyze the use of rational fixed dose combinations (FDCs) in Pune.Methods: 279 prescriptions were evaluated in this study. Information about age, sex, names of the all the drugs prescribed by doctor/ physician, diagnosis for the use of prescriptions and adverse effects were noted in the audit form from the prescriptions of the patients. Rationality of fixed dose combination is evaluated according to WHO Model List of Essential Drugs, 17th updated version, 2011.Results: 56.98 % doctor’s prescriptions in this study were containing of fixed dose combinations and out of this 10.69 % prescriptions were including two or more FDCs in their prescriptions. Only 13.20% FDCs were in accordance with WHO Model List of Essential Drugs. FDCs from antiinflammatory and antirheumatic products, vitamins, minerals, antianaemic preparations, drugs for acid related disorders, antibacterials for systemic use and cough and cold preparations were used more by private non teaching hospitals as compared to SKNMC &amp; GH teaching hospital.  64.61 % prescriptions of private hospitals and 34.08 % prescriptions of teaching hospital were containing more than one drug.Conclusions: This study has shown that about every alternate prescription contains FDC. More than 80 % of prescribed FDCs are not in accordance with Essential Drugs List. Vitamins, minerals, antianaemic preparation FDCs should be prescribed judiciously as they are not free from ADRs. More number of drugs (poly-pharmacy) and FDCs were prescribed by non teaching private hospitals

    Knowledge of Cancer cervix and its causative agents among HIV-infected and HIV-uninfected adolescent women in rural Medical College in Maharashtra, India

    Background: The aim of this study is to examine the knowledge of human papilloma virus (HPV) and cervical cancer among HIV-infected and HIV-uninfected female adolescents attending Antenatal OPD in Department of Obstetrics and Gynecology, Government Medical College, Miraj, Maharashtra, India.Methods: Subjects were recruited randomly from OPD attending patients.  A total of 30 subjects, 15 HIV-infected and 15 HIV-uninfected were selected via randomization and completed a measure of HPV knowledge, based on a previously validated instrument. The study took place in December 2017.Results: The overall mean score on the measure for all subjects was 43.3% (S.D. 10.9). There was no significant difference in HPV knowledge between the HIV-infected and HIV-uninfected groups. Based on results from a previous large-scale study using the same validated measure, this sample scored significantly worse on general HPV knowledge than samples from the other studies.Conclusions: Given the limited knowledge of HPV in this sample, there is greater need for education about the prevention of cervical cancer, specifically among high-risk adolescent women
