    Improved estimators for dispersion models with dispersion covariates

    In this paper we discuss improved estimators for the regression and the dispersion parameters in an extended class of dispersion models (J{\o}rgensen, 1996). This class extends the regular dispersion models by letting the dispersion parameter vary throughout the observations, and contains the dispersion models as particular case. General formulae for the second-order bias are obtained explicitly in dispersion models with dispersion covariates, which generalize previous results by Botter and Cordeiro (1998), Cordeiro and McCullagh (1991), Cordeiro and Vasconcellos (1999), and Paula (1992). The practical use of the formulae is that we can derive closed-form expressions for the second-order biases of the maximum likelihood estimators of the regression and dispersion parameters when the information matrix has a closed-form. Various expressions for the second-order biases are given for special models. The formulae have advantages for numerical purposes because they require only a supplementary weighted linear regression. We also compare these bias-corrected estimators with two different estimators which are also bias-free to the second-order that are based on bootstrap methods.     Background: Uremic solute concentration increases as Glomerular Filtration Rate (GFR) declines. Weak associations were demonstrated between estimated GFR (eGFR) and the concentrations of several small water-soluble and protein-bound uremic solutes (MW500Da). Materials and Methods: In 95 CKD-patients (CKD-stage 2-5 not on dialysis), associations between different eGFR-formulae (creatinine, CystatinC-based or both) and the natural logarithm of the concentration of several LMWP's were analyzed: i.e. parathyroid hormone (PTH), Cystatin C (CystC), interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-alpha), leptin, retinol binding protein (RbP), immunoglobin light chains kappa and lambda (Ig-kappa and Ig-lambda), beta-2-microglobulin (beta M-2), myoglobin and fibroblast growth factor-23 (FGF-23)). Results: The regression coefficients (R-2) between eGFR, based on the CKD-EPI-Crea-CystC-formula as reference, and the examined LMWP's could be divided into three groups. Most of the LMWP's associated weakly (R-2 0.7). Almost identical R-2-values were found per LMWP for all eGFR-formulae, with exception of CystC and beta M-2 which showed weaker associations with creatinine-based than with CystC-based eGFR. Conclusion: The association between eGFR and the concentration of several LMWP's is inconsistent, with in general low R-2-values. Thus, the use of eGFR to evaluate kidney function does not reflect the concentration of several LMWP's with proven toxic impact in CKD

    Development stages of the reproductive tract of male of Panulirus echinatus Smith (Decapoda: Palinuridae)

    Esta pesquisa caracteriza os estádios de desenvolvimento dos testículos e canais deferentes da lagosta Panulirus echinatus Smith, 1869 a partir da relação entre seus aspectos macroscópicos, microscópicos e a relação gonadossomática (RGS). Através de amostragem mensal (novembro de 1999 a outubro de 2000) foram capturados 1716 machos, empregando-se redes de espera de fundo. Retirou-se a região dorsal da carapaça para avaliação dos órgãos reprodutivos. Os testículos e canais deferentes foram dissecados, pesados, fixados em solução de Bouin e submetidos aos procedimentos histológicos. A análise microscópica dos órgãos reprodutivos foi avaliada pela presença ou ausência de espermatozóides nos testículos e canais deferentes. Esta, quando associada a macroscopia (mudança de cor, tamanho, diâmetro e desenvolvimento de espermatóforo) e a relação gonadossomática (RGS), possibilitou a caracterização de três estádios de desenvolvimento: imaturo, intermediário e maturo. Ficou evidenciada que a maturidade dos testículos precedeu a maturidade dos canais deferentes. Para avaliar se a RGS é um bom indicador quantitativo dos estádios de maturidade, um teste t (alfa = 0,05) foi usado e constatou diferença significativa nas médias da RGS. A RGS pode ser utilizada como indicadora dos estádios de maturidade para P. echinatus.This paper characterizes the developmental stages of the testes and vasa deferentia of the Panulirus echinatus Smith, 1869 through comparisons between microscopic findings, macroscopic aspects, and gonadosomatic index (GSR). The lobsters were sampled monthly (November 1999 to October 2000) using seine nets and a total of 1716 males were obtained at Tamandaré Bay. Each carapace was cut to allow evaluation of the reproductive organs; the testes and vasa deferentia were dissected, weighed, fixed in Bouin's solution up to 12 hours and submitted for histological analysis to determine the presence and/or absence of spermatozoa. These measures, along with change in color, size, diameter, development of the spermatophores and the GSR allowed the caracterization of three development stages: immature, intermediate and ripe. In conclusion, the maturity of the testes precedes the maturity of the vasa deferentia. To evaluate if gonadosomatic relation was a good quantitative indicator of the maturity stage, t tests (alpha = 0,05) were used and verified significant difference in the averages of GSR.     Equation of state and transport processes in self--similar spheres

    We study the effect of transport processes (diffusion and free--streaming) on a collapsing spherically symmetric distribution of matter in a self--similar space--time. A very simple solution shows interesting features when it is matched with the Vaidya exterior solution. In the mixed case (diffusion and free--streaming), we find a barotropic equation of state in the stationary regime. In the diffusion approximation the gravitational potential at the surface is always constant; if we perturb the stationary state, the system is very stable, recovering the barotropic equation of state as time progresses. In the free--streaming case the self--similar evolution is stationary but with a non--barotropic equation of state.Comment: 9 pages, 2 figure

    Female Genital Mutilation: Knowledge from Health Professionals

    Portugal is considered by the World Health Organization (WHO) a risk country for the practice of Female Genital Mutilation (FGM). Objectives: To evaluate the knowledge that health professionals from Maternity Dr. Alfredo da Costa (MAC) have regarding FGM. Population and Methods: Analysis of surveys delivered to health professionals from MAC (a hospital dedicated to reproductive health), between April and June 2008, addressing issues related to the knowledge about FGM. Results: Authors collected 112 valid surveys involving 38 doctors, 48 nurses and 26 medical auxiliaries/administrative personnel. From the respondents, 106 (95%) had heard about FMG practice before, the media being the most reported source of information; 59 (53%) replied they could be able to recognize FGM cases in their clinical practice; however, only 31 (28%) claimed to know the FGM type classiication and 32 (29%) admitted to be prepared to recognize and manage these situations in their own clinical practice; 9 had been consulted explicitly by a FGM practice complication and 1 doctor had admitted having been asked to perform/execute FGM; 13 (12%) recognized that the Portuguese legislation its this practice. Regarding the practice of FGM, 100 (89%) of respondentes stated that it should not be maintained and 97 (87%) stated that it should not be tolerated. However, 42 (38%) considered that if these practices were a reality, then they should be medical assisted. Discussion: Health professionals can play an important role in eliminating the practice of FGM, not only by the proper clinical management of this situation, but also by preventing those communities at risk to resort to FGM. Most health professionals are not prepared to deal with FGM in their clinical practice. It is important to promote a better knowledge on the subject and to create protocols for proper clinical management

    The role of recorded and verbal information in health information systems: A case study of the Expanded Program on Immunization in Mozambique

    Background: There is ongoing interest in strengthening the informational component of the EPI as a mean to enhance the efficacy of service delivery. As developing country governments make significant investments in strengthening health information systems, benefits obtained from these initiatives tend to be below their potential. To improve this, it is argued that such systems need to take into account both formal (documented) and informal forms of information. Objective: This study measures the accuracy of people-based information with information recorded in the \"Road to Health Card\". Methods: 840 mothers were screened in a cross sectional study. Information about measles immunization status as documented in the Road to Health card was compared to mothers\' verbal information. The pattern of these answers among mothers presenting the card was compared with those among mothers without the card. Results: Measles vaccine information was 95.7% sensitive and 80.0% specific. Measles vaccination positive answer was 10.0% higher among mothers without the card. In the sample of mothers without the vaccination card, only 2.0% never received a card, 53.0% had forgotten to bring it, and the remaining 44.0% had lost it. Conclusion: A key implication of these findings is the need to develop a mother-specific information system that is based at the facility, and an integration of the verbal and tacit information sources while designing the information system. Further recommendation concerns the need to have more targeted information and education programs about the importance of the Road to Health Cards, especially focused for less educated mothers.The Ethiopian Journal of Health Development Vol. 21 (2) 2007: pp. 136-14

    Copyright © 2003 Blackwell Publishing“One way to describe patterns of species distribution is to plot the frequency histograms using species–range-size data (Gaston, 1994; Brown, 1995; Gaston & Blackburn, 2000). Usually the untransformed geographical ranges of species are distributed following a ‘hollow curve’, i.e. most species have narrow ranges and very few have widespread distributions. This highly ‘right-skewed’ curve has been regarded as unimodal (e.g. Gaston, 1994). In some cases, however, the species-range-size distribution shows a bimodal pattern (Hanski, 1982; Brown, 1984, 1995; Gaston, 1994; Gaston & Blackburn, 2000), in which to the left-hand mode is added a right hand mode generated by the widespread group of species that occur in almost all sampled sites. The ‘core-satellite species hypothesis’ (Hanski, 1982) and the ‘resource usage model’ (Brown, 1984, 1995) were proposed to explain the finding that a few species are regionally common (widespread) and locally abundant (the ‘core’ species in Hanski’s model; the generalists or broad-niched species in Brown’s model), while most species can be regarded as having smaller ranges and low local abundances (the ‘satellite’ species in Hanski’s model; the specialists or narrow-niched species in Brown’s model).
