Economics of Fish Marketing in Central Uganda: A Preliminary Analysis
The paper examines profitability and market performance of small-scale fish traders selected randomly from a cross-section of nine fish markets in four districts in Central Uganda. Data were collected through a structured questionnaire which was designed to solicit information on tradersâ socio-economic characteristics, marketing characteristics, operating costs and returns, and problems associated with fish marketing in the study area. Percentages were used to describe the socio-economic characteristics, market characteristic and problems associated with fish marketing while gross profit and marketing performance models were used to determine profitability, marketing margin and operational efficiency, respectively. The results suggest that fish trade is carried out by both men and women. More men are involved in the trade of fresh fish while more women are involved in the processed (sundried/smoked) fish trade. Some traders dealt in more than one species of fish although a majority sold exclusively in one species. Gross profit was estimated at USh358.40/kg and USh234.73/kg for wholesalers and retailers, respectively, with marketing margins of 19.32% and 16.67% for wholesalers and retailers, respectively. The market operational efficiency was 279.27 percent, implying high efficiency in fish marketing in the study area. Clinical features and survival among children with retinoblastoma in Uganda
AIMS: To characterise the clinical features, treatment and outcome of children diagnosed with retinoblastoma in Uganda. METHODS: The study comprised a 6-year nationwide enrolment with follow-up. RESULTS: In total, 282 cases were enrolled, 26% (72) were bilateral; 6% were lost to follow-up. Almost all diagnoses in the first affected eye were International Classification of Retinoblastoma group E or worse. Histology was available for 92%; of those, 45%, had extraocular tumour at diagnosis. Enucleation of the first eye was done for 271; 94 received radiotherapy to the socket and in the last 2â
years, 70 children received chemotherapy. At close of study, 139 children had died. Survival, as determined in a proportional hazards model adjusted for age, sex, laterality and treatment era (pre or post introduction of chemotherapy), varied by extent of the tumour (p<0.001); children with only intraocular involvement were 80% less likely to die (HR=0.21, 95% CI 0.12 to 0.35) compared with children with extraocular involvement. CONCLUSIONS: Diagnostic delay results in relatively high mortality among children with retinoblastoma in Uganda. There is an urgent need for more effective treatment modalities, particularly chemotherapy, and nationwide efforts to encourage earlier access to medical care

Improving survival of retinoblastoma in Uganda
BACKGROUND: Diagnostic delay results in relatively high mortality among children with retinoblastoma in Uganda, where treatment was limited to surgery and, for some, radiotherapy. In order to improve outcomes, a simple programme of neoadjuvant and adjuvant chemotherapy was introduced. Here we report survival before and after this change to medical practice. METHODS: Affordable standard off-patent chemotherapy agents were administered by trained paramedical staff to groups of patients at the same time. Survival before and after the introduction of chemotherapy was monitored. Between 2006 and 2013 a total of 270 patients with retinoblastoma were included, 181 treated prior to chemotherapy and 89 after (beginning in 2009). We had 94% follow-up and 249 had histological verification of diagnosis. RESULTS: Using a proportional hazards model adjusted for age, sex and laterality, children treated after chemotherapy was introduced had a 37% lower risk of dying (HR 0.63, 95% CI 0.41 to 0.99) compared with children treated before. Prior to the introduction of chemotherapy only 15% of children who survived bilateral disease retained vision after treatment compared with 71% after chemotherapy. CONCLUSIONS: The introduction of chemotherapy proved safe and cost-effective in non-specialist hands and was associated with significant improvements in survival and, among bilateral cases, in preserving vision

Willingness to pay for contagious bovine pleuropneumonia vaccination in Narok South District of Kenya
Contagious bovine pleuropneumonia (CBPP) is an economically important trans-boundary cattle disease which affects food security and livelihoods. A conjoint analysisâcontingent valuation was carried out on 190 households in Narok South District of Kenya to measure willingness to pay (WTP) and demand for CBPP vaccine and vaccination as well as factors affecting WTP. The mean WTP was calculated at Kenya Shillings (KSh) 212.48 (USD 3.03) for vaccination using a vaccine with the characteristics that were preferred by the farmers (preferred vaccine and vaccination) and KSh â71.45 (USD â1.02) for the currently used vaccine and vaccination. The proportion of farmers willing to pay an amount greater than zero was 66.7% and 34.4% for the preferred and current vaccine and vaccination respectively. About one third (33.3%) of farmers would need to be compensated an average amount of KSh 1162.62 (USD 13.68) per animal to allow their cattle to be vaccinated against CBPP using the preferred vaccine and vaccination. About two-thirds (65.6%) of farmers would need to be compensated an average amount of KSh 853.72 (USD 12.20) per animal to allow their cattle to be vaccinated against CBPP using the current vaccine and vaccination. The total amount of compensation would be KSh 61.39 million (USD 0.88 million) for the preferred vaccine and vaccination and KSh 90.15 million (USD 1.29 million) for the current vaccine and vaccination. Demand curves drawn from individual WTP demonstrated that only 59% and 27% of cattle owners with a WTP greater than zero were willing to pay a benchmark cost of KSh 34.60 for the preferred and current vaccine respectively. WTP was negatively influenced by the attitude about household economic situation (p = 0.0078), presence of cross breeds in the herd (p < 0.0001) and years since CBPP had been experienced in the herd (p = 0.0375). It was positively influenced by education (p = 0.0251) and the practice of treating against CBPP (p = 0.0432). The benefit cost ratio (BCR) for CBPP vaccination was 2.9â6.1 depending on the vaccination programme. In conclusion, although a proportion of farmers was willing to pay, participation levels may be lower than those required to interrupt transmission of CBPP. Households with characteristics that influence WTP negatively need persuasion to participate in CBPP vaccination. It is economically worthwhile to vaccinate against CBPP. A benefit cost analysis (BCA) using aggregated WTP as benefits can be used as an alternative method to the traditional BCA which uses avoided production losses (new revenue) and costs saved as benefits
The Gendered Production---Marketing Continuum of NERICA Upland Rice in Hoima District, Uganda
Drawing on a survey covering 302 smallholder households growing NERICA upland rice in Hoima District, Uganda, complimented by qualitative data, this paper will first show that NERICA has become an important food and income earner for these farmers. More than one third of their total cultivated acreage is dedicated to this new crop, and with over three quarters of the harvest being sold off at competitive rates, it makes a significant contribution to the householdsâ income portfolios. Next, the paper will argue that NERICA can provide an important entry point for Ugandan women farmers into more commercially oriented modes of production. The traditional cash crops grown in the district such as tobacco are generally controlled by men, and the food crop surpluses that women usually market fetch low prices. Rice not solely being perceived as a commercial crop but also a food crop seems to have made it more accessible to women as it thereby escapes gendered cultural taboos. Hence NERICA has provided socio-economic leverage for women farmers vis-Ă -vis men farmers when it has expanded the space for women to earn money. It has even made it possible for female-headed households to reach the same level of rice market integration as male-headed households. However, the impact it has had on women in male-headed households depends on the degree of control these women have been able to attain over the proceeds from the crop in relation to their husbands, which nevertheless invariably seems greater than for the traditional cash crops
