    BACKGROUNDS:Preterm birth (PTB)/admission to the neonatal intensive care unit (NICU) is a complex disorder associated with significant neonatal mortality and morbidity and long-term adverse health consequences. Multiple lines of evidence suggest that genetic factors play an important role in its etiology. AIM:Given the role of CD38 in term delivery through oxytocin (OXT) release, we hypothesized that OXT signaling may play a role in the etiology of PTB/admission to the NICU. This study was designed to identify genetic variation in the CD38-oxytocin pathway associated with PTB/admission to the NICU. METHODS:To identify common genetic variants predisposing individuals to PTB/admission to the NICU, we genotyped two single nucleotide polymorphisms (SNPs) in the CD38-oxytocin pathway in 63 case mothers, 55 control mothers, and 188 female volunteers in Nara Medical University Hospital, Japan. RESULTS:Maternal genetic effect analysis of the SNP genotype data revealed a significant association between an SNP in CD38 (rs1800561 (4693C>T): R140W), which was reported to be correlated with diabetes and autism, and the risk of NICU admission. On the other hand, an SNP in the oxytocin receptor (OXTR) (rs2254298) showed no correlation with the risk of NICU admission. CONCLUSION:Our study points to an association between maternal common polymorphisms in the CD38 (rs1800561) gene in Japanese women and susceptibility to PTB/admission to the NICU.     Localized Immunoglobulin Light-Chain Amyloidosis of the Ulnar Nerve

    Amyloidosis is a disorder caused by extracellular tissue deposition of insoluble fibrils. Amyloidosis can be divided into systemic or localized disease. Primary systemic amyloidosis is a multisystem disease caused by the deposition of amyloid in various tissues. Localized amyloidosis has different characteristics than those of systemic amyloidosis. In this paper, we present the case of a middle-aged woman who presented with worsening ulnar nerve palsy. Electrophysiological examination and MRI indicated a tumor surrounding the ulnar nerve in the forearm. However, the operative findings revealed that ulnar nerve fascicles were replaced with a yellow tissue, which was diagnosed as amyloid light-chain λ amyloidosis, based on histopathological examination. Systemic amyloidosis was ruled out after the screening examinations.     <i>In situ</i> observation of strain and phase transformation in plastically deformed 301 austenitic stainless steel

    To inform the design of superior transformation-induced plasticity (TRIP) steels, it is important to understand what happens at the microstructural length scales. In this study, strain-induced martensitic transformation is studied by in situ digital image correlation (DIC) in a scanning electron microscope. Digital image correlation at submicron length scales enables mapping of transformation strains with high confidence. These are correlated with electron backscatter diffraction (EBSD) prior to and post deformation process to get a comprehensive understanding of the strain-induced transformation mechanism. The results are compared with mathematical models for enhanced prediction of strain-induced martensitic phase transformation

    Application of the Hilbert-Huang transform for analyzing SASI induced gravitational waves in a core-collapse supernova

    Through numerical simulations, it is predicted that the gravitational waves (GWs) reflect the characteristics of the core-collapse supernova (CCSN) explosion mechanism. There are multiple GW excitation processes that occur inside a star before its explosion, and it is suggested that the GWs originating from the CCSN have a mode for each excitation process in terms of time-frequency representation. Therefore, we propose an application of the Hilbert-Huang Transform (HHT), which is a high-resolution time-frequency analysis method, to analyze these GW modes for theoretically probing and increasing our understanding of the explosion mechanism. The HHT defines frequency as a function of time, and is not bound by the trade-off between time and frequency resolutions. In this study, we analyze a gravitational waveform obtained from a three-dimensional general-relativistic CCSN model that showed a vigorous activity of the standing-accretion-shock-instability (SASI). We succeed in extracting the SASI induced GWs with high resolution on a time-frequency representation using the HHT and we examine their instantaneous frequencies

    Home intravenous antibiotic treatment for intractable cholangitis in patients with biliary atresia following Kasai portoenterostomies

    PURPOSE: Patients with biliary atresia (BA) treated with Kasai portoenterostomy may later develop intractable cholangitis (IC) that is unresponsive to routine conservative treatment. It may cause biliary cirrhosis and eventually hepatic failure with portal hypertension. Control of IC requires prolonged hospitalization for the administration of intravenous antibiotics. To reduce the hospitalization period, we designed a home intravenous antibiotic treatment (HIVA) which can be administered after initial inpatient treatment. In this study, we reviewed the effects of this treatment. METHODS: We reviewed medical records of 10 patients treated with HIVA for IC after successful Kasai portoenterostomies performed for BA between July 1997 and June 2009. RESULTS: The duration of HIVA ranged from 8 to 39 months (median, 13.5 months). The median length of hospital stay was 5.7 days per month for conventional treatments to manage IC before HIVA and, 1.5 days per month (P = 0.012) after HIVA. The median amount of medical expenses per month was reduced by about one tenth with HIVA. One patient underwent liver transplantation due to uncontrolled esophageal variceal bleeding, but the other nine patients had acceptable hepatic function with native livers. CONCLUSION: HIVA may be an effective primary treatment for IC after Kasai portoenterostomies for BA, and reduce length of hospital stay and medical expenseope

    Manche Zitate sucht man nicht, sondern sie werden gefunden, wie man unversehens über einen Stein oder eine Baumwurzel stolpert. Solche Zufälle der Zitation ergeben Figuren des Ein- und Verrückens von Bedeutung, insofern Zitate immer zwischen Aussagekontexten, zwischen Texten stehen. Denn nicht der Urheber, den die Zitation zuschreibt, spricht, sondern der Zitierende, indem er seine eigene Autorschaft einklammert. Von hier aus ist der Titel einer ›Kreativität des Findens‹ gedacht. Theoretische oder systematische Aspekte einer Kreatologie sind weniger intendiert, wenngleich der Ansatz bei der konkreten Figur fürs Grundsätzliche offen ist – zumal bei ›ästhetischen Objekten‹, dem Gegenstandsbereich dieses Bandes.     「死にたい」と検索する者の相談を促進するインターネット広告の要素は何か?

    本研究では自殺関連語を検索する者の援助要請行動を促しやすいインターネット広告の内容を検討した。広告は基本的内容に加えて、見出しに直接的メッセージ(相談してください)か共感的メッセージ(つらかったですね)のどちらかを含め、説明文に相談手段と支援者情報を組み合わせて8種類を作成した。6種類の自殺関連語を検索した結果として広告一つがランダムに表示されるようにGoogle広告を設定した。広告のリンク先ページからボタンをクリックすると電話相談窓口へ発信することができた。ボタンクリックの有無を従属変数、広告の要素を独立変数としたロジスティック回帰分析を行った結果、見出しは共感的メッセージよりも直接的メッセージの方が約1.6 倍、見出しが共感的メッセージの場合には相談手段を説明に含んだ方が約1.2 倍、ボタンクリックの割合が高くなった。すなわち、自殺の相談を促す広告には直接的メッセージと相談手段を含むことが望ましい

    Enhancing evidence-informed policymaking in medicine and healthcare: stakeholder involvement in the Commons Project for rare diseases in Japan

Rare-disease policy, specifically research priority setting for the allocation of limited research resources, is an area where evidence generation through stakeholder involvement is expected to be effective. We generated evidence for rare-disease policymaking through stakeholder involvement and explored effective collaboration among stakeholders. Methods: We constructed a space called ‘Evidence-generating Commons’, where patients, family members, researchers, and former policymakers can share their knowledge and experiences and engage in continual deliberations on evidence generation. Ten rare diseases were consequently represented. In the ‘Commons’, 25 consecutive workshops were held predominantly online, from 2019 to 2021. These workshops focused on (1) clarification of difficulties faced by rare-disease patients, (2) development and selection of criteria for priority setting, and (3) priority setting through the application of the criteria. For the first step, an on-site workshop using sticky notes was held. The data were analysed based on KJ method. For the second and third steps, workshops on specific themes were held to build consensus. The workshop agendas and methods were modified based on participants’ feedback. Results: The ‘Commons’ was established with 43 participants, resulting in positive effects such as capacity building, opportunities for interactions, mutual understanding, and empathy among the participants. The difficulties faced by patients with rare diseases were classified into 10 categories. Seven research topics were identified as priority issues to be addressed including ‘impediments to daily life’, ‘financial burden’, ‘anxiety’, and ‘burden of hospital visits’. This was performed by synthesising the results of the application of the two criteria that were particularly important to strengthen future research on rare diseases. We also clarified high-priority research topics by using criteria valued more by patients and family members than by researchers and former policymakers, and criteria with specific perspectives. Conclusion: We generated evidence for policymaking in the field of rare diseases. This study’s insights into stakeholder involvement can enhance evidence-informed policymaking. We engaged in comprehensive discussions with policymakers regarding policy implementation and planned analysis of the participants’ experiences in this project
