    Creating connections - the development of a mobile-health monitoring system for heart failure:Qualitative findings from a usability cohort study

    Objective There is significant interest in the role of digital health technology in enabling optimal monitoring of heart failure patients. To harness this potential, it is vital to account for users’ capacity and preferences in the development of technological solutions. We adopted an iterative approach focussed on learning from users’ interactions with a mobile-health monitoring system.Methods We used a participatory mixed methods research approach to develop and evaluate a mobile-health monitoring system. Fifty-eight heart failure patients were recruited from three health care settings in the UK and provided with Internet-enabled tablet computers that were wirelessly linked to sensor devices for blood pressure, heart rate and weight monitoring. One to two home visits were conducted with a subgroup of 29 participants to evaluate the usability of the system over a median follow-up period of six months. The thematic analysis of observational data and 45 interviews was informed by the domestication of technology theory.Results Our findings indicate that digital health technologies need to create and extend connections with health professionals, be incorporated into users’ daily routines, and be personalised according to users’ technological competencies and interest in assuming a proactive or more passive role in monitoring their condition.Conclusions Users' patterns of engagement with health technology changes over time and varies according to their need and capacity to use the technology. Incorporating diverse user experiences in the development and maintenance of mobile-health systems is likely to increase the extent of successful uptake and impacts on outcomes for patients and providers.%U http://dhj.sagepub.com/content/spdhj/2/2055207616671461.full.pd

    Evaluation of acceptability, functionality, and validity of a passive image-based dietary intake assessment method in adults and children of Ghanaian and Kenyan origin living in London, UK

    BACKGROUND: Accurate estimation of dietary intake is challenging. However, whilst some progress has been made in high-income countries, low- and middle-income countries (LMICs) remain behind, contributing to critical nutritional data gaps. This study aimed to validate an objective, passive image-based dietary intake assessment method against weighed food records in London, UK, for onward deployment to LMICs. METHODS: Wearable camera devices were used to capture food intake on eating occasions in 18 adults and 17 children of Ghanaian and Kenyan origin living in London. Participants were provided pre-weighed meals of Ghanaian and Kenyan cuisine and camera devices to automatically capture images of the eating occasions. Food images were assessed for portion size, energy, nutrient intake, and the relative validity of the method compared to the weighed food records. RESULTS: The Pearson and Intraclass correlation coefficients of estimates of intakes of food, energy, and 19 nutrients ranged from 0.60 to 0.95 and 0.67 to 0.90, respectively. Bland-Altman analysis showed good agreement between the image-based method and the weighed food record. Under-estimation of dietary intake by the image-based method ranged from 4 to 23%. CONCLUSIONS: Passive food image capture and analysis provides an objective assessment of dietary intake comparable to weighed food records

    Onset of entanglement

    We have developed a theory of polymer entanglement using an extended Cahn-Hilliard functional, with two extra terms. One is a nonlocal attractive term, operating over mesoscales, which is interpreted as giving rise to entanglement, and the other a local repulsive term indicative of excluded volume interactions. We show how such a functional can be derived using notions from gauge theory. We go beyond the Gaussian approximation, to the one-loop level, to show that the system exhibits a crossover to a state of entanglement as the average chain length between points of entanglement decreases. This crossover is marked by critical slowing down, as the effective diffusion constant goes to zero. We have also computed the tensile modulus of the system, and we find a corresponding crossover to a regime of high modulus.Comment: 18 pages, with 4 figure

    Genetic structure of the South European toothcarp Aphanius fasciatus (Actinopterygii: Cyprinodontidae) populations in the Mediterranean basin with a focus on the Venice lagoon

    The genetic structure of Aphanius fasciatus populations has been analysed using two mitochondrial DNA (mtDNA) markers (16S rRNA and D-loop) obtained from specimens collected in nine sites from the Venice lagoon, Comacchio saltworks and Corsica. Available GenBank sequences were also included, in order to extend the results on a Mediterranean scale. Genetic polymorphism within the Venice lagoon was very low, with most of the specimens analysed (66% for 16S rRNA and 83% for D-loop) sharing the same haplotype for either of the two markers. The genetic homogeneity found within the Venice lagoon may be the consequence of the northward migration of southern Adriatic populations after the Last Glacial Maximum: mismatch analysis showed indeed clear signs of a rapid demographic and spatial expansion. To explain this genetic homogeneity other hypotheses were also suggested, such as adaptation to the high variability of brackish water habitats, and artificial introductions. On a Mediterranean scale, phylogenetic analyses showed the presence of five distinct geographical lineages: Aegean Sea, Greek coast of the Ionian Sea, Adriatic Sea, Tyrrhenian Sea and Southern Sicily. Analysis of molecular variance revealed a genetic partitioning mainly due to differences between groups presumably due to late Miocene geological events, while less polymorphism was present within groups and populations
