    A modulation property of time-frequency derivatives of filtered phase and its application to aperiodicity and fo estimation

    We introduce a simple and linear SNR (strictly speaking, periodic to random power ratio) estimator (0dB to 80dB without additional calibration/linearization) for providing reliable descriptions of aperiodicity in speech corpus. The main idea of this method is to estimate the background random noise level without directly extracting the background noise. The proposed method is applicable to a wide variety of time windowing functions with very low sidelobe levels. The estimate combines the frequency derivative and the time-frequency derivative of the mapping from filter center frequency to the output instantaneous frequency. This procedure can replace the periodicity detection and aperiodicity estimation subsystems of recently introduced open source vocoder, YANG vocoder.     Image synthesis of monoenergetic CT image in dual-energy CT using kilovoltage CT with deep convolutional generative adversarial networks

    Purpose: To synthesize a dual-energy computed tomography (DECT) image from an equivalent kilovoltage computed tomography (kV-CT) image using a deep convolutional adversarial network. Methods: A total of 18,084 images of 28 patients are categorized into training and test datasets. Monoenergetic CT images at 40, 70, and 140 keV and equivalent kVCT images at 120 kVp are reconstructed via DECT and are defined as the reference images. An image prediction framework is created to generate monoenergetic computed tomography (CT) images from kV-CT images. The accuracy of the images generated by the CNN model is determined by evaluating the mean absolute error (MAE), mean square error (MSE), relative root mean square error (RMSE), peak signal-to-noise ratio (PSNR), structural similarity index (SSIM), and mutual information between the synthesized and reference monochromatic CT images. Moreover, the pixel values between the synthetic and reference images are measured and compared using a manually drawn region of interest (ROI). Results: The difference in the monoenergetic CT numbers of the ROIs between the synthetic and reference monoenergetic CT images is within the standard deviation values. The MAE, MSE, RMSE, and SSIM are the smallest for the image conversion of 120 kVp to 140 keV. The PSNR is the smallest and the MI is the largest for the synthetic 70 keV image. Conclusions: The proposed model can act as a suitable alternative to the existing methods for the reconstruction of monoenergetic CT images in DECT from single-energy CT images

    Assessment of biological dosimetric margin for stereotactic body radiation therapy

    Purpose: To develop a novel biological dosimetric margin (BDM) and to create a biological conversion factor (BCF) that compensates for the difference between physical dosimetric margin (PDM) and BDM, which provides a novel scheme of a direct estimation of the BDM from the physical dose (PD) distribution. Methods: The offset to isocenter was applied in 1‐mm steps along left‐right (LR), anterior‐posterior (AP), and cranio‐caudal (CC) directions for 10 treatment plans of lung stereotactic body radiation therapy (SBRT) with a prescribed dose of 48 Gy. These plans were recalculated to biological equivalent dose (BED) by the linearquadratic model for the dose per fraction (DPF) of d = 3–20 Gy/fr and α/β= 3 - 10. BDM and PDM were defined so that the region that satisfied that the dose covering 95% (or 98%) of the clinical target volume was greater than or equal to the 90% of the prescribed PD and BED, respectively. An empirical formula of the BCF was created as a function of the DPF. Results: There was no significant difference between LR and AP directions for neither the PDM nor BDM. On the other hand, BDM and PDM in the CC direction were significantly larger than in the other directions. BCFs of D95% and D98% were derived for the transverse (LR and AP) and longitudinal (CC) directions. Conclusions: A novel scheme to directly estimate the BDM using the BCF was developed. This technique is expected to enable the BED‐based SBRT treatment planning using PD‐based treatment planning systems

    Synthesized effective atomic numbers for commercially available dual-energy CT

    Purpose: The objective of this study was to assess synthesized effective atomic number (Zeff) values with a new developed tissue characteristic phantom and contrast material of varying iodine concentrations using single-source fast kilovoltage switching dual-energy CT (DECT) scanner. Methods: A newly developed multi energy tissue characterisation CT phantom and an acrylic phantom with various iodine concentrations of were scanned using single-source fast kilovoltage switching DECT (GE-DECT) scanner. The difference between the measured and theoretical values of Zeff were evaluated. Additionally, the difference and coefficient of variation (CV) values of the theoretical and measured values were compared with values obtained with the Canon-DECT scanner that was analysed in our previous study. Results: The average Zeff difference in the Multi-energy phantom was within 4.5%. The average difference of the theoretical and measured Zeff values for the acrylic phantom with variation of iodine concentration was within 3.3%. Compared to the results for the single-source Canon-DECT scanner used in our previous study, the average difference and CV of the theoretical and measured Zeff values obtained with the GE-DECT scanner were markedly smaller. Conclusions: The accuracy of the synthesized Zeff values with GE-DECT had a good agreement with the theoretical Zeff values for the Multi-Energy phantom. The GE-DECT could reduce the noise and the accuracy of the Zeff values than that with Canon-DECT for the varying iodine concentrations of contrast medium. Advances in knowledge: The accuracy and precision of the Zeff values of the contrast medium with the GE-DECT could be sufficient with human equivalent materials

    Immune Reactions Against Elongation Factor 2 Kinase: Specific Pathogenesis of Gastric Ulcer from Helicobacter pylori Infection

    Helicobacter pylori (H. pylori) infection is a definite causative factor for gastric ulcers (GUs). In the present study we detected a specific antigen of gastric epithelial cells (HGC-27) using cell ELISA, which was recognized by the sera of GU patients (n = 20) but not in patients with chronic gastritis (CG; n = 20) or in healthy volunteers (HC; n = 10). This antigen was over-expressed by a stressful (heat-stressed) environment, and was identified as elongation factor 2 kinase (EF-2K) by western blotting. The GU patients' lymphocytes stimulated by H. pylori specifically disrupted heat-stressed HGC-27 cells in a cytotoxic assay. In flow cytometry, the effector cells (lymphocytes) from GU patients were significantly differentiated to T helper type 1 lymphocyte (Th1) and cytotoxic T lymphocyte (CTL) as opposed to those from CG patients. The target cells (HGC-27) expressed EF-2K and MHC-class I together with costimulatory molecules from heat stress.     Efficacy and safety of endoscopic submucosal dissection for gastric tube cancer: A multicenter retrospective study

    BACKGROUND Recent improvements in the prognosis of patients with esophageal cancer have led to the increased occurrence of gastric tube cancer (GTC) in the reconstructed gastric tube. However, there are few reports on the treatment results of endoscopic submucosal dissection (ESD) for GTC. AIM To evaluate the efficacy and safety of ESD for GTC after esophagectomy in a multicenter trial. METHODS We retrospectively investigated 48 GTC lesions in 38 consecutive patients with GTC in the reconstructed gastric tube after esophagectomy who had undergone ESD between January 2005 and December 2019 at 8 institutions participating in the Okayama Gut Study group. The clinical indications of ESD for early gastric cancer were similarly applied for GTC after esophagectomy. ESD specimens were evaluated in 2-mm slices according to the Japanese Classification of Gastric Carcinoma with curability assessments divided into curative and non-curative resection based on the Gastric Cancer Treatment Guidelines. Patient characteristics, treatment results, clinical course, and treatment outcomes were analyzed. RESULTS The median age of patients was 71.5 years (range, 57-84years), and there were 34 men and 4 women. The median observation period after ESD was 884 d (range, 8-4040 d). The median procedure time was 81 min (range, 29-334 min), the en bloc resection rate was 91.7% (44/48), and the curative resection rate was 79% (38/48). Complications during ESD were seen in 4% (2/48) of case, and those after ESD were seen in 10% (5/48) of case. The survival rate at 5 years was 59.5%. During the observation period after ESD, 10 patients died of other diseases. Although there were differences in the procedure time between institutions, a multivariate analysis showed that tumor size was the only factor associated with prolonged procedure time. CONCLUSION ESD for GTC after esophagectomy was shown to be safe and effective

    Clinical characteristics and course of sporadic non-ampullary duodenal adenomas A multicenter retrospective study

    Sporadic non-ampullary duodenal adenoma (SNADA) is a rare disease, and therefore, its clinical characteristics have not been comprehensively investigated. Furthermore, owing to the high complication rates and severity of endoscopic resection, treatment strategies vary among facilities. In the present study, we aimed to clarify the clinical characteristics and course of SNADA. We extracted clinical and histological records of SNADA cases diagnosed in 11 hospitals between September 1999 and August 2014. The patients were divided into "no-resection" and "resection" groups based on the initial treatment approach. We investigated the long-term outcome of the "no-resection" group and treatment results of the "resection" group, with particular interest in endoscopic resection. Overall, 299 patients were diagnosed with SNADA. The median age at diagnosis was 67 years (range, 31-88 years), with approximately twice as many men as women. The median tumor size was 8.0 mm (2-60 mm). In total, 161 patients were initially selected for no-resection and 138 underwent resection. Age >70 years and the presence of either severe illness or poor performance status were significantly related to opting for no-resection. In the no-resection group, 101 patients underwent endoscopic follow-up for at least 1 year. During the observational period (2.5 +/- 2.2 years), 27 lesions (27%) disappeared following cold forceps biopsy, and 13 lesions (14%) presented lateral growth. Four lesions (4%) changed to mucosal carcinoma, 3 were treated endoscopically, and 1 was surgically resected. Nineteen patients died; however, no one died of duodenal carcinoma. In the endoscopic resection group, en bloc resection was achieved in 78% of patients. However, the complication rate for perforation was 7%, and endoscopic submucosal dissection was associated with a 36% perforation rate. With the low incidence of cancer development and no disease specific death, the strategy of initially not performing resection could be considered especially for the older adults, poor-prognosis patients, or small lesions

    Effects of surface modification of noble-metal sensing electrodes with Au on the hydrogen-sensing properties of diode-type gas sensors employing an anodized titania film

    Diode-type gas sensors employing an anodized TiO2 film and noble-metal (Pt, Pd and Pd-Pt) sensing electrodes modified with and without a small amount of Au (Au/M/TiO2 and M/TiO2, respectively, M = Pt, Pd, Pd-Pt (Pd:Pt = 58:42 in weight)) have been fabricated, and the effects of surface modification of the electrodes with Au on their H2-sensing properties have been investigated at 250 °C in both air and N2 under dry and wet atmospheres. H2 response of all the M/TiO2 sensors in N2 was much larger than that in air under both dry and wet atmospheres. The Pt/TiO2 sensor showed the smallest H2 response in air among them, but the surface modification of the Pt electrode with Au was quite effective in enhancing the H2 response of the Pt/TiO2 sensor in air. Therefore, the response of the Au/Pt/TiO2 sensor to 8000 ppm H2 in air was comparable to that in N2, especially under wet atmosphere, and thus the response of the Au/Pt/TiO2 sensor to 8000 ppm H2 was almost independent of oxygen concentration under wet atmosphere. In addition, the annealing of the sensor at 600°C did not affect the response of the Au/Pt/TiO2 sensor to 8000 ppm H2. The homogeneous mixing of Au into the Pt electrode also improved the H2 response of the Pt/TiO2 sensor, but the effectiveness was much lower than that of the surface modification of the Pt electrode with Au. On the other hand, the response of the as-fabricated Au/Pd/TiO2 and Au/Pd-Pt/TiO2 sensors to 8000 ppm H2 in air was much larger than that of the Au/Pd/TiO2 and Au/Pd-Pt/TiO2 sensors annealed at 600°C, especially under wet atmosphere. The large H2 response observed with the Au/Pt/TiO2 sensor and the as-fabricated Au/Pd/TiO2 and Au/Pd-Pt/TiO2 sensors can be explained by large suppression of H2 oxidation by Au components remained at the surface of their sensing electrodes and then increased amount of dissociatively adsorbed hydrogen species at the sensing electrode surface

    A dehydrated space-weathered skin cloaking the hydrated interior of Ryugu

    Without a protective atmosphere, space-exposed surfaces of airless Solar System bodies gradually experience an alteration in composition, structure and optical properties through a collective process called space weathering. The return of samples from near-Earth asteroid (162173) Ryugu by Hayabusa2 provides the first opportunity for laboratory study of space-weathering signatures on the most abundant type of inner solar system body: a C-type asteroid, composed of materials largely unchanged since the formation of the Solar System. Weathered Ryugu grains show areas of surface amorphization and partial melting of phyllosilicates, in which reduction from Fe3+ to Fe2+ and dehydration developed. Space weathering probably contributed to dehydration by dehydroxylation of Ryugu surface phyllosilicates that had already lost interlayer water molecules and to weakening of the 2.7 µm hydroxyl (–OH) band in reflectance spectra. For C-type asteroids in general, this indicates that a weak 2.7 µm band can signify space-weathering-induced surface dehydration, rather than bulk volatile loss
