    Numerical Simulation of Oil Jet Lubrication for High Speed Gears

    The Geared Turbofan technology is one of the most promising engine configurations to significantly reduce the specific fuel consumption. In this architecture, a power epicyclical gearbox is interposed between the fan and the low pressure spool. Thanks to the gearbox, fan and low pressure spool can turn at different speed, leading to higher engine bypass ratio. Therefore the gearbox efficiency becomes a key parameter for such technology. Further improvement of efficiency can be achieved developing a physical understanding of fluid dynamic losses within the transmission system. These losses are mainly related to viscous effects and they are directly connected to the lubrication method. In this work, the oil injection losses have been studied by means of CFD simulations. A numerical study of a single oil jet impinging on a single high speed gear has been carried out using the VOF method. The aim of this analysis is to evaluate the resistant torque due to the oil jet lubrication, correlating the torque data with the oil-gear interaction phases. URANS calculations have been performed using an adaptive meshing approach, as a way of significantly reducing the simulation costs. A global sensitivity analysis of adopted models has been carried out and a numerical setup has been defined

    Dupilumab in the treatment of severe uncontrolled chronic rhinosinusitis with nasal polyps (CRSwNP): A multicentric observational Phase IV real-life study (DUPIREAL)

    Background Chronic rhinosinusitis with nasal polyps (CRSwNP) is associated with significant morbidity and reduced health-related quality of life. Findings from clinical trials have demonstrated the effectiveness of dupilumab in CRSwNP, although real-world evidence is still limited. Methods This Phase IV real-life, observational, multicenter study assessed the effectiveness and safety of dupilumab in patients with severe uncontrolled CRSwNP (n = 648) over the first year of treatment. We collected data at baseline and after 1, 3, 6, 9, and 12 months of follow-up. We focused on nasal polyps score (NPS), symptoms, and olfactory function. We stratified outcomes by comorbidities, previous surgery, and adherence to intranasal corticosteroids, and examined the success rates based on current guidelines, as well as potential predictors of response at each timepoint. Results We observed a significant decrease in NPS from a median value of 6 (IQR 5–6) at baseline to 1.0 (IQR 0.0–2.0) at 12 months (p < .001), and a significant decrease in Sino-Nasal Outcomes Test-22 (SNOT-22) from a median score of 58 (IQR 49–70) at baseline to 11 (IQR 6–21; p < .001) at 12 months. Sniffin' Sticks scores showed a significant increase over 12 months (p < .001) compared to baseline. The results were unaffected by concomitant diseases, number of previous surgeries, and adherence to topical steroids, except for minor differences in rapidity of action. An excellent-moderate response was observed in 96.9% of patients at 12 months based on EPOS 2020 criteria. Conclusions Our findings from this large-scale real-life study support the effectiveness of dupilumab as an add-on therapy in patients with severe uncontrolled CRSwNP in reducing polyp size and improving the quality of life, severity of symptoms, nasal congestion, and smell

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Volume Of Fluid (VOF) Analysis of Oil-Jet Lubrication for High-Speed Spur Gears Using an Adaptive Meshing Approach

    In high speed gearbox systems, the lubrication is generally provided using nozzles to create small oil jets that feed oil into the meshing zone. It is essential that the gear teeth are properly lubricated and that enough oil gets into the tooth spaces to permit sufficient cooling and prevent gearbox failure. A good understanding of the oil behaviour inside the gearbox is therefore desirable, to minimize lubrication losses and reduce the oil volume involved, and ensure gearbox reliability. In order to reach these objectives, a comprehensive numerical study of a single oil jet impinging radially on a single spur gear teeth has been carried out using the Volume of Fluid (VOF) method. The aims of this study are to evaluate the resistant torque produced by the oil jet lubrication, and to develop a physical understanding of the losses deriving from the oil-gear interaction, studying the droplets and ligaments formation produced by the breaking up of the jet as well as the formation of an oil film on the surface of the teeth. URANS calculations have been performed with the commercial code ANSYS FLUENT and an adaptive mesh approach has been developed as a way of significantly reducing the simulation costs. This method allows an automatic mesh refinement and/or coarsening at the air-oil interface based on the volume of fluid gradient, increasing the accuracy of the predictions of oil break-up as well as minimizing numerical diffusion of the interface. A global sensitivity analysis of adopted models has been carried out and a numerical set-up has been defined. Finally several simulations varying the oil injection angle have been performed, in order to evaluate how this parameter affects the resistant torque and the lubrication performances.</jats:p

Paradoxical embolism through PAVMs can cause systemic desaturation, cyanosis, and serious cerebrovascular ischemic events (transient ischemic attacks, strokes, and intracranial abscess), even when the abnormal blood vessel is small (diameter 17 s) RLS coming from left pulmonary veins to left atrium while a patent foramen ovale (PFO), small atrial septal defects or septum primum fenestration could not be detected despite several attempts. Contrast Transcranial Doppler (cTCD) confirmed a delayed (>16 s) RLS with two short “shower” patterns corroborating the diagnosis of an extra-cardiac RLS. During the right heart catheterization and pulmonary angiography, it was impossible to cross the interatrial septum with a multipurpose catheter. The patient was finally discharged with off-label thienopyridine agents (clopidogrel 75 mg die) in terms of primary prophylaxis for paradoxical right-to-left embolization of thrombotic material ultimately. Aura migraine symptoms were nearly abolished by P2Y12 platelet inhibition, suggesting a platelet-based mechanism. During 2 years of clinical follow-up on thienopyridine therapy, the resolution of aura migraine episodes was definitively accomplished with significant improvement in her quality of life.</p

    Minor hepatic resection for hepatocellular carcinoma in cirrhotic patients: Kelly clamp crushing resection versus heat coagulative necrosis with bipolar radiofrequency device

    Hemorrhage and postoperative liver insufficiency are frequent and serious complications of hepatic resection in cirrhotic patients. The aim of this study was to assess retrospectively whether the surgical techniques using Kelly clamp crushing resection or heat coagulative necrosis with a bipolar radiofrequency device can reduce the incidence of the above complications and an eventual recurrence of neoplasia on the liver slice. We retrospectively reviewed the results of 35 patients who had undergone resection for monofocal hepatocellular carcinoma at our center. Thirteen patients (Group A) had undergone liver resection with Kelly clamp crushing resection, 22 patients (Group B) had had liver resection assisted with a bipolar radiofrequency device. Radiofrequency-assisted liver resection was associated with diminished blood loss (P < 0.0001), a lower blood transfusion rate (P < 0.005), reduced operative time (P < 0.0001), and better postoperative serum albumin levels (P < 0.03). This nonrandomized retrospective study suggests that radiofrequency-assisted liver resection is associated with better results than the Kelly clamp crushing resection technique in cirrhotic patients with focal hepatocellular carcinoma and preserved liver function. These results should now be assessed prospectively in a randomized clinical trial
