    Predictors of Length of Hospital Stay among Burns Patients in Mulago National Referral Hospital, Kampala- Uganda

    Background: According to WHO (2014), burn injuries are a major problem to health care worldwide. Ninety five per cent of all burn cases occur in LMICs leading to prolonged and expensive hospital stays (WHO, 2009). In Uganda, burn injuries account for 11% of all childhood injuries (Nakitto & Lett, 2010). Burns Unit at Mulago National Referral Hospital is the only specialized burns care unit in the country. However, it was observed that patients in this unit spends unusually longer time in admission than the WHO (2015) recommended time. Thus the objective of this study was to determine the predictors of length of hospital stay among burn patients in Mulago National Referral Hospital Kampala, Burns Care Unit from July, 2014 to June, 2015.Methods: A retrospective study design was used by reviewing medical records of patients discharged from the Burns Care Unit from July, 2014 to June, 2015. Results: More than half (57.1%) of the patients in the study were male with children 5 years and below constituting 55.2%. The majority of patients (86.2%) got burnt at home. The average length of stay for patients was 24.3 days (Âą22.1 days). The degree/ depth of burns (OR=44.22, 95% CI =10.86-180.08, P=0.000) was the single most significant predictor of length of stay of patients with burns at multivariate analysis level.     Ethics as a solution to Corruption : a case study of the construction industry in Kenya

    The construction industry endures a poor reputation and continues to experience ethical problems manifested in collapsed buildings, corrupt deals and a general lack of honesty. In this paper, the authors present results of an investigation into the prevalent ethical problems in the construction industry in Kenya and their impact on projects. A review of literature on corruption shows that emphasis is placed upon putting in place systems and policies for fighting corruption, yet this has not achieved much results. Literature in construction ethics shows that professional codes and ethics are only effective as far as the people are personally ethical. The method of conducting this study was through in-depth interviews of stakeholders to establish prevalent unethical practices in the industry. The authors present the results from a thematic analysis of perceptions of information-rich subjects from different sectors of the industry. The results show that unethical practices such as collusion, kickbacks and supplanting are mostly due to lack of honesty, greed and the desire to get rich quickly. The study concludes that most unethical practices are due to corrupt individual practices and recommends ways of incorporating ethics both at individual and institutional level

    Serologic Surveillance of Anthrax in the Serengeti Ecosystem, Tanzania, 1996–2009

    Bacillus anthracis, the bacterium that causes anthrax, is responsible for varying death rates among animal species. Difficulties in case detection, hazardous or inaccessible carcasses, and misdiagnosis hinder surveillance. Using case reports and a new serologic assay that enables multispecies comparisons, we examined exposure to and illness caused by B. anthracis in different species in the Serengeti ecosystem in Tanzania during 1996–2009 and the utility of serosurveillance. High seroprevalence among carnivores suggested regular nonfatal exposure. Seropositive wildebeest and buffalo showed that infection was not invariably fatal among herbivores, whereas absence of seropositivity in zebras and frequent detection of fatal cases indicated high susceptibility. Exposure patterns in dogs reflected known patterns of endemicity and provided new information about anthrax in the ecosystem, which indicated the potential of dogs as indicator species. Serosurveillance is a valuable tool for monitoring and detecting anthrax and may shed light on mechanisms responsible for species-specific variability in exposure, susceptibility, and mortality rates

    Prospects of Transition Interface Sampling simulations for the theoretical study of zeolite synthesis

    The transition interface sampling (TIS) technique allows to overcome large free energy barriers within reasonable simulation time, which is impossible for straightforward molecular dynamics. Still, the method does not impose an artificial driving force, but it surmounts the timescale problem by an importance sampling of true dynamical pathways. Recently, it was shown that the efficiency of TIS to calculate reaction rates is less sensitive to the choice of reaction coordinate than those of the standard free energy based techniques. This could be an important advantage in complex systems for which a good reaction coordinate is usually very difficult to find.     Genomic epidemiology of Escherichia coli:Antimicrobial resistance through a One Health lens in sympatric humans, livestock and peri-domestic wildlife in Nairobi, Kenya

    BackgroundLivestock systems have been proposed as a reservoir for antimicrobial-resistant (AMR) bacteria and AMR genetic determinants that may infect or colonise humans, yet quantitative evidence regarding their epidemiological role remains lacking. Here, we used a combination of genomics, epidemiology and ecology to investigate patterns of AMR gene carriage in Escherichia coli, regarded as a sentinel organism.MethodsWe conducted a structured epidemiological survey of 99 households across Nairobi, Kenya, and whole genome sequenced E. coli isolates from 311 human, 606 livestock and 399 wildlife faecal samples. We used statistical models to investigate the prevalence of AMR carriage and characterise AMR gene diversity and structure of AMR genes in different host populations across the city. We also investigated household-level risk factors for the exchange of AMR genes between sympatric humans and livestock.ResultsWe detected 56 unique acquired genes along with 13 point mutations present in variable proportions in human and animal isolates, known to confer resistance to nine antibiotic classes. We find that AMR gene community composition is not associated with host species, but AMR genes were frequently co-located, potentially enabling the acquisition and dispersal of multi-drug resistance in a single step. We find that whilst keeping livestock had no influence on human AMR gene carriage, the potential for AMR transmission across human-livestock interfaces is greatest when manure is poorly disposed of and in larger households.ConclusionsFindings of widespread carriage of AMR bacteria in human and animal populations, including in long-distance wildlife species, in community settings highlight the value of evidence-based surveillance to address antimicrobial resistance on a global scale. Our genomic analysis provided an in-depth understanding of AMR determinants at the interfaces of One Health sectors that will inform AMR prevention and control

    Population genomics of <i>Escherichia coli</i> in livestock-keeping households across a rapidly developing urban landscape

    Quantitative evidence for the risk of zoonoses and the spread of antimicrobial resistance remains lacking. Here, as part of the UrbanZoo project, we sampled Escherichia coli from humans, livestock and peri-domestic wildlife in 99 households across Nairobi, Kenya, to investigate its distribution among host species in this rapidly developing urban landscape. We performed whole-genome sequencing of 1,338 E. coli isolates and found that the diversity and sharing patterns of E. coli were heavily structured by household and strongly shaped by host type. We also found evidence for inter-household and inter-host sharing and, importantly, between humans and animals, although this occurs much less frequently. Resistome similarity was differently distributed across host and household, consistent with being driven by shared exposure to antimicrobials. Our results indicate that a large, epidemiologically structured sampling framework combined with WGS is needed to uncover strain-sharing events among different host populations in complex environments and the major contributing pathways that could ultimately drive the emergence of zoonoses and the spread of antimicrobial resistance

    Marsh macrophyte responses to inundation anticipate impacts of sea-level rise and indicate ongoing drowning of North Carolina marshes

    In situ persistence of coastal marsh habitat as sea level rises depends on whether macrophytes induce compensatory accretion of the marsh surface. Experimental planters in two North Carolina marshes served to expose two dominant macrophyte species to six different elevations spanning 0.75 m (inundation durations 0.4–99 %). Spartina alterniflora and Juncus roemerianus exhibited similar responses—with production in planters suggesting initial increases and then demonstrating subsequent steep declines with increasing inundation, conforming to a segment of the ecophysiological parabola. Projecting inundation levels experienced by macrophytes in the planters onto adjacent marsh platforms revealed that neither species occupied elevations associated with increasing production. Declining macrophyte production with rising seas reduces both bioaccumulation of roots below-ground and baffle-induced sedimentation above-ground. By occupying only descending portions of the parabola, macrophytes in central North Carolina marshes are responding to rising water levels by progressive declines in production, ultimately leading to marsh drowning

    We suggest that the vertebrate myosin-I field adopt a common nomenclature system based on the names adopted by the Human Genome Organization (HUGO). At present, the myosin-I nomenclature is very confusing; not only are several systems in use, but several different genes have been given the same name. Despite their faults, we believe that the names adopted by the HUGO nomenclature group for genome annotation are the best compromise, and we recommend universal adoption
