    Analysis of the Editorial Process of the Multidisciplinary Rural Development Journal Tropicultura

    Tropicultura is a multidisciplinary journal which aims mainly at releasing research results relevant to rural development in developing countries and at improving the investigation capacities of the researchers who submit manuscripts to its editorial board. The operating process of the journal and its consequences on its output during the period 2002- 2009 were analysed by considering mainly the factors influencing the duration of the editorial work and the final acceptance of the manuscripts. The factors taken in consideration were: the field of research, the geographic origin of the data analysed, the language of writing and the country of origin of the authors. The available data were analysed using descriptive statistic methods. They were also subjected to parametric and non parametric comparisons. A total of 1,034 papers have been submitted during the investigated period to Tropicultura in different fields of rural development research, with a large proportion of papers in agronomy sensu lato (60%), and livestock production (19%). Most of the papers submitted (85.1%) came from Sub-Saharan Africa, followed by North Africa (11.2%), Asia (1.6%), Latin America (0.6%) and Europe (0.3%). The rate of acceptance (27.4%) was very low compared to other journals, mainly because of a poor design of the works or inappropriate research topics. The average time for final decision was 355 days. The non parametric classification analysis retained as major determinants for the acceptance of papers for publication in decreasing order of influence: (i) time before final decision, (ii) language, (iii) continent, (iv) Belgian cooperation priority countries, (v) Belgian cooperation partner countries, and (vi) the field of research. The data obtained are discussed in the light of the literature related to the editorial process of other scientific journals, taking into account the peculiarities of Tropicultura related to its history and to the history of the rural development actions of the Belgian cooperation. This analysis highlighted a series of possible improvements at the level of the operating process of the journal which should enable it to better achieve its goals

    Peak or sustained antibiotic serum levels for optimal tissue penetration

    A comparative study of the levels of netilmicin in bronchial secretions after intermittent and during continuous intravenous injections was performed. During the first 2 h, the areas under the drug concentration curves in the bolus injection experiment were greater in serum and in bronchial secretions (206 and 150% respectively) than those associated with the continuous infusion (P<0.001); however, during the 8 h experimental period, the areas under the curves were similar in serum and bronchial secretions with the two modes of administration. The percentages of penetration of netilmicin from the blood to the bronchial lumen were also similar with the two modes of administration (19%). After intramuscular administration of amikacin (7.5 and 12.5 mg/kg twice daily), the peak bronchial secretion levels of the drug were 4.4 and 10.1 mg/l respectively and the trough levels about 1 mg/l. During continuous iv injection of amikacin (7-12 mg/kg/8 h), the drug level in the bronchial secretions was only 2.0 mg/l and the percentage of penetration was 14.9% adequate anti-Pseudomonas activity in bronchial secretions was rarely achieved. It is therefore suggested that intermittent injections of aminoglycosides can result in at least transitory higher bronchial secretion levels than continuous injection: the intermittent schedule of antibiotic administration could therefore be recommended in the treatment of bronchopulmonary infections since it has been suggested that the concentration of antibiotic in bronchial secretions could be important in the outcome of these infection

    Primary Care COPD Patients Compared with Large Pharmaceutically-Sponsored COPD Studies:An UNLOCK Validation Study

    Background: Guideline recommendations for chronic obstructive pulmonary disease (COPD) are based on the results of large pharmaceutically-sponsored COPD studies (LPCS). There is a paucity of data on disease characteristics at the primary care level, while the majority of COPD patients are treated in primary care.Objective: We aimed to evaluate the external validity of six LPCS (ISOLDE, TRISTAN, TORCH, UPLIFT, ECLIPSE, POET-COPD) on which current guidelines are based, in relation to primary care COPD patients, in order to inform future clinical practice guidelines and trials.Methods: Baseline data of seven primary care databases (n = 3508) from Europe were compared to baseline data of the LPCS. In addition, we examined the proportion of primary care patients eligible to participate in the LPCS, based on inclusion criteria.Results: Overall, patients included in the LPCS were younger (mean difference (MD)-2.4; p = 0.03), predominantly male (MD 12.4; p = 0.1) with worse lung function (FEV1% MD -16.4; p &lt;0.01) and worse quality of life scores (SGRQ MD 15.8; p = 0.01). There were large differences in GOLD stage distribution compared to primary care patients. Mean exacerbation rates were higher in LPCS, with an overrepresentation of patients with &gt;= 1 and &gt;= 2 exacerbations, although results were not statistically significant. Our findings add to the literature, as we revealed hitherto unknown GOLD I exacerbation characteristics, showing 34% of mild patients had &gt;= 1 exacerbations per year and 12% had &gt;= 2 exacerbations per year. The proportion of primary care patients eligible for inclusion in LPCS ranged from 17% (TRISTAN) to 42% (ECLIPSE, UPLIFT).Conclusion: Primary care COPD patients stand out from patients enrolled in LPCS in terms of gender, lung function, quality of life and exacerbations. More research is needed to determine the effect of pharmacological treatment in mild to moderate patients. We encourage future guideline makers to involve primary care populations in their recommendations.</p

    G4 Resolvase 1 tightly binds and unwinds unimolecular G4-DNA

    It has been previously shown that the DHX36 gene product, G4R1/RHAU, tightly binds tetramolecular G4-DNA with high affinity and resolves these structures into single strands. Here, we test the ability of G4R1/RHAU to bind and unwind unimolecular G4-DNA. Gel mobility shift assays were used to measure the binding affinity of G4R1/RHAU for unimolecular G4-DNA-formed sequences from the Zic1 gene and the c-Myc promoter. Extremely tight binding produced apparent Kd's of 6, 3 and 4 pM for two Zic1 G4-DNAs and a c-Myc G4-DNA, respectively. The low enzyme concentrations required for measuring these Kd's limit the precision of their determination to upper boundary estimates. Similar tight binding was not observed in control non-G4 forming DNA sequences or in single-stranded DNA having guanine-rich runs capable of forming tetramolecular G4-DNA. Using a peptide nucleic acid (PNA) trap assay, we show that G4R1/RHAU catalyzes unwinding of unimolecular Zic1 G4-DNA into an unstructured state capable of hybridizing to a complementary PNA. Binding was independent of adenosine triphosphate (ATP), but the PNA trap assay showed that unwinding of G4-DNA was ATP dependent. Competition studies indicated that unimolecular Zic1 and c-Myc G4-DNA structures inhibit G4R1/RHAU-catalyzed resolution of tetramolecular G4-DNA. This report provides evidence that G4R1/RHAU tightly binds and unwinds unimolecular G4-DNA structure

    Performance of the TOTEM Detectors at the LHC

    The TOTEM Experiment is designed to measure the total proton-proton cross-section with the luminosity-independent method and to study elastic and diffractive pp scattering at the LHC. To achieve optimum forward coverage for charged particles emitted by the pp collisions in the interaction point IP5, two tracking telescopes, T1 and T2, are installed on each side of the IP in the pseudorapidity region 3.1 < = |eta | < = 6.5, and special movable beam-pipe insertions - called Roman Pots (RP) - are placed at distances of +- 147 m and +- 220 m from IP5. This article describes in detail the working of the TOTEM detector to produce physics results in the first three years of operation and data taking at the LHC.Comment: 40 pages, 31 figures, submitted to Int.     Work absence in patients with asthma and/or COPD:a population-based study

    Chronic obstructive pulmonary disease (COPD) and asthma impact on work productivity, but their population-based burden and clinical predictors are understudied. In this observational, real-life study, work absence of 14,383 asthma and/or COPD patients present in the MAJORICA cohort (Spain) was compared with the general population. Using multivariable regression, we studied the association of work absence with demographic and clinical characteristics. Patients with asthma and/or COPD had more work absence than the general population (15.2% vs 8.9%, p <0.0001). Patients with asthma had more often periods of work absence compared to patients with COPD (16.0% vs 12.8%, p <0.0001). The number of days absent were, however, less in asthma than in COPD (median: 15 days [IQR: 5-51] vs 39 days [IQR: 13-134], p <0.001). Patients with asthma-COPD overlap were in between (14.5% with absence; median: 27 days [IQR: 10-82]). Comorbid anxiety, allergic rhinitis, and sleep apnoea were independently associated with more work absence
