    The 'black box' problem in the study of participation

    Research on citizen participation has been guided by two core issues: first, the observation of a widening repertory of modes of participation, and second, the argument that participation is not an undifferentiated phenomenon, but must be conceived as an inherently multidimensional reality. In this article, we argue that conventional participation research has focused too one-sidedly on quantitatively expanding the range of types of activities, while the complex dimensionality is not reflected in the measures used. We formulate a methodological critique by using the metaphor of the 'black box', which refers to the implicit and unquestioned assumption that distinct types of activities and associations represent homogeneous and consistent realities that do not warrant further analytical decomposition. Surveys of participation allocate individuals to different 'participation boxes' by means of a binary logic, leaving a void of what is actually happening inside the boxes. To conclude, we reflect upon the fundamental dilemmas the black box of participation raises for theory and research, and offer conceptual and methodological keys to unlock the participation box

    Implementing US-style anti-fraud laws in the Australian pharmaceutical and health care industries

    This article critically analyses the prospects for introducing United States anti-fraud (or anti-false claims) laws in the Australian health care setting. Australian governments spend billions of dollars each year on medicines and health care. A recent report estimates that the money lost to corporate fraud in Australia is growing at an annual rate of 7%, but that only a third of the losses are currently being detected. In the US, qui tam provisions - the component of anti-fraud or anti-false claims laws involving payments to whistleblowers - have been particularly successful in providing critical evidence allowing public prosecutors to recover damages for fraud and false claims made by corporations in relation to federal and state health care programs. The US continues to strengthen such anti-fraud measures and to successfully apply them to a widening range of areas involving large public investment. Australia still suffers from the absence of any comprehensive scheme that not only allows treble damages recovery for fraud on the public purse, but crucially supports such actions by providing financial encouragement for whistleblowing corporate insiders to expose evidence of fraud. Potential areas of application could include direct and indirect government expenditure on health care service provision, pharmaceuticals, medical devices, defence, carbon emissions compensation and tobacco-related illness. The creation in Australia of an equivalent to US anti-false claims legislation should be a policy priority, particularly in a period of financial stringency

    Simulated X-ray Cluster Temperature Maps

    Temperature maps are presented of the 9 largest clusters in the mock catalogues of Muanwong et al. for both the Preheating and Radiative models. The maps show that clusters are not smooth, featureless systems, but contain a variety of substructure which should be observable. The surface brightness contours are generally elliptical and features that are seen include cold clumps, hot spiral features, and cold fronts. Profiles of emission-weighted temperature, surface brightness and emission-weighted pressure across the surface brightness discontinuities seen in one of the bimodal clusters are consistent with the cold front in Abell 2142 observed by Markevitch et al.Comment: Submitted to Monthly Notices Royal Astronomical Societ

    Breastfeeding is promoted at a public health level by many agencies internationally and nationally. While there is an abundance of literature to guide practice regarding breastfeeding term healthy infants, there is little regarding premature infants, particularly those born before 30 weeks gestation. Data on breastfeeding outcomes in this population is limited in the New Zealand literature. The aims of the study were to identify the breastfeeding outcomes of premature infants born before 30 weeks gestation: to identify key determinants that may contribute to the success of breastfeeding outcomes; and to describe the feeding practices of this population during hospitalisation and post-discharge. This audit used a non-experimental, cross-sectional, descriptive, correlational design using pre-existing medical records, to obtain variables of interest. Independent variables included maternal and infant characteristic and infant feeding characteristic. Key determinant variables were identified as maternal support, maternal milk supply, kangaroo care and bottle use. Feeding problems and growth were also considered as independent variables. Breastfeeding outcomes are comparable to the New Zealand population of breastfed infants in duration but not intensity of breast milk received. Maori and Polynesian infants have poorer breastfeeding outcomes compared to Caucasian and Asian infants and have higher rates of feeding problems. Further research is required to identify the cause of increased feeding problems in these cultures

    Study protocol: addressing evidence and context to facilitate transfer and uptake of consultation recording use in oncology: a knowledge translation implementation study

    Background: The time period from diagnosis to the end of treatment is challenging for newly diagnosed cancer patients. Patients have a substantial need for information, decision aids, and psychosocial support. Recordings of initial oncology consultations improve information recall, reduce anxiety, enhance patient satisfaction with communication, and increase patients' perceptions that the essential aspects of their disease and treatment have been addressed during the consultation. Despite the research evidence supporting the provision of consultation recordings, uptake of this intervention into oncology practice has been slow. The primary aim of this project is to conduct an implementation study to explicate the contextual factors, including use of evidence, that facilitate and impede the transfer and uptake of consultation-recording use in a sample of patients newly diagnosed with breast or prostate cancer. Methods: Sixteen oncologists from cancer centres in three Canadian cities will participate in this three-phase study. The preimplementation phase will be used to identify and address those factors that are fundamental to facilitating the smooth adoption and delivery of the intervention during the implementation phase. During the implementation phase, breast and prostate cancer patients will receive a recording of their initial oncology consultation to take home. Patient interviews will be conducted in the days following the consultation to gather feedback on the benefits of the intervention. Patients will complete the Digital Recording Use Semi-Structured Interview (DRUSSI) and be invited to participate in focus groups in which their experiences with the consultation recording will be explored. Oncologists will receive a summary letter detailing the benefits voiced by their patients. The postimplementation phase includes a conceptual framework development meeting and a seven-point dissemination strategy. Discussion: Consultation recording has been used in oncology, family medicine, and other medicine specialties, and despite affirming evidence and probable applications to a large number of diseases and a variety of clinical contexts, clinical adoption of this intervention has been slow. The proposed study findings will advance our conceptual knowledge of the ways to enhance uptake of consultation recordings in oncology

    Looking beyond stratification: a model-based analysis of the biological drivers of oxygen deficiency in the North Sea

    Low oxygen conditions, often referred to as oxy- gen deficiency, occur regularly in the North Sea, a temperate European shelf sea. Stratification represents a major process regulating the seasonal dynamics of bottom oxygen, yet, low- est oxygen conditions in the North Sea do not occur in the regions of strongest stratification. This suggests that stratifi- cation is an important prerequisite for oxygen deficiency, but that the complex interaction between hydrodynamics and the biological processes drives its evolution. In this study we use the ecosystem model HAMSOM- ECOHAM to provide a general characterisation of the dif- ferent zones of the North Sea with respect to oxygen, and to quantify the impact of the different physical and biological factors driving the oxygen dynamics inside the entire sub- thermocline volume and directly above the bottom. With respect to oxygen dynamics, the North Sea can be subdivided into three different zones: (1) a highly produc- tive, non-stratified coastal zone, (2) a productive, season- ally stratified zone with a small sub-thermocline volume, and (3) a productive, seasonally stratified zone with a large sub- thermocline volume. Type 2 reveals the highest susceptibility to oxygen deficiency due to sufficiently long stratification pe- riods (textgreater60 days) accompanied by high surface productivity resulting in high biological consumption, and a small sub- thermocline volume implying both a small initial oxygen in- ventory and a strong influence of the biological consumption on the oxygen concentration. Year-to-year variations in the oxygen conditions are caused by variations in primary production, while spatial dif- ferences can be attributed to differences in stratification and water depth. The large sub-thermocline volume dominates the oxygen dynamics in the northern central and northern North Sea and makes this region insusceptible to oxygen de- ficiency. In the southern North Sea the strong tidal mixing inhibits the development of seasonal stratification which pro- tects this area from the evolution of low oxygen conditions. In contrast, the southern central North Sea is highly suscep- tible to low oxygen conditions (type 2). We furthermore show that benthic diagenetic processes represent the main oxygen consumers in the bottom layer, consistently accounting for more than 50% of the overall consumption. Thus, primary production followed by rem- ineralisation of organic matter under stratified conditions constitutes the main driver for the evolution of oxygen defi- ciency in the southern central North Sea. By providing these valuable insights, we show that ecosystem models can be a useful tool for the interpretation of observations and the es- timation of the impact of anthropogenic drivers on the North Sea oxygen conditions

    Challenges to delivering university health-based work-integrated learning to students with a disability: A scoping review

    Students with disability experience numerous challenges when engaging in Work-Integrated Learning (WIL). Successful WIL requires stakeholder collaboration to provide an equitable and relevant WIL experience. Stakeholder disparity around disclosure, accommodations, poor attitudes, and behaviours result in negative WIL experiences for students with disability. Understanding stakeholder preparedness and capabilities, in particular host organisations, is key to providing equitable WIL opportunities. Searches of five electronic databases (CINAHL, PubMed, Embase/Scopus, A + Education Informit and Web of Science) were conducted. Twenty-one peer-reviewed articles published between 2005 and 2022 were included in the review. Four themes were identified: Disclosure of the disability; University staff and WIL supervisor attitudes and training; Surviving WIL and Adjusting WIL to the individual. Embedding a strengths-based approach to WIL through strong relationships between student, host organisation and university will produce safe environments that are essential for high quality and fit for purpose WIL experiences for students with disability
