    Remoção de smear layer de superfícies radiculares com aplicação tópica de EDTA com detergente. Análise em microscopia eletrônica de varredura

    The aim of the present study was to compare root surface smear layer removal following topical application of EDTA and EDTA-T (Texapon). Extracted human teeth had their cementum removed and were mechanically scaled. A total of 220 root specimens were obtained and were randomly assigned to the following groups: I-saline solution (control), II-EDTA; III-EDTA-T. Groups II and III specimens were assigned to different EDTA gel concentrations: 5%, 10%, 15%, 20% and 24%. Smear layer removal score was assessed for each specimen by scanning electron microscopy. The results demonstrated that EDTA and EDTA-T gel led to a higher root surface smear layer removal when compared to the control group. The 5% EDTA gel also showed a higher smear layer removal than the 15%, 20% and 24% EDTA gels (p;0.05). EDTA gels had statistically significantly lower smear layer scores than the EDTA-T gels for the 5% and 10% concentrations. The results suggested that topical application of EDTA or EDTA-T gel led to significant smear layer removal of the mechanically treated root surfaces. The addition of a detergent to the EDTA gel formula did not improve smear layer removal of the root surface.O objetivo do presente estudo foi comparar, através de microscopia eletrônica de varredura, a remoção de smear layer de superfícies radiculares após aplicação tópica de gel de EDTA com gel de EDTA-T (Texapon). Foram utilizados dentes humanos que foram submetidos a remoção de cemento radicular e raspagem. As amostras dos dentes foram divididas em 3 grupos: Grupo I- (controle) (n=20); Grupo II- gel de EDTA (pH 7,0) nas concentrações de 5%,10%,15%,20% e 24% (n=100); Grupo II -gel de EDTA-T (pH 7,0) nas concentrações acima descritas (n=100). As fotomicrografias foram avaliadas através de um índice e os dados foram estatisticamente analisados. Ambos os tratamentos com EDTA foram efetivos na remoção de smear layer quando comparados ao grupo controle. O gel de EDTA a 5% demonstrou maior capacidade na remoção de smear layer quando comparado com as concentrações de 15%, 20% e 24% (p;0,05). Os géis de EDTA foram mais efetivos que o EDTA-T nas concentrações de 5 e 10%. Os resultados sugerem que a aplicação tópica de EDTA ou EDTA-T é efetiva na remoção de smear layer, principamente em baixas concentrações e a adição do Texapon não promove vantagens neste tratamento

    The purpose of this study was to compare the removal of root surface smear layer following active application of EDTA gel and EDTA-T (texapon) gel in different concentrations (5%, 10%, 15%, 20% and 24%), using scanning electron microscopy. A total of 220 dentin blocks obtained from the root surfaces of extracted teeth were divided into 3 groups: Group I - (control) application of saline solution (n = 20); Group II - EDTA gel (pH 7.0) was applied in the following concentrations: 5%, 10%, 15%, 20% and 24% (n = 100); Group III - EDTA-T gel (pH 7.0) applied in the same concentrations described above (n = 100). The photomicrographs were evaluated by one calibrated examiner using a smear layer removal index and following statistical analysis (Kruskal-Wallis test). The results demonstrated that the specimens treated with EDTA and EDTA-T gel presented a better smear layer removal than the control group (p ; 0.05). Within the limits of this study, it can be concluded that all treatment modalities effectively removed the smear layer from the root surface. The addition of texapon into the EDTA gel formulation did not increase its effectiveness.O objetivo do presente estudo foi comparar, através de microscopia eletrônica de varredura, o efeito da aplicação de gel de EDTA e de EDTA-T (texapon) em diferentes concentrações (5%, 10%, 15%, 20% e 24%) na remoção de "smear layer" de superfícies radiculares previamente raspadas com instrumentos manuais. Duzentos e vinte espécimes de superfícies radiculares submetidas à raspagem foram divididos em 3 grupos. Grupo I - (controle) solução salina (n = 20); Grupo II - gel de EDTA (pH 7,0) nas concentrações de 5%, 10%, 15%, 20% e 24% (n = 100); Grupo III - gel de EDTA-T (pH 7,0) nas concentrações acima descritas (n = 100). As fotomicrografias obtidas foram avaliadas por um examinador calibrado através da aplicação de um índice de remoção de "smear layer", e os dados foram analisados através de análise estatística (teste de Kruskal-Wallis). Os resultados demonstraram que todos os tratamentos com EDTA e EDTA-T foram mais efetivos na remoção de "smear layer" quando comparados com o grupo controle (p ; 0,05). Dentro dos limites deste estudo, pode-se concluir que todas as modalidades de tratamento foram efetivas na remoção de "smear layer" e a adição de texapon ao gel de EDTA não aumentou a efetividade do tratamento

    The Effectiveness of the Low-Level Laser, Antibiotic and Surgical Therapy in the Treatment of Medication-Related Osteonecrosis of the Jaws: A Case Report

    Introduction: Bisphosphonates consist of a range of drugs used in the treatment of osteopathy or some osteotropic malignancies. Medication-related osteonecrosis of the jaws (MRONJ) is a serious adverse consequence from their use. Conventional treatment is not always effective, so it is necessary to resort to the use of adjuvant therapies. This study aimed to evaluate the effectiveness of the association of surgery, antibiotic therapy and low-level laser (LLL) therapy as a treatment strategy for MRONJ through the presentation of a clinical case.Case presentation: A 49-year-old patient, female gender, presented for diagnosis and treatment of bone lesions in the maxillae. The patient reported that she had used zoledronate for one year. Five years later there were two painless bone lesions in both sides of maxilla, following extraction of the first upper molars. Clinical, tomographic and histopathologic examination established the diagnosis of MRONJ. The treatment consisted of curettage of necrotic bone, antibiotics and thirteen LLL therapy sessions. The integral mucosal healing was observed after two months of follow-up and no symptoms were detected. The patient was evaluated at six and 12 postoperative months without complications.Conclusion: The combination of surgery, antibiotic therapy and LLL therapy has shown be effective in the treatment of MRONJ

    Impact of the burden of caregivers of children with ASD on oral health

    Abstract Introduction the caregiver's demand to meet the needs of individuals with autism spectrum disorder (ASD) can influence their quality of life. Objective the objective of the present study was to compare the burden on caregivers of individuals with ASD with individuals neurotypical (N) and to evaluate the correlation of the impact of oral health with the burden on these caregivers. Material and method participants were divided into two groups: ASD Group (ASD; n=35) and Neurotypical Group (N; n=35). A sociodemographic characteristics questionnaire was administered to caregivers. Caregiver burden was measured using the Burden Interview (BI) questionnaire. The visible plaque index (VPI) of children was analyzed. The data were submitted to statistical analysis (α=5%). Result in the group ASD, the majority of participants were partially dependent, with the majority being dependent on oral hygiene. In group N, the majority were independent and self-sufficient in oral hygiene. Half of the caregivers in the group ASD felt mild to moderate burden (54.3%), while in group N it was found that more than half of them had no burden (65.7%). In the comparative analysis of data from the BI questionnaire, significant differences were observed between the groups in questions (p <0.001), which are directly related to childcare. Regarding the VPI of children in the group ASD and N groups compared to the level of burden, it was observed that there were no significant differences between the groups. Conclusion ASD caregivers have a greater burden when compared to N caregivers and the degree of burden did not influence the oral hygiene of the child with ASD

    Multiple aPDT sessions on periodontitis in rats treated with chemotherapy: Histomorphometrical, Immunohistochemical, Imunological and Microbiological Analyses.

    BACKGROUND: The aim of this study was to evaluate the effect of multiple sessions of antimicrobial photodynamic therapy (aPDT) on the treatment of experimental periodontitis (EP) in rats treated with chemotherapy. METHODS: Chemotherapy using 5-fluorouracil (5-FU) consisted of intraperitoneal administration of 60 and 40 mg/kg of 5-FU. 120 rats were subjected to chemotherapy with 5-FU and divided into groups: PT (periodontal treatment); PT+1aPDT (PT and single aPDT session); PT+4aPDT(PT and 4 sessions of aPDT); 1aPDT (single aPDT session); 4aPDT(4 sessions of aPDT). EP was induced in the mandibular molars via ligature placement. The alveolar bone loss (ABL) area in the furcation region was analysed histometrically. Proliferating cell nuclear antigen (PCNA), tartrate-resistant acid phosphatase (TRAP), receptor activator of nuclear factor kappa-B ligand (RANKL), osteoprotegerin (OPG) and cleaved caspase-3 (CC3) were analysed by immunohistochemistry. Prostaglandin E2 was quantified using an ELISA, tumor necrosis factor (TNF)-α and interleukin (IL)-6 were assessed using a multiplex method. The prevalence of Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, Prevotella nigrescens, Prevotella intermedia and Fusobacterium nucleatum was assessed using PCR. The data were statistically analysed (P < 0.05). RESULTS: The PT+4aPDT group showed lower ABL than the PT or 1aPDT groups on day 7. Rats treated with aPDT showed a higher number of PCNA-positive cells with reduced immunolabeling of RANKL. Significant reductions in Prevotella nigrescens were observed in the PT+4aPDT group and in Aggregatibacter actinomycetemcomitans for the 1aPDT and 4aPDT groups. CONCLUSION: Repeated sessions of aPDT as an adjunct or alternative therapy were effective at reducing ABL, regulating bone metabolism, and reducing Prevotella nigrescens and Aggregatibacter actinomycetemcomitans

    Análise morfológica, através de microscopia eletrônica de varredura, da ação do laser de Er: YAG em superfícies radiculares submetidas à raspagem e aplainamento radicular

    The purpose of this study was to morphologically evaluate, by means of scanning electron microscopy, the effects of Er:YAG laser on the treatment of root surfaces submitted to scaling and root planing with conventional periodontal instruments. Eighteen root surfaces (n = 18), which had been previously scaled and planed, were assigned to 3 groups (n = 6). The control Group (G1) received no further treatment; Group 2 (G2) was irradiated with Er:YAG laser (2.94 mum), with 47 mJ/10 Hz, in a focused mode with air/water spray during 15 s and with 0.57 J/cm² of fluency per pulse; Group 3 (G 3) was irradiated with Er:YAG laser (2.94 mum), with 83 mJ/10 Hz, in a focused mode with air/water spray during 15 s and with 1.03 J/cm² of fluency per pulse. We concluded that the parameters adopted for Group 3 removed the smear layer from the root surface, exposing the dentinal tubules. Although no fissures, cracks or carbonized areas were observed, an irregular surface was produced by Er:YAG laser irradiation. Thus, the biocompatibility of the irradiated root surface, within the periodontal healing process, must be assessed.O objetivo do presente estudo foi analisar morfologicamente, através de microscopia eletrônica de varredura, os efeitos do laser de Er:YAG no tratamento de superfícies radiculares submetidas à raspagem e aplainamento radicular com instrumentos manuais. Foram utilizados 18 espécimes (n = 18) de superfícies radiculares que após ser submetidos à raspagem e aplainamento radicular foram divididos em 3 grupos (n = 6). O grupo controle (G1) não sofreu nenhum tratamento; Grupo 2 (G2) foi irradiado com laser de Er:YAG (2,94 mim) 47 mJ/10 Hz, focalizado,com refrigeração à água durante 15 s e fluência de pulso de 0,57 J/cm²; Grupo 3 (G3) foi irradiado com laser de Er:YAG (2,94 mim), 83 mJ/10 Hz, focalizado, com refrigeração à agua durante 15 s e fluência de pulso de 1,03 J/cm². Através da análise dos resultados podemos concluir que o laser de Er:YAG nos parâmetros utilizados no G3 promove a remoção de "smear layer" da superfície radicular e exposição dos túbulos dentinários, porém o laser promove aspecto irregular na superfície, apesar de não demonstrar crateras, fendas, fraturas e carbonização, necessitando de estudos que demonstrem a biocompatibilidade desta superfície no processo de reparo periodontal

    A histologic assessment of the influence of low-intensity laser therapy on wound healing in steroid-treated animals

    Objective: the aim of the present study was to evaluate the effect of low-intensity laser therapy on the wound healing process treated with steroid. Background Data: Various biological effects have been associated with low-level laser therapy (LLLT). Materials and Methods: Forty-eight rats were used, and after execution of a wound on the dorsal region of each animal, they were divided into 4 groups (n = 12), receiving the following treatments: G1 (control), wounds and animals received no treatment; G2, wounds were treated with LLLT; G3, animals received an intraperitoneal injection of steroid dosage (2 mg/kg of body weight); G4, animals received steroid and wounds were treated with LLLT. The laser emission device used was a GaAIAs (904 nm), in a contact mode, with 2.75 mW gated with 2.900 Hz during 120 sec (33 J/cm(2)). After the period of 3, 7, and 14 days, the animals were sacrificed and the parts sent to histological processing and dyed using hematoxylin and eosin (HE) and Masson trichromium (MT) techniques. Results: the results have shown that the wounds treated with steroid had a delay in healing, while LLLT accelerated the wound healing process. Also, wounds treated with laser in the animals treated with steroid presented a differentiated healing process with a larger collagen deposition and also a decrease in both the inflamatory infiltrated and the delay on the wound healing process. Conclusion: LLLT accelerated healing, caused by the steroid, acting as a biostimulative coadjutant agent, balancing the undesirable effects of cortisone (in the tissue healing process

    Root debridement generates a smear layer which contains microorganisms and toxins that could interfere in periodontal healing. For this reason, different substances have been used to remove it and to expose collagen fibers at the tooth surface. Blood element adhesion to demineralized roots and clot stabilization by collagen fibers are extremely important for the success of periodontal surgery. The aim of this study was to evaluate the different patterns of blood element adsorption and adhesion to root surfaces only irrigated with distilled water and after application of a manipulated or an industrialized EDTA gel. Thirty samples were planed, equally divided into three groups and treated with distilled water (control), a manipulated EDTA gel or an industrialized one. Immediately after, samples were exposed to fresh blood and prepared for scanning electron microscopy. Untreated planed dentin presented the best results with blood cells entrapped in a thick web of fibrin. In the manipulated EDTA group, the web of fibrin was thick with sparse blood elements. The worst result was seen with the industrialized EDTA group, in which no blood elements could be seen. Statistical difference was obtained between control and industrialized EDTA groups. Surfaces only irrigated presented the most organized fibrin network and cell entrapment
