    The deposition of Paraguayan president Fernando Lugo and its repercussions in South American regional organizations

    In June 2012, the Paraguayan Parliament executed a procedure that removed President Fernando Lugo from office. Since 1993, only two presidents have completed their mandates, Juan Carlos Wasmosy (1993-1998) and Nicanor Duarte Frutos (2003-2008). With the exception of the latter, all elected presidents have faced either successful or attempted coups, mostly organized by the military. In the case of Lugo, South American governments reacted by suspending Paraguay from two regional organizations (Unasur and Mercosur). This article has two main objectives: 01. to provide an interpretation of Fernando Lugo's deposition, in the context of the ongoing instability of Paraguay's democratic regime, as well as other similar cases in South America; and 02. to analyze the regional repercussions of the interruption of Lugo's presidential mandate, in particular the reactions from other South American governments and the ramifications for three Mercosur institutions: the Common Market Council (CMC), the Mercosur Parliament (Parlasur), and the Permanent Review Tribunal (TPR). It is argued that, despite the involvement of the parliamentary and judicial institutions of the bloc in the Paraguayan case, the governments have prevailed in both the suspension and reincorporation of Paraguay to the bloc, reinforcing both the intergovernmental nature of these regional initiatives and the prominence of national executives in regional decision-making

    Central venous catheter adverse events are not associated with crowding indicators

    OBJECTIVE: Crowding in the emergency department (ED) impacts a number of important quality and safety metrics. We studied ED crowding measures associated with adverse events (AE) resulting from central venous catheters (CVC) inserted in the ED, as well as the relationship between crowding and the frequency of CVC insertions in an ED cohort admitted to the intensive care unit (ICU). METHODS: We conducted a retrospective observational study from 2008-2010 in an academic tertiary care center. Participants undergoing CVC in the ED or admitted to an ICU were categorized by quartile based on the following: National Emergency Department Overcrowding Scale (NEDOCS); waiting room patients (WR); ED patients awaiting inpatient beds (boarders); and ED occupancy (EDO). Main outcomes were the occurrence of an AE during CVC insertion in the ED, and deferred procedures assessed by frequency of CVC insertions in ED patients admitted to the ICU. RESULTS: Of 2,284 ED patients who had a CVC inserted, 293 (13%) suffered an AE. There was no association between AEs from ED CVCs and crowding scales when comparing the highest crowding level or quartile to all other quartiles: NEDOCS (dangerous crowding [13.1%] vs other levels [13.0%], P = 0.98); number of WR patients (14.0% vs 12.7%, P = 0.81); EDO (13.0% vs 12.9%, P = 0.99); and number of boarding patients (12.0% vs 13.3%), P = 0.21). In a cohort of ED patients admitted to the ICU, there was no association between CVC placement rates in the ED and crowding scales comparing the highest vs all other quartiles: NEDOCS (dangerous crowding 16% vs all others 16%, P = 0.97); WR patients (16% vs 16%, P = 0.82), EDO (15% vs. 17%, P = 0.15); and number of boarding patients (17% vs 16%, P = 0.08). CONCLUSION: In a large, academic tertiary-care center, frequency of CVC insertion in the ED and related AEs were not associated with measures of crowding. These findings add to the evidence that the negative effects of crowding, which impact all ED patients and measures of ED performance, are less likely to impair the delivery of prioritized time-critical interventions

    The reaction of common bean cultivars to common bacterial blight (CBC), caused by Xanthomonasaxonopodis pv. phaseoli, was evaluated under greenhouse conditions. Foliar inoculations were made by themultiple needles method and the disease severity was evaluated at 7, 11, 15 and 19 days after the foliar inoculation(DAI). The area under the common bacterial blight progress curve (AUCBCPC) was also estimated. At the 19th DAI,only the cultivars IPR Juriti, BR 6-Barriga Verde and SCS 202-Guará presented greater resistance levels. CultivarsIPR Juriti, BR 6-Barriga Verde, SCS 202-Guará and Graúna had the lowest AUCBCPC among all cultivars.Foi avaliado o comportamento de cultivares de feijoeiro frente ao crestamento bacteriano comum (CBC), causado por Xanthomonas axonopodis pv. phaseoli, em condições de casa-de-vegetação. Procedeu-se à inoculação foliar pelo método de agulhas múltiplas e a avaliação da severidade da doença ocorreu aos 7, 11, 15 e 19 dias após a inoculação (DAI). Posteriormente, estimou-se a área abaixo da curva de progresso do crestamento bacteriano comum (AACPCBC). Aos 19 DAI, apenas as cultivares IPR Juriti, BR 6-Barriga Verde e SCS 202-Guará mostraram-se com maiores níveis de resistência foliar ao CBC. As cultivares IPR Juriti, BR 6-Barriga Verde, SCS 202-Guará e Graúna apresentaram as menores AACPCBC entre as cultivares avaliadas

    Adverse events related to ultrasound-guided regional anesthesia performed by Emergency Physicians: Systematic review protocol

    The use of ultrasound-guided regional anesthesia for pain management has become increasingly prevalent in Emergency Medicine, with studies noting excellent pain control while sparing opioid use. However, the use of ultrasound-guided regional anesthesia may be hampered by concern about risks for patient harm. This systematic review protocol describes our approach to evaluate the incidence of adverse events from the use of ultrasound-guided regional anesthesia by Emergency Physicians as described in the literature. This project will also seek to document the scope of ultrasound-guided regional anesthesia applications being performed in Emergency Medicine literature, and potentially serve as a framework for future systematic reviews evaluating adverse events in Emergency Medicine

    Measurement of the cosmic ray spectrum above 4×10184{\times}10^{18} eV using inclined events detected with the Pierre Auger Observatory

    A measurement of the cosmic-ray spectrum for energies exceeding 4×10184{\times}10^{18} eV is presented, which is based on the analysis of showers with zenith angles greater than 6060^{\circ} detected with the Pierre Auger Observatory between 1 January 2004 and 31 December 2013. The measured spectrum confirms a flux suppression at the highest energies. Above 5.3×10185.3{\times}10^{18} eV, the "ankle", the flux can be described by a power law EγE^{-\gamma} with index γ=2.70±0.02(stat)±0.1(sys)\gamma=2.70 \pm 0.02 \,\text{(stat)} \pm 0.1\,\text{(sys)} followed by a smooth suppression region. For the energy (EsE_\text{s}) at which the spectral flux has fallen to one-half of its extrapolated value in the absence of suppression, we find Es=(5.12±0.25(stat)1.2+1.0(sys))×1019E_\text{s}=(5.12\pm0.25\,\text{(stat)}^{+1.0}_{-1.2}\,\text{(sys)}){\times}10^{19} eV.Comment: Replaced with published version.     Energy Estimation of Cosmic Rays with the Engineering Radio Array of the Pierre Auger Observatory

    The Auger Engineering Radio Array (AERA) is part of the Pierre Auger Observatory and is used to detect the radio emission of cosmic-ray air showers. These observations are compared to the data of the surface detector stations of the Observatory, which provide well-calibrated information on the cosmic-ray energies and arrival directions. The response of the radio stations in the 30 to 80 MHz regime has been thoroughly calibrated to enable the reconstruction of the incoming electric field. For the latter, the energy deposit per area is determined from the radio pulses at each observer position and is interpolated using a two-dimensional function that takes into account signal asymmetries due to interference between the geomagnetic and charge-excess emission components. The spatial integral over the signal distribution gives a direct measurement of the energy transferred from the primary cosmic ray into radio emission in the AERA frequency range. We measure 15.8 MeV of radiation energy for a 1 EeV air shower arriving perpendicularly to the geomagnetic field. This radiation energy -- corrected for geometrical effects -- is used as a cosmic-ray energy estimator. Performing an absolute energy calibration against the surface-detector information, we observe that this radio-energy estimator scales quadratically with the cosmic-ray energy as expected for coherent emission. We find an energy resolution of the radio reconstruction of 22% for the data set and 17% for a high-quality subset containing only events with at least five radio stations with signal.Comment: Replaced with published version.     Measurement of the Radiation Energy in the Radio Signal of Extensive Air Showers as a Universal Estimator of Cosmic-Ray Energy

    We measure the energy emitted by extensive air showers in the form of radio emission in the frequency range from 30 to 80 MHz. Exploiting the accurate energy scale of the Pierre Auger Observatory, we obtain a radiation energy of 15.8 \pm 0.7 (stat) \pm 6.7 (sys) MeV for cosmic rays with an energy of 1 EeV arriving perpendicularly to a geomagnetic field of 0.24 G, scaling quadratically with the cosmic-ray energy. A comparison with predictions from state-of-the-art first-principle calculations shows agreement with our measurement. The radiation energy provides direct access to the calorimetric energy in the electromagnetic cascade of extensive air showers. Comparison with our result thus allows the direct calibration of any cosmic-ray radio detector against the well-established energy scale of the Pierre Auger Observatory.Comment: Replaced with published version.
