    Polyphenols: Nature's Gift as dietary phytoconstituents against different human ailments

    Recently, researches and scientists are showing a lot of interests in bioactive compounds of plants and its products as these constituents are of high valued. Polyphenols are phytochemical constituents which are the integral components in plants and their products that are associated with defensive mechanism against infections and various oxidative stress by free radicals. Green tea, fruits, vegetables, cereals and red wines are rich sources of polyphenolic constituents which attributes prevention from infections and diseases because of its antioxidant properties, anti-diabetic properties, and radical scavenging properties. Oxidative stress which is the primary reason for different ailments in humans is due to the free radicals that are present even during the normal health condition. Polyphenolic acids like cinnamic acid, romarinic acid, stilbenes like resveratrol, flavonoids like catechin, taxifolin, quercetin, and lignans like sesamin, pinoresinol, podophyllotoxin etc. are found to be effective against wide range of human diseases like oxidative stress, cardiovascular disorder, neurodegenerative diseases, aging, and cancer. These phytoconstituents prevent the diseases and provide relieving sensation via different mechanisms. Here, the study shows the importance of polyphenols with respect to the relevance of human health. As there are the promising applications of various bioactive constituents in a wide range of disease, further research should be encouraged on the mechanism of action and bioavailability of polyphenols

    The development and psychometric properties of oral health assessment instruments used by non-dental professionals for nursing home residents : a systematic review

    Background: Globally, oral health status of the geriatric population residing in nursing homes is poor. The integration of non-dental professionals is vital to monitor oral health, early identification and triaging of oral health problems, and timely referral to dental professionals. The aims of this systematic review were to provide a summary on the development and characteristics of oral health assessment instruments currently used by non-dental professionals for nursing home residents, and to perform a critical appraisal of their psychometric properties. Methods: This review was conducted as per the PRISMA guidelines. CINHAL (EBSCO), Medline (Ovid), and EMBASE (Ovid) were searched systematically. Two reviewers independently screened the title, abstract, and full text of the studies as per the eligibility criteria. Studies describing oral health assessment instruments used to assess oral health of nursing home residents by non-dental professionals were included. Using a methodological framework, each instrument was evaluated for purpose, content, and psychometric properties related to validity, reliability, feasibility, generalisability, and responsiveness. Additionally, the reporting quality assessment of each included study was performed according to the SURGE guidelines. Results: Out of the 819 screened articles, 10 studies were included in this review. The 10 identified instruments integrated 2 to 12 categories to assess oral health, which was scored on a 2 to 5-point scale. However, the measurement content varied widely, and none were able to comprehensively measure all aspects of oral health. Three measurement approaches were identified: performance- based assessment, direct inspection of the oral health status, and interview measures. Only eight instruments provided quality assessment on the basis of validity, reliability, feasibility and generalisability, whereas three instruments- Brief Oral Health Status Examination, Dental Hygiene Registration, and Oral Health Assessment Tool reported good methodological quality on at least one assessment criteria. Conclusions: None of the instruments identified in this review provided a comprehensive assessment of oral health, while three instruments appeared to be valid and reliable. Nonetheless, continuous development of instruments is essential to embrace the complete spectrum of oral health and address the psychometric gaps

    Challenges and opportunities for nurses during COVID-19 pandemic

    Background: Pandemic of COVID-19 coincided with the year of Nurses and Midwives 2020. In war against invisible that has been ongoing during this pandemic, nurses play crucial role. As of June 2020, more than 600 nurses have lost their lives due to COVID-19. Nursing practice takes patients’ safety at paramount through continuum of testing, quarantine and isolation. Thus, there is need to review challenges that pandemic has posed as well as the avenues of opportunities that it has opened for nurses. Method: Review of existing literature on nurses, their challenges and opportunities during COVID-19 was done using electronic database. Freely accessible literatures in English language that clearly mentioned on challenges and opportunities of nurses during COVID-19 were included in the study. Result: One of thechallenges that nurses face during COVID-19 is workforce shortage. With increasing number of people infected with COVID-19, it outstretches the existing nursing health workforce, and disproportionately in the low and middle incomes countries. They are at risk of infection and have high job demand. The incidents of suicide among nurses are representation of physical distress and psychological trauma. Role of nurses in patients care during COVID-19 can be under-rated and stigmatized. Internationally trained nurses and minorities nurses are at increased risk of discrimination. Despite these challenges, COVID-19 puts forward opportunities for nurses on implementing humanistic theory in practice, self-esteem and sharing of real time stories, contingency management and creativity. It opens platform for leading infection prevention measures and advocacy of profession. Conclusion: Being the pertinent stakeholders in patient care, nurses’ faces different challenges as well as some opportunities. COVID-19 has opened avenues for advocacy of nursing profession to get acknowledged for their dedication, perseverance, high morale and contribution during the pandemic management

    Prevalence of dengue and diversity of cultivable bacteria in vector Aedes aegypti (L.) from two dengue endemic districts, Kanchanpur and Parsa of Nepal

    Background: Dengue fever, an endemic arboviral disease, represents one of the major public health concerns in Nepal. It is transmitted by bites of infected Aedes aegypti and Aedes albopictus , the former being primary vector. The bacterial community plays a significant role in biology of mosquitoes; however, the bacterial communities of primary vector A. aegypti remain unstudied in Nepal. The study was designed to determine the rate of dengue seropositivity and to explore the bacterial diversity of A. aegypti from dengue endemic districts, Kanchanpur and Parsa of Nepal. Methods: A cross-sectional study was conducted between June 2013 and November 2013 at two hospitals of Kanchanpur and Parsa. A total of 221 serum samples were collected from patients (inpatients and outpatients) suspected of suffering from dengue fever and attending Mahakali Zonal Hospital, Mahendranagar, Kanchanpur, and Narayani Zonal Hospital, Birgunj, Parsa. Detection of anti-dengue IgM was performed by using human dengue IgM capture ELISA. The larvae and pupae of mosquitoes (A. aegypti) were collected, reared, and emerged. Then, the bacteria were isolated and identified from the gut of identified mosquitoes by using standard methods. Results: Out of total 221 serum samples collected from patients suspected of suffering from dengue fever, 34 (15.38%) were positive for anti-dengue IgM. Gram-negative bacteria were isolated in largest proportion (63%) followed by gram-positive cocci (23.27%) and gram-positive rods (13.73%). The most common cultivable bacteria isolated were Staphylococcus spp., Pseudomonas spp., and Acinetobacter spp. The average bacterial load in the vectors was 3.91 7 104 cfu/ml. Conclusions: High rate of anti-dengue IgM seropositivity was reported in our study. The environmental bacteria were predominantly isolated and identified in A. aegypti. The paratransgenic approach to control vector might be possible by spreading the genetically modified bacteria in larval habitat or shelter of adult mosquitoes

This article examines challenges faced by countries while issuing compulsory licensing; waiver decision with regard to Article 31(f) and 31 (h) of TRIPS Agreement. The article further analyses the causes as to why the waiver solution has not been as effective as it was hoped and envisages an increase in its importance in the coming days

    Serological and Entomological Study of Dengue in Dang and Chitwan Districts of Nepal.

    A descriptive cross-sectional study was carried out among 264 suspected dengue patients in two districts (Dang and Chitwan) of Nepal from June 2013 to November 2013. The anti-dengue IgM positivity was found to be (51/264)19.31% by capture ELISA, of which 21 (41.2%) were male and 30 (58.8%) were female. Symptoms of seropositive cases were fever, anorexia, nausea, headache, retro-orbital pain, skin rashes, and myalgia. Hematological features like thrombocytopenia and leucopenia were found to be significantly associated with the dengue fever (DF). Discarded tires were found as the commonest breeding habitats for the dengue vectors. Higher sero-positivity was recorded from the area having higher Breteau index (BI). The pH, chloride ion concentration and the salinity of the water from breeding habitats were found to be ranging from 6.9±0.82 to 8, 103.33±17.52 mg/L to 140.65 mg/L, and 0.19±0.032 ppt to 0.25 ppt respectively. This study may be helpful for the health authorities and public health workers for early diagnosis of DF and for the improved preventive measures to be adopted in the epidemic and possible epidemic areas

    Knowledge, Attitude, and Practice of Antibiotic Use and Resistance among Poultry Farmers in Nepal

    The abuse and misuse of antibiotics is one of the main drivers of antimicrobial resistance (AMR). Globally, AMR in food-producing animals is a significant public health concern. This study, therefore, assessed the knowledge, attitudes, and practices related to antibiotic usage (AMU) and AMR among poultry farmers in Nepal. We conducted a cross-sectional survey of 605 poultry farmers from six districts of Nepal from May to June 2022 to assess the status of knowledge, attitude, as well as practices toward prudent antibiotic usage (AMU) and AMR. The majority of the participants in our study were from the Chitwan district (31.6%; n = 191/605), aged 30–44 (54.2%; n = 328/605), males (70.4%; n = 426/605), and farmers with a higher secondary (28.76%; n = 174/605) level of education. The tetracyclines (28%, n = 228/828), aminoglycosides (23%, n = 188/828), and fluoroquinolones (15%, n = 126/828) were the most used antibiotics classes among poultry farmers. Although 87.8% (n = 531/605) of poultry farmers used antibiotics, 49.8% (n = 301/605) of them were aware of AMR, and 55.7% (n = 337/605) knew that the misuse of antimicrobials could affect human and environmental health. There were significant differences in the knowledge, attitude, and practices toward prudent AMU and AMR among farmers who reared different birds. The mean knowledge, attitude, and practice score of the respondents were 7.81 ± 3.26, 5.8 ± 2.32, and 7.59 ± 3.38 when measured on a scale of 12, 10, and 15, respectively. Based on a cut-off of 75% of the maximum score, 49.4% (n = 299/605), 62.8% (n = 380/605), and 12.73% (n = 77/605) of the respondents had good knowledge, attitude, and practices toward prudent AMU and AMR, respectively. The multivariable logistic regression analyses revealed that the positive predictors of good knowledge and attitude were male gender, higher level of education, district, and the types of birds (layers). Similarly, those of the male gender (OR: 3.36; 95% CI: 1.38–8.20; p = 0.008) and those that rear layers (OR: 4.63; 95% CI: 1.75–12.25; p = 0.003) were more likely to practice prudent usage of antimicrobials. The findings of this study show poor practice toward prudent antibiotic usage despite good knowledge of AMR. This study provides essential baseline data on the knowledge, attitudes, and practices of poultry farmers in Nepal and offers valuable insights that could help in the design of interventions and policies aimed at addressing illicit AMU and AMR in poultry in Nepal
