    Vulvar Merkel Carcinoma: A Case Report

    This is a new case of Merkel cell carcinoma of the vulva. It is a rare neuroendocrine carcinoma with an aggressive behavior. Because of its rarity in this location, it is not clear whether it behaves differently from the usual neuroendocrine carcinomas of the skin. A case of a 63-year-old patient with vulvar Merkel carcinoma is presented. The clinical presentation, microscopic and immunohistochemical features, and treatment are discussed

    Organizational, regulatory and legal aspects оf european integration of higher medical education in Ukraine: a critical review

    The aim: The main purpose of this research is to conduct the theoretical and applied study of the legal profile enforcement for state attestation of medical graduates, the relevant legislative system, and its effectiveness; to identify the issues of special legislative requirements, as well as the search for ways to resolve it. Materials and methods: Basic methods: analysis, synthesis and comparison. Object of research: system of state certification of graduates of medical sciences. Investigation of this topic in the paper is carried out in the following logical sequence: firstly, the basic principles of the state integrated qualification exam were determined and analyzed; then the legal enforcement for integrated state qualification exam of master’s degree in discipline “22 Health Care” was considered. Also, a critical review of Ukrainian normative base for holding the state attestation of medical graduates was conducted. Conclusions: In view of foregoing, we can make a conclusion that in the modern globalization world, competition of national educational services should be ensured by a range of different activities: legal, organizational, financial, etc. The legal direction, domestic rules should correspond to similar, uniform rules and practices of the most effective state (regional, international) systems

    In vitro fermentation of ten cultivars of barley silage.

    The fermentation characteristics of whole-crop barley silages from ten different cultivars were evaluated by the in vitro gas production technique. The organic matter degradability of barley silage (62.9% in average) was comparable to those reported in our previous trials for oat (59.7%) and sorghum silages (65.5%); while the maximum gas production rate (5.38 ml/h in average) was slightly lower respect to oat (6.71 ml/h) and sorghum silage (6.74 ml/h). The mean nutritive value (4.00 MJ/kg DM) calculated on the basis of both chemical composition and in vitro fermentation data was comparable to that (4.16 MJ/kg DM) obtained in our previous research performed on corn silage, from crop sowed in the same area

    Clinical and Demographic Characteristics and Two-Year Efficacy and Safety Data of 508 Multiple Sclerosis Patients with Fingolimod Treatment

    Introduction: Fingolimod is the first oral immunomodulatory treatment used as secondary care therapy in the treatment of multiple sclerosis for the last 10 years. The objective of our study is to reveal the experiences of the first generic fingolimod active ingredient treatment in different centers across Turkey. Method: The first generic fingolimod efficacy and safety data of patients followed-up in 29 different clinical multiple sclerosis units in Turkey were analyzed retrospectively. Data regarding efficacy and safety of the patients were transferred to the data system both before the treatment and on the 6th, 12th and 24th month following the treatment. The data were analyzed using the IBM SPSS 20.00. P value of <0.05 was considered to be statistically significant. Results: A total of 508 multiple sclerosis patients, 331 of whom were women, were included in the study. Upon comparing the Expanded Disability Status values before and after the treatment, a significant decrease was observed, especially at month 6 and thereafter. Since bradycardia occurred in 11 of the patients (2.3%), the first dose had to be longer than 6 hours. During the observation of the first dose, no issues that could prevent the use of the drug occured. Side effects were seen in 49 (10.3%) patients during the course of fingolimod treatment. Respectively, the most frequent side effects were bradycardia, hypotension, headache, dizziness and tachycardia. Conclusion: The observed results regarding efficacy and safety were similar to clinical trial data in the literature and real life data in terms of the first equivalent with fingolimod active ingredient. © 2023, Turkish Neuropsychiatric Society.     Awn development in barley : data integration for effective virtual crop modelling

    Crop Systems Biology is a promising approach to fulfil challenges in improving complex traits. It combines modern functional genomics and traditional sciences approaches, such as crop physiology and biochemistry, to understand phenotype at the crop level (e.g. grain yield). Data integration thus plays a fundamental role in systems-based approaches and numerous studies are underway to deal with this issue. Our study shows an example of such a working strategy for the analysis of awn development in barley. The role of awn photosynthetic activity, particularly during stress periods, on grain yield has been largely proven: as a consequence awn characteristics have received quite a lot of attention from the breeders. Here, a mutant phenotype with a modified awn shape and increased awn area has been utilize as a black box in comparison with its wild type counterpart. System responses to both genotype and environment changes are being evaluated as a basis for application of virtual crop modelling

    Clinical characteristics, treatment modalities, and potential contributing and prognostic factors in patients with bone metastases from gynecological cancers: A systematic review

    The purpose of this study is to review the clinical characteristics, treatment modalities, and potential contributing and prognostic factors of bone metastases from gynecological cancers (GCs). A systematic literature search on PubMed, Scopus, Web of Science Core Collection and Cochrane Central Register of Controlled Trials databases was conducted. Thirty-one studies, all retrospective, were included in this review, for a total of 2880 patients with GC bone metastases. Primary tumors leading to bone metastases included endometrial cancer (EC), cervical cancer (CC), ovarian cancer (OC), uterine sarcoma (US) and vulvar cancer (VuC), mainly with an International Federation of Gynecology and Obstetrics (FIGO) Stage of III and IV. The main bone metastatic lesion site was the vertebral column, followed by the pelvic bone and lower extremity bones. The median survival rate after bone metastases diagnosis ranged from 3.0 to 45 months. The most frequent treatments were palliative and included radiotherapy and chemotherapy, followed by surgery. The findings of this review give a first dataset for a greater understanding of GC bone metastases that could help clinicians move toward a more “personalized” and thus more effective patient management

    Recovery of phenolic compouns by solid-state fermentation from grape pomace and wheat bran.

    The grape pomace and wheat bran is rich in bioactive compounds that may be conjugated to the plant cell wall, making it difficult to recover. The objective of this study was to produce an enzymatic complex concomitant to the release of phenolics from grape pomace and wheat bran, by solid-state fermentation (SSF), using the mutant strain Aspergillus niger 3T5B8. Both substrates showed potential for the production of hydrolytic enzymes, mainly for xylanase and ?-glucosidase enzymes with grape pomace and wheat bran, respectively.
