    Universal Susceptibility Variations in 1+1 Dimensional Vortex Glass

    We model a planar array of fluxlines as a discrete solid-on-solid model with quenched disorder. Simulations at finite temperatures are made possible by a new algorithm which circumvents the slow glassy dynamics encountered by traditional Metropolis Monte Carlo algorithms.     The Safety Profile of Intentional or Iatrogenic Sacrifice of the Artery of Adamkiewciz and Its Vicinity’s Spinal Segmental Arteries: A Systematic Review

    Study Design: Systematic review. Objectives: There is paucity of consensus on whether (1) the artery of Adamkiewicz (AoA) and (2) the number of contiguous segmental spinal arteries (SSAs) that can be safely ligated without causing spinal cord ischemia. The objective of this review is to determine the risk of motor neurological deficits from iatrogenic sacrifice of the (1) AoA and (2) its vicinity’s SSAs. Methods: Systematic review of the spine and vascular surgery was carried out in accordance to PRISMA guidelines. Outcomes in terms of risk of postoperative motor neurological deficit with occlusion of the AoA, bilateral contiguous SSAs, or unilateral contiguous SSAs were analyzed. Results: Ten articles, all retrospective case series, were included. Three studies (total N = 50) demonstrated a postoperative neurological deficit risk of 4.0% when the AoA is occluded. When 1 to 6 pairs of SSAs (without knowledge of AoA location) were ligated, the postoperative neurological deficit risk was 0.6%, as compared with 5.4% when more than 6 bilateral pairs of SSAs were ligated (relative risk [RR] = 0.105, 95% CI 0.013-0.841, P =.0337). For unilateral ligation of SSAs of two to nine levels, the risk of postoperative neurological deficit does not exceed 1.3%. Conclusion: The current best evidence indicates that (1) occlusion of the AoA and (2) occlusion of up to 6 pairs of SSAs is associated with a low risk of postoperative neurological deficit. This limited number of low quality studies restrict the ability to draw definitive conclusions. Ligation of AoA and SSAs should only be undertaken when absolutely required to mitigate the small but devastating risk of paralysis

    Vortex Glass Phase and Universal Susceptibility Variations in Planar Array of Flux Lines

    Some of the properties of the low temperature vortex-glass phase of randomly-pinned flux lines in 1+1 dimensions are studied. The flux arrays are found to be sensitive to small changes in external parameters such as the magnetic field or temperature. These effects are captured by the variations in the magnetic response and noise, which have universal statistics and should provide an unambiguous signature of the glass phase.Comment: 11 pages and no figures; revtex 3.

    Thermodynamics of Mesoscopic Vortex Systems in 1+1 Dimensions

    The thermodynamics of a disordered planar vortex array is studied numerically using a new polynomial algorithm which circumvents slow glassy dynamics. Close to the glass transition, the anomalous vortex displacement is found to agree well with the prediction of the renormalization-group theory. Interesting behaviors such as the universal statistics of magnetic susceptibility variations are observed in both the dense and dilute regimes of this mesoscopic vortex system.Comment: 4 pages, REVTEX, 6 figures included.     Anomalous Fluctuations of Directed Polymers in Random Media

    A systematic analysis of large scale fluctuations in the low temperature pinned phase of a directed polymer in a random potential is described. These fluctuations come from rare regions with nearly degenerate ``ground states''. The probability distribution of their sizes is found to have a power law tail. The rare regions in the tail dominate much of the physics. The analysis presented here takes advantage of the mapping to the noisy-Burgers' equation. It complements a phenomenological description of glassy phases based on a scaling picture of droplet excitations and a recent variational approach with ``broken replica symmetry''. It is argued that the power law distribution of large thermally active excitations is a consequence of the continuous statistical ``tilt'' symmetry of the directed polymer, the breaking of which gives rise to the large active excitations in a manner analogous to the appearance of Goldstone modes in pure systems with a broken continuous symmetry.Comment: 59 pages including 8 figures ( REVTEX 3.0 )E-mail: [email protected]

    Interacting Arrays of Steps and Lines in Random Media

    The phase diagram of two interacting planar arrays of directed lines in random media is obtained by a renormalization group analysis. The results are discussed in the contexts of the roughening of reconstructed crystal surfaces, and the pinning of flux line arrays in layered superconductors.     Micro RNAs (miRNAs) regulate gene expression at the posttranscriptional level. Here we show that regulatory T (T reg) cells have a miRNA profile distinct from conventional CD4 T cells. A partial T reg cell–like miRNA profile is conferred by the enforced expression of Foxp3 and, surprisingly, by the activation of conventional CD4 T cells. Depleting miRNAs by eliminating Dicer, the RNAse III enzyme that generates functional miRNAs, reduces T reg cell numbers and results in immune pathology. Dicer facilitates, in a cell-autonomous fashion, the development of T reg cells in the thymus and the efficient induction of Foxp3 by transforming growth factor β. These results suggest that T reg cell development involves Dicer-generated RNAs

    Disorder-Induced Depinning Transition

    The competition in the pinning of a directed polymer by a columnar pin and a background of random point impurities is investigated systematically using the renormalization group method. With the aid of the mapping to the noisy-Burgers' equation and the use of the mode-coupling method, the directed polymer is shown to be marginally localized to an arbitrary weak columnar pin in 1+1 dimensions. This weak localization effect is attributed to the existence of large scale, nearly degenerate optimal paths of the randomly pinned directed polymer.     Myogenic cell proliferation and generation of a reversible tumorigenic phenotype are triggered by preirradiation of the recipient site

    Environmental influences have profound yet reversible effects on the behavior of resident cells. Earlier data have indicated that the amount of muscle formed from implanted myogenic cells is greatly augmented by prior irradiation (18 Gy) of the host mouse muscle. Here we confirm this phenomenon, showing that it varies between host mouse strains. However, it is unclear whether it is due to secretion of proliferative factors or reduction of antiproliferative agents. To investigate this further, we have exploited the observation that the immortal myogenic C2 C12 cell line forms tumors far more rapidly in irradiated than in nonirradiated host muscle. We show that the effect of preirradiation on tumor formation is persistent and dose dependent. However, C2 C12 cells are not irreversibly compelled to form undifferentiated tumor cells by the irradiated muscle environment and are still capable of forming large amounts of muscle when reimplanted into a nonirradiated muscle. In a clonal analysis of this effect, we discovered that C2 C12 cells have a bimodal propensity to form tumors; some clones form no tumors even after extensive periods in irradiated graft sites, whereas others rapidly form extensive tumors. This illustrates the subtle interplay between the phenotype of implanted cells and the factors in the muscle environment

    Effectiveness and cost-effectiveness of a web-based cardiac rehabilitation programme for people with chronic stable angina:protocol for the ACTIVATE (Angina Controlled Trial Investigating the Value of the 'Activate your heart' Therapeutic E-intervention) randomised controlled trial

    INTRODUCTION: Chronic stable angina is common and disabling. Cardiac rehabilitation is routinely offered to people following myocardial infarction or revascularisation procedures and has the potential to help people with chronic stable angina. However, there is insufficient evidence of effectiveness and cost-effectiveness for its routine use in this patient group. The objectives of this study are to compare the effectiveness and cost-effectiveness of the 'Activate Your Heart' cardiac rehabilitation programme for people with chronic stable angina compared with usual care.METHODS AND ANALYSIS: ACTIVATE is a multicentre, parallel-group, two-arm, superiority, pragmatic randomised controlled trial, with recruitment from primary and secondary care centres in England and Wales and a target sample size of 518 (1:1 allocation; allocation sequence by minimisation programme with built-in random element). The study uses secure web-based allocation concealment. The two treatments will be optimal usual care (control) and optimal usual care plus the 'Activate Your Heart' web-based cardiac rehabilitation programme (intervention). Outcome assessment and statistical analysis will be performed blinded; participants will be unblinded. Outcomes will be measured at baseline and at 6 and 12 months' follow-up. Primary outcome will be the UK version of Seattle Angina Questionnaire (SAQ-UK), physical limitations domain at 12 months' follow-up. Secondary outcomes will be the remaining two domains of SAQ-UK, dyspnoea, anxiety and depression, health utility, self-efficacy, physical activity and the incremental shuttle walk test. All safety events will be recorded, and serious adverse events assessed to determine whether they are related to the intervention and expected. Concurrent economic evaluation will be cost-utility analysis from health service perspective. An embedded process evaluation will determine the mechanisms and processes that explain the implementation and impacts of the cardiac rehabilitation programme.ETHICS AND DISSEMINATION: North of Scotland National Health Service Research Ethics Committee approval, reference 21/NS/0115.
