    A user preference perception model using data mining on a Web-based Environment

    In a competitive environment, how to provide the information and products to meet the requirements of customers and improve the customer satisfaction will be the key criteria to measure a companyā€™s competitiveness. Customer Relationship Management (CRM) becomes an important issue in any business market gradually. Using information technology, businesses can achieve their requirements for one to one marketing more efficiently with lower cost, labor and time. In this paper, we proposed a user preference perception model by using data mining technology on a web-based environment. First, the usersā€™ web browse records are aggregated. Second, fuzzy set theory and most sequential pattern mining algorithm are used to infer the usersā€™ preference changes in a period. After the test had processed, we use the on-line questionnaire to investigate the customer satisfaction degree from all participators. The results show that the degree of satisfaction was up to 72% for receiving the new information of participants whose preferences had been changed. It indicates that the proposed system can effectively perceive the change of preference for users on a web environment

    Treatment selection for tonsillar squamous cell carcinoma

    AbstractBackgroundThe optimal treatment for tonsillar squamous cell carcinoma (SCC) remains controversial. The purpose of this study was to evaluate long-term treatment outcomes of patients with tonsillar SCC, in order to aid in appropriate treatment selection.MethodsWe conducted a retrospective chart review of 105 patients with curatively treated tonsillar SCC between January 1996 and December 2005. Forty-three patients (41.0%) underwent primary surgery with or without adjuvant therapy (primary surgery group), and 62 patients (59.0%) were treated with radiotherapy/chemoradiotherapy (RT/CRT, organ preservation group). Twenty patients (19%) received tumor tonsillectomy before definitive RT/CRT and were grouped into the organ preservation group.ResultsNo significant differences were observed between the primary surgery and organ preservation groups in terms of local control (pĀ =Ā 0.212), regional control (pĀ =Ā 0.684), distant metastasis (pĀ =Ā 0.627), 5-year disease-specific survival (DSS, pĀ =Ā 0.774), and overall survival rates (OS, pĀ =Ā 0.667). The rates of major complication (pĀ =Ā 0.216), long-term dependency on feeding tubes (pĀ =Ā 0.876), and tracheostomy (pĀ =Ā 0.401) were also similar. Advanced T classification (T3ā€“4) was the only factor associated with significantly worse DSS (pĀ =Ā 0.007) and OS (pĀ =Ā 0.012). However, there was also no difference in final treatment outcomes in T3ā€“4 patients regardless of whether they were treated with primary surgery or RT/CRT. In the organ preservation group, tumor tonsillectomy before RT/CRT did not improve local control (pĀ =Ā 0.520) or other treatment outcomes, including 5-year DSS (pĀ =Ā 0.707) and OS (pĀ =Ā 0.745).ConclusionBoth primary surgery and RT/CRT organ preservation are effective treatments for tonsillar SCC. Single modality treatment, either surgery or RT/CRT, can typically be provided for stage Iā€“II diseases. Although RT/CRT organ preservation is used more frequently for stage IIIā€“IV tonsillar SCC in recent years, primary surgery combined with adjuvant therapy still achieves equivalent outcomes. Multidisciplinary pretreatment counseling and the facilities and personnel available are therefore important for decision-making. In addition, if RT/CRT organ preservation is selected as the primary treatment, tumor tonsillectomy is not indicated

    Genomic sequencing and analyses of Lymantria xylina multiple nucleopolyhedrovirus

    <p>Abstract</p> <p>Background</p> <p>Outbreaks of the casuarina moth, <it>Lymantria xylina </it>Swinehoe (Lepidoptera: Lymantriidae), which is a very important forest pest in Taiwan, have occurred every five to 10 years. This moth has expanded its range of host plants to include more than 65 species of broadleaf trees. LyxyMNPV (<it>L. xylina </it>multiple nucleopolyhedrovirus) is highly virulent to the casuarina moth and has been investigated as a possible biopesticide for controlling this moth. LdMNPV-like virus has also been isolated from <it>Lymantria xylin</it>a larvae but LyxyMNPV was more virulent than LdMNPV-like virus both in NTU-LY and IPLB-LD-652Y cell lines. To better understand LyxyMNPV, the nucleotide sequence of the LyxyMNPV DNA genome was determined and analysed.</p> <p>Results</p> <p>The genome of LyxyMNPV consists of 156,344 bases, has a G+C content of 53.4% and contains 157 putative open reading frames (ORFs). The gene content and gene order of LyxyMNPV were similar to those of LdMNPV, with 151 ORFs identified as homologous to those reported in the LdMNPV genome. Two genes (Lyxy49 and Lyxy123) were homologous to other baculoviruses, and four unique LyxyMNPV ORFs (Lyxy11, Lyxy19, Lyxy130 and Lyxy131) were identified in the LyxyMNPV genome, including a <it>gag-like </it>gene that was not reported in baculoviruses. LdMNPV contains 23 ORFs that are absent in LyxyMNPV. Readily identifiable homologues of the gene <it>host range factor-1 </it>(<it>hrf-1</it>), which appears to be involved in the susceptibility of <it>L. dispar </it>to NPV infection, were not present in LyxyMNPV. Additionally, two putative <it>odv-e27 </it>homologues were identified in LyxyMNPV. The LyxyMNPV genome encoded 14 <it>bro </it>genes compared with 16 in LdMNPV, which occupied more than 8% of the LyxyMNPV genome. Thirteen homologous regions (<it>hr</it>s) were identified containing 48 repeated sequences composed of 30-bp imperfect palindromes. However, they differed in the relative positions, number of repeats and orientation in the genome compared to LdMNPV.</p> <p>Conclusion</p> <p>The gene parity plot analysis, percent identity of the gene homologues and a phylogenetic analysis suggested that LyxyMNPV is a Group II NPV that is most closely related to LdMNPV but with a highly distinct genomic organisation.</p

    Modified Weekly Cisplatin-Based Chemotherapy Is Acceptable in Postoperative Concurrent Chemoradiotherapy for Locally Advanced Head and Neck Cancer

    Background. Triweekly cisplatin-based postoperative concurrent chemoradiotherapy (CCRT) has high intolerance and toxicities in locally advanced head and neck cancer (LAHNC). We evaluated the effect of a modified weekly cisplatin-based chemotherapy in postoperative CCRT. Methods. A total of 117 patients with LAHNC were enrolled between December 2007 and December 2012. Survival, compliance/adverse events, and independent prognostic factors were analyzed. Results. Median follow-up time was 30.0 (3.1ā€“73.0) months. Most patients completed the entire course of postoperative CCRT (radiotherapy ā‰„ 60ā€‰Gy, 94.9%; ā‰„6 times weekly chemotherapy, 75.2%). Only 17.1% patients required hospital admission. The most common adverse effect was grade 3/4 mucositis (28.2%). No patient died due to protocol-related adverse effects. Multivariate analysis revealed the following independent prognostic factors: oropharyngeal cancer, extracapsular spread, and total radiation dose. Two-year progression-free survival and overall survival rates were 70.9% and 79.5%, respectively. Conclusion. Modified weekly cisplatin-based chemotherapy is an acceptable regimen in postoperative CCRT for LAHNC

    Serotonin receptor HTR6-mediated mTORC1 signaling regulates dietary restriction-induced memory enhancement

    Dietary restriction (DR; sometimes called calorie restriction) has profound beneficial effects on physiological, psychological, and behavioral outcomes in animals and in humans. We have explored the molecular mechanism of DR-induced memory enhancement and demonstrate that dietary tryptophan-a precursor amino acid for serotonin biosynthesis in the brain-and serotonin receptor 5-hydroxytryptamine receptor 6 (HTR6) are crucial in mediating this process. We show that HTR6 inactivation diminishes DR-induced neurological alterations, including reduced dendritic complexity, increased spine density, and enhanced long-term potentiation (LTP) in hippocampal neurons. Moreover, we find that HTR6-mediated mechanistic target of rapamycin complex 1 (mTORC1) signaling is involved in DR-induced memory improvement. Our results suggest that the HTR6-mediated mTORC1 pathway may function as a nutrient sensor in hippocampal neurons to couple memory performance to dietary intake

    Development of Interstitial Lung Disease Among Patients With Atrial Fibrillation Receiving Oral Anticoagulants in Taiwan.

    ImportanceThere are emerging concerns from case reports and pharmacovigilance analyses of a possible risk of interstitial lung disease (ILD) associated with the use of factor Xa (FXa) inhibitors.ObjectiveTo evaluate the risk of incident ILD associated with the use of oral anticoagulants (OACs) in patients with nonvalvular atrial fibrillation (NVAF).Design, setting, and participantsThis nationwide retrospective cohort study used data from the Taiwan National Health Insurance Research Database. Patients with NVAF without preexisting lung disease who received OACs from June 1, 2012, to December 31, 2017, were included. Propensity score stabilized weighting (PSSW) was used to balance covariates across the medication groups (FXa inhibitors, dabigatran, and warfarin, with warfarin as the reference). Patients were followed up from the drug index date until the onset of ILD, death, or end of the study (December 31, 2019), whichever occurred first. Data were analyzed from September 11, 2021, to August 3, 2022.ExposuresPatients with NVAF were treated with FXa inhibitors, dabigatran, or warfarin.Main outcomes and measuresNew-onset idiopathic ILD.ResultsAmong the 106ā€Æ044 patients (mean [SD] age, 73.4 [11.9] years; 59 995 men [56.6%]) included in the study, 64ā€Æ555 (60.9%) received FXa inhibitors (apixban [nā€‰=ā€‰15ā€Æ386], edoxaban [nā€‰=ā€‰12ā€Æ413], and rivaroxaban [nā€‰=ā€‰36ā€Æ756]), 22ā€Æ501 (21.2%) received dabigatran, and 18ā€Æ988 (17.9%) received warfarin at baseline. The FXa inhibitors were associated with a higher risk of incident ILD (0.29 vs 0.17 per 100 patient-years; hazard ratio, 1.54 [95% CI, 1.22-1.94]; Pā€‰Conclusions and relevanceResults of this study suggest that FXa inhibitors were associated with lung injury among patients with NVAF who were treated with OACs. Physicians should be vigilant in monitoring for any potential adverse lung outcomes associated with the use of these drugs
