    Discovery of unusual pulsations in the cool, evolved Am stars HD 98851 and HD 102480

    The chemically peculiar (CP) stars HD 98851 and HD 102480 have been discovered to be unusual pulsators during the ``Naini Tal Cape Survey'' programme to search for pulsational variability in CP stars. Time series photometric and spectroscopic observations of these newly discovered stars are reported here. Fourier analyses of the time series photometry reveal that HD 98851 is pulsating mainly with frequencies 0.208 mHz and 0.103 mHz, and HD 102480 is pulsating with frequencies 0.107 mHz, 0.156 mHz and 0.198 mHz. The frequency identifications are all subject to 1 d1^{-1} cycle count ambiguities. We have matched the observed low resolution spectra of HD 98851 and HD 102480 in the range 3500-7400 \AA with theoretical synthetic spectra using Kurucz models with solar metallicity and a micro-turbulent velocity 2 km s1^{-1}. These yield Teff=7000±250T_{eff}=7000\pm250 K, log g=3.5±0.5g=3.5 \pm 0.5 for HD 98851 and Teff=6750±250T_{eff} = 6750 \pm 250 K, log g=3.0±0.5g = 3.0 \pm 0.5 for HD 102480. We determined the equivalent H-line spectral class of these stars to be F1 IV and F3 III/IV, respectively. A comparison of the location of HD 98851 and HD 102480 in the HR diagram with theoretical stellar evolutionary tracks indicates that both stars are about 1-Gyr-old, 2-MM_{\odot} stars that lie towards the red edge of the δ\delta Sct instability strip. We conclude that HD 98851 and HD 102480 are cool, evolved Am pulsators. The light curves of these pulsating stars have alternating high and low amplitudes, nearly harmonic (or sub-harmonic) period ratios, high pulsational overtones and Am spectral types.     Quantifying tropical cyclone's effect on the biogeochemical processes using profiling float observations in the Bay of Bengal

    Physical and biogeochemical observations from an autonomous profiling Argo float in the Bay of Bengal show significant changes in upper ocean structure during the passage of Tropical Cyclone (TC) Hudhud (7–14 October 2014). TC Hudhud mixed water from a depth of about 50 m into the surface layers through a combination of upwelling and turbulent mixing. Mixing was extended into the depth of nutricline, the oxycline and the subsurface‐chlorophyll‐maximum; thus had a strong impact on the biogeochemistry of the upper ocean. Before the storm, the near‐surface layer was nutrient depleted and was thus oligotrophic with the chlorophyll‐a concentration of less than 0.15 mg m‐3. Storm mixing initially increased the chlorophyll by 1.4 mg m‐3, increased the surface nitrate concentration to about 6.6 μM kg‐1, and decreased the sub‐surface dissolved oxygen (30–35 m) to 31 % of saturation (140 μM). These conditions were favorable for phytoplankton growth resulting in an estimated increase in primary productivity averaging 1.5 g C m‐2 day‐1 over 15 days. During this bloom, chlorophyll‐a increased by 3.6 mg m‐3, and dissolved oxygen increased from 111 % to 123 % of saturation. Similar observations during TC Vardah (6–12 December 2016) showed much less mixing. Our analysis suggests that relatively small (high) translation speed and presence of cold (warm) core eddy leads to strong (weak) oceanic response during TC Hudhud (TC Vardah). Thus, although cyclones can cause strong biogeochemical responses in the Bay of Bengal, the strength of response depends on the properties of the storm and the prevailing upper ocean structure such as presence of mesoscale eddies

    Global, regional, and national burden of chronic kidney disease, 1990–2017 : a systematic analysis for the Global Burden of Disease Study 2017

    Background Health system planning requires careful assessment of chronic kidney disease (CKD) epidemiology, but data for morbidity and mortality of this disease are scarce or non-existent in many countries. We estimated the global, regional, and national burden of CKD, as well as the burden of cardiovascular disease and gout attributable to impaired kidney function, for the Global Burden of Diseases, Injuries, and Risk Factors Study 2017. We use the term CKD to refer to the morbidity and mortality that can be directly attributed to all stages of CKD, and we use the term impaired kidney function to refer to the additional risk of CKD from cardiovascular disease and gout. Methods The main data sources we used were published literature, vital registration systems, end-stage kidney disease registries, and household surveys. Estimates of CKD burden were produced using a Cause of Death Ensemble model and a Bayesian meta-regression analytical tool, and included incidence, prevalence, years lived with disability, mortality, years of life lost, and disability-adjusted life-years (DALYs). A comparative risk assessment approach was used to estimate the proportion of cardiovascular diseases and gout burden attributable to impaired kidney function. Findings Globally, in 2017, 1·2 million (95% uncertainty interval [UI] 1·2 to 1·3) people died from CKD. The global all-age mortality rate from CKD increased 41·5% (95% UI 35·2 to 46·5) between 1990 and 2017, although there was no significant change in the age-standardised mortality rate (2·8%, −1·5 to 6·3). In 2017, 697·5 million (95% UI 649·2 to 752·0) cases of all-stage CKD were recorded, for a global prevalence of 9·1% (8·5 to 9·8). The global all-age prevalence of CKD increased 29·3% (95% UI 26·4 to 32·6) since 1990, whereas the age-standardised prevalence remained stable (1·2%, −1·1 to 3·5). CKD resulted in 35·8 million (95% UI 33·7 to 38·0) DALYs in 2017, with diabetic nephropathy accounting for almost a third of DALYs. Most of the burden of CKD was concentrated in the three lowest quintiles of Socio-demographic Index (SDI). In several regions, particularly Oceania, sub-Saharan Africa, and Latin America, the burden of CKD was much higher than expected for the level of development, whereas the disease burden in western, eastern, and central sub-Saharan Africa, east Asia, south Asia, central and eastern Europe, Australasia, and western Europe was lower than expected. 1·4 million (95% UI 1·2 to 1·6) cardiovascular disease-related deaths and 25·3 million (22·2 to 28·9) cardiovascular disease DALYs were attributable to impaired kidney function. Interpretation Kidney disease has a major effect on global health, both as a direct cause of global morbidity and mortality and as an important risk factor for cardiovascular disease. CKD is largely preventable and treatable and deserves greater attention in global health policy decision making, particularly in locations with low and middle SDI

    Expression of Ixodes scapularis Antifreeze Glycoprotein Enhances Cold Tolerance in Drosophila melanogaster

    Drosophila melanogaster experience cold shock injury and die when exposed to low non-freezing temperatures. In this study, we generated transgenic D. melanogaster that express putative Ixodes scapularis antifreeze glycoprotein (IAFGP) and show that the presence of IAFGP increases the ability of flies to survive in the cold. Male and female adult iafgp-expressing D. melanogaster exhibited higher survival rates compared with controls when placed at non-freezing temperatures. Increased hatching rates were evident in embryos expressing IAFGP when exposed to the cold. The TUNEL assay showed that flight muscles from iafgp-expressing female adult flies exhibited less apoptotic damage upon exposure to non-freezing temperatures in comparison to control flies. Collectively, these data suggest that expression of iafgp increases cold tolerance in flies by preventing apoptosis.     Clinical outcomes and patient-matched molecular composition of relapsed medulloblastoma

Methods: Children and infants enrolled on either SJMB03 (NCT00085202) or SJYC07 (NCT00602667) trials who experienced medulloblastoma relapse were analyzed for clinical outcomes, including anatomic and temporal patterns of relapse and postrelapse survival. A largely independent, paired molecular cohort was analyzed by DNA methylation array and next-generation sequencing. Results: A total of 72 of 329 (22%) SJMB03 and 52 of 79 (66%) SJYC07 patients experienced relapse with significant representation of Group 3 and wingless tumors. Although most patients exhibited some distal disease (79%), 38% of patients with sonic hedgehog tumors experienced isolated local relapse. Time to relapse and postrelapse survival varied by molecular subgroup with longer latencies for patients with Group 4 tumors. Postrelapse radiation therapy among previously nonirradiated SJYC07 patients was associated with long-term survival. Reirradiation was only temporizing for SJMB03 patients. Among 127 patients with patient-matched tumor pairs, 9 (7%) experienced subsequent nonmedulloblastoma CNS malignancies. Subgroup (96%) and subtype (80%) stabilities were largely maintained among the remainder. Rare subgroup divergence was observed from Group 4 to Group 3 tumors, which is coincident with genetic alterations involving MYC, MYCN, and FBXW7. Subgroup-specific patterns of alteration were identified for driver genes and chromosome arms. Conclusion: Clinical behavior of relapsed medulloblastoma must be contextualized in terms of up-front therapies and molecular classifications. Group 4 tumors exhibit slower biological progression. Utility of radiation at relapse is dependent on patient age and prior treatments. Degree and patterns of molecular conservation at relapse vary by subgroup.     Enteral lactoferrin supplementation for very preterm infants : a randomised placebo-controlled trial

    Background: Infections acquired in hospital are an important cause of morbidity and mortality in very preterm infants. Several small trials have suggested that supplementing the enteral diet of very preterm infants with lactoferrin, an antimicrobial protein processed from cow's milk, prevents infections and associated complications. Methods: In this randomised, placebo-controlled trial, very preterm infants (born before 32 weeks' gestation) in 37 UK hospitals were allocated randomly (1:1) within 72 hours after birth to receive enteral bovine lactoferrin (150 mg/kg/day; maximum 300 mg/day) versus sucrose (same dose) once daily until 34 weeks' postmenstrual age. Web-based randomisation minimised for recruitment site, gestation (completed weeks), sex, and single versus multifetal pregnancy. Parents, caregivers and outcomes assessors were unaware of group assignment. The primary outcome was microbiologically-confirmed or clinically-suspected lateonset infection (occurring >72 hours after birth). The trial was registered with the International Standard Randomised Controlled Trial Number 88261002. Findings: We recruited 2203 participants between May 2014 and September 2017. Four infants had consent withdrawn or unconfirmed leaving 1098 infants in the lactoferrin group and 1101 in the sucrose group. Primary outcome data for 2182 infants were available for inclusion in the intention-to-treat analyses. In the intervention group, 316/1093 (28.9%) infants acquired a late-onset infection versus 334/1089 (30.7%) in the control group: risk ratio (RR) adjusted for minimisation factors 0.95 (95% confidence interval [CI] 0.86, 1.04). Pre-specified subgroup analyses did not show statistically significant interactions for gestation at birth (completed weeks') or type of enteral milk received (human, formula, or both). Interpretation: Enteral supplementation with bovine lactoferrin does not reduce the incidence of late-onset infection in very preterm infants.
