Partnership for Sustainability: A Case Study of BAZ County
The purpose of the study is to present and discuss a project which can solve the environmental
sustainability problems of companies involving students in their work. The project strengthen
the partnership among the University of Miskolc and the companies operating in the region,
enrich the knowledge of entrepreneurs, improve their environmental performance and help
our students find jobs in the area. With the help of the communication targeted in the project
sustainability can be integrated into the economic and enterprise policy of the region. In the
study an attempt is made to create a new university-company partnership model in order to
promote the practise of sustainability
Experimental and clinical rehabilitation of organic dysphonia
A disszertáciĂł cĂ©lja a hangszálbĂ©nulás Ă©s a teljes gĂ©geeltávolĂtás után lĂ©trejött hangkĂ©pzĂ©si zavarok kĂsĂ©rletes Ă©s klinikai rehabilitáciĂłjának vizsgálata.
Organikus diszfóniát okozhatnak fejlődési rendellenességek, gyulladások, jó és rosszindulatú, gége, hypopharynx daganatok és nyaki térfoglaló folyamatok ( pl. superficialis angiomixómás elváltozások ).
Ideg-izom lebennyel törtĂ©nĹ‘ szelektĂv reinnerváciĂłs technikát alkalmaztuk a cricoarytenoideus posterior izom denerváciĂłja (nervus laryngeus recurrens átmetszĂ©s) után. A reinnerváciĂł cĂ©ljábĂłl ansa cervicalis-sternohyoideus izom lebeny rekonstrukciĂłt vĂ©geztĂĽnk öt keverĂ©k kutya esetĂ©n. Három kutyán csak denerváciĂłs műtĂ©t törtĂ©nt.
Egy év elteltével videolaringoszkópos, EMG, és szövettani vizsgálatokat végeztünk.
A videolaringoszkópos felvételek egyértelműen igazolták az ideg-izom lebeny által reinnervált CAP izom működését. Ezt támasztotta alá az EMG leleteken látott kiváltott polifázisos potenciálok megjelenése. Immunhisztokémiai reakcióval (antineurofilament antitest+biotin) neurofilamentumokat és motoros véglemezeket lehetett látni mind az 5 állat esetén mindkét oldalon.
Az ideg-izom lebeny technika alkalmazása lehetővé teheti az arytenoidectomia és a laterofixáció elkerülését egy-, vagy kétoldali gégebénulások esetén, ezáltal a hangminőség és az életminőség javulása érhető el ezen betegek esetén.
A disszertáciĂł továbbiakban teljes gĂ©geeltávolĂtáson átesett betegek hangrehabilitáciĂłjával, azon belĂĽl is a nyelĹ‘csĹ‘beszĂ©d tanĂtásával, az elĂ©rt hangminĹ‘sĂ©g további javĂtásával foglalkozik. A nyelĹ‘csĹ‘beszĂ©d a hangrehabilitáciĂł legtermĂ©szetesebb formája. A nyelĹ‘csĹ‘ klub cĂ©lja a betegek pszichĂ©s támogatása mellett a helyes lĂ©gvĂ©teli, nyelĂ©si Ă©s hangkĂ©pzĂ©si technikák megtanĂtása. A beszĂ©dtanulás sikeressĂ©gĂ©t szubjektĂv Ă©s objektĂv mĂłdszerekkel - hangelemzĹ‘ program segĂtsĂ©gĂ©vel - elemeztĂĽk. A nyelĹ‘csĹ‘ben kialakult, hangszalagkĂ©nt működĹ‘ nyálkahártya redĹ‘ működĂ©sĂ©t endoszkĂłpos technikával figyeltĂĽk meg. Az intonáciĂłt, beszĂ©dfolytonosságot, beszĂ©dgyorsaságot Ă©s beszĂ©dĂ©rthetĹ‘sĂ©get minden esetben jĂłnak, Ă©s sok esetben közel normálisnak vĂ©lemĂ©nyeztĂ©k a foniáter Ă©s fĂĽl-orr-gĂ©gĂ©sz vizsgálĂłk. Az objektĂv hangelemzĂ©s során az F0 Ă©rtĂ©kek mind a fĂ©rfiak Ă©s nĹ‘k esetĂ©n magasabbak voltak az irodalomban korábban leĂrtaknál. A korábbi megfigyelĂ©sekhez kĂ©pest a hang frekvencia tartomány bĹ‘vĂĽlĂ©sĂ©t sikerĂĽlt elĂ©rni. A korábbi quint helyett egy oktávra sikerĂĽlt bĹ‘vĂteni a hangtartományt.
A garatseb zárása során alkalmazott megfelelő sebészi technika befolyásolta az elért hangminőséget. A legjobb eredményeket a ’gomb’ alakú neoglottis formával rendelkező betegek esetén lehetett megfigyelni.The aim of the thesis was to examine the experimental and clinical aspects of voice rehabilitation in the cases of vocal cord paralysis and after total laryngectomy.
Organic dysphonia can be caused by congenital malformations, inflammations, benign or malignant laryngeal, hypopharyngeal or cervical tumors such as superficial angiomyxoma.
The authors used the application of nerve-muscle pedicle (NMP) technique for the selective reinnervation of the previously denervated posterior cricoarytenoid (PCA) muscle. After vocal cord denervation, the reinnervation was done with an ansa cervicalis-sternohyoid muscle pedicle in five mongrel dogs. Three dogs undervent a shame operation.
Videolaryngoscopy was performed, and electromyographic data were collected after one year in average. Finally histological analysis of the NMP was performed. The video records demonstrated the return of mobility of the PCA muscle reinnervated by a NMP. EMG data with evoked polyphasic potentials showed also evidence of reinnervation of the PCA muscle. With immunohistochemical reaction (antineurofilament antibody+biotin) we could demonstrate neurofilaments and motor endplates in both sides in all five animals. The NMP technique could eliminate the need for arytenoidectomy and laterofixation in patients with unilateral or bilateral vocal fold paralysis. With the use of this reinnervation technique, the voice and the quality of life of those patients’ - who lost their voice because of vocal cord paralysis - can be improved.
In the clinical practice, voice rehabilitation after removal of the larynx is essential for these patients. Esophageal speech is the most natural way for voice rehabilitation. The aim of the esophageal speech training club was to introduce the adequate breathing, swallowing and vocalization technique to these patients with psychological support. The success of acquiring this technique was estimated by means of a voice analyzing program (pitch, sound-holding, loudness, spectrogram), and by the intelligibility via the telephone. Shape of the different types of neoglottis that developed and its functioning during vocalization and continuous speech were observed by nasal endoscopy. Intonation, continuity, speech rate, and intelligibility were found good and in many cases close to normal by phoniatritian specialists. With objective voice quality evaluation, all F0 values of both female and male patients were higher than those published previously (50 Hz) in the literature. Former observations generally gave a frequency range of a quint that could be obtained by esophageal speech. It could be extended to an octave in our training club by singing exercises.
Proper and careful surgical reconstruction of the pharyngeal mucous membrane has a beneficial influence on voice quality parameters. Best results were acquired from the group of patients with ’button’ shape neoglottis form which was the result of the special suture technique in the wound closure
Calibration process for groundwater flow model of a river influenced shallow aquifer
Understanding and simulating the interaction of groundwater and surface water is essential to hydrologists. Water supply and water quality aspects are a few examples of common water-resource issues where understanding the interconnections of groundwater and surface water is fundamental to develop an effective water-resource management and policy. In our study a detailed investigation of a riverbank aquifer was performed to be able to simulate and predict the behavior of the flow system. The continuous hydraulic head measurements in the area of interest showed strong influence on the hydraulic head field caused by intensively changing river head at a distance from the river up to 3,500 m. Based on the results steady state and transient flow calculations were compared, and a great effort has been made to ensure that the model more precisely describe the time and space variable flow field.
