    Environmental and socioeconomic determinants of heart failure

    Background: Over 1.5 million people in Poland suffer from heart failure (HF). The risk of hospitalization is related to environmental, socioeconomic factors and the organization of the health care system. Aims: The study aimed to assess the influence of environmental and socioeconomic factors on the prevalence of hospitalization for HF. Methods: The impact of environmental and socioeconomic factors on HF hospitalizations in Poland in 2012–2019 based on the National Institute of Public Health and Central Statistical Office in Poland data and panel data regression techniques has been estimated. Results: There were 1,618,734 HF-related hospitalizations (51.3% male; 82.6% aged >65 years). An increase in the number of physicians by 10/10000 population and healthcare expenditure of 100 PLN per capita resulted in 3.5% (–0.035; 95% confidence interval [CI], –0.06–[–0.01]; P = 0.002) and 3% (–0.029; 95% CI, –0.04–[–0.013]; P <0.001) decrease in hospitalizations, respectively. For each new outpatient healthcare facility per 10 000 population, there was a 3% (–0.031; 95% CI, –0.048–[–0.014]; P <0.001) decrease in hospitalizations. One percentage point increase in the proportion of green areas resulted in a 2.7% (–0.027; 95% CI, –0.042–[–0.01]; P = 0.049) decrease in hospitalizations. However, an increase of cars by 1000 inhabitants and a 1 percentage point increase in the unemployment rate were associated with a 6% increase in HF hospitalizations (0.064; 95% CI, 0.008–0.121; P = 0.026). Conclusions: The number of HF-related hospitalizations has been increasing in the last decade. This trend is most noticeable in regions with low socioeconomic development and poor medical facilities. Our study indicates that health policy measures including environmental and socioeconomic instruments may result in positive health outcomes. Additional analyzes are needed to compare the impact of socioeconomic and environmental factors against the impact of healthcare alone

    Environmental and cultivar variability in composition, content and biological activity of phenolic acids and alkylresorcinols of winter wheat grains from a multi-site field trial across Europe

    Different factors such as the genotype, environmental conditions, temperature stress, solar radiation and others can influence the phytochemical status of plants. The concentration of phenolic acids and alkylresorciols (ARs) as well as their chemical composition and biological activity have been determined in twelve winter wheat cultivars grown at eight European locations. This was the first winter wheat multi-location field trial of the European Consortium for Open Field Experimentation (ECOFE). Extracts from grain were analyzed using a UPLC-PDA-ESI-MS system (phenolic acids), UPLC-PDA-MS/MS (alkylresorcinols) and TLC-DPPH center dot test with ImageJ program (antiradical activity). The phenolic acid profile consisted of five hydroxybenzoic acid and four hydroxycinnamic acid derivatives, among which ferulic and sinapic acids were predominated. The ARs profile consisted of nine AR derivatives, among which 5-n-heneicosylresorcinol (C21:0) and 5-n-nonadecanylresorcinol (C19:0) were predominated. Our study showed significant differences in phenolic acids and AR content between wheat cultivars, as well as between locations. We observed a positive correlation between the biological activity of extracts and the total amount of phenolic acids and ARs. Two cultivars, Chambo and Julius (average of all sites) and samples from the Spanish site (average of all cultivars) showed the highest content and composition of nutritional substances

    Triterpene Saponins from the Aerial Parts of Trifolium medium L. var. sarosiense

    Seven previously unreported triterpene glycosides (1−7) were isolated from methanol extract of the aerial parts of Trifolium medium var. sarosiense (zigzag clover). Their structures were established by the extensive use of 1D and 2D NMR experiments along with ESI-MS and HRMS analyses. Compounds 1−7 are oleanane derivatives characterized by the presence of a keto group at C-22 of an aglycone and a primary alcoholic function at C-24 and differing functions at C-30. Among these, compounds 1−3 and 6 showed a secondary alcoholic function at C-11, which is methoxylated in compounds 4 and 7. Compound 5 was shown to possess a known aglycone, wistariasapogenol A; however, it is described here for the first time as a saponin constituent of the Trifolium genus. Some aspects of taxonomic classification of zigzag clover are also discussed

    Mechanical thrombectomy in acute stroke – Five years of experience in Poland

    Objectives Mechanical thrombectomy (MT) is not reimbursed by the Polish public health system. We present a description of 5 years of experience with MT in acute stroke in Comprehensive Stroke Centers (CSCs) in Poland. Methods and results We retrospectively analyzed the results of a structured questionnaire from 23 out of 25 identified CSCs and 22 data sets that include 61 clinical, radiological and outcome measures. Results Most of the CSCs (74%) were founded at University Hospitals and most (65.2%) work round the clock. In 78.3% of them, the working teams are composed of neurologists and neuro-radiologists. All CSCs perform CT and angio-CT before MT. In total 586 patients were subjected to MT and data from 531 of them were analyzed. Mean time laps from stroke onset to groin puncture was 250±99min. 90.3% of the studied patients had MT within 6h from stroke onset; 59.3% of them were treated with IV rt-PA prior to MT; 15.1% had IA rt-PA during MT and 4.7% – emergent stenting of a large vessel. M1 of MCA was occluded in 47.8% of cases. The Solitaire device was used in 53% of cases. Successful recanalization (TICI2b–TICI3) was achieved in 64.6% of cases and 53.4% of patients did not experience hemorrhagic transformation. Clinical improvement on discharge was noticed in 53.7% of cases, futile recanalization – in 30.7%, mRS of 0–2 – in 31.4% and mRS of 6 in 22% of cases. Conclusion Our results can help harmonize standards for MT in Poland according to international guidelines

    Electron beam treatment of simulated marine diesel exhaust gases

    The exhaust gases from marine diesel engines contain high SO2 and NOx concentration. The applicability of the electron beam flue gas treatment technology for purification of marine diesel exhaust gases containing high SO2 and NOx concentration gases was the main goal of this paper. The study was performed in the laboratory plant with NOx concentration up to 1700 ppmv and SO2 concentration up to 1000 ppmv. Such high NOx and SO2 concentrations were observed in the exhaust gases from marine high-power diesel engines fuelled with different heavy fuel oils. In the first part of study the simulated exhaust gases were irradiated by the electron beam from accelerator. The simultaneous removal of SO2 and NOx were obtained and their removal efficiencies strongly depend on irradiation dose and inlet NOx concentration. For NOx concentrations above 800 ppmv low removal efficiencies were obtained even if applied high doses. In the second part of study the irradiated gases were directed to the seawater scrubber for further purification. The scrubbing process enhances removal efficiencies of both pollutants. The SO2 removal efficiencies above 98.5% were obtained with irradiation dose greater than 5.3 kGy. For inlet NOx concentrations of 1700 ppmv the NOx removal efficiency about 51% was obtained with dose greater than 8.8 kGy.     Unusual Fernane and Gammacerane Glycosides from the Aerial Parts of <i>Spergula fallax</i>

    The aerial parts of <i>Spergula fallax</i> afforded four glycosides (<b>1</b>–<b>4</b>) based on two new triterpene aglycones (<b>1a</b> and <b>2a</b>), along with the known hopane glycoside succulentoside A. Compound <b>1</b> was identified as belonging to the fernane class, unusual migrated hopane triterpenoids, mainly isolated from ferns and only rarely from higher plants. Compounds <b>2</b>–<b>4</b> were assigned as gammacerane glycosides, having as aglycone a hydroxylated derivative of tetrahymanol. The structures of the isolated compounds <b>1</b>–<b>4</b> and their aglycones <b>1a</b> and <b>2a</b> obtained by acid hydrolysis were elucidated by spectroscopic data interpretation. The growth inhibitory activity of the isolated compounds and their aglycones was evaluated against the HeLa and DLD-1 cancer cell lines
