Evaluation der EffektivitĂ€t unterschiedlicher KleinstbeatmungsgerĂ€te (OxylatorÂź EM- 100/ OxylatorÂź EMX/OxylatorÂź EMX- 100) bei Beatmung ĂŒber EndotrachealtubusEine klinische Evaluation
Die Beatmung von Patienten in der Notfallmedizin, speziell im Rahmen der CPR, geschieht normalerweise unter dem Einfluss von Stress und fordert von dem medizinischen Personal ein sehr hohes MaĂ an ProfessionalitĂ€t, Aufmerksamkeit und Geschicklichkeit. BasismaĂnahmen fĂŒr die kardiopulmonale Wiederbelebung im Notfall ist neben der absolut vordringlichen Herzdruckmassage die Beatmung der Patienten. Eine ausreichende Oxygenierung und Ventilation hĂ€ngen zum Teil von den verabreichten Tidal- und Atemminutenvolumina ab.
In der Notfallmedizin wird meistens die Beutel-Masken-Beatmung primĂ€r zur Beatmung eingesetzt. Deren erfolgreiche DurchfĂŒhrung erfordert spezifische Kenntnisse und Fertigkeiten und kann insbesondere unter Notfallbedingungen erschwert sein.
Der OxylatorÂź soll daher, bedingt durch sein spezielles und sehr einfach funktionierendes Prinzip, die LĂŒcke zwischen Beutel-Masken-Beatmung und transportablen BeatmungsgerĂ€ten in der Notfallmedizin schlieĂen.
Ziel dieser vorliegenden Untersuchung war es, an lungengesunden, narkotisierten und intubierten Patienten mit vollstÀndiger Muskelrelaxation zu untersuchen, ob die OxylatorenŸ EM-100, EMX und EMX-100 in der Lage sind, eine adÀquate Ventilation und Oxygenierung zu ermöglichen und aufrechtzuerhalten. Zur Messung der spirometrischen Daten wurden 68 Patienten in eine prospektiv randomisierte Studie unter standardisierten Bedingungen eingeschlossen.
Oben genannte Parameter wurden zu insgesamt 13 verschiedenen Messzeitpunkten auf einem fĂŒr diese Studie erstellten Erhebungsbogen dokumentiert.
Alle drei OxylatorenÂź , betrieben durch eine Sauerstoffquelle, konnten eine ausreichende Oxygenierung bei lungengesunden, narkotisierten, muskelrelaxierten, intubierten Patienten aufrechterhalten. Allerdings ergaben sich hinsichtlich einer adĂ€quaten Ventilation groĂe Unterschiede: Die Einstellung eines Pinsp von entweder 15 cmH2O oder eines Pinsp höher als 20 cmH2O fĂŒhrten im ersten Fall regelmĂ€Ăig zu einer Hypoventilation (Median = 47 mmHg) oder im zweiten Fall regelmĂ€Ăig zu einer Hyperventilation (Median = 25 mmHg). Es ist annehmbar, dass diese Effekte bei der Ventilation von Notfallpatienten mit ungeschĂŒtztem Atemweg und geringer Lungenfunktion noch deutlicher und nachteiliger sind.
Zudem wurde in dieser Studie in einer Testlungeneinstellung mit dem OxylatorÂź EMX mit einem inspiratorischen Atemwegspitzendruck von 20 cmHâO ventiliert, um die AbhĂ€ngigkeit der spirometrischen Parameter Vt, AMV, I:E, AF von einer isolierten Abnahme der Compliance, eines isolierten Anstiegs der Resistance sowie einer kombinierten VerĂ€nderung der respiratorischen Mechanik darzustellen.
Trotz signifikanter Abnahme der Atemzugvolumina konnte bei allen drei Simulationen einer pathologischen Lungenfunktion an unserer Testlunge eine suffiziente Ventilation und Oxygenierung erreicht werden.
Die wichtigste Feststellung dieser Untersuchung ist, dass die Ergebnisse bei Ventilation der Patienten mit einem inspiratorischen Atemwegdruck von 20 cmH2O hinsichtlich der Tidal- und Atemminutenvolumina und der Inspirationszeit am besten zu den aktuellen Empfehlungen von AHA und ERC passen.
Die Verwendung von OxylatorenÂź zur Beatmung mit der Gesichtsmaske ist eher zurĂŒckhaltend zu beurteilen aufgrund der bekannten Risiken und Probleme mit der Maskenbeatmung unter notfallmedizinischen Bedingungen.
Weitere Untersuchungen an gröĂeren Kollektiven unter Einsatz von supraglottischen Atemweghilfen sollten durchgefĂŒhrt werden, um die Anwendbarkeit der OxylatorenÂź in der Notfallmedizin als primĂ€res BeatmungsgerĂ€t detaillierter beurteilen zu können. Konsequente technische Weiterentwicklungen und kontinuierliches Training könnten den Routineeinsatz ermöglichen
Radiation therapy attenuates lymphatic vessel repair by reducing VEGFR-3 signalling
Introduction: Surgery and radiotherapy are key cancer treatments and the leading causes of damage to the lymphatics, a vascular network critical to fluid homeostasis and immunity. The clinical manifestation of this damage constitutes a devastating side-effect of cancer treatment, known as lymphoedema. Lymphoedema is a chronic condition evolving from the accumulation of interstitial fluid due to impaired drainage via the lymphatics and is recognised to contribute significant morbidity to patients who survive their cancer. Nevertheless, the molecular mechanisms underlying the damage inflicted on lymphatic vessels, and particularly the lymphatic endothelial cells (LEC) that constitute them, by these treatment modalities, remain poorly understood.Methods: We used a combination of cell based assays, biochemistry and animal models of lymphatic injury to examine the molecular mechanisms behind LEC injury and the subsequent effects on lymphatic vessels, particularly the role of the VEGF-C/VEGF-D/VEGFR-3 lymphangiogenic signalling pathway, in lymphatic injury underpinning the development of lymphoedema.Results: We demonstrate that radiotherapy selectively impairs key LEC functions needed for new lymphatic vessel growth (lymphangiogenesis). This effect is mediated by attenuation of VEGFR-3 signalling and downstream signalling cascades. VEGFR-3 protein levels were downregulated in LEC that were exposed to radiation, and LEC were therefore selectively less responsive to VEGF-C and VEGF-D. These findings were validated in our animal models of radiation and surgical injury.Discussion: Our data provide mechanistic insight into injury sustained by LEC and lymphatics during surgical and radiotherapy cancer treatments and underscore the need for alternative non-VEGF-C/VEGFR-3-based therapies to treat lymphoedema

CDK4/6 inhibition triggers anti-tumor immunity
Cyclin-dependent kinases 4 and 6 (CDK4/6) are fundamental drivers of the cell cycle and are required for the initiation and progression of various malignancies1,2. Pharmacologic inhibitors of CDK4/6 have shown significant activity against several solid tumors3,4. Their primary mechanism of action is thought to be the inhibition of phosphorylation of the retinoblastoma (RB) tumor suppressor, inducing G1 cell cycle arrest in tumor cells5. Here, we use murine models of breast carcinoma and other solid tumors to show that selective CDK4/6 inhibitors not only induce tumor cell cycle arrest, but also promote anti-tumor immunity. We confirm this phenomenon through transcriptomic analysis of serial biopsies from a clinical trial of CDK4/6 inhibitor treatment for breast cancer. The enhanced anti-tumor immune response has two underpinnings. First, CDK4/6 inhibitors activate tumor cell expression of endogenous retroviral elements, thus increasing intracellular levels of double-stranded RNA. This in turn stimulates production of type III interferons and hence enhances tumor antigen presentation. Second, CDK4/6 inhibitors markedly suppress the proliferation of regulatory T cells (Tregs). Mechanistically, the effects of CDK4/6 inhibitors on both tumor cells and Tregs are associated with reduced activity of the E2F target, DNA methyltransferase 1. Ultimately, these events promote cytotoxic T cell-mediated clearance of tumor cells, which is further enhanced by the addition of immune checkpoint blockade. Our findings indicate that CDK4/6 inhibitors increase tumor immunogenicity and provide rationale for new combination regimens comprising CDK4/6 inhibitors and immunotherapies as anti-cancer treatment
High-Throughput Live and Fixed Cell Imaging Method to Screen Matrigel-Embedded Organoids
Technical advances in microscopy and automation have enabled image-based phenotypic screening of spheroids and organoids to become increasingly high throughput and high content at the same time. In particular, matrix-embedded 3D structures can recapitulate many aspects of parent (e.g., patient) tissues. Live-cell imaging of growing structures allows tremendous insight into population heterogeneity during drug treatment. However, screening for targeted markers and more detailed morphological analyses typically require fixation of 3D structures, and standard formaldehyde (FA) incubation conditions can dissolve collagen-based extracellular matrices such as Matrigel. The dislocation and clumping of the spheroids make image-based segmentation very difficult and the tracking of structures from the live cell stage to their fixed cell location virtually impossible. In this method, we present a fixation and staining protocol that is gentle enough to maintain 3D structures exactly in their live-cell location and does not alter their morphology. This opens up analytical strategies that connect the spheroid’s growth kinetics and heterogeneity of treatment responses with the more targeted fixed cell stains. Furthermore, we optimized the automated seeding and imaging of spheroids so that screening and phenotypic characterization can be performed in high-throughput at either low or high magnification and yield the same result, independent of the microscope used
Mechanisms of idiosyncratic hepatotoxicity - Impact of drug independent stress factors on reative metabolite formation and cellular stress
Idiosynkratische LeberschĂ€digung durch Arzneimittel (z.B. Diclofenac) stellt trotz ihres seltenen Auftretens eine erhebliche Komplikation in der Arzneimittelentwicklung und -therapie dar. Die zu idiosynkratischen Reaktionen fĂŒhrenden, komplexen chemischen und biologischen AblĂ€ufe sind noch weitgehend unklar. Inzwischen wird jedoch vermutet, dass die ToxizitĂ€t eines Arzneimittels durch Arzneistoff-unabhĂ€ngige Risikofaktoren, wie Krankheiten, EntzĂŒndungsreaktionen, Co-Medikation oder Alkohol, erhöht werden kann. Mögliche Mechanismen könnten hierbei eine vermehrte Bildung reaktiver Metaboliten bzw. eine verĂ€nderte zellulĂ€re Stress- und Immunantwort sein. Um tiefere Einblicke in die Bedeutung möglicher Arzneistoff-unabhĂ€ngiger Risikofaktoren zu erhalten, wurde in der vorliegenden Arbeit der Einfluss drei verschiedener Stressfaktoren auf die ToxizitĂ€t von Diclofenac (Dcl) untersucht. Bei diesen Stressfaktoren handelte es sich um Lipopolysaccharid (LPS) und Poly I:C (PIC) zur Simulation einer bakteriellen bzw. viralen EntzĂŒndung sowie um Buthionin-Sulfoximin (BSO) zur Depletion zellulĂ€ren Glutathions. ZusĂ€tzlich wurde getestet, ob eine durch Stressfaktoren ausgelöste Erhöhung der ToxizitĂ€t von Dcl in Ratten mit VerĂ€nderungen in der Biotransformation bzw. mit einer Hochregulation co-stimulatorischer Faktoren (z.B. Zytokine oder Alarmsignale) einhergeht. Die Kombination einer einwöchigen therapeutisch dosierten Dcl-Behandlung mit einer einmaligen LPS-Dosis erzeugte in den Tieren eine ausgeprĂ€gte HepatotoxizitĂ€t, die mit erhöhten AktivitĂ€ten der Aminotransferasen im Serum einherging. Diese adversen Effekte konnten jedoch nicht durch LPS oder Dcl alleine, bzw. in Kombination mit PIC oder BSO erzeugt werden. Es besteht die Annahme, dass die Bioaktivierung von Diclofenac zu 5-OH-Dcl oder Dcl-Acylglucuronid (AG) sowie die folgende Bildung kovalenter Proteinaddukte zur Entwicklung von LebertoxizitĂ€t beitrĂ€gt. Mittels LC-MS/MS-Messungen konnten wir jedoch nachweisen, dass die Gabe von LPS + Dcl keine erhöhte Bildung reaktiver Metaboliten oder Dcl-AG-abhĂ€ngiger Proteinaddukte auslöst. Im Einklang damit wurden Enzyme, die fĂŒr die Bio-aktivierung von Dcl zu reaktiven Metaboliten verantwortlich sind (z.B. Cyp2C11, Cyp2C7 und UGT2B1), sowie die MRP-Effluxtransporter der Leber durch die Co-Behandlung mit LPS in ihrer Genexpression gehemmt. ZusĂ€tzliche qRT-PCR-Analysen Nrf2-abhĂ€ngiger Gene, als Sensor fĂŒr elektrophilen oder oxidativen Stress, zeigten keine Hochregulation zytoprotektiver Faktoren und unterstĂŒtzen die Schlussfolgerung, dass Arzneistoff-unabhĂ€ngige Stress-faktoren keine erhöhte Bildung toxischer Dcl-Metaboliten auslösen. SchlieĂlich ergaben unsere Analysen, dass eine Aktivierung co-stimulatorischer NFÎșB- und MAPK-Signalwege mit Hochregulation co-stimulatorischer Faktoren (z.B. IL-1ÎČ, TNF-α, CINC-1, iNOS) und Akkumulation neutrophiler Granulozyten in der Leber sowohl durch Behandlung mit LPS + Dcl als auch mit PIC + Dcl induziert wurde. Nur die Kombination von LPS und Diclofenac bewirkte jedoch darĂŒber hinaus eine massive Freisetzung pro-inflammatorischer Zytokine, Chemokine sowie toxizitĂ€tsfördernder Alarmsignale (z.B. IL-1ÎČ, TNF-α, CINC-1, HMGB1, LTB4) ins Plasma. ZusĂ€tzlich waren schĂŒtzende negative Feed-back-Mechanismen, wie die Hitzeschockreaktion, in den mit LPS und Dcl behandelten Tieren gehemmt. Zusammenfassend zeigen unsere Ergebnisse, dass eine metabolische Aktivierung von Dcl bzw. eine Akkumulation reaktiver Dcl-Metaboliten an der Entwicklung idiosynkratischer LeberschĂ€digung nicht ausschlaggebend beteiligt ist. Im Gegensatz zu PIC oder BSO fĂŒhrte in den verabreichten Dosen nur die Gabe von LPS als Stressfaktor zu einer Aktivierung co-stimulatorischer Signalwege sowie zu einer Hemmung protektiver Systeme, wodurch die leberschĂ€digende Wirkung von Dcl potenziert wurde.Idiosyncratic drug reactions (IDRs) are rare but major complications of drug therapy and development. A basic understanding of the chemical and biological events leading to IDRs is still lacking. However, it appears that drug-independent risk factors may be critical determinants in the response to an otherwise non-toxic drug. It has been speculated that stress factors like an underlying disease, inflammation, co-medication or alcohol may increase reactive metabolite formation and/or alter cellular stress and immune response. Thus, we were interested to determine the impact of various drug-independent stress factors on the toxicity of diclofenac (Dcl), a model drug associated with rare but significant cases of serious hepatotoxicity. We tested the hypothesis that co-treatment with various drug-independent risk factors may enhance Dcl toxicity. These included lipopolysaccharide (LPS) and poly I:C (PIC) simulating bacterial and viral inflammation, respectively, and buthionine sulfoximine (BSO) as a model for cellular glutathione depletion. Additionally, we were interested to understand if stress factor-induced modulation of Dcl toxicity involves alterations in drug metabolism and/or up-regulation of co-stimulatory molecules thought to constitute âdanger signalsâ. Co-treatment of rats repeatedly given therapeutic doses of Dcl for 7 days with a single dose of LPS resulted in severe liver toxicity accompanied by elevated serum aminotransferase activity. Neither LPS nor diclofenac alone or in combination with PIC or BSO had such an effect. It is thought that bioactivation to reactive 5-OH-Dcl or Dcl acyl glucuronides (AG) with subsequent protein adduct formation contribute to Dcl induced liver injury. However, LC-MS/MS analyses did not reveal increased formation of reactive metabolites or Dcl-AG-dependent protein adducts in animals treated with LPS + Dcl. Consistent with this, co-treatment with LPS induced down-regulation of enzymes responsible for Dcl bioactivation to reactive metabolites (e.g. Cyp2C11, Cyp2C7 and UGT2B1), as well as liver MRP efflux transporters. Furthermore, qRT-PCR analyses of Nrf2-dependent genes, as a sensor of electrophilic or oxidative stress, showed no up-regulation of cytoprotective factors, supporting the conclusion that drug-independent stress factors do not enhance formation of toxic Dcl metabolites. Hepatic gene expression analyses revealed activation of NFÎșB and MAPK pathways with up-regulation of co-stimulatory molecules (IL-1ÎČ, TNF-α, CINC-1, iNOS) and accumulation of neutrophil granulocytes in liver tissue by LPS + Dcl as well as by PIC + Dcl. However, only LPS + Dcl lead to extensive release of pro-inflammatory cytokines, chemokines and cytotoxic danger signals (IL-1ÎČ, TNF-α, CINC-1, HMGB1, LTB4) into plasma. Furthermore, down-regulation of protective factors (SOD2, HSPs, PGE2) suggested an impairment of negative feedback mechanisms in animals treated with LPS + Dcl. In summary our results show no major role of metabolic Dcl bioactivation or accumulation of reactive Dcl metabolites in the pathogenesis of idiosyncratic hepatotoxicity. In contrast to PIC or BSO only administration of LPS as stress factor lead to activation of co-stimulatory pathways as well as impairment of protective systems, resulting in potentiation of liver toxicity of therapeutic Dcl doses

Examining the supportâsupply and budâpacking hypotheses for the increase in toothed leaf margins in northern deciduous floras
Premise
The proportion of woody dicots with toothed leaves increases toward colder regions, a relationship used to reconstruct past mean annual temperatures. Recent hypotheses explaining this relationship are that (1) leaves in colder regions are thinner, requiring thick veins for support and water supply, with the resulting craspedodromous venation leading to marginal teeth (supportâsupply hypothesis) or that (2) teeth are associated with the packing of leaf primordia in winter buds (budâpacking hypothesis).
Methods
We addressed these hypotheses by examining leaf thickness, number of primordia in buds, growing season length (mean annual temperature, MAT), and other traits in 151 deciduous woody species using georeferenced occurrences and a Bayesian model controlling for phylogeny. We excluded evergreen species because longer leaf life spans correlate with higher leaf mass per area, precluding the detection of independent effects of leaf thickness on leafâmargin type.
Results
The best model predicted toothed leaves with 94% accuracy, with growing season length the strongest predictor. Neither leaf thickness nor number of leaves preformed in buds significantly influenced margin type, rejecting the supportâsupply and budâpacking hypotheses.
Conclusions
A direct selective benefit of leaf teeth via a carbon gain early in the spring as proposed by Royer and Wilf (2006) would match the strong correlation between toothed species occurrence and short growing season found here using Bayesian hierarchical models. Efforts should be directed to physiological work quantifying seasonal photosynthate production in toothed and nontoothed leaves.ISSN:1914-2016:ISSN:0002-912
Data from: A systems toxicology approach for the prediction of kidney toxicity and its mechanisms in vitro
The failure to predict kidney toxicity of new chemical entities early in the development process before they reach humans remains a critical issue. Here, we used primary human kidney cells and applied a systems biology approach that combines multidimensional datasets and machine learning to identify biomarkers that not only predict nephrotoxic compounds but also provide hints towards their mechanism of toxicity. Gene expression and high content imaging phenotypical data from 46 diverse kidney toxicants were analyzed using Random Forest machine learning. Imaging features capturing changes in cell morphology and nucleus texture along with mRNA levels of HMOX1 and SQSTM1 were identified as the most powerful predictors of toxicity. These biomarkers were validated by their ability to accurately predict kidney toxicity of 4 out of 6 candidate therapeutics that exhibited toxicity only in in late stage preclinical/clinical studies. Network analysis of similarities in toxic phenotypes was performed based on live-cell high-content image analysis at seven time points. Using compounds with known mechanism as reference, we could infer potential mechanisms of toxicity of candidate therapeutics. In summary, we report an approach to generate a multidimensional biomarker panel for mechanistic de-risking and prediction of kidney toxicity in vitro for new therapeutic candidates and chemical entities

A thiolâbound drug reservoir enhances APRâ246âinduced mutant p53 tumor cell death
Abstract The tumor suppressor gene TP53 is the most frequently mutated gene in cancer. The compound APRâ246 (PRIMAâ1Met/Eprenetapopt) is converted to methylene quinuclidinone (MQ) that targets mutant p53 protein and perturbs cellular antioxidant balance. APRâ246 is currently tested in a phase III clinical trial in myelodysplastic syndrome (MDS). By in vitro, ex vivo, and in vivo models, we show that combined treatment with APRâ246 and inhibitors of efflux pump MRP1/ABCC1 results in synergistic tumor cell death, which is more pronounced in TP53 mutant cells. This is associated with altered cellular thiol status and increased intracellular glutathioneâconjugated MQ (GSâMQ). Due to the reversibility of MQ conjugation, GSâMQ forms an intracellular drug reservoir that increases availability of MQ for targeting mutant p53. Our study shows that redox homeostasis is a critical determinant of the response to mutant p53âtargeted cancer therapy
