    Workmen's Compensation in Massachusetts.

    Thesis (M.A.)--Boston UniversityWorkmen's Compensation in Massachusetts Workmen's Compensation is a type of social insurance which provides medical care and partial wage compensation for wage earners or their dependents for economic losses caused by industrial injuries or diseases arising out of, or received during the course of employment. Massachusetts is no exception to the rule that industrial injuries are of major importance in our present day economy. Therefore, the purpose of the thesis is to explain the workings of the Massachusetts Workmen's Compensation Act. The principle began in Germany about 1883 and spread to other parts of the world. The present compensation act is a result of the evolution of the employer's liability for negligence. The history of negligence law shows that in the middle of the 19th century, employers were so burdened by the concept that they sought ways and means of lessening it effect on them. The methods they used consisted of employer's liability insurance and the common law defenses of contributory negligence, fellow servant negligence, and the assumption of risk. The common law defenses worked great hardship on injured workers. The general effects were: to lessen the number of injuries compensated, and to lower the amount of compensation. It has been estimated that about 80% of the cases were lost by the workers, while many settled outside the court for small lump sum payments. There was a definite need for a workmen's compensation law, then, one which would utilize a new concept- that work injuries are a cost of production and should be paid for in the same manner as broken machinery is paid for. The Mass. Compensation Act was passed in 1911 and became effective in July, 1912. There have been amendments to the law since its passage which continue into 1948. This thesis deals with the present law and reviews the original act only to show specific and important changes. At present the coverage is good. About 92% of those who could be covered are actually under the act. Those excluded, who could conceivably be incorporated into the law, are farm laborers, domestics and casuals. Farm laborers account for only 1.2% of the total eligibles- so we can say that the coverage is good. Inaddition to the coverage- about 98% of the covered are also insured. The benefits of the law have been changed from time to time. In general the benefit scale states that workers may be compensated at 3/4 of the average weekly wage, but not more than $25 a week. Provisions are also made for children and widows. Maximum dollar limits are sometimes set on total payments and at other times the limit refers to a given number of weeks of payment. For medical care, benefits are unlimited in time or amount. My criticism of the benefit system is that there should not be a maximum weekly dollar limit on payments. Rather, the payments should be allowed to remain at 3/4 of the weekly salary. Under the present system the worker suffers financially both in prosperity and in depression. Chapter III, describing the practices and procedures under the compensation law, makes it obvious that the rights of both the employer and the employee are fully considered. Neither has lost his legal right of appeal through the courts. The Industrial Accident Board has taken over all the functions previously preformed by the common law courts and it is only in a few instances that compensation cases are appealed beyond the board. This last statement is an indication of the fairness and effectiveness of the board and the ability of its officers. From time to time the question has arisen concerning a state operated fund to provide the insurance for the compensation act. In the last analysis the problem resolves itself into the question of whether or not the state fund should be exclusive, i.e. operated to the exclusion of private insurance carriers. The proponents favoring an exclusive state fund argue on the grounds of economy. An investigation of the costs of compensation insurance by stock companies, competitive state funds, and an exclusive state fund, revealed that the exclusive state funds cost only 1.6% of the premium while the competitive funds ranged from 6.2% to 9%, and the cost to commercial stock companies ranged from 35-40%. In addition to other arguments the proponents claim that no one should be allowed profits and commissions on the necessity of the employer and the suffering of the worker. The opponents oppose the state funds on numerous grounds. One statement claims that it is only one step closer to communism. Other arguments are that benefits are left to the discretion of a few political officials; it is not sound public policy to leave the same officials who determine benefits to tax the employer; state-managed insurance breaks the relationship between employer and employee; and finally the only cost which is not common to the state fund is the commission and acquisition costs, but these are upheld by the services rendered by the agents and the writing of the business. In conclusion, only one system seems desirable and that is the exclusive state fund. The merit rating is a system used by insurance carriers whereby employers with more modern plants are allowed a lower insurance rate. This system gives the incentive to employers to reduce the number of industrial injuries in order to obtain lower costs of the premiums. The conclusions reached in the thesis have been: 1) the old theory of common law defenses is inapplicable to modern conditions of employment; 2) there was a definite need in Mass. for a workmen's compensation act; 3) the coverage of the act is good, but the benefits need revision such that the compensation paid will always be equal to 3/4 of the average weekly pay; 4) the meriti rating is a highly desirable system of computing insurance premiums under the act, for it gives an incentive to reduce accidents through safety work; 5) the state should operate an exclusive state fund; 6) the practices and procedures under the act are such that all the legal rights of appeal are maintained unimpaired for both the employer and the employee

    Optical Fibre Based Real-Time Measurements During an LDR Prostate Brachytherapy Implant Simulation: Using a 3D printed anthropomorphic phantom

    An optical fbre sensor based on radioluminescence, using the scintillation material terbium doped gadolinium oxysulphide (Gd2O2S:Tb) is evaluated, using a 3D printed anthropomorphic phantom for applications in low dose-rate (LDR) prostate brachytherapy. The scintillation material is embedded in a 700 µm diameter cavity within a 1 mm plastic optical fbre that is fxed within a brachytherapy needle. The high spatial resolution dosimeter is used to measure the dose contribution from Iodine-125 (I-125) seeds. Initially, the efects of sterilisation on the sensors (1) repeatability, (2) response as a function of angle, and (3) response as a function of distance, are evaluated in a custom polymethyl methacrylate phantom. Results obtained in this study demonstrate that the output response of the sensor, pre- and post-sterilisation are within the acceptable measurement uncertainty ranging from a maximum standard deviation of 4.7% pre and 5.5% post respectively, indicating that the low temperature sterilisation process does not damage the sensor or reduce performance. Subsequently, an LDR brachytherapy plan reconstructed using the VariSeed treatment planning system, in an anthropomorphic 3D printed training phantom, was used to assess the suitability of the sensor for applications in LDR brachytherapy. This phantom was printed based on patient anatomy, with the volume and dimensions of the prostate designed to represent that of the patient. I-125 brachytherapy seeds, with an average activity of 0.410 mCi, were implanted into the prostate phantom under transrectal ultrasound guidance; following the same techniques as employed in clinical practice by an experienced radiation oncologist. This work has demonstrated that this sensor is capable of accurately identifying when radioactive I-125 sources are introduced into the prostate via a brachytherapy needle

    Long-term voluntary wheel running does not alter vascular amyloid burden but reduces neuroinflammation in the Tg-SwDI mouse model of cerebral amyloid angiopathy

    Background: Cardiovascular exercise (CVE) has been shown to be protective against cognitive decline in aging and the risk for dementias, including Alzheimer’s Disease (AD). CVE has also been shown to have several beneficial effects on brain pathology and behavioral impairments in mouse models of AD; however, no studies have specifically examined the effects of CVE on cerebral amyloid angiopathy (CAA), which is the accumulation of amyloid-beta (Aβ) in the cerebral vasculature. CAA may be uniquely susceptible to beneficial effects of CVE interventions due to the location and nature of the pathology. Alternatively, CVE may exacerbate CAA pathology, due to added stress on already compromised cerebral vasculature. Methods: In the current study, we examined the effects of CVE over many months in mice, thereby modeling a lifelong commitment to CVE in humans. We assessed this voluntary CVE in Tg-SwDI mice, a transgenic mouse model of CAA that exhibits behavioral deficits, fibrillar vascular Aβ pathology, and significant perivascular neuroinflammation. Various “doses” of exercise intervention (0 h (“Sedentary”), 1 h, 3 h, 12 h access to running wheel) were assessed from ~ 4 to 12 months of age for effects on physiology, behavior/cognitive performance, and pathology. Results: The 12 h group performed the greatest volume of exercise, whereas the 1 h and 3 h groups showed high levels of exercise intensity, as defined by more frequent and longer duration running bouts. Tg-SwDI mice exhibited significant cerebral vascular Aβ pathology and increased expression of pro-inflammatory cytokines as compared to WT controls. Tg-SwDI mice did not show motor dysfunction or altered levels of anxiety or sociability compared to WT controls, though Tg-SwDI animals did appear to exhibit a reduced tendency to explore novel environments. At all running levels, CAA pathology in Tg-SwDI mice was not significantly altered, but 12-h high-volume exercise showed increased insoluble Aβ burden. However, CVE attenuated the expression of pro-inflammatory cytokines TNF-α and IL-6 and was generally effective at enhancing motor function and reducing anxiety-like behavior in Tg-SwDI mice, though alterations in learning and memory tasks were varied. Conclusions: Taken together, these results suggest that CAA can still develop regardless of a lifespan of substantial CVE, although downstream effects on neuroinflammation may be reduced and functional outcomes improved

    The understanding of international war, like many complex social events, may be - and has been - ap proached from a range of theoretical perspectives and via a variety of research strategies. Outside of the work of Bloch (1898), Sorokin (1936), Richardson (1941), and Wright (1942), however, there was little re search of a scientific nature until the mid-1960s. And while these past fifteen years have certainly not given us a compelling theory of international war, they have seen a steady growth in cumulative knowledge regar ding the correlates of war.     HDP—A Novel Heme Detoxification Protein from the Malaria Parasite

    When malaria parasites infect host red blood cells (RBC) and proteolyze hemoglobin, a unique, albeit poorly understood parasite-specific mechanism, detoxifies released heme into hemozoin (Hz). Here, we report the identification and characterization of a novel Plasmodium Heme Detoxification Protein (HDP) that is extremely potent in converting heme into Hz. HDP is functionally conserved across Plasmodium genus and its gene locus could not be disrupted. Once expressed, the parasite utilizes a circuitous “Outbound–Inbound” trafficking route by initially secreting HDP into the cytosol of infected RBC. A subsequent endocytosis of host cytosol (and hemoglobin) delivers HDP to the food vacuole (FV), the site of Hz formation. As Hz formation is critical for survival, involvement of HDP in this process suggests that it could be a malaria drug target

    Lung cancer CT screening: psychological responses in the presence and absence of pulmonary nodules

    Objectives: To determine the psychological response (thoughts, perceptions and affect) to a diagnosis of pulmonary nodules following a novel antibody blood test and computed tomography (CT) scans within a UK population. Materials and methods: This study was nested within a randomised controlled trial of a blood test (Early CDT®-Lung test), followed by a chest x-ray and serial CT-scanning of those with a positive blood test for early detection of lung cancer (ECLS Study). Trial participants with a positive Early CDT®-Lung test were invited to participate (n=338) and those agreeing completed questionnaires assessing psychological outcomes at 1, 3 and 6 months following trial recruitment. Responses of individuals with pulmonary nodules on their first CT scan were compared to those without (classified as normal CT) at 3 and 6 months follow-up using random effects regression models to account for multiple observations per participant, with loge transformation of data where modelling assumptions were not met. Results: There were no statistically significant differences between the nodule and normal CT groups in affect, lung cancer worry, health anxiety, illness perceptions, lung cancer risk perception or intrusive thoughts at 3 or 6 months post-recruitment. The nodule group had statistically significantly fewer avoidance symptoms compared to the normal CT group at 3 months (impact of events scale avoidance (IES-A) difference between means -1.99, 95%CI -4.18, 0.21) than at 6 months (IES-A difference between means 0.88, 95%CI -1.32, 3.08; p-value for change over time =0.003) with similar findings using loge transformed data. Conclusion: A diagnosis of pulmonary nodules following an Early CDT®-Lung test and CT scan did not appear to result in adverse psychological responses compared to those with a normal CT scan

    Lung cancer screening: does pulmonary nodule detection affect a range of smoking behaviours?

    Background Lung cancer screening can reduce lung cancer mortality by 20%. Screen-detected abnormalities may provide teachable moments for smoking cessation. This study assesses impact of pulmonary nodule detection on smoking behaviours within the first UK trial of a novel auto-antibody test, followed by chest x-ray and serial CT scanning for early detection of lung cancer (Early Cancer Detection Test-Lung Cancer Scotland Study). Methods Test-positive participants completed questionnaires on smoking behaviours at baseline, 1, 3 and 6 months. Logistic regression compared outcomes between nodule (n=95) and normal CT groups (n=174) at 3 and 6 months follow-up. Results No significant differences were found between the nodule and normal CT groups for any smoking behaviours and odds ratios comparing the nodule and normal CT groups did not vary significantly between 3 and 6 months. There was some evidence the nodule group were more likely to report significant others wanted them to stop smoking than the normal CT group (OR across 3 and 6 month time points: 3.04, 95%CI 0.95, 9.73; p=0.06). Conclusion Pulmonary nodule detection during lung cancer screening has little impact on smoking behaviours. Further work should explore whether lung cancer screening can impact on perceived social pressure and promote smoking cessation

    Two Alleles of NF-κB in the Sea Anemone Nematostella vectensis Are Widely Dispersed in Nature and Encode Proteins with Distinct Activities

    BACKGROUND. NF-κB is an evolutionarily conserved transcription factor that controls the expression of genes involved in many key organismal processes, including innate immunity, development, and stress responses. NF-κB proteins contain a highly conserved DNA-binding/dimerization domain called the Rel homology domain. METHODS/PRINCIPAL FINDINGS. We characterized two NF-κB alleles in the sea anemone Nematostella vectensis that differ at nineteen single-nucleotide polymorphisms (SNPs). Ten of these SNPs result in amino acid substitutions, including six within the Rel homology domain. Both alleles are found in natural populations of Nematostella. The relative abundance of the two NF-κB alleles differs between populations, and departures from Hardy-Weinberg equilibrium within populations indicate that the locus may be under selection. The proteins encoded by the two Nv-NF-κB alleles have different molecular properties, in part due to a Cys/Ser polymorphism at residue 67, which resides within the DNA recognition loop. In nearly all previously characterized NF-κB proteins, the analogous residue is fixed for Cys, and conversion of human RHD proteins from Cys to Ser at this site has been shown to increase DNA-binding ability and increase resistance to inhibition by thiol-reactive compounds. However, the naturally-occurring Nematostella variant with Cys at position 67 binds DNA with a higher affinity than the Ser variant. On the other hand, the Ser variant activates transcription in reporter gene assays more effectively, and it is more resistant to inhibition by a thiol-reactive compound. Reciprocal Cys<->Ser mutations at residue 67 of the native Nv-NF-κB proteins affect DNA binding as in human NF-κB proteins, e.g., a Cys->Ser mutation increases DNA binding of the native Cys variant. CONCLUSIONS/SIGNIFICANCE. These results are the first demonstration of a naturally occurring and functionally significant polymorphism in NF-κB in any species.     Effects of antiplatelet therapy on stroke risk by brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases: subgroup analyses of the RESTART randomised, open-label trial

    Background Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage.     Mucedorus: the last ludic playbook, the first stage Arcadia

    This article argues that two seemingly contradictory factors contributed to and sustained the success of the anonymous Elizabethan play Mucedorus (c. 1590; pub. 1598). First, that both the initial composition of Mucedorus and its Jacobean revival were driven in part by the popularity of its source, Philip Sidney's Arcadia. Second, the playbook's invitation to amateur playing allowed its romance narrative to be adopted and repurposed by diverse social groups. These two factors combined to create something of a paradox, suggesting that Mucedorus was both open to all yet iconographically connected to an elite author's popular text. This study will argue that Mucedorus pioneered the fashion for “continuations” or adaptations of the famously unfinished Arcadia, and one element of its success in print was its presentation as an affordable and performable version of Sidney's elite work. The Jacobean revival of Mucedorus by the King's Men is thus evidence of a strategy of engagement with the Arcadia designed to please the new Stuart monarchs. This association with the monarchy in part determined the cultural functions of the Arcadia and Mucedorus through the Interregnum to the close of the seventeenth century
