Study and Optimization of Design Parameters in Water Loop Heat Pump Systems for Office Buildings in the Iberian Peninsula
Water loop heat pump (WLHP) air conditioning systems use heat pumps connected to a common water circuit to fulfill the energy demands of different thermal zones in a building. In this study, the energy consumption was analyzed for the air conditioning of an office building in the typical climate of four important cities of the Iberian Peninsula. The energy consumption of one water loop heat pump system was compared with a conventional water system. Two design parameters, the range in the control temperatures and the water loop thermal storage size, were tested. Energy redistribution is an important advantage of the WLHP system, but significant savings came from high efficiency parameters in the heat pumps and minor air flow rates in the cooling tower. The low thermal level in the water loop makes this technology appropriate to combine with renewable sources. Using natural gas as the thermal energy source, a mean decrease in CO2 emissions of 8.1% was reached. Simulations showed that the installation of big thermal storage tanks generated small energy savings. Besides, the total annual consumption in buildings with high internal loads can be reduced by keeping the water loop as cool as possible
Choice of the initial antiretroviral treatment for HIV-positive individuals in the era of integrase inhibitors
BACKGROUND: We aimed to describe the most frequently prescribed initial antiretroviral therapy (ART) regimens in recent years in HIV-positive persons in the Cohort of the Spanish HIV/AIDS Research Network (CoRIS) and to investigate factors associated with the choice of each regimen. METHODS: We analyzed initial ART regimens prescribed in adults participating in CoRIS from 2014 to 2017. Only regimens prescribed in >5% of patients were considered. We used multivariable multinomial regression to estimate Relative Risk Ratios (RRRs) for the association between sociodemographic and clinical characteristics and the choice of the initial regimen. RESULTS: Among 2874 participants, abacavir(ABC)/lamivudine(3TC)/dolutegavir(DTG) was the most frequently prescribed regimen (32.1%), followed by tenofovir disoproxil fumarate (TDF)/emtricitabine (FTC)/elvitegravir(EVG)/cobicistat(COBI) (14.9%), TDF/FTC/rilpivirine (RPV) (14.0%), tenofovir alafenamide (TAF)/FTC/EVG/COBI (13.7%), TDF/FTC+DTG (10.0%), TDF/FTC+darunavir/ritonavir or darunavir/cobicistat (bDRV) (9.8%) and TDF/FTC+raltegravir (RAL) (5.6%). Compared with ABC/3TC/DTG, starting TDF/FTC/RPV was less likely in patients with CD4100.000 copies/mL. TDF/FTC+DTG was more frequent in those with CD4100.000 copies/mL. TDF/FTC+RAL and TDF/FTC+bDRV were also more frequent among patients with CD4<200 cells//μL and with transmission categories other than men who have sex with men. Compared with ABC/3TC/DTG, the prescription of other initial ART regimens decreased from 2014-2015 to 2016-2017 with the exception of TDF/FTC+DTG. Differences in the choice of the initial ART regimen were observed by hospitals' location. CONCLUSIONS: The choice of initial ART regimens is consistent with Spanish guidelines' recommendations, but is also clearly influenced by physician's perception based on patient's clinical and sociodemographic variables and by the prescribing hospital location.The RIS cohort (CoRIS) is supported by the Instituto de Salud Carlos III through the Red Temática de Investigación Cooperativa en Sida (RD06/006, RD12/0017/0018 and RD16/0002/0006) as part of the Plan Nacional I+D+i and cofinanced by ISCIII-Subdirección General de Evaluación and the Fondo Europeo de Desarrollo Regional (FEDER). Efecto terapéutico de la biomodulina T homeopática con pacientes portadores de enfermedad pulmonar obstructiva crónica
Se realizó un Ensayo Clínico Controlado, fase II, aleatorizado y abierto en el Centro de Investigaciones sobre Longevidad, Envejecimiento y Salud, durante el período comprendido entre el año 2008 y 2009, con el objetivo de evaluar el efecto terapéutico de la Biomodulina T en presentación homeopática, con pacientes adultos mayores portadores de Enfermedad Pulmonar Obstructiva Crónica que sufrieran infección respiratoria a repetición. Se incluyó 100 pacientes, que dieron su consentimiento informado. Se distribuyeron en dos grupos: uno, recibió el tratamiento en estudio y el otro, el tratamiento control. La variable principal de respuesta fue la incidencia y severidad de sepsis respiratoria, durante y después del tratamiento. Se utilizó el paquete estadístico SPSS versión 13. La Biomodulina T en presentación homeopática tuvo buen efecto terapéutico en la reducción de infecciones respiratorias, así como en la disminución de las recidivas. Logró mejorar la comorbilidad en estos pacientes y mostró baja frecuencia de aparición de reacciones adversas.Palabras clave:Ensayo clínico, efecto terapéutico, biomodulina T, presentación homeopática</p

El coste total incluía los costes de TAR, del manejo clínico y del manejo de efectos adversos (EA). Se aplicó la perspectiva del pagador (Sistema Nacional de Salud), considerando solo costes directos.
Resultados: En el análisis por intención de tratar, el coste anual varió entre 8.501 € para TDF/FTC/EFV y 12.840 € para TDF/FTC+RAL. De media, el coste farmacológico supone el 87,6% del coste total, variando entre 83,8 % para TDF/FTC/EFV y 91,1% para TDF/FTC+RAL. El coste del manejo clínico supone el 11,5% del coste total (8,6% para TDF/FTC+RAL y 13,9% para ABC/3TC/DTG). Sólo el 0,9% del coste de la pauta se debe al manejo de EA, más elevados con TDF/FTC/EFV (2,6%) y TDF/FTC+DRV/r (1,1%).
Conclusión: El coste para el sistema sanitario del primer año de TAR se debe, fundamentalmente, al precio de los fármacos. Las pautas con menor coste fueron TDF/FTC/EFV, TDF/FTC/RPV y ABC/3TC/DTG, siendo TDF/FTC/EVG/COBI y TDF/FTC+RAL las de coste superior

Analysis of Knowledge of Smoking-Related Diseases in Spanish Nursing Students
[EN] Smoking causes significant morbidity and mortality worldwide, mainly in developed
countries. In addition, it is the cause of numerous diseases in the body, despite the fact that the
prevalence of tobacco use is decreasing. Nursing students, as future professionals, should be aware of
action plans for cessation and information designed for smokers. To determine the level of knowledge
among nursing students about smoking-related diseases and analyze the prevalence of student who
smoke at the University of Leon, Spain, a descriptive cross-sectional study was carried out in which
students were given an anonymous questionnaire, which was previously validated, during the
2021–2022 academic year. In a sample of 477 (79.5%) nursing students, a smoking prevalence of
17.6% was obtained. In addition, students’ knowledge about the diseases directly caused by tobacco
consumption and others associated with exposure to environmental smoke was assessed, and in
both cases (8.03 points of 9 for consumption and 5.24 of 6 to exposure), scores were obtained that
allow us to state that students do not know for sure the types of diseases that are related to tobacco
use and passive smoking. In spite of this, it is necessary to continue to reduce the prevalence of
smoking through different programs implemented in schools and universities, as it is also necessary
to improve teaching plans when explaining smoking-related diseases, so that students, in the future,
will be able to advise patients correctly.SIThis research received no external funding
Análisis de la intimidación y acoso experimentado por estudiantes de Enfermería de cuarto curso durante sus prácticas clínicas
[ES]FUNDAMENTOS // Los estudiantes de Enfermería, con su inexperiencia y el desafío frecuente de encontrarse con nuevos entornos,
son un grupo potencialmente vulnerable para sufrir intimidación y acoso en el ámbito del trabajo. El objetivo de este trabajo fue
analizar la intimidación y/o acoso padecidos por los estudiantes de Enfermería durante sus prácticas clínicas.
MÉTODOS // Se realizó un estudio descriptivo transversal en la Universidad de León. La población a estudio fueron los estudiantes
del Grado en Enfermería de cuarto curso. El instrumento de medida fue un cuestionario que constó de 24 items y que se aprobó por el
Comité de Ética de la Universidad. Para el análisis estadístico se utilizó la prueba de chi-cuadrado, t de Student y la U de Mann Whitney.
RESULTADOS // Se observó una prevalencia del 26,5% (n=83) de estudiantes que sufrieron episodios de intimidación y/o acoso; los
estudiantes de menor edad padecieron estos hechos por parte de los médicos, pacientes y familiares y/o acompañantes en mayor
medida. Esto produjo un impacto en su bienestar psicológico e hizo que se sintieran deprimidos, humillados e incapaces, lo que afectó
negativamente al nivel de atención prestado a los pacientes.
CONCLUSIONES // El problema global estudiado presenta una prevalencia inferior a la obtenida en otros trabajos llevados a cabo
en el resto del mundo. Como consecuencia, existe un impacto en el bienestar psicológico de los estudiantes que afecta a su nivel de
atención en las tareas que están llevando a cabo y a su forma de trabajar con los demás.

Effectiveness and tolerability of dolutegravir/lamivudine for the treatment of HIV-1 infection in clinical practice
Objectives: To assess the effectiveness and tolerability of dolutegravir (DTG)/lamivudine (3TC) among treatment-naive and virologically suppressed treatment-experienced individuals in the multicentre cohort of the Spanish HIV/AIDS Research Network (CoRIS) during the years 2018-2021. Methods: We used multivariable regression models to compare viral suppression (VS) [HIV RNA viral load (VL) <50 copies/mL] and the change in CD4 cell counts at 24 and 48 (±12) weeks after initiation with dolutegravir/lamivudine or other first-line ART regimens. Results: We included 2160 treatment-naive subjects, among whom 401 (18.6%) started with dolutegravir/lamivudine. The remaining subjects started bictegravir (BIC)/emtricitabine (FTC)/tenofovir alafenamide (TAF) (n = 949, 43.9%), DTG + FTC/tenofovir disoproxil fumarate (TDF) (n = 282, 13.1%), DTG/3TC/abacavir (ABC) (n = 255, 11.8%), darunavir (DRV)/cobicistat(COBI)/FTC/TAF (n = 147, 6.8%) and elvitegravir (EVG)/COBI/FTC/TAF (n = 126, 5.8%). At 24 and 48 weeks after starting dolutegravir/lamivudine, 91.4% and 93.8% of the subjects, respectively, achieved VS. The probability of achieving VS with dolutegravir/lamivudine was not significantly different compared with any other regimen at 24 or 48 weeks, with the exception of a lower chance of achieving VS at 24 weeks for DRV/COBI/FTC/TAF (adjusted OR: 0.47; 95% CI: 0.30-0.74) compared with dolutegravir/lamivudine.For the analysis of treatment-experienced virally suppressed subjects we included 1456 individuals who switched to dolutegravir/lamivudine, among whom 97.4% and 95.5% maintained VS at 24 and 48 weeks, respectively. During the first 48 weeks after dolutegravir/lamivudine initiation, 1.0% of treatment-naive and 1.5% of treatment-experienced subjects discontinued dolutegravir/lamivudine due to an adverse event.
