    Assessing Microfinance: The Bosnia and Herzegovina Case

    Microfinance is often hailed both as a tool for fighting poverty and as a tool for post-conflict reconciliation. This paper explores the use of microfinance in post-civil war Bosnia and Herzegovina, assessing its results in terms of both goals. As it combined high unemployment with a highly educated population in an institutionally open context, Bosnia and Herzegovina provides a crucial test of the effect of microfinance. If unambiguous signs of success cannot be found in a case with such favorable conditions, this would raise serious questions about the potential benefits of microfinance. The paper draws together evidence from a series of independent reviews of microfinance in Bosnia and Herzegovina, to assess its impact in terms of economic performance, the economic system, social welfare and post-conflict integration.     Can there be efficient and natural FHE schemes?

    In 1978, Rivest, Adleman and Dertouzos asked for algebraic systems for which useful privacy homomorphisms exist. To date, the only acknownledged result is noise based encryption combined with bootstrapping. Before that, there were several failed attempts. We prove that fully homomorphic schemes are impossible for several algebraic structures. Then we develop a characterisation of all fully homomorphic schemes and use it to analyse three examples. Finally, we propose a conjecture stating that secure FHE schemes must either have a significant ciphertext expansion or use unusual algebraic structures

    Optimizing the use of quicklime (CaO) for sea urchin management — A lab and field study

    Mass blooms of sea urchins sometimes cause kelp forest collapses that can last for decades. Quicklime has historically been used to reverse those conditions, but the efficacy of liming has varied along latitudinal and temperature gradients for reasons that are not fully understood. To evaluate the feasibility and ecological impacts of liming in a high latitude area in Northern Norway (70°N), we conducted a field pilot study in 2008–2011, a follow-up lab study in 2017, and a further field study in 2018–2019, with the latter evaluating and implementing the previous results in a site high in refuges. It was found that liming can reduce sea urchin densities sufficiently for macroalgal revegetation to occur, and that the mobile fauna species richness and abundance increased in the re-vegetated in comparison to the barren control fields. Also, the remaining sea urchins in the treated fields increased their roe content to commercial levels after 2 years. The lab experiments in 2017 indicated that the liming method is season/temperature-independent, as mortality remained at the same level irrespective of whether treatment started in the spring, when the sea temperatures were 2 °C, or in autumn when the temperatures were closer to 10 °C. The most important factor in treatment efficacy in the lab was particle size. With similar doses, the particles in the smallest size range (0–0.5 mm) caused 100% mortality, while the 0.5–2 mm and 2–4 mm fractions caused only 13% and 2% mortality respectively. In 2018–2019 we tested the fine CaO fraction (0.1–0.6 mm) and the medium fraction (0.5–2 mm) in a field experiment in areas characterized by high levels of refuges. Within 11 days, the sea urchin densities in the three fields treated with the fine lime were reduced to levels that theoretically should allow revegetation, but only in one of those fields was that potential partly realized after 1 year. The lack of effect in the two other fields was probably due to urchins protected by the substrate during treatment reappearing in sufficient numbers to prevent macroalgal regrowth, demonstrating that CaO treatment can be less effective on substrates where part of the sea urchin population hides among stones. Of the three variables held up as potential explanations for the different effects of CaO treatment in previous studies, we conclude based on our experiments that the presence of refuges and particle size were probably more important than temperature.     Fine-scale spatial variation of northern shrimp and Atlantic cod across three Norwegian fjord systems and implications for management

    The spatial complexity of coastal ecosystems represents a challenge for the management of inshore resources. Here we compared two large fjord systems in northern Norway that have been closed for all bottom trawling for 50 years to a fjord with continuous shrimp fishery with bottom trawls. No significant differences were found between fjords with and without commercial trawling in population density and stock composition of northern shrimp (Pandalus borealis) and their main predator, Atlantic cod (Gadus morhua). Shrimp density was instead linked with bottom depth, while shrimp size and stage composition as well as cod density were explained by seasonal effects and shrimp density. For shrimp, a large degree of the observed variation was captured by spatial correlation that could not be explained by other covariates. The results underline the complex ecology in heavily structured coastal habitats and indicate that coastal shrimp dynamics are shaped by an interplay of multiple ecological and environmental drivers, possibly in concert with local genetic adaptations. The substantial fine-scale spatial variation adds to the challenges of assessing and managing fisheries resources in these fjord ecosystems. Because shrimp are an important forage species, notably as prey for cod, there are potential management conflicts between rebuilding cod stocks and reopening closed shrimp trawling areas.publishedVersio

    Reduction in omission events after implementing a Rapid Response System: a mortality review in a department of gastrointestinal surgery

    Background Hospitals worldwide have implemented Rapid Response Systems (RRS) to facilitate early recognition and prompt response by trained personnel to deteriorating patients. A key concept of this system is that it should prevent ‘events of omission’, including failure to monitor patients’ vital signs, delayed detection, and treatment of deterioration and delayed transfer to an intensive care unit. Time matters when a patient deteriorates, and several in-hospital challenges may prevent the RRS from functioning adequately. Therefore, we must understand and address barriers for timely and adequate responses in cases of patient deterioration. Thus, this study aimed to investigate whether implementing (2012) and developing (2016) an RRS was associated with an overall temporal improvement and to identify needs for further improvement by studying; patient monitoring, omission event occurrences, documentation of limitation of medical treatment, unexpected death, and in-hospital- and 30-day mortality rates. Methods We performed an interprofessional mortality review to study the trajectory of the last hospital stay of patients dying in the study wards in three time periods (P1, P2, P3) from 2010 to 2019. We used non-parametric tests to test for differences between the periods. We also studied overall temporal trends in in-hospital- and 30-day mortality rates. Results Fewer patients experienced omission events (P1: 40%, P2: 20%, P3: 11%, P = 0.01). The number of documented complete vital sign sets, median (Q1,Q3) P1: 0 (0,0), P2: 2 (1,2), P3: 4 (3,5), P = 0.01) and intensive care consultations in the wards ( P1: 12%, P2: 30%, P3: 33%, P = 0.007) increased. Limitations of medical treatment were documented earlier (median days from admission were P1: 8, P2: 8, P3: 3, P = 0.01). In-hospital and 30-day mortality rates decreased during this decade (rate ratios 0.95 (95% CI: 0.92–0.98) and 0.97 (95% CI: 0.95–0.99)). Conclusion The RRS implementation and development during the last decade was associated with reduced omission events, earlier documentation of limitation of medical treatments, and a temporal reduction in the in-hospital- and 30-day mortality rates in the study wards. The mortality review is a suitable method to evaluate an RRS and provide a foundation for further improvement.publishedVersio

    Klotho and Fibroblast Growth Factor 23 Are Independent of Vitamin D, and Unlike Vitamin D, Are Not Associated With Graft- and Patient Survival After Kidney Transplantation

    Background: Short-term survival after kidney transplantation is excellent but long-term survival remains suboptimal. The aim of the study was to explore the relationship between soluble α-Klotho (sKlotho) and intact fibroblast growth factor 23 (iFGF23) measured 8 wk and 1 y posttransplant with long-term graft- and patient survival in a cohort of kidney transplant recipients with deficient and nondeficient vitamin D (25[OH]D) levels. Methods: Vitamin D, sKlotho, and iFGF23 were measured 8 wk and 1 y posttransplant in 132 recipients transplanted between November 2012 and October 2013. Results: Of the 132 kidney transplant recipients, 49 had deficient vitamin D levels (<30 nmol/L) and 83 had nondeficient vitamin D levels (≥30 nmol/L) at 8 wk posttransplant. The mean age was 51 y and the median follow-up was 7.4 y. At 1 y posttransplant, vitamin D increased significantly. There were no significant differences in sKlotho or iFGF23 levels between the 2 vitamin D groups neither at 8 wk nor 1 y. sKlotho increased significantly and iFGF23 decreased significantly in the whole cohort. During the follow-up, there were 36 graft losses (27%) and 27 deaths (20%). Ninety-four percent of the transplant recipients with nondeficient vitamin D levels were alive with a well-functioning graft after 5 y using Kaplan-Meier survival estimates, compared with 84% of the patients with deficient vitamin D levels (P = 0.014). Klotho and FGF23 levels did not influence graft- and patient survival. Conclusions: In this nationwide cohort of kidney transplant recipients, long-term graft- and patient survival were significantly better in patients with vitamin D ≥30 nmol/L 8 wk posttransplant compared with those with vitamin D <30 nmol/L. sKlotho levels increased and iFGF23 levels decreased from 8 wk to 1 y posttransplant.
