    Modelling of Temperature in Full-scale Systems: a Review of the Biological Heat Generation Within Windrow Composting

    Windrow composting is the most common method for organic solid waste treatment. Temperature is an important state variable in modelling, which could be simulated by estimating the change of heat balance components at any moment. Biological heat energy is the most significant term in the heat balance. In this paper biological heat generation of the composting process are examined and their applicability for a full-scale windrow system is evaluated. It is found that, to date, the accuracy of predicting the rate of substrate degradation has been a major challenge. The use of an inductive approach based on either first-order kinetic expression or empirical kinetics relation was found to be more feasible for practical purposes. However a scale-up correction factor for particular processes and types of substrate may need to be further investigated to narrow the discrepancies of kinetic performances between laboratory and full-scale systems

    The efficacy of lymph node fine needle aspiration cytology

    Fine needle aspiration cytology (FNAC) of lymph nodes is a safe, easy, cheap and quick diagnostic tool, which involves the examination of a random sample of cells from a lymph node. To assess the distribution of diagnostic categories and the efficacy of lymph node fine needle aspiration cytology at our institution. These were compared to the literature. Methodology: All of lymph node FNAC cases taken between the 1st January 2012 and the 31st December 2013 were retrieved from our Laboratory Information System. A total of 300 cases were retrieved and then placed into one of six categories; Category 1: Non-diagnostic, 2: Reactive, 3: Probably reactive but lymphoma cannot be excluded, 4: Non-Hodgkin lymphoma, 5: Hodgkin lymphoma, and 6: Metastasis. These were then correlated with the histology of the lymph node excision specimens. The proportion of diagnoses placed under categories 1, 2, 3, 4, 5 and 6 represent 14%, 53%, 4.3%, 5.7%, 1.7% and 21.3% of the total respectively. The overall efficacy of FNAC showed a sensitivity of 84.5%, specificity of 99.3%, a false negative rate of 10%, a false positive rate of 0.7%, accuracy of 93.1%, positive predictive value of 98.8% and negative predictive value of 89.9%. FNAC of lymph nodes is a very useful and effective tool in triaging patients with lymphadenopathy.peer-reviewe

    Combination of pharmacotherapy and lidocaine analgesic block of the peripheral trigeminal branches for trigeminal neuralgia: a pilot study

    Classical trigeminal neuralgia (CTN) is treated predominantly by pharmacotherapy but side effects and unsuccessful occurs. The current study was carried out to evaluate the therapeutic effect of combination of pharmacotherapy and lidocaine block. Thirteen patients with CTN managed with pharmacotherapy were recruited and assigned either to no additional treatment (Group I) or to additional analgesic block (Group II). The primary endpoint was the reduction in the frequency of pain episodes in a month assessed at 30 and 90 days. Comparisons of measurements of pain, general health and depression scales were secondary endpoints. The results from the follow-up visits at 30 and 90 days showed the Group II to have larger reduction in the frequency of pain and exhibited a bigger improvement in the scores of the pain, general health and depression scales. The results from this preliminary study suggest a clinical benefit of the combination of pharmacotherapy and lidocaine block

    An Idiom for India: Hindustani and the Limits of the Language Concept

    This essay explores the cultural legacy of Hindustani, which names the intimate overlap between two South Asian languages, Hindi and Urdu. Hindi and Urdu have distinct religious identities, national associations and scripts, yet they are nearly identical in syntax, diverging to some extent in their vocabulary. Hindi and Urdu speakers, consequently, understand each other most of the time, but not all of the time, though they can never read each other’s texts. Their shared space, Hindustani, finds no official recognition in India or in Pakistan, but it denotes, particularly in the early twentieth century, an aspiration for Hindu–Muslim unity: the dream of a shared, syncretic culture, crafted from the speech genres of everyday life. Beginning with the colonial project of Hindustani, the essay focuses on a discussion of the works of early twentieth-century writers like Nehru, Premchand and Sa’adat Hasan Manto. I argue that the aesthetic project of Hindustani attempted to produce, not a common language, but a common idiom: a set of shared conventions, phrases and forms of address, which would be legible to Indians from all religions and all regions. By theorizing Hindustani as an idiom, and not a language, I explain its persistence in Bollywood cinema well after its abandonment in all literary and official registers. Bollywood, I argue, is Hindustani cinema, not only because of its use of a mixed Hindi–Urdu language in its dialogues, but also because of its development of a set of clearly recognizable, easily repeatable conventions that can surmount linguistic differences

    Treatment and seroconversion in a cohort of children suffering from recent chronic Chagas infection in Yoro, Honduras

    Between 1999-2002, Médécins Sans Frontières-Spain implemented a project seeking to determine the efficacy and safety of benznidazole in the treatment of recent chronic Chagas disease in a cohort of seropositive children in the Yoro Department, Honduras. A total of 24,471 children were screened for Trypanosoma cruzi IgG antibodies through conventional enzyme-linked immunosorbent assays (ELISA) on filter paper. Recombinant ELISA (0.93% seroprevalence) showed 256 initially reactive cases, including 232 confirmed positive cases. Of these, 231 individuals were treated with benznidazole (7.5 mg/kg/day) for 60 days and were followed with a strict weekly medical control and follow-up protocol. At the end of the project, 229 patients were examined by the Honduras Secretariat of Health for post-treatment serological assessments; 88.2% seroconverted after 18 months and 93.9% seroconverted after three years. No differences were found in the seroconversion rates according to age or sex. Most of the side effects of the treatment were minor. These results support the argument that in areas where T. cruzi I is predominant and in areas affected by T. cruzi II, when vector transmission has been interrupted, Chagas disease diagnosis and treatment are feasible, necessary and ethically indisputable
