    Insufficient access to harm reduction measures in prisons in 5 countries (PRIDE Europe): a shared European public health concern

    Background: Prisoners constitute a high-risk population, particularly for infectious diseases. The aim of this study was to estimate the level of infectious risk in the prisons of five different European countries by measuring to what extent the prison system adheres to WHO/UNODC recommendations. Methods: Following the methodology used in a previous French survey, a postal/electronic questionnaire was sent to all prisons in Austria, Belgium, Denmark and Italy to collect data on the availability of several recommended HIV-HCV prevention interventions and HBV vaccination for prisoners. A score was built to compare adherence to WHO/UNODC recommendations (considered a proxy of environmental infectious risk) in those 4 countries. It ranged from 0 (no adherence) to 12 (full adherence). A second score (0 to 9) was built to include data from a previous French survey, thereby creating a 5-country comparison. Results: A majority of prisons answered in Austria (100 %), France (66 %) and Denmark (58 %), half in Belgium (50 %) and few in Italy (17 %), representing 100, 74, 89, 47 and 23 % coverage of the prison populations, respectively. Availability of prevention measures was low, with median adherence scores ranging from 3.5 to 4.5 at the national level. These results were confirmed when using the second score which included France in the inter-country comparison. Overall, the adherence score was inversely associated with prison overpopulation rates (p = 0.08). Conclusions: Using a score of adherence to WHO/UNODC recommendations, the estimated environmental infectious risk remains extremely high in the prisons of the 5 European countries assessed. Public health strategies should be adjusted to comply with the principle of equivalence of care and prevention with the general community

    Availability, accessibility, and coverage of needle and syringe programs in prisons in the European Union

    Needle and syringe programs (NSPs) are among the most effective interventions to control infection transmission among people who inject drugs in prisons. This review aimed to evaluate the availability, accessibility, and coverage of NSP in prisons in the European Union countries. In line with the “Preferred Reporting Items for Systematic Reviews and Meta-Analyses” criteria, four databases of peer-reviewed publications (PubMed/Medline, ISI Web of Science, EBSCO, and ScienceDirect), and 53 databases for grey literature were systematically searched to collect data published from January 2008 to August 2018. A total of 23,969 documents (17,297 papers and 6,672 grey documents) were identified, of them 26 were included into the study. In 2018, imprisonment rates in 28 EU countries ranged between 51 per 100,000 in Finland and 235 per 100,000 in Lithuania. Only four countries namely Germany (in one prison), Luxemburg (no coverage data were found), Romania (available in more than 50% of prisons), and Spain (in all prisons) have needle and syringe programs in prisons. Portugal stopped the program after a six-months pilot phase. Despite the protective impact of the prison-based NSP on infection transmission, only four EU countries distribute sterile syringes among people who inject drugs in prisons, and coverage of the program within these countries is very low. Since most prisoners will eventually return to the community, lack of NSP in EU prisons is not only a threat to the health of prisoners but also endangers public health

    Potential novel drug carriers for inner ear treatment: hyperbranched polylysine and lipid nanocapsules

    AIM: Treatment of sensorineural hearing loss could be advanced using novel drug carriers such as hyperbranched polylysine (HBPL) or lipid nanocapsules (LNCs). This study examined HBPL and LNCs for their cellular uptake and possible toxicity in vitro and in vivo as the first step in developing novel nanosized multifunctional carriers. METHOD: Having incubated HBPL and LNCs with fibroblasts, nanoparticle uptake and cell viability were determined by confocal laser scanning microscopy, fluorescence measurements and neutral red staining. In vivo, electrophysiology, confocal laser scanning microscopy and cytocochleograms were performed for nanoparticle detection and also toxicity studies after intracochlear application. RESULTS: Both nanoparticles were detectable in the fibroblasts\u27 cytoplasm without causing cytotoxic effects. After in vivo application they were visualized in cochlear cells, which did not lead to a change in hearing threshold or loss of hair cells. Biocompatibility and traceability were demonstrated for HBPL and LNCs. Thus, they comply with the basic requirements for drug carriers for potential application in the inner ear

This will be achieved by selective membranes with high permeability for CO2, O2 or H2, so that high-purity CO2 is obtained in a readily condensable form.     Filariasis of the Axilla in a Patient Returning from Travel Abroad: A Case Report

    Background: The term filariasis comprises a group of parasitic infections caused by helminths belonging to different genera in the superfamily Filaroidea. The human parasites occur mainly in tropical and subtropical regions, but filariae are also found in temperate climates, where they can infect wild and domestic animals. Humans are rarely infected by these zoonotic parasites. Patients and Methods: A 55-year-old patient presented with a new-onset, subcutaneous, non-tender palpable mass in the right axilla. Ultrasonography showed a 1.3-cm, solid, singular encapsulated node. Sonography of the breast on both sides, axilla and lymphatic drainage on the left side, lymphatic drainage on the right side, and mammography on both sides were without pathological findings. The node was excised under local anesthesia as the patient refused minimal invasive biopsy. Results: On histopathological examination, the tail of a parasite of the group of filariae was found. The patient revealed that she had stayed in Africa and Malaysia for professional reasons. 6 months before the time of diagnosis, she had also suffered from a fever and poor general condition after a trip abroad. The patient was referred for further treatment to the Institute for Tropical Medicine at the University of Dusseldorf, where a treatment with ivermectin was conducted on the basis of positive staining with antibodies against filariae. Conclusion: Our case demonstrates the importance of interdisciplinary collaboration between breast center, pathology, and other specialties such as microbiology and tropical medicine

    Unintended consequences of drug policies experienced by young drug users in contact with the criminal justice systems

    The aim of this paper is to assess to what extent prohibitive drug policies hamper the management of drug problems from the perspective of young people who have experience with the criminal justice systems (CJS). Qualitative, in-depth interviews were carried out in six European countries (Austria, Denmark, Germany, Italy, Poland, and the UK) following a common interview guide to obtain comparative data on the life trajectories of drug experienced youth. Altogether 198 interviews with people aged 14–25 years were collected and analysed by national teams following a common coding book. Unintended consequences of drug policies for the individual and society were identified. Individual consequences included health consequences and traumatic experiences with law enforcement. Social consequences included those affecting social relations such as stigmatisation and those impacting on institutions, for example, focusing on drug use and neglecting other problems. This paper confirmed earlier research indicating unintended consequences of prohibitive drug policies but also added to the literature its cross-national perspective and use of young people narratives as a source of analyses. There are, however, policy measures available that may reduce the volume and range of unintended effects. Their implementation is crucial to reduce the array of unintended consequences of prohibitive drug policies

    Glial cell line-derived neurotrophic factor and chronic electrical stimulation prevent VIII cranial nerve degeneration following denervation

    As with other cranial nerves and many CNS neurons, primary auditory neurons degenerate as a consequence of loss of input from their target cells, the inner hair cells (IHCs). Electrical stimulation (ES) of spiral ganglion cells (SGCs) has been shown to enhance their survival. Glial cell line-derived neurotrophic factor (GDNF) has also been shown to increase survival of SGCs following IHC loss. In this study, the combined effects of the GDNF transgene delivered by adenoviral vectors (Ad- GDNF ) and ES were tested on SGCs after first eliminating the IHCs. Animal groups received Ad- GDNF or ES or both. Ad- GDNF was inoculated into the cochlea of guinea pigs after deafening, to overexpress human GDNF . ES-treated animals were implanted with a cochlear implant electrode and chronically stimulated. A third group of animals received both Ad- GDNF and ES (GDNF/ES). Electrically evoked auditory brainstem responses were recorded from ES-treated animals at the start and end of the stimulation period. Animals were sacrificed 43 days after deafening and their ears prepared for evaluation of IHC survival and SGC counts. Treated ears exhibited significantly greater SGC survival than nontreated ears. The GDNF/ES combination provided significantly better preservation of SGC density than either treatment alone. Insofar as ES parameters were optimized for maximal protection (saturated effect), the further augmentation of the protection by GDNF suggests that the mechanisms of GDNF- and ES-mediated SGC protection are, at least in part, independent. We suggest that GDNF/ES combined treatment in cochlear implant recipients will improve auditory perception.     Evaluation of phosphorus in thermally converted sewage sludge: P pools and availability to wheat

    Aims: Dried sewage sludge (SS) and the by-products of four SS thermal conversion processes (pyrolysis, incineration and two types of gasification) were investigated for phosphorus (P) availability. Methods: A sequential extraction was used to determine the distribution of P among different P pools. After mixing materials with soil, availability of the P was determined with soil P extractions and in a growth experiment with wheat. Results: Thermally converted SS contained a greater proportion of P within recalcitrant pools than dried SS. Despite having very different P pool distributions, the incinerated and dried SS provided similar amounts of P to plants. Plant P supply from dried and incinerated SS was lower than the comparable soluble P treatment (50 mg P kg⁻Âč), but higher than a soluble treatment at a lower rate (20 mg P kg⁻Âč). Plant P uptake in gasified and pyrolysed treatments was only marginally greater than uptake in a control (no P) treatment. Plant P uptake correlated most closely with diffusive gradients in thin films (DGT) P analysis of soil-material mixes. Phosphorus availability in the dried and incinerated SS treatments increased over time.
