    SafetyKit: first aid for measuring safety in open-domain conversational systems

    The social impact of natural language processing and its applications has received increasing attention. In this position paper, we focus on the problem of safety for end-to-end conversational AI. We survey the problem landscape therein, introducing a taxonomy of three observed phenomena: the Instigator, Yea-Sayer, and Impostor effects. We then empirically assess the extent to which current tools can measure these effects and current systems display them. We release these tools as part of a “first aid kit” (SafetyKit) to quickly assess apparent safety concerns. Our results show that, while current tools are able to provide an estimate of the relative safety of systems in various settings, they still have several shortcomings. We suggest several future directions and discuss ethical considerations

    One year follow-up of physical performance and quality of life in patients surviving COVID-19: a prospective cohort study

    INTRODUCTION: The coronavirus disease (COVID-19) continues to affect many countries globally, with the long-term impact of the disease now being recognized. According to the latest research, some of the affected individuals continue to experience functional limitations, reduced physical performance and impaired health-related quality of life (HRQoL) even after eight months. This prospective cohort study aimed to describe the longer-term recovery of physical performance and HRQoL in COVID-19 survivors over one year. METHOD: A cohort (n = 43; 32-84 years old) hospitalized with COVID-19 between March and June 2020 was followed over one year and assessed at three time points: hospital discharge, 3 months and 12 months post-admission. Participants experienced mild (10/43) to critical (6/43) pneumonia and stayed in the hospital for a median of 10 days (IQR 9). Participants were assessed for physical performance (six-minute walk test), HRQoL (EQ-5D-5L), COVID-19 related limitations in functionality (PCFS), hospital-related anxiety and depression (HADS-A/-D), lung function (FEV1, FVC) and dyspnea during activity (mMRC). All assessments were conducted by physiotherapists trained in cardio-respiratory rehabilitation. RESULTS: After discharge, 8/34 showed reduced physical performance, 9/42 had lower HRQoL and 14/32 had COVID-19 induced limitations in functionality on the PCFS scale. Physical performance did not change significantly between discharge and 12-month follow-up, but 15/34 participants showed clinically relevant improvements in walking distance (>30 m). However, 16/34 had a decreased walking distance >30 m when comparing 3-month to 12-month follow-up. At 12 months, 12/41 of participants still perceived COVID-19 related limitations in daily life on the PCFS scale. For HRQoL, 12/41 participants still perceived moderate-to-severe symptoms of pain and discomfort and 13/41 slight-to-severe symptoms of anxiety and depression. CONCLUSION: This cohort of adult patients hospitalized for mild to severe COVID-19 in Switzerland was generally mildly affected but still reported some limitations after one year. These results offer preliminary indications for ongoing support after hospitalization and point towards the need for specific, individualized follow-up to support their recovery.     Exercise training undertaken by people within 12 months of lung resection for non-small cell lung cancer

    Background: Decreased exercise capacity and impairments in health-related quality of life (HRQoL) are common in people following lung resection for non-small cell lung cancer (NSCLC). Exercise training has been demonstrated to confer gains in exercise capacity and HRQoL for people with a range of chronic conditions, including chronic obstructive pulmonary disease and heart failure, as well as in people with cancers such as prostate and breast cancer. A programme of exercise training for people following lung resection for NSCLC may confer important gains in these outcomes. To date, evidence of its efficacy in this population is unclear.Objectives: The primary aim of this study was to determine the effects of exercise training on exercise capacity in people following lung resection (with or without chemotherapy) for NSCLC. The secondary aims were to determine the effects on other outcomes such as HRQoL, lung function (forced expiratory volume in one second (FEV1)), peripheral muscle force, dyspnoea and fatigue as well as feelings of anxiety and depression.Search methods: We searched the Cochrane Central Register of Controlled Trials (CENTRAL) (The Cochrane Library 2013, Issue 2 of 12), MEDLINE (via PubMed) (1966 to February 2013), EMBASE (via Ovid) (1974 to February 2013), SciELO (The Scientific Electronic Library Online) (1978 to February 2013) as well as PEDro (Physiotherapy Evidence Database) (1980 to February 2013).Selection criteria: We included randomised controlled trials (RCTs) in which study participants with NSCLC, who had recently undergone lung resection, were allocated to receive either exercise training or no exercise training.Data collection and analysis: Two review authors screened the studies and identified those for inclusion. Meta-analyses were performed using post-intervention data for those studies in which no differences were reported between the exercise and control group either: (i) prior to lung resection, or (ii) following lung resection but prior to the commencement of the intervention period. Although two studies reported measures of quadriceps force on completion of the intervention period, meta-analysis was not performed on this outcome as one of the two studies demonstrated significant differences between the exercise and control group at baseline (following lung resection).Main results: We identified three RCTs involving 178 participants. Three out of the seven domains included in the Cochrane Collaboration's 'seven evidence-based domains' table were identical in their assessment across the three studies (random sequence generation, allocation concealment and blinding of participants and personnel). The domain which had the greatest variation was ‘blinding of outcome assessment’ where one study was rated at low risk of bias, one at unclear risk of bias and the remaining one at high risk of bias. On completion of the intervention period, exercise capacity as measured by the six-minute walk distance was statistically greater in the intervention group compared to the control group (mean difference (MD) 50.4 m; 95% confidence interval (CI) 15.4 to 85.2 m). No between-group differences were observed in HRQoL (standardised mean difference (SMD) 0.17; 95% CI -0.16 to 0.49) or FEV1 (MD -0.13 L; 95% CI -0.36 to 0.11 L).     Medical sciences have long since established an ethics code for experiments, to minimize the risk of harm to subjects. Natural language processing (NLP) used to involve mostly anonymous corpora, with the goal of enriching linguistic analysis, and was therefore unlikely to raise ethical concerns. As NLP becomes increasingly wide-spread and uses more data from social media, however, the situation has changed: the outcome of NLP experiments and applications can now have a direct effect on individual users’ lives. Until now, the discourse on this topic in the field has not followed the technological development, while public discourse was often focused on exaggerated dangers. This position paper tries to take back the initiative and start a discussion. We identify a number of social implications of NLP and discuss their ethical significance, as well as ways to address them

    Divergence and convergence in nutrition science

    Nutrigenomics diverged from mainstream nutrition science, ideologically, instrumentally and culturally, due to the establishment of a protective niche. That protection is fading. This article chronicles a case in which convergence between nutrigenomics and nutrition science is pursued. Here we report the opportunities and hurdles that researchers within two large Dutch research consortia encountered when trying to engineer collaboration. The most salient hurdles are the complexity of and unfamiliarity with nutrigenomics, unclear and unshared notions of relevance; difficulties in organising data exchange; and the existence of two research cultures. Overcoming these technical and social hurdles will require the development of a shared 'future' for the relationship between nutrigenomics and nutrition science. (C) 2015 Elsevier Ltd.     Contentious governance of wind energy planning: strategic dilemmas in collaborative resistance by local governments and citizen action groups

    Local governments are at the heart of implementing increasingly ambitious national plans for wind energy. While support by local governments for these plans has been studied extensively, only few studies have looked into local governments’ contestation of wind energy development. In this paper we analyze contentious governance processes in which local governments join efforts with citizen action groups to oppose projects proposed by developers or the national government. We focus on the strategic dilemmas that local governments face while engaging in contentious governance. Uncovering strategic dilemmas helps to see contestation by local governments as part of a web of governance relationships, thus moving beyond dichotomous understandings of the players involved in wind farm conflicts. Strategic dilemmas also allow understanding of how stances of local governments and their citizen allies change over time, and how strategic and tactical considerations emerge from interaction patterns.Organisation and Governanc

    When controversies cascade: Analysing the dynamics of public engagement and conflict in the Netherlands and Switzerland through “controversy spillover”

    Energy controversies have been widely studied. Such studies are, however, generally based on either single case studies, providing rich and in-depth understanding of (local) dynamics of planning and implementation processes, or they focus on understanding responses to a specific technology (not bound to a location). Therefore these studies tend to overlook a key dynamic in controversy, namely that publics respond to projects by drawing on earlier experiences with a similar technology elsewhere, or with earlier experiences with other technologies in their vicinity. We refer to this dynamic as controversy spillover. The notion of controversy spillover helps to understand how the discursive space of controversy changes over time. In case studies, other controversies are usually considered as context, i.e. as an external condition. However, in order to understand the temporal dynamics of public engagement with energy projects, spillover from other controversies deserves to be investigated more as an object of interest, rather than as an external condition. The aim of this paper is to conceptualize controversy spillover as an important dynamic in controversies and to develop a research agenda. We identify three different types of spillover: 1) geographical (i.e. between the same energy technology in different locations), 2) historical (i.e. with respect to earlier experiences at the same location), 3) technology (i.e. between different technologies). Three empirical examples serve to illustrate the three types of spillover. We finalize the paper with a research agenda for further conceptualization and empirical analysis of the notion of controversy spillover.ISSN:2214-629
